Background: Efforts made by the reduction of MMR and IMR with equalization ofmidwife health worker and continuing education. Continuing education will improvethe competence and quality of midwife health personnel. In ensuring the quality ofhealth personnel, the government requires midwife health personnel to followcompetency test. Percentage of graduation Competency Test Diploma of Midwifery72% by 2015. The Association of Indonesian Midwifery Education Institutions(AIPKIND) as the stakeholder of midwifery education has a role in developing andfostering the Midwifery Education Institution in order to meet the standards. In supportof AIPKIND's role, it is necessary to have accurate data and information from itsmembers, so that it is necessary to build the design of Database Information SystemMidwifery Institution in AIPKIND. Method: Development of database system in thisresearch using System Development Life Cycle (SDLC) approach by using Prototypingmethod. Results: Design prototype database system based on client-server and web-based so that clients can access using a web browser on all communication devicesconnected to the internet. Presentation of data and information in the form of graphs andtables. Conclusions: Prototype of Database Information System of Online MidwiferyInstitution provides accurate and timely data and information of AIPKIND members.AIPKIND may monitor and evaluate profiles, educators and learners present inmidwifery education institutions.Key words: information system; prototype;Database; AIPKIND.
Asuhan Keperawatan (Aske merupakan suatu pendekatan yang dilakukan seorang perawat terhadap kliennya. Motu Asuhan keperawatan di RSUD Dr. H. Abdul Moeloek Lampung masih diternokan pendokumentasian asuhan keperawatan pada rekam medik pasien yang asih belum terisi; masih adanya masyarakat yang merasa belum puas terhadap pelayanan yang dibe ·kan oleh perugas diruangan (20,3%); kelohan tentang pelayanan sebanyak 21,9%; 70% dari 20 pasien mengatakan perawat yang kurang ramah dalam menangani pasien, 75% dari 20 pasien merasa tidak diperhatikan oleh perawat. Hal ini perlu sekali mendapat perhatian dari semua pihak terkait:; dan menempatkan Pemantauan Motu Askep (PMA) sebagai kegiatan yang penting dilaksanakan secara berkesinambungan. Dalam pelaksanaannya, PMA belum didukung dengan sistem informasi pengolahan data yang masih dilakukan secara manual, belum ada analisis tiap penelitian adalah Ruang Rawat Inap yang ada di Rumah Sakit Umum Daerah Dr. H. Abdul Moeloek Propinsi Lampung. Sebagai hasil dari penelitian ini adalah diperolehnya prototipe Sistem lnformasi Pemantauan Mutu Asuhan Keperawatan yang diharapkan dapat effektif dan effisien dalam penyediaan informasi yang dibutuhkan. lnformasi yang diperoleh akan dirancang agar benar-benar relevan. cepat, tepat dan akurat, serta dapat lebih bennanfaat. terutama untuk kepentingan perencanaan peningkatan mutu pada umumnya, dan pemantauan Askep pada khususnya, sehingga dapat tercapai optimalisasi pemantauan mutu Askep di rumah sakit tersebut Agar pelaksanaan sistem infonnasi ini dapat berjalan dengan baik dan berkelanjutan, dibutuhkan pengembangan prototipe yang berbasis jaringan.

For that require to develop of monitoring. This research was result a prototype of Information System Monitoring Quality of Askep. This system is expected to supply any needed information effectively earn and effektif of efficient in is ready of required information. obtained infonnation will be designed to really relevant, quickly, precisely and is accurate, and also earn more useful, especially for planning of make-up of quality in general, and monitoring of Askep especially, so that can reach by monitoring quality of Askep. For implementation of this information system can walk better and have continuation, required by development of prototype being based on network.
Tujuan : Tujuan dari penelitian ini adalah mengembangkan basis data di puskesmas dan Dinas Kesehatan yang bermanfaat sebagai alat bantu dalam menghasilkan informasi yang dapat dijadikan acuan dalam menentukan kebijakan, perencanaan, monitoring dan evaluasi program dalam rangka menurunkan prevalensi karies dan meningkatkan cakupan UKGS.
Metoda : Metoda yang digunakan adalah metoda pendekatan System Development Life Cicle (SDLC) dengan cara observasi dan wawancara mendalam dengan informan.
Hasil : Pengembangan sistem dimulai dari penetapan kebutuhan sistem, pemodelan sistem serta penetapan perangkat lunak dan keras yang digunakan dengan harapan dapat menghasilkan informasi dalam menyusun perencanaan, monitoring dan evaluasi program UKGS.Sistem informasi UKGS dikembangkan dengan rancangan formulir input dan tampilan output yang berisi cakupan UKGS, Indikator, grafik dan Peta sebaran tenaga Kesehatan gigi di Kabupaten Sumedang.
Kesimpulan : Pelaksanaan sistem informasi ini agar berjalan dengan baik dan berkelanjutan membutuhkan komitmen dan kebijakan yang kuat dari penentu kebijakan, aturan yang jelas tentang organisasi pelaksana, motivasi yang kuat dari pelaksana dan dukungan dana yang berkesinambungan.

Background: The result from a quick survey on dental health at Sumedang district in 2004 shows that 64 % prevalency caries affected children from the 6th grade students, DMF-T average 1.78 and PTI value at 5,2 %. However, In 2010 the Departement of Health targetting prevalency caries < 50 %. Indeks DMF-T average < 1.00 and PTI > 50%. On the evaluation in report of UKGS activities in 2003, it is said that 50% of Puskesmas did not report their UKGS activities. The reason for this was because the expected UKGS activities was not running well.
Objective : The purpose of this research is to develop information data base in Puskesmas and Dinas Kesehatan, as a result will be useful as a guide line in making policy, planning, monitoring and evaluating program in effort to reduce the presentage of prevalency of caries. In addition increase UKGS objectivities.
Methodes: The method used in this research is called development life cycle system (SDLC) by observing and extensivelly interviewing the informant.
Results: In developing a system it should begin with an implication of required model and hardware and software. This system will be used to obtain information of arranging a plan, monitoring and evaluation of UKGS program. UKGS information system is developed with the input design form and output sheet that contains UKGS covarage, indicator, graphic and distribution map of dental health personnel in Sumedang District.
Conclution: Good and continuity of system implementation need the policy and commitment of decision maker, clear regulation of system organization, good motivation of the system operator and continuity of financial support.
