Abstrak:
Rumah Sakit Santa Elisabeth Baloi Batam merupakan suatu contoh bagaimana besarnya dampak dari Pandemi COVID-19 mampu melumpuhkan pelayanan kesehatan dengan akibat 22% tenaga keperawatannya terkonfirmasi COVID-19. Melalui instrumen penelitian yang dibuat berdasarkan Systematic Literature Review, dan desain penelitian mixed method, didapatkan bahwa kesiapan tenaga Keperawatan RS Santa Elisabeth Baloi Batam dalam menghadapi gelombang baru pandemi COVID-19 pada tahun 2022, dengan melihat pada nilai rata-rata yang ada pada 4 aspek yang didapat adalah 76.5% yang menyatakan diri siap, pada aspek internal yang berupa aspek riwayat pelatihan hanya memberikan nilai 70% yang menyatakan siap dalam menghadapi gelombang baru pandemi COVID-19. Adanya beban ganda dalam penanganan pasien-pasien non COVID-19 dan pasien COVID-19 dalam menghadapi pandemi COVID-19, membuat managamen belum mampu untuk memfokuskan lebih pada pengembangan SDM khusus terkait kesiapan COVID-19

Abstrak:
Infeksi virus SARS-Cov-2 pada pandemi Covid-19 dapat mengakibatkan ibu berhenti menyusui namun dapat pula melakukan relaktasi pascapulih. Relaktasi merupakan proses mengembalikan bayi kembali menyusu setelah sebelumnya berhenti. Menyusui bukan sekadar memberi ASI merupakan alasan penting mengapa relaktasi perlu dilakukan. Penelitian fenomenologi deskriptif ini bertujuan untuk dapat mengetahui dan mendeskripsikan pengalaman hidup (lived-experience) ibu menyusui di Jadebotabek dalam menjalani relaktasi pascaterinfeksi Covid-19. Data diperoleh melalui wawancara mendalam terhadap 15 partisipan yang kemudian dianalisis dengan metode Colaizzi. Ditemukan tujuh tema yang berkaitan dengan pengalaman relaktasi tersebut, yaitu 1) Indikasi, 2) Motivasi, 3) Strategi, 4) Perasaan ibu, 5) Dukungan, 6) Pengetahuan, dan 7) Harapan. Penelitian ini menunjukkan bahwa relaktasi di situasi pandemi Covid-19 adalah hal yang mungkin dan dapat dilakukan. Relaktasi dapat dilakukan oleh semua ibu tanpa melihat status pendidikan, status pekerjaan, usia bayi, usia ibu, status paritas, dan waktu berhentinya menyusui. Penelitian ini menyarankan perlunya panduan relaktasi yang dapat digunakan sebagai rujukan bagi ibu menyusui dan tenaga kesehatan. Diperlukan pula layanan telemedicine atau breastfeeding helpline, dan homecare melalui fasilitas kesehatan yang terjangkau baik secara akses maupun biaya kesehatan bagi ibu yang menjalani relaktasi. Abstrak:
Kemampuan rumah sakit untuk bertahan dan menjalankan fungsinya sebagai penyedia pelayanan kesehatan kepada masyarakat dalam menghadapi tantangan dan situasi pandemi Covid-19, kemampuan bertahan dalam kondisi ketidakpastian membuat rumah sakit harus membuat perencanaan kontingensi. Penelitian ini bertujuan merancang perencanaan kontingensi Rumah Sakit Grha Permata Ibu Depok dalam menghadapi Pandemi Covid-19. Penelitian ini mengunakan pendekatan kualitatif dengan design kasus melalui wawancara mendalam, telaah dokumen observasi dan decision making group. Hasil gambaran umum kesiapsiagaan RS GPI dalam menghadapi era pandemi Covid-19 menunjukkan bahwa secara umum rumah sakit memiliki tingkat kesiapsiagaan yang adekuat untuk berespon terhadap Covid-19. Komponen yang memiliki performa kurang baik adalah komponen komunikasi yang cepat dan keterlibatan masyarakat, Komponen Kesehatan kerja, kesehatan mental, dan dukungan psikososial; komponen kesinambungan layanan dukungan penting dan identifikasi dan diagnosis cepat. Rumah sakit belum memiliki program kesehatan mental karyawan yang komprehensif terutama bagi tenaga kesehatan yang menangani Covid-19. Kesiapsiagaan dan respon rumah sakit terhadap pandemi tentunya harus dapat dipertahankan, ditingkatkan, dan dievaluasi sehingga disusunlah rencana kontingensi dalam menghadapi pandemi Covid-19 untuk mempertahankan keberlangsungan bisnis operasionalnya. Rencana kontingensi ini terdiri dari pra pandemi Covid-19, pandemi Covid-19 terkendali dan pandemi Covid-19 tidak terkendali. Diperlukan penelitian lebih lanjut tentang perencanaan kontingensi secara lebih komprehensif.berdasarkan komponen kesiapsiagaan

