Analisis budaya keselamatan pasien menghasilkan 4 dimensi kategori budaya sedang yang memerlukan perbaikan keselamatan pasien serta 7 dimensi kategori budaya baik yang menjadi kekuatan dalam keselamatan pasien. Pengorganisasian ketenagaan, beban kerja dan pola kerja; konseling pasien; keterbukaan komunikasi; dan respons terhadap kesalahan menjadi kelemahan budaya keselamatan pasien yang menjadi prioritas perbaikan. Tingkat pelaporan kejadian masih rendah dan harus mendapat perbaikan.

This research uses sequential explanatory design started from quantitative analysis using questionnaire The Pharmacy Survey on Patient Safety Culture (PSOPSC) from AHRQ followed by focus group discussion to formulate strategy to build patient safety culture.
Analysis of patient safety culture resulted in 4 dimensions of moderate cultural categories that require improvement and 7 dimensions of good cultural categories that be strength of the patient safety culture. Staffing, Work Pressure and Pace; Patient counseling; Communication openness; and Response to Mistakes is weakness of the patient safety culture that become priority improvement. Level of incident reporting is still low and need improvement.

it is necessary to measure patient safety culture especially among nurses.This study aims to describe the nurses perception about patient safety culture andhow the implementation of patient safety goals at Setia Mitra Hospital. Thisdescriptive research conducted with quantitative and qualitative approaches.Population in this study is all nurses in Setia Mitra hospital that consist of 77 nurseswith sample 68 nurses.The result of study shows the nurse's perception about patient safety in SetiaMitra hospitals is 72.1% good. Dimension with the most good nurse perception ishospital handoffs and transitions dimension by 75% and at least perceived good ismanagement support dimension of 1.5%. Statistically, age (p value = 0.048), workperiod (p value = 0.016) and the level of nurses (p value = 0.049) have arelationship with the nurses perception on patient safety culture (p <0.05). In theimplementation of patient safety goals in Setia Mitra Hospital, namely 55.15% with agood understanding. It is recommended that patient safety team in the Setia MitraHospital further optimized its duties and functions so that the patient safety culture inhospitals can be improved.Keywords : Patient Safety Culture, Nurse Perception, Patient Safety Goal.
Rumah sakit Islam adalah sebuah media pelayanan kesehatan untuk mengartikulasikan Islam dalam pelayanan sehari-hari. Dalam falsafahnya Rumah sakit Islam Jakarta adalah perwujudan dari iman dan amal sholeh kepada Allah dan menjadikannya sebagai sarana ibadah. Di setiap sudut ruang di RS1 Jakarta terparnpang hadis senyum adalah sedekah, tetapi setiap penulis berkunjung kesana jarang mendapat senyarnan. Di misi RSI Jakarta ingin mewujudkan Pelayanan kesehatan yang Islami, profesional dan bermutu dengan tetap peduli pada kaum dhu'afa. Di sisi lain obat-obatan yang dipakai masih mengandung alkohol, pelayanan pasien kandungan masih ditangani dokter spesialis pria. Di jam kerja penulis sering menemui karyawan RSI Jakarta di tempat parkir. Tujuan penelitian ini adalah didapatkannya gambaran Kepemimpinan, Budaya Organisasi Islami dan Produktivitas Karyawan di Rumah Sakit Islam Jakarta. Penelitian ini menggunakart metode cross sectional dengan jumlah responden sebanyak 310 responden dan satu orang inforrnan yaitu direktur SDM dan Pembinaan Rohani RSI Jakarta. Pada penelitian ini didapatkan basil bahwa, nilai-nilai Islam barn diterapkan dalam bentuk simbol seperti pakaian muslimah tetapi di sisi lain RSI Jakarta masih menggunakan simbol Aesculap yang tidak Islami. Untuk Pelayanan belum dilakukan pernisahan antara petugas pria untuk pasien pria demikian pula sebaliknya. Keteladanan juga didapati bet= kuat di RS1 Jakarta. Sedangkan untuk produktivitas karyawan sudah baik. Pada saat ini Kepemimpinan di RSI Jakarta belum kuat, demikian pula dengan Budaya Organisasi Islaminya, sedangkan untuk Produktivitas Karyawan sudah balk. Untuk itu sebaiknya RS1 Jakarta para pimpinannya lebih berkornitinen untuk menjadi teladan bagi para stafnya. Mulai dirintis pelayanan pasien dengan menggunakan sistem gender, bila ha] itu tidak bisa dilakukan lebih balk rnemperbanyak petugas wanita mengingat batasan aurat wanita yang bokh dilihat yaitu wajah dan telapak tangan. Memperbanyak dialog antara pimpinan dengan staf, staf dengan staf baik di dalam unit kerja yang sama maupun dengan unit kerja lainnya.
Islam Hospital is a health service media for articulating Islam in daily services. In philosophy of Islam Hospital Jakarta is a form of faith and good deed to Allah and make it as religious medium. In every corner of RS1 Jakarta, shown hadis of smile is alms, but every the writer come he rarely got smile. RS1 Jakarta mission is realizing Islamic health service, professional and certifiable by still caring dhu'afa people. In the other side, used medicine was still containing alcohol, obstetrical patient service still performed by man specialty doctor. In work hour, writer often meet RS1 Jakarta staffs in parking area. This research purpose is obtaining description of leadership, Islamic organizational culture and staffs productivity at RS1 Jakarta. This research is using cross sectional method with total respondents of 310 people and I informant whose director of SDM and spiritual construction of RSI Jakarta. From this research obtained that Islamic values implemented in the form of symbolic such as rnuslimah cloths but in the other side RS1 Jakarta still using non Islamic Aesculap symbol. For services not yet performed separation between man staffs for man patient and in the contrary. Compliance obtained not yet strong in RSI Jakarta. While, staffs productivity has already well. Recently leadership in RSI Jakarta not yet strong, thus with Islamic organizational culture, while for staffs productivity is well. Therefore, it is better for leader of RSI Jakarta more committed to become model for their staffs. Patient services start be pioneered by using gender system, if it could not performed, it is better to increase woman staffs considering woman aurat are face and hand palm. Increasing dialogues between chief-staffs and staffs-staffs whether in the same working unit and other working unit.

