Pneumonia merupakan penyakit penyebab kematian kedua tertinggi setelah diare diantara balita. Hal ini menunjukkan bahwa pneumonia merupakan penyakit yang menjadi masalah kesehatan masyarakat utama yang berkontribusi terhadap tingginya angka kematian balita di Indonesia. Intervensi yang dapat dilakukan untuk menurunkan angka mortalitas neonatal antara lain promosi menyusui dini dan diikuti dengan pemberian ASI eksklusif. ASI merupakan minuman alami bagi bayi baru lahir pada bulan pertama kehidupannya yang bermanfaat untuk kesehatannya. Namun sayang, cakupan ASI eksklusif di Indonesia masih sangat rendah. tingginya angka pneumonia dan rendahnya cakupan ASI eksklusif ini perlu mendapatkan perhatian. Penelitian ini menggunakan disain kasus kontrol. Kasus adalah anak usia 1-5 tahun yang didiagnosa pneumonia oleh dokter/bidan/perawat yang berkunjung ke puskesmas terpilih. Kontrol adalah anak usia 1-5 tahun yang didiagnosa tidak pneumonia dan penyakit berat yang berkunjung ke puskesmas terpilih. Analisis yang dilakukan adalah analisis univariat, bivariat, stratifikasi, dan multivariat dengan logistik regresi. Dan hasil penelitian diperoleh bahwa anak yang diberi ASI tidak eksklusif memiliki Rasio Odds 6,699 kali (95% CI: 3.204-14.007) untuk mengalami kejadian pneumonia dibandingkan anak yang diberi ASI eksklusif setelah dikontrol dengan variabel adanya perokok, adanya asap pembakaran, pendidikan ibu, riwayat imunisasi DPT lengkap dan status gizi. Hasil penelitian ini membuktikan hipotesis penelitian yaitu pemberian ASI tidak eksklusif meningkatkan risiko kejadian Pneumonia dibandingkan yang diberi ASI eksklusif pada anak usia 1-5 tahun di Kabupaten Bogor. Intervensi melalui promosi ASI eksklusif selama enam bulan, program anti rokok, penggunaan bahan bakar untuk masak yang sempurna pembakarannya, pemberdayaan ibu rumah tangga, imunisasi DPT dan perbaikan gizi, perlu dilakukan secara terintegritas untuk menurunkan morbiditas dan mortalitas penyakit Pneumonia.
Pneumonia is one of main cause after diarrhea among under-five year children. It was showed that Pneumonia had become a major public health problem which contributed toward the high of mortality among under-five year children. The intervention action we can do is breastfeeding initiation promotion and giving exclusive breastfeeding. Breastfeeding is a natural food for newborn particularly in the first month of life, that very important for their health in the future. Unfortunately, the practice of exclusive breastfeeding in Indonesia was not really good. The high prevalence of Pneumonia and the low of exclusive breastfeeding practice, so we should give more attention for this problem. The objective of study was to identify the role of exclusive breastfeeding practice with lower respiratory tract infection in under-five children in Bogor District. This study used case control design. Sample was selected by purposive (for cases) and random sampling (for control). Each group consist 107 cases and 107 controls. Case definition is children suffered Pneumonia whom diagnosed by doctor/midwives/nurse seeking for treatment at selected health center. Control definition is children without Pneumonia whom diagnosed by doctor/midwives/nurse seeking for treatment at selected health center. Data analysis used univariate, bivariate, stratification, and multivariate using logistic regression. And the result of study showed that children whom not given exclusive breastfeeding had the risk Rasio Odds 6,699 kali (95% CI: 3.204-14.007) greater to Pneumonia than children whom given exclusive breastfeeding in under-five year children after controlled by smoker at home, air pollution from cooking at home, level education of mother, immunization status and nutrition status. This result succeed to prove the hypothesis that not giving exclusive breastfeeding practice increasing the risk to Pneumonia than whom given exclusive breastfeeding in under-five year children. Intervention through exclusive breastfeeding promotion, anti-cigarette program, use of stove with perfect burst, mother empowerment, immunization, improving nutrition status should applied at each family for decreasing morbidity and mortality caused by lower respiratory tract infection in under-five year children.
Pneumonia merupakan penyebab kematian terbesar pada anak di seluruh dunia. Setiap tahunnya diestimasikan sekitar 18% kematian anak di bawah usia 5 tahun di seluruh dunia disebabkan oleh pneumonia. Faktor risiko pasti yang berkontribusi diantaranya yaitu balita yang tidak mendapatkan ASI eksklusif.
