Pemberian ASI Eksklusif memiliki banyak manfaat untuk bayi dan ibu. UNICEF sendiri menargetkan setiap negara mampu mencapai 80% dalam memberikan ASI Eksklusif. Terdapat beberapa faktor yang menyebabkan masih rendahnya angka pencapaian terhadap pemberian ASI secara Eksklusif di berbagai negara. Salah satu kendalnya adalah ibu melahirkan melalui operasi sesar. Peningkatan angka persalinan dengan metode sesar dapat mempengaruhi kesehatan ibu, anak dan kehamilan berikutnya. Penelitian ini bertujuan untuk mengetahui faktor determinan kegagalan ASI Eksklusif pada ibu melahirkan sesar di Provinsi Sumatera Utara. Penelitian ini menggunakan data sekunder yang berasal dari data sekunder Survey Demografi dan Kesehatan Indonesia (SDKI) tahun 2017 dengan menggunakan metode kuantitatif dengan desain studi cross sectional. Hasil penelitian menunjukkan ada hubungan antara status pekerjaan ibu dan kegagalan ASI Eksklusif (OR=2,616; 95% CI=1.428-4.790). Tidak ada hubungan antara usia ibu dengan kegagalan ASI eksklusif (OR=0,552; 95% CI=0,284-1,073); pendidikan (OR=1,163; 95% CI=0,588-2,299); paritas (OR=1,301; 95% CI=0,701-2,415); status ekonomi (OR=0,668; 95% CI=0,379-1,249); daerah tinggal (OR=0,833; 95% CI=0,450-1,542); jenis kelamin (OR=0,837; 95% CI=0,464-1,508) ; IMD (OR=0,815; 95% CI=0,416-1,597) dan ANC (OR=1,097; 95% CI=0,423-2,845). Faktor yang paling dominan terhadap kegagalan ASI Eksklusif adalah Status Pekerjaan (p value 0,002 OR= 2,616), ibu yang tidak bekerja memiliki risiko 2,616 kali lebih tinggi untuk gagal ASI Eksklusif dibandingkan ibu yang bekerja. Diperlukan adanya kolaborasi multisektoral dalam meningkatkan kesadaran ibu terhadap pentingnya pemberian ASI eksklusif bagi ibu dan bayi.
Exclusive breastfeeding has many benefits for babies and mothers. UNICEF itself targets that every country be able to achieve 80% coverage of exclusive breastfeeding. There are several factors that contribute to the low achievement rate of exclusive breastfeeding in various countries. One of the constraints is the mother giving birth by cesarean section. The increase in the number of deliveries by caesarean method can affect the health of mothers, children, and subsequent pregnancies. This study aims to determine the determinants of the failure of exclusive breastfeeding in women giving birth by cesarean section in North Sumatra Province. This study uses secondary data derived from secondary data from the 2017 Indonesian Demographic and Health Survey (IDHS) using a quantitative method with a cross-sectional study design. The results showed that there was a relationship between the mother's employment status and exclusive breastfeeding failure (OR=2.616; 95% CI=1,428-4,790). There is no relationship between maternal age and failure of exclusive breastfeeding (OR=0,552; 95% CI=0,284-1,073); education (OR=1.163; 95% CI=0,588-2,299); parity (OR=1,301; 95% CI=0,701-2,415); economic status (OR=0,688; 95% CI=0,379-1,249); area of residence (OR=0,833; 95% CI=0,450-1,542); baby’s gender (OR=0.837; 95% CI=0,464-1,508); IMD (OR=0,815; 95% CI=0,416-1,597) and ANC (OR=1,097; 95% CI=0,423-2,845). The most dominant factor for failure of exclusive breastfeeding is employment status (p value 0.002 OR = 2.616), mothers who do not work have a 2.616 times higher risk of failure of exclusive breastfeeding than working mothers. There is a need for multi-sectoral collaboration to increase mothers awareness of the importance of exclusive breastfeeding for both mother and baby.

