Disparitas antara layanan kesehatan diabetes melitus antara kota dan kabupaten masih menjadi tantangan di Indonesia meskipun sudah ada Jaminan Kesehatan Nasional. Akses ini menjadi penting karena pasien sangat bergantung terhadap manajemen diabetes melitus tipe 1 untuk menjaga kualitas hidupnya. Penelitian ini bertujuan untuk mendapatkan gambaran akses layanan kesehatan pada pasien diabetes melitus tipe 1 di Indonesia tahun 2015-2022 berdasarkan kabupaten/kota. Penelitian ini menggunakan data sampel BPJS Kesehatan tahun 2015-2022 dengan desain penelitian cross sectional. Analisis statistik deskriptif dilakukan untuk melihat proporsi pada setiap karakteristik pasien berdasarkan kabupaten/kota. Hasil penelitian menunjukkan sebanyak 3,8% pasien memiliki akses adekuat dengan mayoritas adalah laki-laki, berada dalam segmentasi PBI, berobat ke wilayah kota apabila tinggal di wilayah kabupaten, berobat ke wilayah kabupaten apabila tinggal di wilayah kota, dan memiliki komorbid hipertensi. Perluasan jaminan manfaat program Prolanis dan pedoman manajemen penyakit diabetes melitus tipe 1 menjadi hal yang penting untuk meningkatkan akses pada pasien.
Healthcare access disparity between cities and regencies in diabetes mellitus healthace is a challenging issue despite Indonesia already have National Health Insurance. This access become an important subject because type 1 diabetes mellitus patients depend on good management to maintain their quality of life. The aim of this study is to describe the healthcare access among type 1 diabetes mellitus patients based on regency-city status. This study used Indonesia National Health Insurance 2015-2022 Sample Data with cross sectional as its study design. The results showed that 3,8% type 1 diabetes mellitus in patients in Indonesia have adequate access with majority of them are men, in PBI scheme, seeking treatment in the city if they lived in the regency, seeking treatment in the regency if they lived in the city, and have hypertension as a comorbid. The expansion of Prolanis program and type 1 diabetes mellitus management guidelines are important to improve patients’ access to healthcare.

Diabetes melitus tipe 2 adalah penyakit tidak menular yang dapat diobati dan konsekuensinya dapat dihindari atau ditunda dengan pola makan, aktivitas fisik, pengobatan, serta skrining dan pengobatan komplikasi secara teratur. Namun, penyakit ini didiagnosis beberapa tahun setelah timbul sehingga komplikasi dan komorbid telah muncul dan menjadi kasus penyakit yang sering sekali masuk dalam daftar 10 besar penyakit yang menjalani rawat inap. Penelitian ini bertujuan untuk mengetahui tingkat keparahan dan komorbiditas terhadap lama rawat inap pasien diabetes melitus tipe 2 di FKRTL pada peserta BPJS Kesehatan tahun 2023, dengan dikontrol oleh variabel tipe FKRTL, kepemilikan FKRTL, segmentasi, kelas rawat, usia, dan jenis kelamin. Penelitian ini menggunakan data sampel BPJS Kesehatan 2023 dengan studi potong-lintang. Analisis yang dilakukan mencangkup analisis univariat, bivariat, dan multivariat. Analisis bivariat menghasilkan hubungan yang signifikan antara tingkat keparahan terhadap lama rawat inap (p-value= 0,001) dan hasil yang tidak signifikan untuk komorbiditas dengan lama rawat inap (p-value= 0,285). Tingkat keparahan sedang dan berat dan komorbiditas dengan skor CCI lebih dari sama dengan 1 beresiko lebih tinggi untuk menjalani lama rawat inap panjang dan beresiko lebih rendah untuk menjalani rawat inap pendek daripada ideal (RRR=4,95; 95%CI=0,82–29,85; RRR=0,46; 95%CI=0,29–0,72 | RRR=1,11; 95%CI= 0,25–4,92; RRR=0,67; 95%CI=0,41–1,10). Analisis multivariat untuk mengontrol variabel tipe FKRTL, kepemilikan FKRTL, segmentasi, kelas rawat, usia, dan jenis kelamin didapatkan hasil yang tetap signifikan antara tingkat keparahan dengan lama rawat inap setelah dikontrol oleh variabel tipe FKRTL dan kepemilikan FKRTL dan komorbiditas tetap tidak signifikan walaupun setelah dikontrol oleh variabel kontrol. Upaya peningkatan program deteksi dini klinis derajat tingkat keparahan dan komorbid DM tipe 2 agar tidak memperpanjang durasi rawat inap akibat komplikasi dan keparahan yang menimbulkan beban kesehatan yang berarti.