Tujuan studi ini untuk mengetahui hubungan pemberian ASI eksklusif terhadap kejadian pneumonia balita usia 12 -23 bulan setelah dikontrol terhadap confounder. Studi kasus kontrol ini dilakukan di tiga wilayah puskesmas Kota Cimahi berdasarkan angka insidens kasus pneumonia balita yang tertinggi di tahun 2012. Kasus adalah balita usia 12 - 23 bulan yang berkunjung ke sarana puskesmas penelitian periode Januari - Desember 2012 dan didiagnosa sebagai kasus pneumonia. Kontrol merupakan tetangga dari kasus, dengan perbandingan jumlah kasus dan kontrol yaitu 1:1. Besar sampel minimal sebanyak 133 untuk masing - masing kelompok. Analisis multivariat dengan menggunakan regresi logistik. Besar asosiasi balita yang tidak mendapat ASI eksklusif memiliki OR untuk terjadinya pneumonia sebesar 3,58 kali (95% CI: 2,08 - 6,19) dibandingkan yang mendapat ASI eksklusif setelah dikontrol terhadap confounder.
Penelitian ini memperkuat penelitian terdahulu yang membuktikan kekuatan hubungan pemberian ASI eksklusif terhadap kejadian pneumonia pada balita. Berfokus pada daerah dengan angka insiden kasus penumonia yang tinggi, pihak dinas kesehatan dan puskesmas dapat lebih meningkatkan upaya promosi dan fasilitasi ASI eksklusif, menciptakan kawasan tanpa asap rokok di tingkat rumah tangga, pengurangan adanya paparan asap pembakaran di dalam rumah, peningkatan pengetahuan ibu berkaitan faktor risiko pneumonia.
Pneumonia is the biggest cause of death in children worldwide. Each year approximately 18% of estimated deaths of children under five worldwide are caused by pneumonia. Definite risk factors that contribute to them are children under five who are not exclusively breastfed.
The purpose of this study to determine the relationship of exclusive breastfeeding on the incidence of pneumonia children under five age 12 -23 months after controlling for confounders.Case-control study was conducted in three areas of public health centers Cimahi City based incidence rates were highest children under five cases of pneumonia in 2012. Cases were children aged 12-23 months who visited the research public health centers means the period of January to December 2012 and was diagnosed as a case of pneumonia. Control is a neighbor of the case, the ratio of the number of cases and controls is 1:1. Minimum sample size for each of as many as 133 - each group. Multivariate analysis using logistic regression. Major association children under five who are not exclusively breastfed for the occurrence of pneumonia had an OR of 3.58 (95% CI: 2.08 to 6.19) than those who are breastfed exclusively after controlling for confounders.
This study reinforces previous research that proves the strength of association of exclusive breastfeeding on the incidence of pneumonia in infants. Focusing on areas with a number of high incidence of cases of pneumonia, the health department and public health center could further enhance the promotion and facilitation of exclusive breastfeeding, creating a smoke-free area at the household level, reduction in exposure to combustion fumes in the house, increasing maternal knowledge of risk factors associated pneumonia.
Stunting is a public health problem that needs to be addressed both in Indonesia and the World. The real impact of this problem is hampered motor development, mental growth and increased risk of morbidity and death. One of the risk factors for stunting in children is the lack of nutritional intake of infants, especially exclusive breastfeeding. This study aims to determine the relationship between exclusive breastfeeding with stunting in toddlers aged 12-36 months in Tamansari sub-district, Bogor district, 2019. The research design used was cross-sectional using primary data. The sample in this study amounted to 500 toddlers obtained by Proportional Random Sampling. This research was conducted in July 2019. Data collection was carried out by measuring height, body length, interviews with questionnaires and food recall. Stunting status was assessed based on the TB / U indicator <-2 z-score. The analysis used in this study uses multivariate cox regression analysis and the magnitude of the effect is expressed in the prevalence ratio (PR) with a 95% confident interval (CI). The results showed that the prevalence of stunting in toddlers aged 12-36 months in Tamansari sub-district, Bogor district in 2019 was 39.2%. The results of multivariate analysis showed that toddlers who were not given exclusive breastfeeding had a chance of 1.37 (95% CI: 1.03-1.83) times stunting compared to toddlers who were given exclusive breastfeeding after being controlled by maternal education and toddler calorie intake. It is necessary to improve maternal education at least high school and to provide toddlers food intake that is high in energy and protein and affordable by the community.