Pandemi Covid-19 menjadi situasi yang menantang bagi tenaga kesehatan karena menempatkan mereka sebagai populasi berisiko tinggi untuk terinfeksi dan mendapatkan permasalahan terkait kondisi kerja yang berpengaruh terhadap tingkat distres. Tujuan penelitian ini ingin menganalisis faktor-faktor yang berhubungan dengan tingkat distres pada tenaga kesehatan di Kabupaten Pandeglang selama pandemi Covid-19. Penelitian ini menggunakan desain studi cross-sectional yang dilakukan pada Oktober-Desember 2021 secara daring. Populasi dalam penelitian ini adalah seluruh tenaga kesehatan di Kabupaten Pandeglang yang bekerja di Rumah Sakit, Puskesmas dan Klinik. Risiko psikososial diukur menggunakan Pandemic-Related Perceived Stress Scale of COVID-19 (PSS-10-C). Data dianalisis menggunakan Chi-squere dan regresi logistik ganda. Tenaga kesehatan memiliki tingkat distres rendah (30,49%) dan sedang (69,51%). Analisis regresi logistik ganda menunjukan bahwa faktor dominan yang berhubungan dengan tingkat distres yaitu ketersediaan APD dan dukungan rekan kerja. Tenaga kesehatan di Kabupaten Pandelang mengalami tingkat distres rendah dan sedang selama pandemi Covid-19. Pasokan APD yang memadai dan pembagian tugas yang jelas antar rekan kerja dibutuhkan untuk mencegah meningkatnya tingkat distres pada tenaga kesehatan selama pandemi Covid-19

This study aims to analyze the factors associated with the level of distress among health workers in Pandeglang during the Covid-19 pandemic. This study uses a cross-sectional study design that was conducted in October-December 2021. The population in this study were all health workers in Pandeglang who worked in hospitals, public health centers, and clinics. The distress level is measured using the Pandemic-Related Perceived Stress Scale of COVID-19 (PSS-10-C). Data are analyzed using Chi-square and multiple logistic regression. Health workers have low (30.49%) and moderate (69.51%) levels of distress. Angka kejadian kasus DBD masih tinggi setiap tahunnya meskipun jumlah kasus kematiannya cenderung menurun, namun DBD menyebabkan gejala penyakit yang lebih berat dan sulit penanganannya bila telah menjadi koinfeksi bagi virus COVID-19. Penelitian ini bertujuan untuk Mengetahui sebaran kejadian kasus DBD Kota Palembang dan menganalisa kewaspadaan sebelum dan selama masa pandemi COVID-19. Metode penelitian kualitatif desktiptif dengan teknik wawancara mendalam terhadap 3 informan, serta melakukan telaah dokumen dari instansi Direktorat Pencegahan dan Pengendalian Penyakit Tular Vektor dan Zoonosis, Dinas Kesehatan Provinsi Sumatera Selatan, Dinas Kesehatan Kota Palembang, dan Puskesmas terkait. Hasil menunjukkan Kejadian DBD di Kota Palembang sebelum masa pandemi COVID-19 pada Tahun 2018 memiliki nilai IR 39,06 per 100.000 penduduk hal ini meningkat di tahun 2019 mencapai 41,91 per 100.000 penduduk, namun angka ini tidak melebihi target kejadian DBD yaitu 49 per 100.000 penduduk. Sedangkan kejadian DBD di Kota Palembang selama masa pandemi COVID-19 pada tahun 2020 