Abstrak: Skripsi ini membahas aplikasi kesehatan kerja yang sudah dilakukan oleh PT.Bridgestone Tire Indonesia Bekasi pada tahun 2012 yang dibandingkan denganKemenkes nomor 1758 tahun 2003 dan Kep Dirjen P2K nomor 22 tahun 2008.Penelitian ini adalah penelitian kualitatif dengan disain deskriptif. Hasil penelitianmenyarankan bahwa PT. Bridgestone Tire Indonesia Bekasi harus melakukanpengukuran lingkungan kerja selama 8 jam terutama pada pengukuran kebisingandan kuallitas udara; melakukan test Spirometri pada pekerja area Banbury, FinalInspection dan Tube; harus menambah pelatihan P3K sebanyak 6 orang; harusmelakukan pemeriksaan kesehatan pra mutasi dan pra mutasi intern; harusmelakukan recruitment 1 orang dokter S2K3, 1 orang Perawat hiperkes, 1 oranglaborat, 1 orang industrial hygien; harus melengkapi peralatan seperti alat P3K,Komputer, buku panduan, personal dust sampler; juga harus membuat insidentrate penyakit akibat kerja (PAK) dan kecelakaaan kerja (KK), prevalensi ratePAK dan KK, serta angka absensi sakit akibat penyakit umum, PAK dan kecelakaan kerja.Kata kunci : Kesehatan kerja, penyakit akibat kerja, kecelakaaan kerja
This minithesis discusses the application of occupational health that have beenconducted by PT. Bridgestone Tire Indonesia Bekasi in 2012 compared withDecision of the Health Ministers No. 1758 of 2003 and Decision of the DirectorGeneral of Labour Inspection Guidance No. 22 of 2008. This research is aqualitative descriptive design. The results suggest that PT. Bridgestone TireIndonesia Bekasi have to take measurements for 8 hours of work environments,especially in the measurement of noise and air quality, perform spirometry test inBanbury area workers, Final Inspection and Tube, should add as many as 6 peopleFirst Aid training, need a medical examination pre-mutation and pre-mutationinternal, should do recruitment 1 Physician with Magister Of Occupational Healthand Safety, 1 Nurse with training of Hiperkes, 1 lab, 1 industrial hygienist, mustcomplete tools such as First Aid equipment, computers, manual of occupationalhealth services, personal dust sample, should also make insident rate ofoccupational diseases and occupational accidents, the prevalence rate ofoccupational diseases and occupational accidents, as well as the number ofsickness absence due to common diseases, occupational diseases and occupationalaccidents.Key Word:Occupational health, occupational diseases, occupational accidents

Abstrak:

Employees as human resources play an important role in pacify and expand a production process. They carry out a necessary working activity that in some cases are exposed by the risk of dangerous situation. Occupation accident in an industry, especially in a manufacturing company constitute as undesired evidence. Labor protection principle in occupational accident not only evaluate a potential risk of danger but also find out the cause of emergence. This thesis emphasizes on the cause factors of accident that might suddenly happen in preceding to the working activity. This study would be explored in the production unit of Bridgestone Tire Indonesia. It uses the Fault Tree Analysis Method with a qualitative research. The result of this research shows that the occupation accident occurring in that company has various types following Heinrich theories that the cause of accident are due to fail of carefulness during working activities and unsafe working conditions. To pay attention on the statistics of accident, based on the yearly report, the evidence has increased about 72,7% from 1999 to 2001, at the time patrol safety clothes, the figure fluctuate. That research only focus on flow think or flow chart for unsafe acts and unsafe conditions with secundair data result in fact with field in Fault tree analysis. From the data field result unsafe condition caused of operation machine, materiil conditions and environment condition. Accupation accident from operation machine caused of operation didn't follow on procedures, abnorm condition and didn't connect. Abnorm conditions may be caused of requirement function unprotect, trouble machine or lable in machine can't understand becaused Kanji letters. On trouble may be caused of didn't function of emergency stop, rope emergency can't caught, trouble operator to find manual stop or less of requirement gived. Environment conditions may be floor not smooth, unbalance cleaning service, not keep on Healhty or narrow office in work, in unsafe acts caused of operator working unfollowed with standard and machine abnorm condition. Operator working unfollowed with standard caused of neglected on working, work without safety clothes, work in machine oparation, work withough requirement protect, and hurry working, work unsafe body position, work trouble, unsafe requirement. Work without safety clothe s caused of safety clothes broken, unwilling safety clothes, safety clothes unrelative give, working with safety clothes unpleasant or safety clothes not enough stock. In hurry working caused of in hurry finish or in hurry order target. Trouble working caused of don't know risk or low educated. Hopefully this thesis could contribute to decrease the figure of accident in the manufacturing companies.Bibliography : 35 ( 1980-2000)