However, this disease is often diagnosed several years after onset, by which time complications and comorbidities may have developed, making it one of the top 10 causes of hospitalizations. This study aims to determine the effect of severity and comorbidity on the length of hospital stay among patients with type 2 diabetes mellitus in advanced referral health facilities (FKRTL) among BPJS Kesehatan participants in 2023, controlled for variables such as FKRTL type, FKRTL ownership, segmentation, care class, age, and gender. This research used 2023 BPJS Kesehatan sample data with a cross-sectional study design. The analysis included univariate, bivariate, and multivariate methods. Bivariate analysis showed a significant relationship between severity and length of stay (p-value = 0.001), while comorbidities were not significantly associated with length of stay (p-value = 0.285). Moderate to severe severity and comorbidities with a CCI score of ≥1 were associated with a higher risk of prolonged hospitalization and a lower risk of short hospitalization compared to the ideal length of stay (RRR = 4.95; 95% CI = 0.82–29.85; RRR = 0.46; 95% CI = 0.29–0.72 | RRR = 1.11; 95% CI = 0.25–4.92; RRR = 0.67; 95% CI = 0.41–1.10). Background: Type 2 diabetes mellitus (T2DM) is a non-communicable disease with one of the highest health burdens in Indonesia, with prevalence nearly doubling over a decade, from 6.9% (2013) to 11.7% (2023), placing Indonesia as the country with the fifth-highest number of diabetes patients in the world (20.4 million in 2024). T2DM patients face a threefold higher risk of cardiovascular disease and a tenfold higher risk of kidney disease, with complication-related costs accounting for approximately 74% of total direct medical costs covered by BPJS Kesehatan. The effectiveness of prevention programs such as Prolanis depends heavily on understanding the factors that accelerate complication onset. Therefore, this study aimed to determine the survival of T2DM patients enrolled in the National Health Insurance (JKN) program with respect to complication onset, along with its associated determinants. Methods: This study used BPJS Kesehatan Sample Data from 2020, employing a retrospective closed-cohort design with an observation period of 2020–2025. Analyses were conducted using the Kaplan-Meier method and Cox Proportional Hazard regression. Results: Of 2,704 T2DM patients included in the study, 54.1% experienced complications, with a 4-year survival rate of 0.473 (95% CI: 0.45–0.49) and a median time-to-event of 3.639 years. The Kaplan-Meier and log rank analysis found out that age at diagnosis (p = 0,041), hypertension (p < 0,0001), and dyslipidemia (p < 0,0001) variables significantly differs the time of complication onset among T2DM patients. Based on the Cox Proportional Hazard analysis, factors that significantly accelerated complication onset were having hypertension (aHR=1,370; 95% CI: 1,231–1,525) and having dyslipidemia (aHR=1,522; 95% CI: 1,363–1,647), while being ≥65 of age when first diagnosed is a protective factor (aHR=0,868; CI: 0,771 – 0,978). Conclusion: Survival from complications among JKN-enrolled T2DM patients in Indonesia remains relatively low, with hypertension and dyslipidemia as the main determinants. Comprehensive efforts to prevent and manage these comorbidities are therefore needed, including through optimized risk stratification within the Prolanis program. Penderita DM tipe 2 menghadapi risiko tiga kali