Latar belakang: Inisiasi menyusu dini memberikan efek yang menentukan bagi kelanjutan pemberian ASI Eksklusif hingga 6 bulan pada bayi. Beberapa penelitian menyimpulkan bahwa bayi yang berhasil menyusu dalam satu jam pertama setelah lahir cenderung akan terus menyusu lebih lama. Satu jam pertama merupakan periode emas yang akan menentukan keberhasilan seorang ibu untuk dapat menyusui bayinya secara optimal. Keberhasilan memberikan ASI Eksklusif yang diawali oleh keberhasilan dalam memberikan kesempatan dalam satu jam pertama ini berkaitan dengan refleks menghisap (suckling reflex) pada bayi. Dimana pada jam-jam pertama setelah lahir refleks menghidap bayi sangat kuat dan setelah itu bayi akan tertidur (Sidi et al, 2007). Penelitian ini dilakukan untuk mengetahui adanya pengaruh antara inisiasi menyusu dini terhadap kelangsungan pemberian ASI Eksklusif pada baya berusia 6 - 12 bulan di Kabupaten Kuantan Singingi. Metodologi: Penelitian merupakan studi observasional yang menggunakan desain kros seksional. Namun dalam penelitian ini terdapat variabel waktu yang merupakan periode follow up yang peroleh melalui recall (ingatan) responden. Temporal ambiguity dapat dihindari, karena event terjadi setelah exposure. Analisis dilakukan dengan life fable, uji log rank dan Kaplan Meier serta cox proportional hazard. Hasil dan Pembahasan: Inisiasi menyusu dini di Kabupaten Kuantan Singingi dilakukan bervariasi dengan waktu tercepat 15 menit setelah bayi lahir. Sebagian besar (73,1 %) bayi di Kabupaten Kuantan Singingi baru diberikan kesempatan untuk inisiasi menyusu dini > 1 jam setelah lahir, sedangkan bayi yang diberikan kesempatan menyusu pada < 1 jam pertama setelah lahir 26,9%. Ini jauh lebih besar dari angka nasional yang baru mencapai 3,7%. Ini dimungkinkan terjadi karena tingkat pandidikan dan sosial ekonomi masyarakat yang relatif lebih baik serta low birth weight yang sangat kecil. Rata - rata pemberian ASI Eksklusif bayi usia 6-12 di Kabupaten Kuantan Singingi 6,23 minggu (1,51 bulan). Jika merujuk pada standar yang ditetapkan Depkes RI (6 bulan), maka pemberian ASI eksklusif di Kabupaten Kuantan Singingi 4,49 bulan jauh dibawah yang diharuskan. Bayi yang inisiasi menyusu dini < 1 jam cenderung lebih lama menyusu secara eksklusif dibandingkan >1 jam (p=O,OOO). Dari pengujian hazard ralio bayi yang inisiasi menyusu dini 1 jam (HRcrude 5,17 dan HRadjusted 4,98). Hasil ini sama dengan penelitian yang dilakukan Fikawati dan Syafiq tahun 2003 menyebutkan bahwa bayi yang diberikan kesempatan untuk menyusu dini delapan kali akan lebih berhasil menyusu ASI eksklusif. Kesimpupan: Inisiasi menyusu dini merupakan faktor yang mempengaruhi kelangsungan pemberian ASI Eksklusif pada bayi 6 - 12 bulan dengan HRadjusted 4,98 (95% CI 3,74-6,64). Hanya 11,50% bayi di Kabupaten Kuantan Singingi diberikan ASI Eksklusif hingga 26 minggu (6 bulan), pemberian ASI eksklusif menurun drastis pada akhir 4 bulan.