Abstrak: Tangki timbun WT16 pada PT. Bridgestone Tire Indonesia memiliki risiko kebakaran yang tinggi dikarenakan merupakan cairan mudah terbakar. Walaupun jumlah kejadian mengenai kebakaran pada tangki timbun WT16 ini tidak ditemukan di Indonesia,namun penilaian risiko kebakaran merupakan aktifitas mendasar yang harus dilakukan oleh industri kimia, petrokimia dan industri yang menggunakan hidrokarbon dalam proses produksinya. Penelitian ini bersifat deskriptif dengan menggunakan data sekunder berupa keadaan tangki timbun WT16. Penelitian ini menggunakan penilaian risiko secara kualitatif yang menghasilkan bahwa kebakaran pada tangki timbun WT16 ini termasuk kedalam high risk. Kemudian berdasarkan perhitungan dengan ALOHA software di dapatkan hasil radiasi panas pada jarak 71 meter dari tangki dengan intensitas radiasi 10 Kw/ m2, 97 meter dari tangki dengan intensitas radiasi panas 5 Kw/m2 dan 147 meter dari tangki dengan intensitas 2 Kw/m2 pada tangki nomor 3 dan 10 Kw/m2 pada jarak 68 meter, intensitas panas 5 Kw/m2 pada jarak 94 meter dan dengan intensitas panas 2 Kw/m2 pada jarak 142 meter pada tangki nomor 2. Serta jarak aman berdasarkan perhitungan menggunakan acceptable separation distance calculator didapatkan hasil jarak aman bagi manusia dari tangki nomor 3 dan bangunan masing masing 179, 16 meter dan 35,38 meter untuk dan untuk tangki nomor 2 jarak aman berada pada jarak 128, 60 meter dan 24,48 meter untuk manusia dan bangunan masing masing.
 

WT16 Solvent storage tank has a potential fire risk because this tank contain flammable liquid solvent. Even though, there is no record that this case ever happens in Indonesia. But, fire risk assessment is the essential activities that need to do for chemical industry, petrochemical industry or any other industry that use hydrocarbon in their industrial activity. The method used for this paper is descriptive method with secondary data about the storage tank. This paper use qualitative risk assessment result that fire risks for WT16 the storage tank is high. Then based on the calculation of the ALOHA software in getting the results of the thermal radiation in tank number 3 have 71 meters with 10 Kw/m2 intensity of radiation, 97 meters with 5 Kw/m2 intensity of radiation, 147 meter with Kw/m2 intensity of radiation and for tank number 2 have 68 meters with 10 Kw/m2 intensity of radiation, 94 meters with 5 Kw/m2 intensity of radiation, 142 meters with 2 Kw/m2 intensity of radiation. Then, a safe distance by calculating using the acceptable separation distance calculator results obtained within safe for humans 179, 16 meters and 35.38 meters for buildings for tank number 3 and for the tank number 2 the safe distance is at 128, 60 meters for humans and 24, 48 feet for the building.