lebih tinggi mengalami penyakit kardiovaskular dan sepuluh kali lebih tinggi mengalami penyakit ginjal, dengan beban biaya komplikasi yang mencakup sekitar 74% dari total biaya medis langsung yang ditanggung BPJS Kesehatan. Efektivitas program pencegahan seperti Prolanis sangat bergantung pada pemahaman mengenai faktor-faktor yang mempercepat terjadinya komplikasi. Oleh karena itu, penelitian ini dilakukan untuk mengetahui kesintasan komplikasi pasien DM tipe 2 peserta Jaminan Kesehatan Nasional (JKN) beserta faktor-faktor yang memengaruhinya. Latar belakang: Diabetes melitus tipe 2 masih menjadi masalah kesehatan global yang serius. Prevalensi DMT2 selalu meningkat dari tahun 2000-2021 yakni dari 4,55% menjadi 10,6%. Angka kematian DMT2 juga mengalami peningkatan sebanyak 57,94% dari tahun 2011 ke tahun 2021. Walaupun pemerintah sudah berupaya dalam mengendalikan DMT2, beban penyakit ini diproyeksikan meningkat hingga tahun 2045. DMT2 merupakan penyakit dengan angka mortalitas dan morbiditas yang tinggi. Beban penyakit DMT2 ini dapat tercermin dalam ukuran DALYs. Penelitian ini bertujuan untuk melihat tren DALYs penyakit DMT2 dan faktor risikonya di Indonesia tahun 1990-2021. Metode: Penelitian ini menggunakan metode systematic review dengan data sekunder Global Burden of Disease 2021. Populasi dari penelitian ini adalan penduduk usia ≥ 35 tahun di 34 provinsi di Indonesia dengan unit analisis provinsi. Variabel dalam penelitian ini adalah DALYs DMT2, DALYs DMT2 akibat IMT tinggi, dan DALYs DMT2 akibat perilaku merokok. Hasil: Penelitian menjukkan DALYs DMT2 mengalami kenaikan sebesar 59,6% dari tahun 1990-2021. Faktor yang paling berkontribusi dalam DALYs DMT2pada tahun 2021 yaitu IMT tinggi (41,7%) dan perilaku merokok (15,8%). DALYs DMT2 yang disebabkan oleh faktor risiko tersebut juga mengalami kenaikan, dengan rincian 133,21% untuk IMT tinggi dan 69,2% untuk perilaku merokok. Kesimpulan: DALYs DMT2, DALYs DMT2 akibat IMT tinggi, dan DALYs DMT2 akibat perilaku merokok selalu meningkat dari tahun 1990-2021. Hal ini menunjukkan kewaspadaan masyarakat terhadap dampak DMT2 yang masih rendah.
Background: Type 2 diabetes mellitus remains a serious global health problem. The prevalence of T2DM has always increased from 2000-2021, from 4.55% to 10.6%. The mortality rate of T2DM also increased by 57.94% from 2011 to 2021. Despite the government's efforts to control T2DM, the disease burden is projected to increase until 2045. T2DM is a disease with high mortality and morbidity. The disease burden of T2DM can be reflected in the DALYs measure. This study aims to examine the trend of T2DM disease DALYs and its risk factors in Indonesia from 1990 to 2021. Methods: This study used systematic review methods with secondary data from the Global Burden of Disease 2021. The population of this study was the population aged ≥ 35 years in 34 provinces in Indonesia. The variables in this study were T2DM DALYs, T2DM DALYs due to high BMI, and T2DM DALYs due to smoking behaviour. Results: The study showed that T2DM DALYs increased by 59.6% from 1990-2021. The most contributing factors to T2DM DALYs in 2021 were high BMI (41.7%) and smoking behaviour (15.8%). T2DM DALYs caused by these risk factors also increased, with 133.21% for high BMI and 69.2% for smoking behaviour. Conclusion: T2DM DALYs, T2DM DALYs due to high BMI, and T2DM DALYs due to smoking behaviour always increased from 1990-2021. This shows that public awareness of the impact of T2DM is still low.