Meskipun manfaat dari ASI eksklusif sudah tidak diragukan lagi, cakupan pemberian ASIeksklusif di Indonesia masih rendah, yaitu hanya 42% pada tahun 2012. Pekerjaan ibuterbukti merupakan faktor yang berisiko tinggi untuk menghambat ibu memberikan ASIeksklusif. Maka dari itu, penelitian ini berusaha menggunakan konsep positive devianceuntuk menelusuri faktor determinan keberhasilan pemberian ASI eksklusif padakelompok ibu yang memiliki risiko tinggi untuk tidak ASI eksklusif, yaitu ibu bekerja.Penelitian dengan metode cross-sectional ini menganalisis data sekunder dari 376 ibubekerja dengan bayi usia 0-5 bulan di Indonesia pada SDKI tahun 2017. Analisis denganuji chi-square dan regresi logistik ganda dengan confidence interval 95% dilakukan untukmenganalisis faktor yang berhubungan signifikan dengan pemberian ASI eksklusif. Diantara 376 ibu yang bekerja, 45,5% nya diidentifikasi sebagai pelaku positive deviance,yang berhasil memberikan ASI eksklusif. Pada analisis multivariat, ditemukan kunjunganANC, kunjungan PNC, dan status ekonomi sebagai faktor yang berhubungan secarasignifikan dengan pemberian ASI eksklusif di antara ibu bekerja. Kunjungan ANCmerupakan faktor dominan yang berhubungan dengan ASI eksklusif dengan OR 1,831(95% CI: 1,075-3,118). Perilaku melakukan kunjungan ANC dan PNC, termasukmendapatkan edukasi dan konseling terkait menyusui dari petugas kesehatan saatkunjungan tersebut dapat mendukung pemberian ASI eksklusif di antara ibu bekerja diIndonesia.

Abstrak:
Pemberian ASI eksklusif menjadi pelindung terhadap kejadian stunting pada anak balita, memberikan dampak pertumbuhan yang lebih baik pada bayi dalam enam bulan pertama, serta menunjukkan IQ poin yang lebih tinggi daripada yang tidak mendapat ASI eksklusif. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan pemberian ASI eksklusif di Provinsi Jawa Barat Tahun 2018. Penelitian ini merupakan penelitian kuantitatif dengan pendekatan desain potong lintang. Data yang digunakan adalah data Riskesdas 2018 yang diperoleh dari laboratorium Manajemen Data Balitbangkes Kemenkes RI. Populasi penelitian ini berjumlah 604 ibu yang memiliki bayi usia 6 - 11 bulan, sampel penelitian adalah yang memiliki data riwayat pemberian ASI sebagai variabel dependen penelitian sebanyak 601. Hasil penelitian menunjukkan sebanyak 29,8% ibu memberikan ASI eksklusif selama enam bulan. Analisis multivariat menunjukkan bahwa variabel yang berhubungan dengan pemberian ASI eksklusif adalah IMD (p=0,010; OR=2,204) setelah dikontrol oleh variabel frekuensi ANC dan konseling. Variabel paling dominan yang berhubungan dengan pemberian ASI eksklusif adalah IMD, ibu yang melakukan IMD berpeluang memberikan ASI Eksklusif sebanyak 2,204 kali dibandingkan ibu yang tidak melakukan IMD. Saran: Meningkatkan kegiatan edukasi pada ibu hamil maupun calon ibu tentang IMD dan manfaat pemberian ASI eksklusif melalui Posyandu serta bersinergi dengan lintas sektor

Abstrak:
Makanan prelakteal adalah makanan atau minuman selain ASI yang diberikan kepada bayi sebelum menyusui dalam 3 hari pertama kehidupan yang dapat menyebabkan gagalnya pemberian ASI Eksklusif dan mendorong risiko infeksi dan malnutrisi yang kemudian berdampak pada stunting. Satu dari dua bayi yang pernah diberikan ASI di Indonesia pernah diberikan makanan prelakteal. Penelitian ini bertujuan untuk mengetahui faktor determinan perilaku pemberian makanan prelakteal pada bayi di pulau Sumatera. Penelitian ini menggunakan data sekunder yang berasal dari SDKI tahun 2017 dengan rancangan studi cross sectional. Sampel penelitian ini adalah ibu yang memiliki bayi usia 0 ? 23 bulan yang memenuhi kriteria inklusi dengan jumlah 1.224 responden. Hasil penelitian menunjukkan bahwa terdapat 54,4 persen ibu yang memberikan makanan prelakteal. Pada analisis hubungan didapatkan hasil bahwa usia ibu, paritas, IMD, dan jenis persalinan memiliki hubungan yang signifikan dengan pemberian makanan prelakteal pada bayi (p-value < 0,05). Hasil uji regresi logistik ganda menunjukkan IMD sebagai faktor dominan yang menyebabkan pemberian makanan prelakteal pada bayi di pulau Sumatera (OR 6,06) yakni ibu yang terlambat memberikan IMD berpeluang 6 kali lebih berisiko untuk memberikan makanan prelakteal pada bayi di pulau Sumatera setelah dikontrol oleh variabel berat lahir, ANC, jenis persalinan, usia ibu, paritas, dan tenaga penolong persalinan.

