Abstrak:
Tesis ini membahas tentang evaluasi kegiatan investigasi kecelakaan yang dilakukan diPT. HPU pada tahun 2018. Jenis penelitian termasuk penelitian kualitatif denganpendekatan phenomology. Tujuan dari penelitian ini dimaksudkan untuk mengetahuihasil implementasi metode investigasi dua level yang digunakan semenjak tahun 2015.Dari hasil penelitian diketahui bahwa secara teoritis, metode investigasi yang diterapkanmasih masuk ke dalam metode investigasi yang sesuai berdasarkan karakteristik sistempada industri tambang. Dalam proses investigasi insiden terlihat bahwa kegiataninvestigasi belum berjalan sesuai dengan tujuan implementasi investigasi kecelakaan dualevel. Investigator masih belum begitu sempurna dalam memahami penggunaan metodeinvestigasi level dua akibat pengaruh dari metode investigasi level pertama yang sudahsangat dikuasai. Dan pada komponen output, terlihat bahwa metode investigasi level duamampu secara lebih baik menghasilkan laporan yang dapat menggambarkan faktorpenyebab kecelakaan secara lebih lengkap dibandingkan metode level pertama. Dalampelaksanaan kegiatan investigasi, metode level kedua masih memerlukan adanya panduanyang lebih jelas bagi investigator untuk memudahkan dalam penggunaannya. Perlu jugadilakukan peningkatan pengetahuan bagi investigator untuk lebih memahami model yangmelandasi metode investigasi yang digunakan.

This thesis discusses the evaluation of accident investigation activities conducted at PT.HPU in 2018. The type of the research is qualitative research with phenomologyapproach. The purpose of this research is to describe the result of the implementation oftwo-level investigation method used since 2015 in PT. HPU, with interview anddocument observation as a research tools. From the results of the research it is knownthat based on theoretically, the investigation methods applied still suitable withappropriate investigation method characteristics of the mining industry system. In theinvestigation process, it is showing that the investigation activities have not been run inaccordance with the purpose of the implementation of two-level accident investigation.There is lack of understanding about how to apply the second-level investigation methodsby investigator due to the influence of the already highly-trained first-level investigationmethods in the past. On the output component, it is showing that the second-levelinvestigation methods are able to better generate reports that can be illustrate the accidentcausal factors more comprehensive than the first level methods. In the investigationprocess, the second level methods still requires a guidance about how the methods shouldbe implementing in the investigation activities. It is also necessary if an investigatorhaving better understanding about the underlying model as a basic model framework forthe investigation methods.

Metode yang digunakan untuk menganalisa data kecelakaanan adalah Tripod Beta. Tripod Beta memiliki beberapa kelebihan dibandingan metode yang digunakan PT. X untuk melakukan investigasi kecelakaan antara lain : adanya gambaran yang berbeda antara target, bahaya, dan kejadian, serta hambatan yang akan dijelaskan dengan penyebab langsung, kondisi awal, dan penyebab dasarnya. Selain itu dengan metode Tripod Beta ini urutan kejadian kecelakaan bisa terlihat dengan jelas. Kata Kunci: Kecelakaan, metode investigasi kecelakaan, Tripod Beta, penyebab dasar kecelakaan.

Kompleksitas dalam industri penerbangan menjadikan penyebab kecelakaan masih melibatkan faktor manusia, teknologi, dan organisasi. Sehingga, diperlukan adanya analisis yang mendalam mengenai contributing factors, underlying (root) causes, dan organizational failures untuk mengurai kompleksitas industri penerbangan serta mencegah faktor penyebab kecelakaan terulang kembali kedepannya. Penelitian ini menganalisis faktor yang berkontribusi dalam insiden kecelakaan pesawat dengan menggunakan metode Incident Cause Analysis Method (ICAM) dengan pedoman analisis yang dikhususkan untuk untuk menganalisis contributing factors, underlying (root) causes, dan organizational failures. Bentuk penelitian yang dilakukan adalah deskriptif analitis. Untuk kasus kecelakaan yang diteliti adalah Boeing 737-500 yang sudah dilakukan investigasi oleh KNKT serta data dari sumber lainnya. Dari hasil analisis menggunakan metode ICAM didapati hasil mengenai faktor yang berkontribusi terhadap kecelakaan yaitu Prosedur, Keterlambatan Sistem CTSM, Faktor Manusia, Pelatihan terkait Upset Recovery, sistem Manajemen Keselamatan pihak PT X, dan Regulasi Direktorat Jenderal Perhubungan Udara. Saran yang dapat ditarik dari analisis ICAM berada dalam area peralatan, manusia, dan manajemen atau organisasi sesuai dengan akar yang berkontribusi terhadap kecelakaan terutama pada prosedur, peralatan, pelatihan pilot, sistem manajemen keselamatan, dan kebijakan.
The complexity of the aviation industry means that the causes of accidents still involve human, technological and organizational factors. Thus, an in-depth analysis of contributing factors, underlying (root) causes, and organizational failures is needed to unravel the complexity of the aviation industry and prevent accident-causing factors from recurring in the future. This research analyzes contributing factors in aircraft accident incidents using the Incident Cause Analysis Method (ICAM) with analysis guidelines specifically for analyzing contributing factors, underlying (root) causes, and organizational failures. The form of research conducted is descriptive analytical. The accident case studied is the Boeing 737-500 accident which has been investigated by the KNKT and data from other sources. From the results of the analysis using the ICAM method, it was found that the factors that contributed to the accident were Procedures, Delays in the CTSM System, Human Factors, Training related to Upset Recovery, PT X Air's Safety Management system, and Directorate General of Civil Aviation regulations. Suggestions that can be drawn from the ICAM analysis are in the areas of equipment, people, and management or organization according to the roots that contribute to accidents, especially in procedures, equipment, pilot training, safety management systems, and policies.
