Abstrak:
Pneumonia pada balita masih merupakan masalah kesehatan utama baik di dunia maupun di Indonesia. Di Indonesia, Pneumonia merupakan penyebab kematian nomor dua pada bayi dan anak balita. Kecamatan Cakung merupakan salah satu daerah yang memiliki kasus pneumonia pada balita yang cukup banyak. Penelitian ini bertujuan untuk mengetahui faktor risiko yang berhubungan dengan kejadian pneumonia pada balita di wilayah Puskesmas Kecamatan Cakung. Penelitian ini menggunakan desain studi case control. Populasi penelitian adalah balita usia 12-59 bulan yang berada di Wilayah Puskesmas Kecamatan Cakung. Hasil penelitian menunjukkan ada hubungan antara paparan asap rokok dalam rumah (OR=4,67; 1,19-18,33); tingkat konsumsi rokok (OR=2,77; 1,12-6,86), pencahayaan alami dalam rumah (OR=5,16; 1,94-13,70); pengetahuan ibu (OR=3,85; 1,12-13,25), status gizi (OR=9,14; 1,90-43,89), riwayat imunisasi (OR=3,85; 1,12-13,25) dan riwayat ASI ekslusif (OR=3,11; 1,24-7,78) terhadap kejadian pneumonia pada balita di wilayah Puskesmas Kecamatan Cakung. Faktor yang diprediksi paling dominan mempengaruhi kejadian pneumonia adalah status gizi (OR=5,607; 1,082-29,058).

Pneumonia in children under five is still major public health problem in the world or in Indonesia. In Indonesia, Pneumonia is the number two cause of death in infants and children under five. Cakung sub-district is one of the areas that have quite a lot cases of pneumonia in children under five. This study aimed to determine the risk factors associated with the incidence of pneumonia in children under five in the region of Cakung sub-district health center. This study uses a case control study design. The population in this study are all of children aged 12 month until 59 months who lived in the region of Cakung sub-district health center. The results of this study indicate that there was a significant correlation between exposure to secondhand smoke in the home (OR = 4.67; 1.19 to 18.33); the number of ciggarates smoked per day (OR=2,77; 1,12-6,86), lighting in the home (OR = 5.16; 1.94 to 13.70), knowledge of mothers (OR = 3.85; 1.12 to 13.25), nutritional status (OR = 9.14; 1.90 to 43.89), immunization history (OR = 3.85; 1.12 to 13 , 25) and a history of exclusive breastfeeding (OR = 3.11; 1.24 to 7.78) with the incidence of pneumonia among children under five in the region of Cakung sub-district health center. The variable that predicted the most dominant cause of pneumonia is the nutritional status (OR = 5.607; 1.082 to 29.058).