Indonesia merupakan salah satu negara dengan prevalensi anemia ibu hamil yang tergolong tinggi dibandingkan dengan negara-negara berkembang lain. Padahal anemia pada ibu hamil menyumbang 40% penyebab kematian ibu yang tinggi di Indonesia, selain itu juga berdampak pada pertumbuhan janin yang terhambat, meningkatkan risiko BBLR dan bayi lahir prematur, serta mengakari stunting dan anemia dini. Salah satu cara yang direkomendasikan oleh WHO untuk mencegah anemia pada ibu hamil adalah melakukan suplementasi Tablet Tambah Darah (TTD) oral setiap hari sebagai bagian dari perawatan antenatal pada ibu hamil. Kemenkes (2015) menganjurkan untuk ibu hamil mengonsumsi TTD minimal 90 tablet selama masa kehamilan sebagai bentuk pencegahan anemia. Berdasarkan survei SDKI 2017, provinsi dengan tingkat konsumsi TTD terendah adalah Provinsi Sulawesi Tenggara (14%) disusul dengan Provinsi Sumatera Utara (15%). Pada hasil riset SDKI 2012 sebelumnya Provinsi Sumatera Utara juga berada di posisi 4 terendah konsumsi TTD ibu hamil sesuai anjuran, dengan persentase 8,7%. Dengan demikian tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang berhubungan dengan kepatuhan ibu hamil dalam mengonsumsi TTD. Penelitian ini menggunakan desain cross sectional dan data yang diambil adalah data sekunder SDKI 2017 di Provinsi Sumatera Utara dengan sampel sebanyak 464 orang. Variabel dalam penelitian adalah: umur, pekerjaan, pendidikan, wilayah tempat tinggal, paritas, jarak antar kelahiran, frekuensi ANC, jenis tenaga dan tempat pemeriksaan kehamilan, dan ketersediaan TTD di rumah. Hasil analisis univariat adalah ibu hamil di Sumatera Utara menunjukkan bahwa lebih banyak persentase ibu yang masuk dalam kelompok umur berisiko, memiliki pendidikan rendah dan menengah, dengan status bekerja, tinggal di perkotaan, memiliki riwayat paritas multipara, memiliki jarak antar kelahiran ≥24 bulan, frekuensi ANC sesuai, nakes sebagai tenaga ANC, faskes sebagai tempat ANC dan memiliki ketersediaan TTD di rumah. Hasil dari penelitian bivariat menunjukkan tiga variabel terbukti secara statistik memiliki hubungan yang bermakna dengan kepatuhan ibu mengonsumsi TTD yakni pendidikan (p=0,002, OR=2,153), paritas (p=0,020, OR=3,544) dan frekuensi ANC (p=0,001, OR=2,419). Sedangkan variabel yang tidak menunjukkan hubungan yang bermakna antara lain umur (p=0,444), pekerjaan (p=0,236), wilayah tempat tinggal (p=0,523), jarak antar kelahiran (p=0,968), tenaga ANC (p=0,517), tempat ANC (p=1,000) dan ketersediaan TTD di rumah (tidak dapat dianalisis). Saran bagi instansi terkait dapat melakukan kolaborasi lintas sektor untuk mengatasi rendahnya tingkat pendidikan wanita di Sumatera Utara, menggencarkan promosi dan informasi TTD di sosial media, menggencarkan penyuluhan dan edukasi mengenai KB, membuat program layanan tambahan mengenai TTD untuk ibu hamil dengan pendidikan rendah. Selain itu bagi peneliti lain dapat melakukan penelitian yang lebih baik dari segi pengujian data, metode dan sumber data.
