Program Jaminan Persalinan merupakan suatu terobosan untuk menurunkan Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) sehingga dapat mempercepat capaian target Millenium Development Goals (MDGs). Penelitian ini bertujuan untuk menganalisa implementasi kebijakan program Jampersal di Kota Bekasi. Metode penelitian yang digunakan adalah pendekatan kualitatif dan pengumpulan data menggunakan wawancara mendalam dan telaah dokumen. Hasil penelitian menunjukan bahwa pelaksanan program belum berjalan optimal sebagaimana yang diharapkan, sehingga menyebabkan cakupan masih rendah. Agar implementasi program Jampersal dapat berjalan dengan optimal diperlukan koordinasi Dinas Kesehatan Kota Bekasi dengan Pemerintah Daerah dan lintas sektor terkait seperti Ikatan Bidan Indonesia (IBI). Selain itu perlu melakukan monitoring dan evaluasi rutin ke RSUD Kota Bekasi, Puskesmas dan Bidan Praktik Mandiri.
Delivery Security Program is a breakthrough to reduce Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in order to accelerate the achievement of the Millennium Development Goals (MDGs). This study aims to analyze the implementation of program policies Delivery Security in Bekasi. The research method used was a qualitative approach and data collection using in-depth interviews and document review. The results showed that the optimal conduct of the program has not run as expected, resulting in coverage is still low. Delivery Security program implementation in order to run optimally required coordination with the City Health Office Bekasi local government and across relevant sectors such as the Indonesian Midwives Association (IBI). In addition to the need to perform regular monitoring and evaluation to Bekasi City Hospital, health centers and midwives Independent Practice.
Prelacteal is anything other breastmilk that given to infants before breastfeeding in the first 3 days of life which can cause failure of exclusive breastfeeding and may increased risk of infection and malnutrition which then will impact on stunting. One in two babies who have been breastfed in Indonesia have been given prelacteal. This study aims to determine the determinants of prelacteal feeding behavior in infants on the island of Sumatra. This study uses data from the 2017 IDHS with a cross sectional study design. The sample of this study was mothers who had babies aged 0-23 months with the inclusion criteria with a sum of 1,224 respondents. The results showed that there were 54.4% of mothers who gave prelacteal. From correlations analysis it was found that maternal age, parity, IMD, and type of delivery were associated with prelacteal feeding to infants (p-value < 0.05). The results of the logistic regression analysis showed that IMD as the dominant factor that causes prelacteal feeding (OR: 6.06) where mothers who are late in giving IMD are 6 times more likely to giving prelacteal to infants after being controlled by weight. birth, ANC, type of delivery, maternal age, parity, and birth attendants.
ABSTRAK Nama : Ernawati Roeslie Program Studi : Ilmu Kesehatan Masyarakat Judul : Analisis Kesiapan Implementasi Program Indonesia Sehat dengan Pendekatan Keluarga (Indikator 8: Kesehatan Jiwa) di Kota Depok tahun 2018 Pembimbing : dr. Adang Bachtiar, MPH., DSc. Program Indonesia Sehat dengan Pendekatan Keluarga (PIS-PK) adalah program prioritas Kementerian Kesehatan yang dilaksanakan oleh Puskesmas. Indikator 8: Kesehatan Jiwa belum mendapat perhatian khusus di Kota Depok, kasus Orang dengan Gangguan Jiwa (ODGJ) berat mengalami peningkatan dari 3986 kasus pada tahun 2016 menjadi 5768 kasus pada tahun 2017, dimana kasus skizofrenia dan gangguan psikotik kronik lainnya mengalami kenaikan dari 1687 kasus pada 2016 menjadi 2342 kasus pada 2017. Analisis kesiapan implementasi PIS-PK (Indikator 8: Kesehatan Jiwa) di Kota Depok tahun 2018 merupakan tahapan penting sebagai penentu keberhasilan kinerja Pemerintah Daerah dalam bidang kesehatan. Penelitian ini adalah penelitian kualitatif dengan desain deskriptif. Tujuan penelitian ini adalah untuk mengetahui kesiapan implementasi program PIS-PK (Indikator 8: Kesehatan Jiwa) di Kota Depok Tahun 2018 dilihat dari variabel komunikasi, disposisi, sumber daya dan struktur birokrasi menggunakan Teori Edward III. Metode pengumpulan data dilakukan dengan wawancara mendalam, Focus Grup Discussion (FGD) dan telaah dokumen. Hasil penelitian didapatkan kesiapan implementasi PIS-PK (Indikator 8: Kesehatan Jiwa) di Kota Depok berdasarkan 4 (empat) variabel implementasi menurut teori Edward III, yaitu komunikasi, sumber daya, disposisi dan struktur birokrasi dinilai belum siap untuk dilaksanakan. Rekomendasi pada penelitian ini yaitu keberhasilan implementasi akan dicapai bila dilakukan perbaikan dari kekurangan, baik dari sisi komunikasi, sumber daya, disposisi dan struktur birokrasi. Di samping itu hambatan program yang ada bisa diatasi dengan tersedianya pendanaan yang cukup. Saran dari penelitian ini adalah agar meningkatkan pemberdayaan peran keluarga dan potensi masyarakat dengan metode pelatihan untuk peningkatan kesehatan jiwa dan mengurangi stigma di masyarakat. This research is a qualitative research with descriptive design. The purpose of this research is to determine the implementation readiness of PIS-PK (Indicator 8: Mental Health) in Depok 2018 reviewed from communication, disposition, resources and bureaucratic structure using Edward III theory. Data collection method was performed using in-depth interviews, Focus Group Discussion (FGD) and document review. The research result indicates that PIS-PK implementation (Indicator 8: Mental Health) in Depok were not