Diabetes melitus tipe 2 telah menjadi masalah kesehatan masyarakat yang serius dan merupakan penyebab penting dari angka kesakitan, kematian, kecacatan dan kerugian ekonomi di seluruh dunia. Penelitian ini bertujuan untuk mengestimasi prevalensi, faktor-faktor risiko dan model prediksi kejadian diabetes melitus tipe 2 di daerah urban Indonesia. Penelitian ini menggunakan data Survei Riset Kesehatan Dasar 2007. Kriteria diagnostik menggunakan metode Tes Toleransi Glukosa Oral (TTGO) menurut World Health Organization (WHO) 1999 dan American Diabetes Association (ADA) 2003. Dari 19.960 responden berusia 15 tahun keatas hanya 18.746 responden yang dianalisis. Analisis data menggunakan regresi logistik dengan desain sampel dua tahap. Dari analisis data didapatkan prevalensi diabetes melitus sebesar 5,98% (95%CI 5,40% - 6,62%), prevalensi diabetes melitus tertinggi pada kelompok umur diatas 45 tahun sebesar 12,41% (95%CI 11,13% - 13,81%). Dengan mengontrol tingkat pendidikan, pekrjaan dan umur didapatkan odds ratio kegemukan sebesar 1,52 (OR = 1,52; 95%CI 1,27 - 1,82), odds ratio obesitas sebesar 2,40 (OR = 2,40; 95%CI 1,80 - 3,19) dan odds ratio obesitas sentral sebesar 1,92 (OR = 1,92; 95%CI 1,62 - 2,26). Dengan menghindari kejadian obesitas sentral dapat mencegah 22,6% (95% CI 18,2% - 26,5%) kejadian diabetes melitus tipe 2 di populasi, atau sekitar 474.922 kasus diabetes melitus dapat dicegah jika obesitas sentral diintervensi.

The result showed that diabetes mellitus prevalence was 5.98% (95%CI 5.40% - 6.62%), and the highest prevalence was 12.41% (95%CI 11.13% - 13.81%) in an above 45 year-old age group. We estimate odds ratio by adjusted education level, occupation and age. The odds ratio of overweight is 1.52 (OR = 1.52; 95%CI 1.27 - 1.82), the odds ratio of general obesity is 2.40 (OR = 2.40; 95%CI 1.80 - 3.19) and the odds ratio of central obesity is 1.92 (OR = 1.92; 95%CI 1.62 - 2.26). By prevent central obesity we could prevent 22.6% (95% CI 18.2% - 26.5%) the expected diabetes mellitus cases in the population, or above 474,922 diabetes mellitus cases could prevent.

Pada tahun 2021, Indonesia menempati peringkat ke-6 di dunia dengan kasus kematian akibat diabetes terbanyak. Tingginya angka kematian diabetes disebabkan oleh banyak hal, salah satunya adalah komorbiditas. Tujuan penelitian ini adalah untuk melihat pengaruh komorbid terhadap tingkat survival penderita diabetes tipe II di Indonesia. Penelitian ini menggunakan Data sampel kontekstual Diabetes Mellitus BPJS tahun 2015-2021 dengan desain kohort retrospektif dan didapatkan jumlah sampel 96.379 terbobot. Dilakukan analisis statistik univariat, bivariat dengan kurva Kaplan Meier dan Chi-square, dan multivariat dengan uji cox proportional hazard. Hasil penelitian menunjukan bahwa penderita diabetes tipe II yang memiliki komorbid mempunyai cumulative survival probability (CSP) yang lebih rendah dan memiliki risiko kematian yang lebih tinggi dibandingkan dengan yang tidak memiliki komorbid, terutama pada penderiya yang memiliki komorbid tidak sesuai (CSP 85,6%; 95% CI 84,4% - 86,7%; aHR 1,64; 95% CI 1,150 – 1,390). Maka dari itu, penting untuk memperhatikan perawatan yang diberikan kepada penderita diabetes tipe II yang memiliki komorbid tidak sesuai agar perawatan yang diberikan tidak memperburuk kondisi salah satu penyakit
Diabetes is a chronic disease that occurs when the pancreas can no longer make insulin, or when the body cannot properly use the insulin it does make. In 2021, Indonesia is the 6th country in the world with the most cases of diabetes deaths. The high mortality rate of diabetes is caused by many things, especially comorbidities. The purpose of this study was to see the effect of comorbidities on the survival rate of type II diabetes patients in Indonesia. This study used BPJS Diabetes Mellitus contextual sample data in 2015-2021 with a retrospective cohort design and obtained a sample size of 96,379 weighted. Conducted univariate, bivariate statistical analysis with Kaplan Meier curve and Chi-square, and multivariate with Cox proportional hazard test. The results showed that patients with type II diabetes who have comorbidities have a lower cumulative survival probability (CSP) and have a higher risk of death compared to those without comorbidities, especially in patients who have discordant comorbidities (CSP 85.6%; 95% CI 84.4% - 86.7%; aHR 1.64; 95% CI 1.150 - 1.390). Therefore, it is important to pay attention to the care given to patients with type II diabetes who have discordant comorbidities so that the care given does not worsen the condition of one of the diseases.

