Stroke adalah penyakit penyebab kematian tertinggi setelah penyakit jantung di dunia. Analisis faktor yang mempengaruhi lama perawatan pada rawat inap dengan populasi Indonesia masih jarang dilakukan. Penelitian ini menggunakan desain cross-sectional untuk mengevaluasi hubungan antara hiperglikemia dan hiperkloremia dengan lama perawatan pada pasien stroke iskemik akut di unit rawat inap RSUD Pasar Minggu, Jakarta Selatan, Indonesia. Data sekunder rekam medis dari pasien stroke iskemik akut yang dirawat antara 1 Januari 2022 hingga 31 Desember 2022 digunakan untuk analisis. Metode non-probability sampling tipe purposive digunakan untuk merekrut sampel. Analisis statistik yang dilakukan termasuk analisis deskriptif, bivariat, stratifikasi, dan multivariat. Hasil penelitian menunjukkan bahwa proporsi pasien dengan stroke iskemik akut yang masuk ke unit rawat inap RSUD Pasar Minggu adalah 84.5%. Proporsi hiperglikemia pada pasien tersebut adalah 27%, sedangkan proporsi hiperkloremia adalah 10.2%. Median lama perawatan pasien adalah 5 hari. Terdapat hubungan yang signifikan secara statistik antara durasi perawatan ≥ 5 hari dengan lama perawatan setelah dikontrol dengan variabel kovariat, (adjusted PR 2.21, 95% CI 1.70-2.88, p-value 0.00). Sementara hubungan antara hiperkloremia dan lama perawatan tidak signifikan secara statistik (adjusted PR 1.02; 95% CI 0.66-1.45, p-value 0.56) setelah dikontrol dengan variabel kovariat. Diperlukannya kampanye kesehatan publik, program pemeriksaan berkala, serta penyediaan sistem monitoring pengobatan bagi pasien dengan gangguan metabolik seperti diabetes. Penting untuk memberikan edukasi, mengembangkan protokol penanganan yang jelas, dan melakukan pemantauan glukosa darah serta elektrolit secara teratur pada pasien stroke iskemik akut.
Stroke is the leading cause of death after heart disease in the world. Analysis of factors affecting the length of hospital stay among the Indonesian population is still rarely conducted. This study employed a cross-sectional design to assess the association between hyperglycemia and hyperchloremia with the length of stay in acute ischemic stroke patients at the inpatient unit of RSUD Pasar Minggu Hospital, South Jakarta, Indonesia. Secondary data from acute ischemic stroke patients admitted between January 1, 2022, and December 31, 2022, were analyzed. A purposive non-probability sampling method was used to recruit the sample. Statistical analyses included descriptive, bivariate, stratification, and multivariate analyses. The results revealed that 84.5% of patients admitted to the RSUD Pasar Minggu Hospital had acute ischemic stroke. The proportion of hyperglycemia among these patients was 27%, while hyperchloremia was 10.2%. The median length of stay was 5 days. After controlling for covariates, the association between hyperglycemia and length of stay remained statistically significant (adjusted PR 2.21, 95% CI 1.70-2.88, p-value 0.00), while the association between hyperchloremia and length of stay remained not statistically significant (adjusted PR 1.02; 95% CI 0.66-1.45, p-value 0.56). There is a need for public health campaigns, regular screening programs, and integrated treatment monitoring systems for patients with metabolic disorders such as diabetes. It is essential to provide education, develop clear treatment protocols, and regularly monitor blood glucose and electrolytes in acute ischemic stroke patients.

