Transition of epidemiology has made a non-communicable diseases becoming the new health problems in the world. Cervical cancer is one of the problems. Globocan study in 2008 have shown that cervical cancer is the second most common cancer that leading causes more than 80% of deaths in women living in developing countries. In Indonesia there are 15.000 new cases of cervical cancer reported each year.
Cervical cancer is commonly occurs in woman. More than 1.4 million ofwomen suffer from this disease.In Indonesia the Incidence Rate of cervical canceris 13/100.000 woman. The result of early detection cervical cancer program withIVA method in Karawang revealed 246 cases from 2010-2013. In 2014 until Junethere are 56 cases. The increasing of divorced rate (200 cases/year)andprostitutions di Karawang District are the risk factor of cervical cancer. Thepurpose of this research is to know the risk factor of changed sex partner towardscervical cancer in Karawang District.A case control study design was conducted with two controls per case. Thesource of data identified from public health centre register and confirmed casesfrom hospital register of Karawang during 2014. Information collected onparticipants using pretested questionnaires, during household interviews includedage, education level, occupation, income, family cervical cancer history, age offirst sex, smoking, using hormonal contraception, parity and consumption pattern.We estimated odds ratio (OR) and confidence interval (CI 95%) usingmultivariate logistic regression.Fifty-two cases and 104 controls were enrolled. The bivariate model ofchanging sex partner towards cervical cancer showed (OR: 3.57 CI 95%: 1.67-7.6). The multivariate model included age, level education, income, familycervical cancer history, smoking active and passive, parity, age of first time sex,using hormonal contraception, and food contain carcinogen consumption. Theodds ratio adjusted (OR adj : 2.68, CI 95%: 1.03-6.9). The variable of age, level ofeducation, family cervical cancer history, active smoking and using hormonalcontraception are the confounders in relation between changing sex partner withcervical cancer.Woman with changing sex partner behaviour were more likely to havecervical cancer 2.68 times compared with women who did not changing sexpartner. District Health office of Karawang should increasing the health campaignof the risk factor of cervical cancer and preventing cervical cancer with earlydetection in public health centre.Key words: cervical cancer, changing sex partner, case control study
Cervical cancer ranks second highest at 9.2 per 100,000 population, with an average death rate of 9.0 per 100,000 population. Cilegon has not reached the designated target for IVA screenings, with percentages from 2019 to 2022 reaching 0.99%, 1.78%, 2.05%, and 2.35%, thus not achieving the national target of 80% and the health SPM target of 100%, with a target population of 71,139 individuals. The research objective is to analyze the achievement of early detection of cervical cancer at the Cilegon City Health Center from 2019 to 2022. The research design is qualitative with a case study design, conducted in April - May 2023. The research findings indicate that the program's achievement in detecting cervical cancer in Cilegon City from 2019 to 2022 has not reached the SPM target. In terms of structural components, human resources, standard operating procedures, funding, and infrastructure are already available, but there is still a slight obstacle due to the imbalance between the number of human resources and the target. In terms of process components, each health center has implemented planning, implementation, recording, reporting, and monitoring and evaluation processes according to procedures. Efforts have been made to facilitate access, actively promote awareness, employ innovative approaches, and foster cross-program cooperation, but cross-sector collaboration has not significantly improved the early detection of cervical cancer at the Cilegon City Health Center. This is due to discrepancies in perceptions of target calculation, suboptimal promotion and advocacy activities, inappropriate communication approaches, ineffective media usage, underutilization of potential resources in Cilegon as an industrial city, and a population that is either afraid of or unaware of IVA screenings.