Masa neonatal yakni 28 hari pertama kehidupan merupakan periode paling kritis bagi kelangsungan hidup bayi karena tingginya risiko untuk mengalami kematian pada fase ini. Berdasarkan laporan SKI 2023, Indonesia berada di posisi ketiga untuk AKN tertinggi di Asia Tenggara yakni 9,3 kematian per 1.000 kelahiran hidup. Data terkini mengungkapkan peningkatan yang cukup signifikan, dimana kasus kematian neonatal melonjak dari 20.882 pada tahun 2022 menjadi 29.954 pada tahun 2023. Penelitian ini bertujuan untuk mendapatkan faktor-faktor apa saja yang berhubungan dengan kematian neonatal pada peserta BPJS Kesehatan Tahun 2015-2022. Metode penelitian menggunakan desain cross sectional dengan menganalisis data sampel BPJS Kesehatan tahun 2015-2022, mencakup bayi baru lahir (0-28 hari) yang melakukan kunjungan ke Fasilitas Kesehatan Rujukan Tingkat Lanjut (FKRTL). Hasil penelitian menunjukkan bahwa faktor sosioekonomi (status ekonomi dan tempat tinggal) dan faktor neonatus (jenis kelamin, usia saat kunjungan dan berat badan lahir) memiliki hubungan yang signifikan (p-value: 0,000) terhadap kematian neonatal. Dengan hasil berat badan lahir rendah memiliki risiko 4,1 kali lebih berisiko untuk mengalami kematian neonatal (OR: 4,1 95% CI: 3,74-4,55), kemudian neonatus yang melakukan kunjungan di usia 0-7 hari 3,4 kali berisiko mengalami kematian neonatal (OR: 3,4 95% CI: 2,64-4,43), neonatus perempuan memiliki risiko 0,8 kali lebih rendah untuk mengalami kematian neonatal (OR: 0,8 95% CI: 0,74-0,88), dan untuk neonatus yang berada di luar pulau jawa memiliki risiko 1,3 kali lebih berisiko untuk mengalami kematian neonatal (OR: 1,31, 95% CI: 1,21-1,43) serta neonatus dengan status ekonomi kurang memiliki risiko 1,8 kali lebih untuk mengalami kematian neonatal (OR: 1,89, 95% CI: 1,79-2,06).

According to SKI 2023, Indonesia ranks third for the highest neonatal mortality rate in Southeast Asia at 9.3 deaths per 1,000 live births. Recent data shows a significant increase, with neonatal deaths rising from 20,882 cases in 2022 to 29,954 in 2023. This study aims to identify factors associated with neonatal mortality among BPJS Kesehatan participants from 2015-2022. Using a cross-sectional design, we analyzed BPJS Kesehatan data of newborns (0-28 days) visiting Advanced-Level Health Facilities (FKRTL). Results show that socioeconomic factors (economic status and residence) and neonatal factors (sex, age at visit, and birth weight) significantly correlate with neonatal mortality (p-value: 0,000). With low birth weight having a 4.1 times higher risk of experiencing neonatal death (OR: 4,1 95% CI: 3,74-4,55), then neonates who have visits at 0-7 days old have a 3.4 times higher risk of experiencing neonatal death (OR: 3,4 95% CI: 2,64-4,43), female neonates have a 0.8 times lower risk of experiencing neonatal death (OR: 0,8 95% CI: 0,74-0,88), and neonates who are outside of Java Island have a 1,3 times higher risk of experiencing neonatal death (OR: 1,31, 95% CI: 1,21-1,43) as well as neonates with poor economic status having a 1,8 times higher risk of experiencing neonatal death (OR: 1,89, 95% CI: 1,79-2,06).