Abstrak:
Informed consent bukanlah suatu pemberian tandatangan pada formulir, melainkan sebuah proses komunikasi di mana pasien diberi informasi tentang pilihannya untuk tes kesehatan, perawatan, atau prosedur, dan kemudian memilih opsi yang paling sesuai untuk tujuan dan nilainya. Informed consent sangat penting untuk hubungan terapeutik antara dokter dan pasien. Proses ini memungkinkan pasien, atau mereka yang bertanggung jawab secara hukum atas perawatan mereka, untuk membuat keputusan berdasarkan informasi tentang perawatan atau prosedur yang dimaksud. Di RSUP Sanglah Denpasar pada tahun 2016, tingkat kepatuhan pengisian informed consent masih rendah yaitu 58%, tingkat ketidakpatuhan penulisan singkatan sebanyak 42%.
Tujuan dari penelitian ini adalah untuk mengetahui gambaran kelengkapan terhadap persetujuan setelah penjelasan (informed consent) pada tindakan bedah secara menyeluruh di ruang rawat inap bedah RSUP Sanglah Denpasar pada tahun 2017. Metode penelitian ini merupakan penelitian kuantitatif dan kualitatif dengan pendekatan retrospektif dan crossectional. Jumlah sampel dokter bedah yang menjadi subjek penelitian terdiri dari 57 dokter bedah, 647 informed consent dan tiga orang informan untuk pengumpulan data secara kualitatif.
Dari penelitian ini didapatkan bahwa kelengkapan informed consent tindakan bedah di ruang rawat inap bedah RSUP Sanglah pada tahun 2017 hanya mencapai 30%. Hasil penelitian ini menunjukkan adanya hubungan signifikan antara jumlah kasus yang ditangani dan proses pemberian informed consent yang baik dengan kelengkapan pemberian informed consent tindakan bedah dengan p-value berturut-turut 0,02 dan 0,01. Dari penelitian ini dapat disimpulkan bahwa kelengkapan pemberian informed consent berhubungan dengan jumlah kasus yang ditangani dan proses pemberian informed consent yang baik.

Informed consent is essential for therapeutic relationships between physicians and patients. This process allows patients, or those who are legally responsible for their care, to make informed decisions about the treatment or procedure in question. In RSUP Sanglah Denpasar in 2016, compliance level of informed consent is still low ie 58%, non-compliance rate of writing abbreviation as much as 42%.

Review rekam medis merupakan suatu proses yang penting dalam rangka peningkatan mutu layanan Rumah Sakit sesuai akreditasi Internasional. Review rekam medis yang dilakukan belum mencapai 100%. Penelitian ini merupakan penelitian deskriptif kualitatif. Pengumpulan data dengan wawancara dan penelusuran dokumen. Analisa data dengan content analysis. Hasil penelitian menunjukkan standar pelaksanaan review rekam medis sudah dibuat sesuai rekomendasi tim akreditasi Internasional. Sosialisasi standar belum optimal, pemahaman tentang review rekam medis oleh dokter dan petugas gizi masih kurang, keterlibatan sumber daya manusia belum sesuai, belum adanya format laporan dan jadwal pelaporan dari instalasi rekam medis ke direksi dan belum berjalannya sistem evaluasi tindak lanjut sesuai PDCA (Plan Do Check Action). Kesimpulan: review rekam medis belum berjalan sesuai standar yang dibuat. Saran: pelaksanaan review rekam medis dimasukkan sebagai salah satu tugas pokok pejabat yang berwenang, supervisi dilakukan secara berkesinambungan, tingkatkan sosialisasi standar review rekam medis, review rekam dapat dilakukan secara rutin untuk semua rumah sakit sebagai siklus perbaikan kualitas rekam medis.
Review of medical records is an important process in order to improve the quality of service correspond Hospital International accreditation. Review of medical records that do not reach 100%. This research is a qualitative descriptive study. Data collection with interviews and document searches. Data analysis with content analysis. The results showed a standard implementation review of medical records has been made according to the International accreditation team's recommendations. Socialization of standards is not optimal, an understanding of the medical record review by physicians and nutrition workers are still lacking, the involvement of human resources is not appropriate, the lack of reporting formats and reporting schedules of the medical record installation to the directors and the evaluation system of follow-up not accordance to PDCA (Plan Do Check Action). Conclusion: a review of medical records have not been going according to the standards set. Suggestion: the implementation of medical record review included as one of the main tasks the competent authority, supervision is done on an ongoing basis, increase socialization standard medical record review, and medical record review can be performed routinely for all hospitals as medical record quality improvement cycle.

Kepuasan pelanggan menjadi aspek yang sangat penting sehingga harus dikeloladengan baik. Rendahnya kepuasan dalam suatu proses layanan rumah sakit dapatberdampak tidak terselesaikannya pembayaran oleh pasien (minggat), yang berimbaspada pasien tidak akan datang kembali atau terjadi kehilangan pelanggan. Beberapamasalah yang masih sering dikeluhkan oleh pasien adalah lamanya waktu tunggu.Waktu tunggu dicari dengan melakukan pengamatan terhadap pasien yang melakukanproses pelayanan di rawat jalan, kemudian diukur kepuasannya, dan ditanyakan pulamengenai niat kunjugan ulangnya.