This study aims to analys the hospital of patient safety culture of Awal Bros Hospital Batam in 2016. The concept used was the concept of patient safety culture from AHRQ (2004) which is adopted from Puspitasari M. research (2009), then for improvement used the concept of system reliability form Marx D. (2010). The research design was sequential explanatory, used questionnaire from AHRQ which has been translated to Indonesia language, followed by FGD to formulate the weak dimension improvement effort. Patient safety culture status categorized into medium culture, average of positive perception 70,82%. The greatest strengths are in organizational learning and continuous improvement, feedback and communication about patient safety, communication openness. Weaknesses are primarily in staffing, non-punitive responses to errors, handover and transitions must be fixed immediately. Improvement suggestions by reducing non core job assignments, employee retention programs, hotline service internal, leader lead tracer, investigator training.

Keselamatan pasien (patient safety) rumah sakit adalah suatu sistem dimana rumah sakit membuat asuhan pasien lebih aman. Sistem tersebut meliputi: assesmen resiko, identifikasi dan pengelolaan hal yang berhubungan dengan risiko pasien, dan seterusnya. Sejak dideklarasikannya pelaksanaan Patient Safety di Rumah Sakit X pada tahun 2009 hingga tahun 2011, tercatat Insiden Keselamatan Pasien (IKP) sebanyak 171 kasus, dimana IKP paling banyak yaitu sekitar 60% terjadi di pelayanan rawat inap. Melalui penelitian ini, dianalisis penyebab terjadinya IKP di ruang perawatan Rumah Sakit X. Studi dilakukan terhadap 100 perawat pelaksana dengan menggunakan desain cross sectional untuk melihat bentuk hubungan antara variabel individu, kompleksitas pengobatan, kerjama, gangguan/ interupsi, komunikasi, Standar Prosedur Operasional, dan kenyamanan tempat kerja terhadap kejadian IKP.
Hasil penelitian menunjukkan variabel karakteristik individu, yang terdiri dari usia, masa kerja, dan kompetensi; dan variabel kerja sama yang memiliki hubungan yang signifikan terhadap kejadian IKP dengan nilai P value masing-masing sebesar 0.028, 0.010, 0.028, dan 0.012. Dengan kata lain variabel yang paling berpengaruh terhadap kejadian IKP adalah variabel karakteristik individu sehingga hasil studi ini bisa menjadi pertimbangan bagi Bagian SDM, Komite Keperawatan dan Bagian Keperawatan Rumah Sakit X dalam melakukan seleksi dan pengembangan SDM Keperawatan dalam upaya meningkatkan keselamatan pasien.
Patient safety is a system to make patient care become safer. The systems include risk assessment, identifying and managing the risks associated with patient, and so on. Since the patient safety program has been declared in "X" Hospital in 2009 until 2011, there are 171 cases recorded as a number of the patient safety incident (PSI), most cases about 60% occur in inpatient unit. Through this study, determinants of PSI in inpatient unit X Hospital are analyzed. Study is applied to 100 nursing staffs by cross sectional study design in order to observe the correlation between variable of individual characteristic, medication complexity, teamwork, interruption, communication, standard of procedure operational, and work place comfortable to PSI.
Result shows that there is a significant correlation between variable of individual characteristic (include age, working time, and levels of competence) and teamwork to PSI, with the P value: 0.028, 0.010, 0.028, and 0.012. In other word, the most significant variable to PSI is individual characteristic variable so it could be a consideration to recruit and do improvement based on patient safety by Human Resources, Nursing Committee and Nursing Unit of X Hospital.