Temporal Ambiguity can be reside, because the event is happened after the exposure. The analysis of the study is conduct through life table, log rank test, Kaplan Arfeir and Cox Proportional Hazard. Results and Discussion: EarJy innitiation of beastfeeding in Kuantan Singingi District is conduct varied with shortest time is 15 minutes after the infant is gavebirth. Most of the infant in Kuantan Singingi District (73,1%) is let to innitiate the breastfeed after 1 hour nf birth, meanwhie infant who let to have breastfeed before the first I hour of birth is 26,9%. This number is bigger than National number which reach 3.7%. This is possible because the education and socioeconomic level ofthe society is higher and the low birth weight infants is lower. The mean of exclusive breastfeeding upon the 6-12 months old baby in Kuantan Singingi Districts is 6,23 weeks (l,51 months). Referred to the standard setting by Department of health (6 months), the mean of Exclusive Breastfeeding in Kuantan Singingi is 4,49 months below the standard setting. The infant who innitiate the early breastfeed before 1 hour afterbirth is tend to have longer exclusive breasfeeding period compare with more than 1 hour after birth. Based on the Hazard test, the ratio of infant innitiating of early breastfeeding before and 1 hour of time breastfeed exclusively longer compared with more than 1 hour afterbirth. (BRerude 5.17 and HRacijusled 4.98). This result is similar with the study conducted by Pikawati and Syafiq in 2003 which showed that the infant let to have earJy breastfeed 8 times will more succeed in Exclusive breastfeed. Conclusions: Early innitiation of breastfeeding is a factor influencing a continuity of Exclusive breastfeeding on the 6-12 months old baby with HRadjusled 4,98 (95% CI 3.74-6.64). Only 11.50% infants Kuantan Singingi District is given exclusive beastfeading until 26 weeks old (6 months), Exclusive breasfeeding is decreasing dramatically in the end of 4 months.

Tuberculosis is one of the causes of morbidity and death that often occurs in children.WHO 2018 data states that there are 1.1 million TB cases in children occur each year.One of the causes of TB in children is nutritional status. Poor nutritional status can makea child's immunity vulnerable and can be affected by pulmonary tuberculosis. This studyaims to see whether there is a relationship between nutritional status and the incidenceof pulmonary tuberculosis (TB) in children aged 1-5 years in Indonesia. This research isa quantitative study with cross-sectional study design using Riskesdas 2018 data. Thesample of the study is children aged 1-5 years with a total sample of 27779. Variableconfounding, like as sex,, residence area, BCG immunization status, parental educationstatus, parental employment status old age, the existence of smokers, and the physicalcondition of the house. Bivariate analysis using Chi-Square test The results of bivariateanalysis found that there was a relationship between nutritional status and pulmonarytuberculosis of children aged 1-5 years (p <0.05) with PR 1.78 (95% CI; 1.1-2.9 ).Children who have less nutritional status are 1.78 times at risk of developing pulmonaryTB compared to children with normal nutritional status. Indonesia has a nutritional problem is characterized by the magnitude of theprevalence of malnutrition among children under five. Malnutrition in childrenfrom birth to age three years will greatly affect the growth and development ofglial cells and brain myelination process, and therefore contributes to the qualityof his brain. In Karawang district proportion malnutrition (weight / age) infantsweighing in July 2013 was approximately 0.4 % , and 35.76 % of that number ischildren aged 6-35 months. The purpose of this study was to determine therelation of exclusive breastfeeding history with severe malnutrition status(weight/age) children aged 6-35 months in Karawang district in 2013 after beingcontrolled by the variable birth weight, child 's health status, dietary intake,maternal education, knowledge mother, maternal employment status, familyincome, number of family members and liveliness visit the neighborhood healthcenter. This study was conducted in August 2013 in Karawang district using case-control design. Cases were children aged 6-35 months in Karawang measuredweight on the weighing in July 2013 and have severe nutritional status (weight /age) and controls were children aged 6-35 months in Karawang measured weightonthe weighing in July 2013 and had a good nutritional status. In this study a totalsample of 276 (138 cases and 138 controls). Data were analyzed by multiplelogistic regression. The results showed a relation of exclusive breastfeedinghistory with severe malnutrition status (weight/age) children aged 6-35 months inKarawang districtin 2013. Children aged 6-35 months who had a history ofexclusive breastfeeding risk 0.26 times (95% CI 0,12-0,55) exposed to severemalnutrition (weight/age) compared with no history of exclusive breastfeedingafter controlled by food intake, maternal knowledge, and liveliness visit theneighborhood health center. History of exclusive breastfeeding decrease the riskof severe malnutrition (weight/age) in children aged 6-35 months in Karawangdistrict in 2013 by 74%.Efforts to prevent malnutrition in infants one of which isthe exclusive breastfeeding.Necessary of increases the health promotion ofexclusive breastfeeding with good and correct way to the target group effectivelyin order to get a good nutritional status.
