Ketersedian dan kualitas air bersih memainkan peran penting dalam kehidupan dan kualitas hidup manusia. PT YZ membangun Water treatment Plant (WTP) untuk memenuhi kebutuhan air bersih di kawasan perindustrian di Kabupaten Bintan. Penilaian risiko pada proses penjernihan air di WTP PT YZ masih terintegrasi dengan beberapa plant lain di dalam satu departemen yang sama dengan WTP yaitu departemen estate. Penelitian ini bertujuan untuk mengidentifikasi bahaya dan menilai risiko keselamatan dan kesehatan kerja pada proses penjernihan air di area WTP PT YZ secara komprehensif. Penelitian ini menggunakan desain studi deskriptif observasional dengan metode analisis semi-kuantitatif. Hasil penelitian ditemukan terdapat 34 bahaya dengan 63 risiko pada proses penjernihan air di WTP. Tingkat risiko didapatkan 2 risiko sangat tinggi, 11 risiko tinggi, dan 50 risiko sedang. Pengendalian risiko dilibatkan dalam penilaian risiko sehingga nilai risiko dapat diturunkan berdasarkan pengendalian K3 yang dilakukan oleh perusahaan dan rekomendasi pengendalian K3 tambahan yang diberikan peneliti.
The availability and quality of clean water play a crucial role in human life and quality of life. PT YZ built a Water Treatment Plant (WTP) to meet the clean water needs in the industrial area of Bintan Regency. The risk assessment of the water purification process in PT YZ's WTP is still integrated with several other plants within the same department as the WTP, namely the estate department. This study aims to comprehensively identify hazard and assess occupational health and safety risks in the water purification process at PT YZ's WTP. This study uses a descriptive observational study design with a semi-quantitative analysis method. The research findings identified 34 hazards with 63 risks in the water purification process at the WTP. The risk levels were found to be 2 very high risks, 11 high risks, and 50 medium risks. Risk control is involved in the risk assessment so that risk values can be reduced based on the occupational health and safety controls implemented by the company and additional control recommendations provided by the researchers.

Kegiatan penambangan yang dilakukan oleh perusahaan akan menghasilkan limbah sisa pengolahan batu-batuan yang mengandung kontaminan logam berat yang selanjutnya disebut sebagai tailing. Pemerintah mengeluarkan kebijakan bahwa perusahaan wajib melakukan pemanfaatan tailing guna mengurangi volume tailing di lingkungan. Proses pemanfaatan tailing merupakan bagian yang tidak terpisahkan dari proses bisnis pertambangan dan memiliki sejumlah potensi bahaya dan risiko yang perlu dikelola. Sehingga, penelitian ini dilakukan dengan tujuan untuk mengidentifikasi dan menilai risiko keselamatan dan kesehatan kerja pada proses pemanfaatan tailing di industri pertambangan. Penelitian ini menggunakan desain deskriptif dengan penilaian kualitatif berdasarkan Standar AS/NZS 4360:2004. Penilaian risiko dilakukan melalui analisis tahapan proses kerja, bahaya, unexpected event, dan risiko untuk mengevaluasi probabilitas, konsekuensi, dan tingkat risiko. Hasil penelitian mengidentifikasi sejumlah 86 unexpected events dari total 47 jenis bahaya dalam proses pemanfaatan tailing, yang terbagi menjadi 29 bahaya fisik, 10 bahaya kimia, dan 8 bahaya ergonomi. Penilaian risiko menunjukkan bahwa pada risiko dasar (basic risk) terdapat 8 bahaya rendah, 25 bahaya sedang, 10 bahaya tinggi, dan 4 bahaya ekstrem, sedangkan pada risiko sisa (residual risk) terdapat 32 bahaya rendah, 10 bahaya sedang, 4 bahaya tinggi, dan 1 bahaya ekstrem. Berdasarkan hasil penilaian yang diperoleh, terdapat bahaya yang memerlukan pengendalian tambahan untuk mengurangi risiko, serta prioritas bahaya yang membutuhkan tindakan segera dan perhatian utama dari manajemen.
Mining activities conducted by the company produce waste from the processing of ore containing heavy metal contaminants, referred to as tailings. The government issued a policy that requires companies to utilize tailings to reduce the volume of tailings in the environment. The tailings utilization process is an inseparable part of the mining business process and has a number of potential hazards and risks that need to be managed. Therefore, this research was conducted with the aim of identifying and assessing occupational safety and health risks in the tailings utilization process in the mining industry. This research uses a descriptive design with qualitative assessments based on Standard AS/NZS 4360:2004. Risk assessment was carried out by analyzing the stages of the work process, hazards, unexpected events, and risks to evaluate probability, consequences, and risk levels. The research identified a total of 86 unexpected events from 47 types of hazards in the tailings utilization process, which were divided into 29 physical hazards, 10 chemical hazards and 8 ergonomic hazards. The risk assessment shows that in the basic risk (basic risk) there are 8 low hazards, 25 moderate hazards, 10 high hazards and 4 extreme hazards, while in the residual risk (residual risks) there are 32 low hazards, 10 moderate hazards, 4 high hazards, and 1 extreme danger. Based on the assessment results, certain hazards require additional controls to reduce risks, as well as prioritized hazards that need immediate action and primary attention from management.

