Angka Kematian Ibu (AKI) di Kota Depok meningkat tiga kali lipat lebih tinggi di tahun 2021 dibandingkan dengan tahun-tahun sebelumnya khususnya yang diakibatkan oleh COVID-19. Penelitian ini bertujuan untuk melihat penerapan pelayanan kesehatan maternal yang sudah disesuaikan di masa COVID-19 untuk deteksi dini komplikasi serta rujukan maternal di Kota Depok tahun 2021 dari eksplorasi keluarga ibu yang meninggal dunia. Studi kasus ini diharapkan merupakan upaya penting dilakukan dalam upaya menurunkan kematian maternal, baik di tingkat fasilitas maupun di tingkat kabupaten/kota. Jenis penelitian ini adalah penelitian kualitatif, menggunakan desain penelitian studi kasus untuk memberikan informasi detail terkait deteksi dini komplikasi serta rujukan maternal di Kota Depok tahun 2021, penelitian dilakukan di Kota Depok tahun pada bulan Juni sampai Juli 2023 dengan jumlah informan 12 orang, data diperoleh dengan wawancara mendalam dan telaah dokumen yang terdiri dua kelompok informan. Kelompok kasus adalah keluarga dari ibu hamil, bersalin, dan ibu nifas yang sudah meninggal di Kota Depok periode Januari – Desember 2021, kelompok control adalah ibu hamil, bersalin, dan ibu nifas yang tidak meninggal di Kota Depok periode Januari – Desember 2021. Hasil penelitian menunjukkan bahwa pelaksanaan deteksi dini komplikasi dan di Kota Depok sebagian besar sudah baik untuk ANC namun untuk telehealth belum terintegrasi karena masih menggunakan aplikasi yang tidak memiliki data base yang baik. Terkait rujukan maternal di Kota Depok pada tahun 2021 secara umum sudah baik ditandai dengan aspek jarak ke fasilitas kesehatan tidak memiliki masalah sama sekali, namun masih terkendala pada aspek pelayanan kesehatan optimal. Hal ini dipengaruhi oleh pandemi COVID-19 yang menyebabkan kendala kapasitas rumah sakit dalam hal ketersediaan tempat tidur dan kapasitas pelayanan. Keberhasilan deteksi dini dan rujukan maternal akan terwujud jika ada komitmen yang kuat dari semua pihak sehingga perlu penguatan peran dan komitmen tenaga kesehatan dalam pelaksanaan deteksi dini dan rujukan maternal sebagai bentuk dari upaya penurunan Angka Kematian Ibu .
The Maternal Mortality Ratio (MMR) in Depok City will be three times higher in 2021 compared to previous years, especially those caused by COVID-19. This study aims to look at the implementation of maternal health services that have been adjusted during the COVID-19 era for early detection of complications and maternal referrals in Depok City in 2021 from exploration of the families of mothers who died. It is hoped that this case study will be an important effort to reduce maternal mortality, both at the facility level and at the district ort city level. This type of research is qualitative research, using a case study research design to provide detailed information regarding early detection of complications and maternal referrals in Depok City in 2021, the research was conducted in Depok City from June to July 2023 with 12 informants, data obtained by in-depth interviews and document review consisting of two groups of informants. The case group was the families of pregnant, giving birth and postpartum women who had died in Depok City for the period January - December 2021, the control group were pregnant, giving birth and postpartum women who did not die in Depok City for the period January - December 2021. The results showed that the implementation of early detection of complications and in Depok City was mostly optimal for ANC but for telehealth it had not been integrated because it still used applications that did not have a good data base. Regarding maternal referrals in the city of Depok in 2021, in general, it has been good, marked by the aspect of distance to health facilities that has no problems at all, but is still constrained by aspects of optimal health services. This was influenced by the COVID-19 pandemic which caused constraints on hospital capacity in terms of bed availability and service capacity. The success of early detection and maternal referral will be realized if there is a strong commitment from all parties so that it is necessary to strengthen the role and commitment of health workers in implementing early detection and maternal referral as a form of efforts to reduce maternal mortality. Abstrak:
Keterlibatan masyarakat menjadi salah satu kunci penting penanganan masalah kesehatan masyarakat, khususnya saat pandemi. Besarnya upaya yang dilakukan oleh pemerintah perlu diimbangi dengan partisipasi aktif masyarakat dalam respon krisis penanganan pandemi COVID-19 agar tertangani dengan cepat. Praktik baik di beberapa negara dengan kesadaran masyarakat tinggi guna terlibat aktif dalam mendukung penanganan COVID-19, tingkat keberhasilan mengatasi dampak COVID-19 cukup tinggi jika dibandingkan dengan negara dengan warga yang terbilang cukup pasif atau bahkan tidak mau terlibat sama sekali (apatis). Pemerintah memiliki keterbatasan yaitu tidak mampu menjangkau seluruh komponen kehidupan masyarakat. Dengan demikian, keterlibatan dan partisipasi masyarakat akan memudahkan tugas pemerintah dalam menjalankan kebijakan yang telah dibuat. Desain penelitian ini adalah kuantitatif dengan metode cross sectional untuk menilai korelasi antara kinerja kader Surveilans Berbasis Masyarakat (SBM) dengan variabel independen yang dilakukan di 11 Puskesmas di Kota Depok. Guna memperkaya hasil pembahasan dan implementasi peran instansi terkait, peneliti menambahkan informasi yang bersumber dari wawancara mendalam pada informan kunci. Hasil menunjukkan bahwa variabel yang berhubungan dengan penemuan dan pelaporan COVID 19 secara dini oleh kader, antara lain status pernikahan (p value = 0,0001), lama waktu menjadi kader (p value = 0,038), status pelatihan (p value = 0,002), dan perilaku professional p value = 0,033). Selain itu, faktor-faktor yang paling dominan mempengaruhi kinerja kader dalam penemuan dan pelaporan COVID 19 secara dini ialah status pernikahan setelah dikontrol oleh variabel lain. Variabel confounding dalam penelitian ini ialah lama menjadi kader. Status pernikahan memiliki OR 15,34 artinya status menikah meningkatkan 15 kali kinerja kader dalam penemuan dan pelaporan COVID-19 secara dini (95%CI=1,9-118,8) setelah dikontrol oleh variabel lain dengan p-value 0,009. Selanjutnya adanya pelatihan surveilans berbasis masyarakat meningkatkan 3 kali kinerja kader dalam penemuan dan pelaporan COVID-19 secara dini (95%CI=1,3-5,05). Namun, jika dilihat dari p value, maka status pelatihan mendapatkan angka yang paling kecil yakni 0,006, sehingga dapat dikatakan bahwa pelatihan menjadi variabel paling berpengaruh pada kinerja kader SBM dalam penemuan dan pelaporan kasus COVID-19 di Kota Depok.
Community involvement is one of the important keys to handling public health problems, especially during a pandemic. The magnitude of the efforts made by the government needs to be balanced with the active participation of the community in the crisis response to the handling of the COVID-19 pandemic so that it can be handled quickly. Good practice in several countries with high public awareness to be actively involved in supporting the handling of COVID-19, the success rate in overcoming the impact of COVID-19 is quite high when compared to countries with citizens who are quite passive or even do not want to be involved at all (apathetic). The government has limitations, namely not being able to reach all components of people's lives. Thus, community involvement and participation will facilitate the government's task in carrying out the policies that have been made. The design of this study was quantitative with a cross sectional method to assess the correlation between the performance of Community-Based Surveillance (SBM) cadres and independent variables conducted at 11 Puskesmas in Depok City. In order to enrich the results of the discussion and implementation of the role of relevant agencies, the researcher added information sourced from in-depth interviews with key informants. The results showed that variables related to the early detection and reporting of COVID 19 by cadres, including marital status (p value = 0.0001), length of time being a cadre (p value = 0.038), training status (p value = 0.002), and professional behavior p value = 0.033). In addition, the most dominant factors influencing the performance of cadres in the early detection and reporting of COVID-19 is marital status after being controlled by other variables. The confounding variable in this study is the length of time being a cadre. Marital status had an OR of 15.34, meaning that marital status increased 15 times the performance of cadres in early detection and reporting of COVID-19 (95%CI=1.9-118.8) after being controlled by other variables with a p-value of 0.009.