Kanker leher rahim merupakan penyakit berbahaya pada wanita yang dapat ditangani semenjak awal dengan melakukan deteksi dini. Capaian deteksi dini kanker leher rahim di Indonesia yang rendah menyebabkan peningkatan angka kesakitan dan kematian akibat kanker leher rahim. Puskesmas Batuceper merupakan salah satu puskesmas di Kota Tangerang dengan capaian deteksi dini kanker leher rahim metode IVA terendah pada tahun 2022. Penelitian ini bertujuan untuk mengetahui determinan perilaku wanita usia 30-50 tahun dalam melakukan deteksi dini kanker leher rahim metode IVA di Puskesmas Batuceper tahun 2024 berdasarkan teori Health Belief Model (HBM). Desain penelitian menggunakan studi cross-sectional. Data dikumpulkan dengan metode wawancara menggunakan kuesioner kepada 172 wanita usia 30-50 yang dipilih dengan teknik consecutive sampling. Data dianalisis secara univariat, bivariat dan multivariat dengan uji regresi logistik ganda. Hasil penelitian menunjukkan 11,6% wanita usia 30-50 tahun telah melakukan deteksi dini metode IVA dalam 3 tahun terakhir. Faktor yang berhubungan dengan perilaku deteksi dini kanker leher rahim metode IVA adalah persepsi hambatan dan isyarat bertindak, dimana persepsi hambatan merupakan faktor yang paling dominan. Responden dengan persepsi hambatan yang rendah memiliki peluang 4,68 kali untuk melakukan deteksi dini kanker leher rahim metode IVA dibandingkan responden dengan persepsi hambatan yang tinggi setelah dikontrol oleh isyarat bertindak (aOR=4,68 95% CI 1,45-15,10). Oleh karena itu, penting dilakukan upaya untuk mengurangi hambatan dan meningkatkan isyarat bertindak dengan memperluas kegiatan pemberian edukasi dan informasi mengenai bahaya kanker leher rahim serta pentingnya pemeriksaan IVA kepada calon pengantin, suami/pasangan, orangtua siswa di sekolah serta lintas sektor. Cervical cancer is a dangerous disease in women that can be treated early by screening. The low achievement of cervical cancer screening in Indonesia has led to an increase in the incidence of pain and death from cervical cancer. The Batuceper Health Center is one of the health centers in Tangerang City with the lowest achievement of cervical cancer screening with IVA method in 2022. This study aims to determine the behavioral determinants of women aged 30-50 in conducting early detection of cervical cancer using the IVA method at the Batuceper Health Center in 2024 based on the Health Belief Model (HBM) theory. The research design uses a cross-sectional study. Data was collected by interview method using a questionnaire to 172 women aged 30-50 who were selected using consecutive sampling techniques. The data were analyzed univariately, bivariate, and multivariate with multiple logistic regression tests. The results showed that 11.6% of women aged 30-50 had successfully detected the IVA method early in the last 3 years. Factors related to the behavior of early detection of cervical cancer IVA method are the perception of obstacles and action signals, where the perception of obstacles is the most dominant factor. Respondents with a low perception of obstacles had 4.68 times the chance of cervical cancer screening with IVA method compared to respondents with a high perception of obstacles after being controlled by action signals (aOR=4.68, 95% CI 1.45-15.10). Therefore, it is important to make efforts to reduce the barriers and increase action signals by expanding education and information activities about the dangers of cervical cancer as well as the importance of IVA examination to brides-to-be, husbands/partners, and parents of students at school and across sectors. Breast (30.8%) and cervical cancer (17.2%) are leading causes of cancer deaths among Indonesian women (Globocan 2022). Despite high treatability when detected early, 70% of cases are diagnosed at advanced stages. The 2023 transformation of community health centers (Puskesmas) into Integrated Primary Services (ILP) under Ministerial Decree HK.01.07/Menkes/2015/2023 requires accurate unit cost calculations for screening services. This study analyzes actual costs of SADANIS and IVA/HPV-DNA screening at Tebet Puskesmas, an ILP pilot for Jakarta, using Activity-Based Costing (ABC) and ThinkWell's Prasana tool (April-June 2025 data). Results show unit costs of IDR 6,540 for SADANIS and IDR 66,465 for IVA/HPV-DNA per patient, with medical consumables and human resources as major cost drivers. A gap between actual and normative costs reflects differences in equipment lists and wage assumptions. Key recommendations include: adopting actual cost data for ILP budgeting; optimizing IVA screening claims under BPJS tariff schemes (MoH Regulation 3/2023); and integrating screenings into free health check-up programs to improve coverage.