Kompleksitas industri manufaktur mesin dan perlengkapan melibatkan beragam jenis proses produksi, salah satunya adalah pekerjaan pengelasan. Proses pekerjaan pengelasan merupakan salah satu proses industri terpenting dalam menggabungkan komponen logam atau baja. Beragam bahaya ditemukan pada proses pengelasan seperti percikan logam las, percikan api las, emisi asap las, dan manual handling selama pekerjaan berlangsung menimbulkan risiko keselamatan dan kesehatan kerja bagi pekerja. Penelitian ini bertujuan untuk mengidentifikasi dan menilai risiko keselamatan dan kesehatan kerja pada proses pekerjaan pengelasan di area fabrikasi PT XYZ. Tahap identifikasi bahaya dan risiko dilakukan secara kualitatif melalui observasi, kegiatan brainstorming, wawancara, dan telaah dokumen perusahaan menggunakan metode Hazard and Operability Study (Studi HAZOP). Setelah itu, dilakukan analisis penilaian risiko semi kuantitatif dengan menilai perkalian dari kriteria probability dan severity berdasarkan matriks penilaian risiko PT XYZ. Hasil penelitian menunjukkan bahwa terdapat 12 tahapan proses pekerjaan pengelasan. Teridentifikasi 52 bahaya dengan dengan 49 risiko dari proses pekerjaan pengelasan. Tingkat risiko yang didapatkan meliputi 23 risiko dengan kategori sangat tinggi, 9 risiko dengan kategori tinggi, dan 17 risiko dengan kategori sedang. Penilaian risiko melibatkan pengendalian risiko sehingga nilai tingkat risiko dapat diturunkan melalui pengendalian yang telah ada dan rekomendasi pengendalian yang diberikan peneliti.

Welding is one of the most important industrial processes in joining metal or steel components. Various hazards found in the welding process such as welding metal sparks, welding sparks, welding fumes emissions, and manual handling during work pose risks to occupational safety and health for workers. This study aims to identify and assess occupational safety and health risks in the welding work process in the PT XYZ fabrication area. The hazard and risk identification stage is carried out qualitatively through observation, brainstorming activities, interviews, and reviewing company documents using the Hazard and Operability Study (HAZOP Study). After that, a semi-quantitative risk assessment analysis was carried out by assessing the multiplication of probability and severity criteria based on PT XYZ's risk assessment matrix. Skripsi ini berisi tentang penilaian risiko keselamatan dan kesehatan kerja pada proses produksi textile chemical di PT. Huntsman Indonesia Divisi Textile Effect Tahun 2019. Pada penelitian ini, identifikasi bahaya dilakukan menggunakan Job Safety Analysis (JSA) dan analisis risiko menggunakan ukuran standar yang telah dimodifikasi dari AS/NZS 4360:2004 dan telah disesuaikan dengan kondisi di lapangan. Desain studi yang digunakan bersifat deskriptif dan eksploratif dengan pendekatan analisis kualitiatif. Penilaian risiko didapatkan dari perkalian dari probabilitas dan konsekuensi per aktivitas dengan mempertimbangkan pengendalian yang sudah dilakukan oleh perusahaan. Hasil penelitian ini mendapatkan 3 tahapan kerja yang terdiri dari 17 aktivitas kerja memiliki 30 potensi bahaya yang dapat menimbulkan 56 risiko yang terdiri dari 16 risiko rendah, 21 risiko sedang, 16 risiko tinggi, dan 3 risiko sangat tinggi.

Penelitian ini menggunakan desain studi cross sectionaldengan pendekatan observasi dan wawancara. Proses peleburan merupakan proses pencairan dan pematangan logam di dalam tungku dengan suhu mencapai lebih dari 1600oC untuk menghasilkan logam cair yang akan dicetak menjadi sebuah komponen penyusun alat berat. Skripsi ini bertujuan untuk menilai risiko kesehatan kerja pada proses peleburan di pabrik pengecoran perusahaan alat berat. Penelitian menggunakan desain sequential explanatory dengan menggabungkan metode kuantittaif untuk melakukan penilaian risiko berdasarkan metode penilaian semi-kuantitatif W.T. Fine dan metode kualitatif untuk menginterpretasikan penilaian risiko. Pengumpulan data dilakukan dengan observasi, wawancara informan, dan telaah data perusahaan. Hasil penelitian menunjukkan adanya 87 risiko dari 7 tahapan proses kerja. Risiko prediktif pada kategori substantial, priority II, dan acceptable masing-masing sebanyak 45, 24, dan 18 risiko. Bahaya dengan tingkat risiko prediktif kategori substantial adalah bising, asap logam, dan debu logam, sehingga masih dibutuhkan perhatian lebih. Penyebab tingginya tingkat risiko pada bahaya kebisingan, asap, dan debu dianalisis pada penelitian ini.
Melting process of metals in the furnace is carried out at temperature exceeding 1600oC to produce molten metal that will be molded into components for heavy equipment. This thesis aims to assess occupational health risks in the melting process at heavy equipment manufacturer’s foundry plant. The research uses a sequential explanatory design, combining quantitative methods to conduct risk assessments based on the W.T. Fine semi-quantitative assessment method, and qualitative methods to interpret the risk assessments. Data collection was conducted through observation, informant interviews, and company data review. The research findings indicate the presence of 87 risks from 7 stages of the work process. The predictive risks fall into the substantial, priority II, and acceptable categories, with 45, 24, and 18 risks respectively. Hazards with substantial predictive risk levels include noise, metal fumes, and metal dust, requiring further attention. The causes of the high level of risk in noise, metal fume, and metal dust hazards were analyzed in this study.