Latar Belakang : Diare merupakan penyebab kematian terbesar kedua pada anak dibawah usia lima tahun yang membunuh sekitar 252.000 balita per tahun secara global. Di Indonesia sendiri, prevalensi diare pada balita menurut RISKESDAS 2018 mengalami kenaikan dari 2,4% pada tahun 2013 menjadi 11%. Provinsi Banten tercatat menjadi provinsi kedua dengan angka cakupan pelayanan diare pada balita tertinggi sebesar 44,3% menurut Profil Kesehatan Indonesia 2020. Kota Tangerang merupakan salah satu kota di provinsi Banten yang melayani kejadian diare pada balita dengan temuan kasus sebanyak 3,26% (9.799 kasus) dari jumlah sasaran target temuan pada tahun 2020. Dari tiga puluh delapan puskesmas di Kota Tangerang, Puskesmas Cibodasari yang terletak di Kecamatan Cibodas menjadi salah satu puskesmas yang melayani kasus diare pada balita sebanyak 5,47% (592 kasus) dari jumlah sasaran target temuan. Belum diketahui faktor apa yang menjadi penyebab tingginya angka temuan kejadian diare pada balita di Puskesmas Cibodasari sehingga penelitian berikut bertujuan untuk mencari tahu terkait faktor risiko kejadian diare pada balita di wilayah kerja Puskesmas Cibodasari Kota Tangerang tahun 2022. Metode : Penelitian ini merupakan penelitian kuantitatif dengan menggunakan desain case control. Metode sampling yang digunakan adalah teknik purposive sampling yang ditentukan berdasarkan diagnosa penyakit berupa diare dan non-diare (diluar gejala diare) yang didapat dari rekam medik pasien. Pengumpulan data dilakukan dengan wawancara secara door to door dengan alat bantu kuesioner. Analisis univariat digunakan untuk memberikan gambaran tiap variabel dan analisis bivariat dengan uji chi-square untuk melihat hubungan antara variabel dependen dan independen. Hasil : Didapati variabel yang berkontribusi secara berurut dari paling besar ke kecil dalam kejadian diare pada balita adalah perilaku cuci tangan (OR: 6.19; 95% CI: 2.19-17.5), riwayat ASI Eksklusif (OR: 3.91; 95% CI: 1.40-10.8), dan pengetahuan (OR: 2.78; 95% CI: 1.07-7.22). Kesimpulan : Ketiga variabel independen yakni pengetahuan, perilaku cuci tangan, serta riwayat ASI Eksklusif memiliki hubungan dengan kejadian diare pada balita. Kurangnya kesadaran masyarakat perihal perilaku cuci tangan, pemberian ASI Eksklusif, serta minimnya pengetahuan terhadap kejadian diare pada balita menjadi tantangan bagi tenaga kesehatan di Puskesmas Cibodasari untuk kembali mengedukasi ulang masyarakat setempat terkait variabel yang diteliti dan hubungannya dengan diare pada balita.
Background: Diarrhea is the second biggest cause of death which kills around 252,000 children under five per year globally. The prevalence of diarrhea among toddlers in Indonesia according to RISKESDAS 2018 has increased from 2.4% in 2013 to 11%. Banten Province was recorded as the second province with the highest coverage rate of diarrhea services among toddlers at 44.3% according to Indonesian Health Profile 2020. Tangerang City is one of the cities in Banten Province which served the diarrhea incidence among toddlers with 3.26% of case findings (9.799 cases) from the targeted finding number of cases in 2020. From thirty-eight public health centers in Tangerang City, Cibodasari Public Health Center which is located in Cibodas District became one of the public health centers which served the diarrhea incidence among toddlers with 5.47% of case findings (592 cases) from the targeted finding number of cases in 2020. The factor which caused the diarrhea incidence among toddlers in Cibodasari Public Health Center remains unknown, thus the following research aims to find out the risk factor of diarrhea incidence among toddlers in Cibodasari Public Health Center's Working Area, Tangerang City in 2022. Methods: This is a quantitative study with a case-control design and used primary data which was collected through door-to-door interviews. The sampling method used was the purposive sampling technique which was determined based on the diagnosis of diarrheal and non-diarrheal disease (excluding the symptoms of diarrhea) obtained from the patient?s medical record. The univariate analysis was used to describe each variable and the bivariate analysis (using the chi-square test) was used to find out the relation between the dependent and independent variables. Result: The variables that contributed sequentially to the diarrhea incidence among toddlers from the largest to the smallest were hand-washing behavior (OR = 6.19; 95% CI = 2.19-17.5), exclusive breastfeeding history (OR = 3.91; 95% CI = 1.40-10.8), and knowledge (OR = 2.78; 95% CI = 1.07-7.22). Conclusion: These three independent variables which are knowledge, hand-washing behavior, and exclusive breastfeeding had a relation with diarrhea incidence among toddlers as a result. The lack of awareness about hand-washing behavior and exclusive breastfeeding, also the knowledge of toddler's diarrheal becomes a challenge for the Cibodasari Public Health Center?s health workers to re-educate people about the three main variables of research and their relation to toddler?s diarrheal.

Pneumonia adalah salah satu penyakit infeksi saluran pernapasan akut (ISPA) yang menyerang alveoli dan disebabkan oleh bakteri, virus, dan jamur. Pneumonia menjadi penyakit utama yang membunuh anak dibawah usia lima tahun (balita). Kelurahan Ancol menunjukkan tren fluktuatif kasus pneumoniaa dan didominasi oleh permukiman padat, menjadikannya wilayah yang rentan terhadap penyakit. Penelitian ini bertujuan untuk menganalisis hubungan antara faktor balita, perilaku anggota keluarga dan rumah dengan kejadian pneumonia pada balita di wilayah kerja Puskesmas Kelurahan Ancol. Penelitian ini menggunakan desain studi kasus kontrol dengan total sampel 36 responden. Pengambilan data dilakukan dengan wawancara dan observasi lingkungan. Analisis data yang dilakukan meliputi analisis univariat, analisis bivariat, dan analisis multivariat. Hasil penelitian menunjukkan variabel usia memiliki hubungan yang signifikan terhadap kejadian pneumonia dan menjadi faktor yang dipredisksi paling dominan mempengaruhi kejadian pneumonia. Puskesmas Kelurahan Ancol dapat meningkatkan penyuluhan rutin kepada keluarga balita mengenai pentingnya ventilasi dan pencahayaan alami rumah, bahaya asap rokok, dan asap dari obat nyamuk bakar.

Pneumonia remains a leading cause of death among children under five years old. Kelurahan Ancol shows a fluctuating trend of pneumonia cases and is dominated by densely populated settlements, making it a vulnerable area for disease transmission. This study aims to analyze the relationship between child, family behavioral, housing conditions factors with the incidence of pneumonia among children under five in the working area of Puskesmas Kelurahan Ancol. A case-control study design was employed, involving a total of 36 respondents in both case and control groups. Data were collected through interviews and environmental observations. The analysis included univariate, bivariate, and multivariate approaches. The results revealed that age had a significant association with pneumonia incidence and was identified as the dominant predictive factor influencing its occurrence. Puskesmas Kelurahan Ancol can enhance routine health education for families with children under five regarding the importance of home ventilation and natural lighting, as well as the dangers of cigarette smoke and mosquito coil smoke.
