Abstrak:
Cardiovascular disease has become the first cause of death. Highest morbidity is found in the age, and among cardiovascular diseases, prevalence of hypertension is the highest. Many studies on the relationship between nutritional factors and hypertension have been done, but studies to observe determinants of hypertension in Indonesia are lacking. Therefore, there is an urgent need to elaborate information on various hypertension risk factors in the Indonesian elderly, which will allow the policy makers to provide appropriate intervention programs. The primary purpose of this study was to investigate various determinants of hypertension in the Indonesian elderly with different nutritional status. A cross sectional study was undertaken in Jakarta and 5 other cities with total sample of 1261 elderly using multistage random sampling. Subjects were recruited from elderly population in Jakarta, Padang, Bandung, Jogyakarta, Denpasar and Makasar. Data were collected through interview using structured questionnaires, anthropometrics measurements, biochemical blood and urine analysis, and blood pressure measurements. Daily nutrients intake was analyzed using WorldFood2 Dietary Assessment Program. Data were analyzed by using SPSS programs for Windows version 7.5; General Linear Model, Multiple linear regression and logistic regression analysis were performed to determine the predictive power of independent variables for outcome variables. Prevalence of hypertension found in the study was quite high, more than 50% of the study population for both men and women. This study showed significant differences of determinant and predictive factors of blood pressure between elderly with Body Mass Index (BMI) < 25 kg/m2 and BMI? 25 kg/m2. Prevalence of systolic and diastolic hypertension was higher in the elderly with BMI < 25 kg/lm2 than in the elderly with BMI 25 kg/m2. BMI was a significant determinant for diastolic blood pressure in elderly with BMI > 25 kg/m2. There was a positive association between blood pressure and Waist to hip ratio (WHR) irrespective of BMI value. Plasma LDL cholesterol >160 mg/dl increased the risk of having systolic hypertension 1.5 to 2 times in the elderly with BMI < 25 kg/m2 after the age of 65 years and increased the risk of having diastolic hypertension 1.5 times. Plasma triglycerides > 200 mg/dl increased the risk of having systolic hypertension 1.7 and 2.5 times in elderly with BMI 25 kg/m2 and in elderly with BMI? 25 kg/m2 respectively, after the age of 65 years and increased the risk of having diastolic hypertension Ft, 1.7 times. Ratio of total cholesterol to HDL cholesterol > 5 increased the risk of having hypertension 1.8 times in elderly with BMI? 25 kg/m2. Plasma HDL cholesterol < 35 mg/dl in elderly with BMI < 25 kg/m2 increased the risk of hypertension approximately 2.4 times. In elderly with BMI < 25 kgmm2, monounsaturated fatty acid (MUFA) had negative correlation (protecting effect) with diastolic and systolic blood pressure. Saturated fatty acid (SFA) had positive correlation with diastolic blood pressure and systolic blood pressure. Cholesterol intake had positive correlation with diastolic blood pressure. Multivariate analyses in this study did not find significant correlation between energy intakes with blood pressure. Although no significant correlation was found between protein intakes with blood pressure, this study showed that arginine intake had protecting effect against hypertension. The study also showed that calcium and potassium intake had negative correlation with DBP and SBP respectively in elderly with BMI > 25 kg/m2. In elderly with BMI < 25 kg/m2 sodium intake had positive correlation with SBP. This study also demonstrated that sport index had negative correlation (protecting effect) with diastolic blood pressure. Nutrition education to elderly group should emphasize healthy nutrients with protecting effect against hypertension and avoid nutrients with positive correlation to hypertension. Suggestion for sodium restriction especially in the elderly with BMI < 25 kg/m2, and proper physical/sport activity as a protecting factor against hypertension is very important for the elderly. Regular check of blood pressure and plasma lipid should be conducted and Public Health Centers equipped with appropriate laboratory facilities, for early detection of hypertensive risk factors. BMI category should be considered in hypertension program since there were differences of determinant factors of hypertension between different categories of BMI. Future studies should be directed on public health and nutrition intervention to the elderly community.

