The efforts of care for people with HIV/AIDS implemented in Hospitals MawarClinic dr. Abdul Aziz, Singkawang, West Kalimantan province since 2004 untilnow. Mawar clinic is one of the purposes of referral for patients with HIV/AIDSwho came from the District of Sambas and Bengkayang which was the borderregion between Indonesia with Malaysia.Design research with cross sectional was held to analyze the mobile vct servicesrelationship with adherence/compliance people with HIV/AIDS in the treatmentof Mawar ART at the clinic since 2004 until February 2016. Of the entire peoplewith HIV/AIDS are in clinical on register the Mawar note that only 55,9% aredoing the treatment of ART while the complience or adhere to ART amounting to24,5%. For a univariate analysis was carried out, the analysis of missing data, themultivariate analysis bivariat and to know the description ofadherence/compliance people with HIV/AIDS in the treatment of ART.Multivariate analysis using logistic regression test showed that the static VCTservices was a protective factor for mobile VCT services for people with HIV /AIDS to adhere to antiretroviral treatment is influenced by education, place ofresidence , ethnicity and duration of ART .Keywords: HIV / AIDS, ART, adherence, vct.
HIV is a cause of disease infection that will be suffered a lifetime. Not all people with HIV have the the same timeframe in the showing symptoms clinicayl, so that the transmission may still occur during the patients in the period of asymptomatic. This research was aimed to determine the factors associated with HIV status VCT clients (Voluntary Counseling and Testing) at RSUD Mandau Bengkalis In 2012. This study is a further analysis of the medical records of HIV VCT clinic at 897 people with HIV VCT clients in RSUD Mandau. The data was collected through observation VCT form using the checklist sheet. Results of this study get 4.2% of VCT clients infected with HIV and work related with HIV status, where clients who work associated with risk factors nearly 16 times for HIV infection than clients who work not associated with risk factors. Need more concern, prevention and suppression of all parties, including government, health workers, and society.
The progression of Human Immunodeficiency Virus (HIV) infection in theworld is very progressive. Since found in 1981 until 2016 the number of cases hasreached tens of millions of lives. The number of new HIV infections in CirebonRegency has the same as the condition of the world. In 2017 the number of new casesincreased by 50% compared to 2009. The spread of HIV infection is still concentratedin the key population with the main transmission pattern through unsafe sex. Primaryprevention undertaken are early detection of a HIV status and counselling of riskfactors through Voluntary Counseling and Testing (VCT) activities. This study aims todetermine the magnitude of the association between risky sexual behavior with HIVinfection on VCT Clients in Cirebon Regency.This was cross sectional study using secondary data of VCT in 2017. Thepopulation is clients who visit VCT clinic, doing pre-test counselling, HIV test andpost-test counselling. Clients are key populations (gay / MSM, customer sex workers,IDUs, sex workers, transgender, and prisoners), TB patients and legaly sex partner.Logistic regression analysis was used to estimate association between risky sexbehavior and HIV infection after controlled covariate variables.Nearly 3.0%. (85/2,858) of tested clients were positif HIV and 80.4%(2,299/2.858) client had risky sexual behavior. There was a significant associationbetween risky sex behavior and HIV infection on VCT clients in Cirebon Regency(Adjusted POR=2.23 (1.019-4.899) after controlling to gender.The proportion of risky sex behaviors in VCT clients is very high, VCT clientswho engage in sex-risk behaviors had a risk of 2.23 times for HIV infection comparedto VCT clients who do not engage in risky sexual behavior. It is recommended to theCirebon Health Office to improve promotive and preventive programs to enhancingcommunity knowledge and skills in preventing HIV infection, conducting outreachtraining in terms of regulatory and advocacy techniques, increasing the frequency ofVCT and improving the duration and quality of counselling in VCT.Key words:HIV, VCT, Risky sexual behavior, Cirebon Regency.
ABSTRAK
Voluntary Counseling and Testing (VCT) merupakan pintu masuk ke seluruh layanan HIV AIDS. Target VCT di Rutan Klas IIA Jakarta Timur 38%. Untuk peningkatan utilisasi VCT, disahkanlah Peraturan MenkumHAM RI berupa Rencana Aksi Nasional Penanggulangan HIV AIDS. Tujuan penelitian mengetahui faktor-faktor dalam kebijakan VCT, yaitu proses tahapan, SDM, dan sarana-prasarana, desain studi kuantitatif dan semi kualitatif dengan responden WBP, dan informan birokrat/petugas klinik. Hasil penelitian (106 orang) diketahui proses tahapan dan SDM berhubungan signifikan, sedangkan sarana prasarana tidak. Terjadi kesenjangan antara target dengan capaian utilisasi VCT . Disarankan melibatkan rutan dalam pembuatan kebijakan, perubahan alur, serta pembenahan SDM dan sarana prasarana.
ABSTRACT
Voluntary Counseling and Testing (VCT) is a key point to enter comprehensive HIV AIDS service. Target of VCT utilization at East Jakarta Prison Class IIA is 38%. To increase VCT utilization, Indonesian Ministry of Law and Human Rights approved policy which is called National Planned Action Countermeasures of HIV AIDS. The aim is to know factors of VCT policy, such as process, human resources, and infrasturctures, by used quantitative and semi-qualitative method and used WBP as respondents, policy makers and clinic officers as informants. The study (from 106 respondents) revealed that VCT utilization associated significantly with process and human resources, but not assosiated significantly with infrasturctures. There is gap between target and attainment of VCT utilization. It is recommended to involve prison in making policy, changing step, and improving human resources and infrasturctures.
HIV/AIDS has became a major health problem among Jakarta’s population. To cope with this problem, many VCT clinics has been established in Jakarta, one of which is Gambir VCT Clinic. This research was conducted using cross-sectional method by using entries from Gambir VCT clinic’s forms between January-December 2011. There was 95 clients from the entries, from which, 20 clients got HIV positive status. There is no significant association between HIV test result and age, education, work status, client status, reason for testing, and history of testing. The factors associated with HIV test result are: male(PR= 4,6, 95% CI= 1,67-12,84), has not married(PR= 3, 95% CI=1,39-6,62), IDUs(PR= 5,9, 95% CI= 2,51-13,81), sharing needles in past 6 months PR= 2,8, 95% CI= 1,31-6,13), got the information of testing from field officers(PR= 2,5, 95% CI= 1,18-5,27), having unsafe sex in past 6 months(PR= 0,22, 95% CI=0,1-0,49), and got the information of testing from doctors/midwives(PR= 0,39, 95% CI= 0,18-0,85). From those results, it is recommended to improve the recording and counseling system in the VCT clinic and also the health promotion regarding HIV/AIDS in Jakarta, it is also necessary to consider doing an advanced research with better design and more valid analysis to eliminate the probability of bias and confounding.