mencapai 26,07 per 100.000 penduduk, angka ini tidak melebihi target kejadian DBD yaitu 49 per 100.000 penduduk. Pada tahun 2020 semua kecamatan di kota Palembang tidak ada yang melebihi target kejadian DBD, hal ini menunjukkan bahwa selama masa pandemi COVID-19 terjadinya penurunan kasus pada setiap kecamatan di Kota Palembang. Tingkat kewaspadaan pada tahun 2018-2019 dalam kategori baik sekali, sedangkan tahun 2020 dalam kategori cukup. Berdasarkan Analisa terhadap kejadian DBD kota Palembang dapat disimpulkan bahwa terjadinya penurunan kasus DBD selama masa pandemi COVID-19 dibandingkan sebelum masa pandemi COVID-19, karena pada masa pandemi COVID-19 adanya peraturan yang ketat yaitu PSBB, dan kewaspadaan petugas terhadap penyakit DBD menjadi teralihkan karena lebih mementingkan penurunan kasus COVID-19

Pada awal bencana pandemi COVID-19 menyebabkan banyak Rumah Sakit tidak siap, terutama saat terjadi lonjakan kapasitas. Adanya kebijakan Pemantauan dan Evaluasi Kesiapan Rumah Sakit dalam menghadapi pandemi dan penetapan sebagai RS Rujukan COVID-19 membuat RSUD Kabupaten Bekasi harus mengimplementasikan kebijakan tersebut. Tujuan penelitian ini adalah untuk mengetahui sejauh mana kesiapan RSUD Kabupaten Bekasi dalam menghadapi pandemi COVID-19, faktor-faktor apa yang berperan dalam keberhasilan implementasi kebijakan, bagaimana peran pimpinan dan pelaksananya, bagaimana kebijakan dan strategi yang dilaksanakan. Metode: penelitian ini adalah desain deskriptif analitik dengan pendekatan kualitatif melalui wawancara mendalam dan validitas dengan Triangulasi sumber melalui observasi, telaah dokumen dan cross-check dengan penilaian terhadap 12 komponen kunci menggunakan checklist Monev Kesiapan RS yang diadaptasi dari WHO. Penelitian dilakukan pada Februari-Mei 2022. Hasil: penelitian ini adalah RSUD Kabupaten Bekasi mendapatkan hasil skor 96% pada saat penilaian kondisi triwulan 4 Tahun 2021. Faktor-faktor yang mempengaruhi keberhasilan implementasi kebijakan adalah Faktor Lingkungan, Sumber Daya, Hubungan antar organisasi terkait, serta Karakteristik dan kapabilitas pimpinan dan pelaksana. Kesimpulan penelitian bahwa RSUD Kabupaten Bekasi siap dalam menghadapi pandemi COVID-19. Faktor yang mempengaruhi keberhasilan implementasi kebijakan adalah Lingkungan, Sumber Daya, Hubungan antar organisasi, serta karakteristik dan kapabilitas para pelaksana. COVID-19 mengganggu layanan imunisasi, hal ini memungkinkan jutaan anak berpotensi terjangkit wabah penyakit seperti difteri, campak, polio dan komplikasinya. Berdasarkan teori Health Belief Model (HBM) dimana persepsi berpengaruh terhadap respon, pandemi COVID-19 diasumsikan dapat mempengaruhi persepsi orang tua dan pola pencarian imunisasi dasar. Penelitian ini menggunakan analisis kualitatif untuk melihat kaitan antara persepsi dan pola pencarian imunisasi dasar oleh orang tua saat pandemi COVID-19 di Kelurahan Bojong Menteng dan Kelurahan Margajaya Kota Bekasi. Hasil penelitian ini menunjukkan status ekonomi saat pandemi melatarbelakangi penundaan imunisasi anak, walau bukan menjadi faktor tunggal. Dukungan kader dan petugas kesehatan terhadap imunisasi berperan dalam keputusan untuk mengimunisasi anaknya. Penerapan strategi yang disesuaikan dengan karakter sosial budaya di tiap wilayah menjadi sangat penting. Saran: meningkatkan pemahaman masyarakat mengenai COVID-19 dan informasi mengenai sistem layanan kesehatan yang aman saat situasi kedaruratan khusus seperti pandemi perlu disampaikan secara utuh untuk memberikan kepercayaan layanan imunisasi yang aman.
