Stroke is one of the most prevalent diseases in Indonesia, particularly in Central Java. In 2023, BPJS Kesehatan allocated a budget of Rp5.2 trillion to cover 3.4 million stroke cases, with an average cost of Rp1.53 billion per case. This study aims to analyse the total stroke claim costs for JKN participants at FKRTL in Central Java Province in 2023, as well as the factors influencing them. The study employs a quantitative method using secondary data from the BPJS Health 2024 Sample Data and a cross-sectional study design. The study provides an overview of the relationship between total stroke claim costs through univariate and bivariate analyses using the Mann Whitney and Kruskal-Wallis tests. The results show that the total stroke claim costs for JKN participants at FKRTL in Central Java Province in 2023 were IDR 5,592,314,702 (5.6 billion), with the breakdown of total stroke claim costs for advanced outpatient services amounting to IDR 95,315, 500 million (95.3 million) and advanced inpatient care at Rp5,496,999,202 (5.5 billion), with significantly associated factors including age, residential area, participant segmentation, class of care entitlement, ownership of FKRTL, type of stroke, severity of the case, length of hospital stay, and type of FKRTL service. This study concluded that The PROLANIS programme and increased monitoring of claims are necessary to improve the health and quality of life of JKN participants and to ensure cost efficiency and accuracy.