Compared to other developing countries, Indonesia is one of the high prevalence countries with anemia among pregnant women. Whereas anemia in pregnant women causes 40% of high maternal mortality in Indonesia, it also impact on retarding fetal growth, increasing the risk of low birth weight and premature birth, as well as rooting stunting and early anemia. WHO recommends to prevent anemia in pregnant women by iron supplementation every day as part of antenatal care. Kemenkes RI (2015) recommends pregnant women to consume a minimum 90 tablets of iron supplement during pregnancy as a form of anemia prevention. Based on the IDHS 2017 survey, the province with the lowest level of iron consumption was Southeast Sulawesi (14%) followed by North Sumatra (15%). In the previous 2012 IDHS research results, North Sumatra was also in the 4th lowest position in the consumption of iron for pregnant women with 8.7%. Thus, the purpose of this study was to determine what factors influence the adherence to Iron Supplement consumption. This can be seen from several factors such as age, occupation, education, area of residence, parity, distance between births, frequency of ANC, type ANC workers, ANC place, and availability of iron tablets at home. This study uses a cross sectional design and using secondary data from the IDHS 2017 in North Sumatra with a sample of 464 people. The results of univariate analysis were pregnant women in North Sumatra showed that there was a higher percentage of mothers who belonged to the risk age group, had low and secondary education, working, lived in urban areas, multiparity parity, had a birth interval of 24 months, the frequency of ANC appropriate, health workers as ANC personnel, health facilities as ANC places and have the availability of iron tablets at home. The results of bivariat analysis showed that three variables were statistically proven to have a significant relationship with adherence of taking iron tablets among pregnant women, such as education (p=0.002, OR=2.153), parity (p=0.020, OR=3.544) and frequency of ANC (p=0.001, OR =2.419). While the variables that did not show a significant relationship were age (p=0.444), occupation (p=0.236), area of residence (p=0.523), distance between births (p=0.968), ANC personnel (0.517), ANC location (p=1,000) and availability of iron at home (can not be analyzed). From this study there is some suggestions for the government can collaborate across sectors to overcome the low level of women's education in North Sumatra, intensify promotion and information on iron tablets on social media, intensify counseling and education about family planning, create additional service programs regarding iron tablets for pregnant women with low education. In addition, other researchers can do better research in terms of testing data, methods and data sources.

Angka Kematian Bayi merupakan salah satu indikator keberhasilan program kesehatan Ibu dan anak. Pemberian ASI eksklusif kepada bayi merupakan salah satu cara efektif agar dapat menurunkan Angka Kematian Bayi di Indonesia. Cakupan ASI eksklusif berdasarkan Riskesdas 2018 sebesar 37,3%, hal tersebut belum memenuhi minimal 50% dari target nasional. Penelitian ini bertujuan untuk menentukan faktor yang mempengaruhi pemberian ASI eksklusif 6 bulan pada anak usia 6-23 bulan di Pulau Jawa. Penelitian ini menggunakan data sekunder dari Riskesdas tahun 2018 dengan desain cross-sectional. Sampel penelitian berjumlah 3.623 yang terdiri dari ibu berusia 15-49 tahun yang memiliki anak berusia 6-23 bulan terakhir. Analisis data berupa analisis bivariabel dengan metode chi-square dan analisis multivariabel dengan metode regresi logistik berganda. The infant mortality rate was an indicator of the success of maternal and child health programs. Giving exclusive breast milk to babies is one effective way to reduce the infant mortality rate in Indonesia. Exclusive breastfeeding coverage based on Riskesdas 2018 is 37.3%. This number does not meet the minimum 50% of the national target. This study aims to determine the factors that influence exclusive breastfeeding for Six months in children aged 6-23 months on the island of Java. This research uses secondary data from the 2018 Riskesdas with a cross-sectional design. The research sample was 3,623 consist of mothers aged 15-49 years who had children aged 6-23 months. Data analysis took the form of bivariable analysis using the chi-square method and multivariable analysis using the multiple logistic regression method. Factors that influence exclusive breastfeeding for six months are the mother's age, mother's education, parity, baby's