Abstrak:
Penyakit kardiovaskuler merupakan penyebab utama kematian yaitu sebesar 30% kematian di dunia. Tahun 2013 prevalensi penyakit jantung koroner di Indonesia berdasarkan wawancara terdiagnosis dokter adalah sebesar 0,5%, dan berdasarkan terdiagnosis dokter atau gejala sebesar 1,5%. Sebanyak 68% orang yang menderita penyakit diabetes melitus meninggal karena komplikasi penyakit jantung koroner. Prevalensi orang dengan DM di Indonesia pada tahun 2013 sebanyak 6,9% dan pada tahun 2015. Penelitian ini bertujuan untuk mengetahui hubungan penyakit diabetes melitus (DM) dengan prevalensi penyakit jantung koroner (PJK) di Indonesia. Penelitian ini merupakan analisis lanjut Riskesdas 2013 dengan desain studi Cross Sectional. Responden dalam penelitian ini adalah seluruh penduduk di Indonesia usia ≥15 tahun yang memiliki data variabel penelitian lengkap. Berdasarkan penelitian ini diperoleh bahwa responden yang menderita diabetes melitus memiliki risiko 3,07 kali lebih besar untuk menderita penyakit jantung koroner dibandingkan dengan responden yang tidak menderita diabetes melitus setelah dikontrol variabel usia, hipertensi, obesitas sentral, obesitas, stress, variabel interaksi diabetes melitus dengan usia, dan variabel interaksi diabetes melitus dengan obesitas sentral. . Kata Kunci : Penyakit Jantung Koroner, Diabetes Melitus, Riskesdas 2013 Cardiovascular disease is the leading cause of death which contributes to about 30% of deaths in the world. In 2013, the prevalence of coronary heart disease in Indonesia, based on medical diagnosis was 0.5% and based on medical diagnosis or symptoms was 1,5%. There were 68% of people who suffered from diabetes mellitus died from complications of coronary heart disease. The prevalence of people with diabetes in Indonesia in 2013 was about 6.9% and in 2015. The aim of this study to determine the relationship between diabetes mellitus (DM) and the prevalence of coronary heart disease (CHD) in Indonesia. This study is a further analysis of Riskesdas (Indonesia Basic Health Research) 2013 designed with a cross-sectional study. The respondents of this research were all residents in Indonesia at age ≥ 15 years, those who had completed research variable data. Based on the survey results revealed that respondents with diabetes mellitus are at 3.07 times higher risk of suffering coronary heart disease compared to respondents without diabetes mellitus after controlled by age, hypertension, central obesity, obesity, stress, interaction variable between diabetes mellitus and age, and interaction variable between diabetes mellitus and central obesity. Latar belakang: Kanker payudara merupakan penyebab utama kematian akibat kanker pada perempuan di Indonesia, dengan data Global Cancer Observatory 2022 mencatatkan 68.271 kasus baru, 209.748 kasus dalam lima tahun terakhir, dan 22.598 kematian. Angka kesintasan 5 tahun pasien kanker payudara di Indonesia relatif rendah dibandingkan dengan negara lain. Berbagai faktor, termasuk faktor individu dan layanan kesehatan, dapat memengaruhi kesintasan pasien. Oleh karena itu, penelitian ini dilakukan untuk mengetahui kesintasan pasien kanker payudara peserta Jaminan Kesehatan Nasional (JKN) beserta faktor-faktor yang memengaruhinya. Metode: Penelitian ini menggunakan Data Sampel BPJS Kesehatan tahun 2018-2023 dengan desain studi kohort retrospektif. Analisis dilakukan dengan metode Kaplan-Meier dan uji Cox Proportional Hazard. Hasil: Hasil penelitian menunjukkan tingkat kesintasan 5 tahun sebesar 52,2% (95% CI: 46,4-58,7%), yang menunjukkan tingkat kesintasan yang masih rendah. Faktor individu yang berpengaruh terhadap kesintasan adalah status kawin/cerai (aHR = 1,632; 95% CI: 1,102 – 2,416), wilayah tinggal di Regional 4 (aHR = 2,230; 95% CI: 1,497 – 3,321), dan adanya penyakit penyerta ≥1 (aHR = 1,498; 95% CI: 1,182 – 1,899). Sementara itu, faktor penyedia layanan kesehatan yang memengaruhi kesintasan adalah tingkat keparahan II (aHR = 5,566; 95% CI: 3,396 – 9,12) dan tingkat keparahan III (aHR = 11,118; 95% CI: 6,706 – 18,432). Kesimpulan: Kesintasan 5 tahun pasien kanker payudara di Indonesia masih tergolong rendah. Oleh karena itu, diperlukan peningkatan upaya penanggulangan kanker payudara secara komprehensif, mulai dari masyarakat, BPJS Kesehatan, hingga pemangku kebijakan.

The analysis was conducted using the Kaplan-Meier method and Cox Proportional Hazard test.
