Abstrak:
Kelainan refraksi merupakan masalah kesehatan masyarakat yang terjadi secara global dan banyak terjadi di negara dengan penghasilan menengah kebawah. Kondisi ini termasuk ke dalam masalah kesehatan mata yang dapat dicegah dan ditangani dengan adanya pelayanan kesehatan yang komprehensif. Namun, faktor sosioekonomi menjadi salah satu kendala yang dialami oleh para penderita kelainan refraksi. Sehingga, integrasi pelayanan kelainan refraksi ke dalam pelayanan di fasilitas kesehatan tingkat primer menjadi penting untuk dilakukan. Penelitian ini menggunakan metode literature review dengan model IPO. Pencarian studi yang relevan dilakukan dengan beberapa database, yaitu PubMed, Proquest, ScienceDirect, dan Wiley Online Library. Hasil penelitian ini menunjukkan bahwa pelayanan kelainan refraksi sudah terintegrasi sebagai suatu pelayanan di fasilitas kesehatan primer berbagai negara meskipun dengan beberapa keterbatasan, khususnya dari sisi ketersediaan tenaga kesehatan, peralatan kesehatan dan ketersediaan alat bantu optik. Upaya yang dilakukan berkaitan dengan task-shifting kepada tenaga kesehatan dan penggunaan kacamata siap pakai. Namun, untuk jaminan asuransi kesehatan, belum ditemukan adanya penjaminan untuk pelayanan kesehatan mata secara komprehensif, khususnya untuk biaya alat bantu kacamata. Kesimpulan dari penelitian ini adalah belum adanya pelaksanaan pelayanan kelainan refraksi yang komprehensif di berbagai negara.

Refractive error are public health problems that occur globally and in many middle to low-income countries. This condition is categorized as can be prevented and treated with comprehensive health services. However, the socioeconomic factor is one of the obstacles experienced by people with refractive error. Thus, integration of refractive error into services at primary health facilities is important. This study uses the literature review method with the IPO model. The search for relevant studies was carried out with several databases, namely PubMed, Proquest, ScienceDirect, and Wiley Online Library. The results of this study indicate that refractive error services delivery has been integrated into primary health facilities in various countries, although with some limitations, especially in terms of the availability of health workers, medical equipment, and the availability of optical aids. Efforts are made relating to task-shifting health workers and the use of ready-made glasses. However, for the health insurance coverage scheme, there has not yet been found a comprehensive guarantee for eye health services, especially for the cost of glasses. The conclusion of this research is the absence of comprehensive refraction services in various countries.

Penelitian ini menganalisis utilisasi pelayanan kesehatan mental depresi di fasilitas kesehatan pada peserta JKN tahun 2023. Desain penelitian ini non-eksperimental dengan pendekatan cross sectional. Hasil penelitian menunjukkan bahwa mayoritas peserta memanfaatkan pelayanan kesehatan mental depresi dengan frekuensi kunjungan yang rendah atau 1-3 kali kunjungan dalam satu tahun, yaitu sebanyak 70.351 (88,3%) peserta. Faktor-faktor yang menunjukkan hubungan yang signifikan yaitu umur, jenis kelamin, status perkawinan, segmentasi peserta, jenis fasilitas kesehatan, kepemilikan fasilitas kesehatan, dan wilayah fasilitas kesehatan. Determinan utama adalah umur, dengan peserta yang berumur 45-54 tahun memiliki peluang tertinggi untuk berada pada kategori frekuensi kunjungan yang tinggi (OR: 2,583; 95% CI: 2,042-3,268). Temuan ini menunjukkan bahwa diperlukan edukasi, intervensi, peningkatan kualitas layanan dan tenaga kesehatan, serta pemerataan fasilitas dan tenaga kesehatan.

Penelitian ini menganalisis utilisasi fasilitas kesehatan tingkat pertama pelayanan kesehatan mental di Indonesia. Penelitian menggunakan desain cross-sectional dengan menggunakan data sampel BPJS tahun 2015-2022 yang berisi data kunjungan tahun 2022. Penelitian ini menggunakan Chi-square. Dalam penelitian ini dapat diobservasi bahwa utilisasi pelayanan kesehatan tingkat pertama oleh peserta JKN pada tahun 2015-2022 sebesar 331.008.686 kunjungan dengan total kunjungan ke pelayanan kesehatan mental di FKTP sebesar 1.158.843 kunjungan. Proporsi utilisasi pelayanan kesehatan mental di FKTP di Indonesia lebih besar pada jenjang umur dewasa dengan besar 994.553 kunjungan, jenis kelamin laki-laki sebesar 639.442 kunjungan, status belum kawin sebesar 632.421, tinggal di Provinsi Jawa Tengah sebesar 234.583 kunjungan, berkunjung ke puskesmas sebesar 935.204 kunjungan, dan segmen peserta PBI APBN sebesar 470.619 kunjungan. Oleh karena itu, memastikan ketersediaan fasilitas pelayanan kesehatan mental di FKTP beserta tenaga ahli kesehatan mental yang terlatih dan sesuai standar harus ditingkatkan, terlebih daerah dengan kunjungan tertinggi.
This study analyses the utilisation of first-level health facilities for mental health services in Indonesia. The study used a cross-sectional design using BPJS sample data for 2015-2022 which contained visit data for 2022. This study used Chi-square. It was observed that the utilisation of primary healthcare services by JKN participants in 2015-2022 was 331,008,686 visits with a total of 1,158,843 visits to mental health services at primary healthcare facilities. The proportion of mental health service utilisation at primary healthcare facilities in Indonesia is greater in the adult age group with 994,553 visits, male gender with 639,442 visits, unmarried status with 632,421 visits, living in Central Java Province with 234,583 visits, visiting puskesmas with 935,204 visits, and the PBI APBN participant segment with 470,619 visits. Therefore, ensuring the availability of mental health service facilities at FKTP along with trained and standardised mental health experts must be improved, especially in areas with the highest visits.