COVID-19 has hit the world, has been declared an emergency status for the outbreak in Indonesia. COVID-19 disrupts immunization services, this allows millions of children to potentially contract outbreaks such as diphtheria, measles, polio and its complications. The theory of the Health Belief Model (HBM) about perception can influences response, the COVID-19 pandemic is assumed to be able to influence parents' perceptions and basic immunization search patterns. This study used a qualitative analysis to see the pattern of basic immunization search for parents during the COVID-19 pandemic in Bojong Menteng and Margajaya, Kota Bekasi. The results of this study indicate that the economic status during the pandemic is behind the delay in immunization of children, although it is not a single factor. COVID-19 has hit the world, has been declared an emergency status for the outbreak in Indonesia. COVID-19 disrupts immunization services, this allows millions of children to potentially contract outbreaks such as diphtheria, measles, polio and its complications. The theory of the Health Belief Model (HBM) about perception can influences response, the COVID-19 pandemic is assumed to be able to influence parents' perceptions and basic immunization search patterns. This study used a qualitative analysis to see the pattern of basic immunization search for parents during the COVID-19 pandemic in Bojong Menteng and Margajaya, Kota Bekasi. The results of this study indicate that the economic status during the pandemic is behind the delay in immunization of children, although it is not a single factor. Abstrak:
Kemampuan rumah sakit untuk bertahan dan menjalankan fungsinya sebagai penyedia layanan kesehatan kepada masyarakat menghadapi tantangan dalam situasi darurat dan bencana. Rumah sakit harus mampu menghadapi pandemi COVID-19 dan bertahan sebagai salah satu bagian sentral dari ekosistem kesehatan. Penelitian ini dilakukan untuk menganalisis respon Rumah Sakit Awal Bros Batam terhadap pandemi COVID-19. Penelitian ini menggunakan pendekatan kualitatif dengan desain studi kasus melalui wawancara mendalam, telaah dokumen, observasi dan focus group discussion (FGD). Hasil penelitian menunjukkan bahwa secara umum rumah sakit dinilai memiliki tingkat kesiapsiagaan yang adekuat untuk berespon terhadap COVID-19. Komponen yang memiliki performa kurang baik adalah Komponen Kesehatan kerja, kesehatan mental, dan dukungan psikososial; Komponen Manajemen Pasien, dan Komponen Surge Capacity. Rumah sakit belum memiliki program kesehatan mental karyawan yang komprehensif terutama bagi tenaga kesehatan yang menangani COVID-19. Penggunaan terapi baru yang belum terdaftar juga belum dilakukan pemantauan dan kajian dilema etik dengan mengembangkan protokol pemantauan terapi. Selain itu, rumah sakit juga belum melakukan penetapan jumlah optimal sumber daya yang dibutuhkan untuk menghadapi kemungkinan kapasitas lonjakan di masa mendatang. Kesiapsiagaan dan respon rumah sakit terhadap pandemi tentunya harus dapat dipertahankan, ditingkatkan, dan dievaluasi sehingga disusunlah strategi mitigasi risiko prioritas yang menitikberatkan pada subkomponen yang memiliki nilai Risk Priority Number (RPN) paling tinggi. Selain strategi mitigasi risiko, telah disusun pula serangkaian Key Performance Outcome Indicator yang akan digunakan untuk melakukan pengukuran dan pemantauan keberhasilan rumah sakit dalam bersiapsiaga, berespon terhadap pandemi COVID-19 dan mempertahankan keberlangsungan bisnis operasionalnya. Diperlukan penelitian lebih lanjut tentang status kesehatan mental tenaga kesehatan yang bekerja di rumah sakit pada masa pandemi COVID-19 serta analisis hubungannya dengan status kesiapsiagaan rumah sakit serta melakukan evaluasi dari pemantauan penggunaan obat yang tidak terdaftar dan dampaknya terhadap outcome pasien COVID-19
Hospital ability to survive and maintain its function as a health service provider to the community faces challenges in emergency and disaster situations. Hospitals must be able to deal with the COVID-19 pandemic and survive as a central part of the health ecosystem. This research was conducted to analyze Awal Bros Batam Hospital responses to the COVID-19 pandemic. This study used a qualitative approach with a case study design through in-depth interviews, document review, observation, and focus group discussion (FGD). The results showed that in general, hospitals were considered to have an adequate level of preparedness to respond to COVID-19. Underperforming components are the components of Occupational Health, Mental Health, And Psychosocial Support; Patient Management Components, and Surge Capacity Components. The hospital have not develop a comprehensive employee mental health program, especially for health workers who handle COVID-19 patients. The use of new unregistered has also not been adequately monitored and studied ethical dilemmas by developing therapy monitoring protocols. Besides, the hospital has not determined the optimal amount of human resources needed to deal with possible future capacity spikes. Hospital preparedness and response to pandemics must of course be maintained, improved, and evaluated so that a priority risk mitigation strategy is formulated that focuses on the subcomponent that has the highest Risk Priority Number (RPN) value. In addition to risk mitigation strategies, a series of Key Performance Outcome Indicators have also been prepared which will be used to measure and monitor the success of hospitals in preparing, responding to the COVID-19 pandemic, and maintaining the sustainability of its operational business. Further research is needed on the mental health status of health workers working in hospitals during the COVID-19 pandemic and its relationship with hospital preparedness status, also research to evaluate the unregistered drug use monitoring and its impact on COVID-19 patient outcomes