Hipertensi merupakan salah satu penyakit tidak menular dengan prevalensi tinggi yang memerlukan pemantauan dan pengobatan secara berkelanjutan melalui layanan kesehatan. Provinsi Jawa Barat memiliki prevalensi hipertensi yang tinggi, yaitu sebesar 34,4% berdasarkan hasil pengukuran tekanan darah dan 10,7% berdasarkan diagnosis dokter. Meskipun demikian, pemanfaatan layanan kesehatan primer pada penderita hipertensi masih belum optimal dan menunjukkan adanya kesenjangan antar kelompok masyarakat. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan pemanfaatan layanan kesehatan primer serta ketimpangan pemanfaatannya pada penderita hipertensi usia ≥18 tahun di Provinsi Jawa Barat. Penelitian ini menggunakan desain cross-sectional dengan data Survei Kesehatan Indonesia (SKI) tahun 2023. Analisis data dilakukan secara univariat menggunakan distribusi frekuensi, bivariat menggunakan uji chi-square, multivariat menggunakan regresi logistik ganda, serta analisis ketimpangan menggunakan Concentration Index. Hasil penelitian menunjukkan bahwa dari 4.357 penderita hipertensi usia ≥18 tahun, sebanyak 40,6% memanfaatkan layanan kesehatan primer dan 59,4% tidak memanfaatkannya. Faktor yang berhubungan dengan pemanfaatan layanan kesehatan primer meliputi usia, jenis kelamin, tingkat pendidikan, kepemilikan jaminan kesehatan, status sosial ekonomi, komorbiditas, dan status gizi (IMT). Analisis ketimpangan menunjukkan bahwa pemanfaatan layanan kesehatan primer masih terkonsentrasi pada kelompok sosial ekonomi yang lebih tinggi (pro-rich). Penelitian ini menyimpulkan bahwa pemanfaatan layanan kesehatan primer pada penderita hipertensi di Jawa Barat masih rendah dan belum merata. Oleh karena itu, diperlukan optimalisasi pelaksanaan skrining dan pemantauan hipertensi secara berkala, penguatan edukasi kesehatan, serta perluasan cakupan kepesertaan jaminan kesehatan untuk meningkatkan pemerataan pemanfaatan layanan kesehatan primer.
Hypertension is one of the most prevalent non-communicable diseases and requires continuous monitoring and long-term treatment through health care services. West Java has a high prevalence of hypertension, with 34.4% based on blood pressure measurements and 10.7% based on physician diagnosis. However, the utilization of primary health care services among patients with hypertension remains suboptimal and unequally distributed. This study aimed to analyze the determinants of primary health care utilization and socioeconomic inequality among adults aged ≥18 years with hypertension in West Java. This cross-sectional study used data from the 2023 Indonesian Health Survey (Survei Kesehatan Indonesia [SKI]). Data were analyzed using frequency distributions, chi-square tests, multiple logistic regression, and the Concentration Index. Among 4,357 adults with hypertension, 40.6% utilized primary health care services, while 59.4% did not. Factors significantly associated with primary health care utilization were age, sex, educational level, health insurance coverage, socioeconomic status, comorbidity, and body mass index (BMI). The Concentration Index indicated that primary health care utilization was disproportionately concentrated among individuals with higher socioeconomic status (pro-rich). In conclusion, primary health care utilization among patients with hypertension in West Java remains low and unequally distributed. Strengthening regular hypertension screening and follow-up, improving health education, and expanding health insurance coverage are needed to promote equitable utilization of primary health care services.

Abstrak:
The new approach to the Indonesia Air Transportation Policy, introduced in 1974, raises a number of issues which are of vital importance to the development of the country's air transportation system and the tourist sector. These issues concern the effectiveness of the alternative measures which were introduced as strategy in the new air transportation policy to achieve rational economic resource utilization as well as the influence of these measures on other national policy objectives, such as culture, security, foreign relations, and related national goals. Indonesia's potential and resources for domestic and international tourism development are among the world's greatest. Yet the lack of an effective air transportation policy and system, which are the lifeblood of international tourism, has apparently prevented the country from further capitalizing on these generous natural capital endowments. The study is an attempt to analyze whether the new policy approach has been effective in attaining the desired multi-objectives, and to advance some policy decision-making approaches towards solving the complex air transportation policy problems within the overall national setting . The ultimate goals of the new policy in air transportation are the formulation and implementation of a policy which will be both conducive to the development of the national air transport industry and tourism, and at the same time give due protection against possible future pitfalls in other related sectors of the economy .however because of social, cultural, political, and other considerations or constraints, a certain trade-off among the objectives is inevitable. The policy-maker, striving towards an efficient management of air transport, tourist, and other related national resources has tried to devise a "policy-mix approach" to attain those multiple goals, some conflicting, some complementary, and a few irreconcilable or in-compatible. In order to be able to more objectively evaluate how far the new policy has been able to achieve those policy goals, in other words, how far the policy has been formulated effectively so as to provide the best compromise between the conflicting policy objectives and accommodate most of the supplementary interests which are reconcilable, the dual trade-off analysis has been presented and developed in this study . This is further complemented with other policy decision-making approaches, including games theory, adapted in a participatory air access policy- options display.