birth weight, delivery method, place of birth, IEB, frequency of ANC visits, and breastfeeding counseling during PNC. Cakupan ASI eksklusif berdasarkan Riskesdas 2018 sebesar 37,3%, hal tersebut belum memenuhi minimal 50% dari target nasional. Penelitian ini bertujuan untuk menentukan faktor yang mempengaruhi pemberian ASI eksklusif 6 bulan pada anak usia 6-23 bulan di Pulau Jawa. Penelitian ini menggunakan data sekunder dari Riskesdas tahun 2018 dengan desain cross-sectional. Sampel penelitian berjumlah 3.623 yang terdiri dari ibu berusia 15-49 tahun yang memiliki anak berusia 6-23 bulan terakhir. ASI eksklusif terbukti menjadi makanan terbaik yang dapat diberikan ibu kepada anaknya selama 6 bulan pertama. Rendahnya cakupan ASI eksklusif di Indonesia perlu menjadi perhatian mengingat tingginya risiko kesehatan yang dapat mengancam pertumbuhan, kesehatan, hingga menyebabkan kematian bayi jika tidak ASI eksklusif. Berbagai faktor ditemukan menjadi penentu dalam praktik pemberian ASI eksklusif. Penelitian ini dilakukan untuk melihat faktor dominan yang berhubungan dengan pemberian ASI eksklusif hingga 6 bulan pada anak usia 6-23 bulan di Indonesia. Desain yang digunakan adalah cross-sectional dengan menggunakan data sekunder IFLS-5 tahun 2014-2015 yang memiliki sampel anak usia 6-23 bulan sebanyak 1550 orang. Data dianalisis menggunakan uji chi square dan uji regresi logistik ganda. Hasil analisis menunjukkan prevalensi pemberian ASI eksklusif hingga usia minimal 5 bulan adalah sebesar 24,9%. Analisis bivariat menemukan beberapa faktor yang berhubungan signifikan dengan pemberian ASI eksklusif, yaitu usia ibu, pendidikan ibu, berat badan lahir, tempat persalinan, penolong persalinan, dan kunjungan ANC. Faktor status pekerjaan, status perkawinan, paritas, pengetahuan terkait ASI eksklusif, jenis kelamin, wilayah tempat tinggal, dan kunjungan PNC ditemukan tidak berhubungan signifikan dengan pemberian ASI eksklusif dalam penelitian ini. Hasil analisis multivariat menemukan usia ibu sebagai faktor dominan pemberian ASI eksklusif pada ibu dengan anak usia 6-23 bulan di Indonesia dengan OR 2,13. Penelitian ini menunjukkan bahwa optimalisasi praktik menyusui pada usia reproduktif dapat meningkatkan keberhasilan pemberian ASI eksklusif selama 6 bulan hingga 2,1 kali lebih tinggi.

Pemberian ASI eksklusif memiliki dampak positif baik bagi ibu maupun bayinya, namun persentase cakupan pemberian ASI eksklusif pada bayi usia 0-5 bulan di Nusa Tenggara Barat berdasarkan data Riskesdas 2018, yaitu hanya sebesar 20,3%. Hal ini masih dibawah target pencapaian indikator ASI Eksklusif yang ditetapkan oleh Kementerian Kesehatan, yaitu 80%. Penelitian ini bertujuan untuk mengetahui faktor dominan dan faktor-faktor yang berhubungan dengan pemberian ASI eksklusif pada bayi usia 0-23 bulan di NTB. Penelitian ini merupakan penelitian kuantitatif dengan desain studi cross sectional dengan sampel berjumlah 802 bayi usia 0-23 bulan yang tinggal di NTB serta menggunakan data Riset Kesehatan Dasar 2018. Analisis data yang digunakan adalah analisis univariat, analisis bivariat menggunakan chi square, dan analisis multivariat menggunakan regresi logistik berganda. Hasil menunjukan bahwa dari 802 subjek, 56,7% diberikan ASI eksklusif. Analisis bivariat juga menunjukan antara paritas dan IMD dengan pemberian ASI eksklusif (p-value < 0,05). Analisis multivariat menunjukan bahwa paritas merupakan faktor dominan pemberian ASI eksklusif (p-value = 0,002 ; OR : 1,6 ; 95% CI : 1,1 – 2,1).
EBF has a positive impact for both the mother and the baby, but based on the Indonesia Basic Health Research (Riskesdas) in 2018, only 20,3% of infants aged 0-5 months in West Tenggara Province are exclusive breastfed. It is still has not reached target indicator set by the Ministry of Health, which is 80%. This study aims to identify the determinant factor and related factor associated with EBF in infants aged 0-23 months in West Tenggara Province. This research is a quantitative study with a cross sectional study design with a sample of 802 infants aged 0-23 months who live in West Nusa Tenggara. The data was obtained from the Indonesia Basic Health Research 2018. The association between risk factor and EBF were measured through chi square bivariate analysis. Multivariate analysis was done using multiple logistic regression. Among 802 subjects, 56,7% breastfed exclusively. This study found that parity and early initiation of breastfeeding (EIBF) were significantly associated with EBF (p-value < 0,05). Parity was the most dominant risk factor of EBF (p-value = 0,002 ; OR : 1,6 ; 95% CI : 1,1 – 2,1).