ready based on 4 (four) implementation variables according to Edward III theory, ie communication, resources, disposition and bureaucracy structure. The research recommends to improve the all aspect of communication, resources, disposition and bureaucratic structure in order to achieve the successful implementation. In addition, the program contraints can be reduced by sufficient funding availability. The research suggests to increase the empowerment of family role and community contribution using training method in order to improve the mental health and reduce the stigma in society. The local government of Kudus Regency has committed to accelerating the handling of stunting cases and is trying to realize Kudus zero stunting by issuing Regional Regulation Number 53/2020 concerning the Acceleration of Stunting Prevention in Kudus Regency. However, the implementation of convergence in Kudus has not run optimally. The results of the work meeting on the assessment of the integrated stunting prevention convergence action in Central Java in 2020, the capacity for implementing situation analysis and inputting as well as the quality of situation analysis with cross-sectoral involvement in Kudus Regency has not been optimal, the stunting discussion has not been carried out intensively, and there are no regulations to strengthen publications of stunting prevention. In addition, the COVID-19 pandemic has led to a refocusing of the budget from other services and programs, including stunting prevention. The purpose of this study is to analyze the implementation of policies to accelerate stunting prevention during the COVID-19 pandemic in Kudus Regency. This research is qualitative research. Data were collected through in-depth interviews with informants and document review with the Edward III framework covering the variables of communication, resources, disposition, and bureaucratic structure and Van Meter and Van Horn covering variables of the economic, social and political environment. Research informants are representatives from regional apparatus who are members of the stunting prevention acceleration team at the district, sub-district, and village/kelurahan levels as well as the Human Development Cadre (KPM). The study was conducted from April to May 2022. The results show that policy implementation has been running but sensitive nutrition interventions have not been maximized, communication is limited due to the COVID-19 pandemic, the available resources are adequate, and there are policy guidelines that are known and understood by policy implementers, coordination is not optimal. , the economic and social environment is not yet sufficiently supportive and the political environment is sufficiently supportive of policies. In conclusion, the implementation of the policy to accelerate stunting prevention during the COVID-19 pandemic has not run optimally, with constraints on human resource variables, coordination, as well as the economic and social environment. Thus, the implementation of convergence still needs to be improved by carrying out activities in a coordinated manner between institutions. Socialization to the community continues to be intensified so that policy objectives can be achieved. This policy need to be adjusted with the latest Presidential Regulation on accelerating stunting reduction.
Bekasi Regency is ranked first in the city/regency with the highest total daily waste volume and the highest total annual waste volume in West Java Province, with a total daily waste volume of 1,900 tons/day and total waste generation in 2020 of 693,586 tons/year. The Bekasi district government stipulates a derivative regulation from Presidential Regulation Number 97 of 2017 and West Java Governor Regulation Number 91 of 2018 in the form of Bekasi Regent Regulation Number 33 of 2019 concerning Regional Policies and Strategies (JAKSTRADA) Bekasi Regency in the Management of Household Waste and Waste Similar to Household Waste. This study aims to analyze the implementation of household waste management policies in Bekasi Regency. The results of the study show that the legal framework contained in household waste management policies is available from the highest regulatory hierarchy at the national, provincial, to district level regulations. The technical problems faced are quite diverse, namely the absence of technology used in the final waste management process at the TPA, the TPA Land that has been overloaded and the lack of management infrastructure. The characteristics of the policy indicate that the policy objectives are clear and detailed discussing the technical main tasks and functions of each policy implementing agency, as well as policy targets and achievements. The large budget allocation seems disproportionate to the implementation of waste management in Bekasi Regency which is still faced with problems in technology and facilities and infrastructure. There are still sectoral egos or actions that prioritize their respective agencies in coordination between the hierarchies of policy implementing agencies. One important aspect of the policy environment that still needs to be improved is the availability of technology in the final management of household waste. At this stage in the policy implementation process, the output issued as a derivative form of household waste management policy in Bekasi Regency is in the form of programs and activities of each implementing agency. Discipline and Public Awareness related to Household Waste Management Policy is still low. The target group's violations are in the form of throwing garbage into rivers and illegal dumping sites.
