The focus of this research is to analyze all occupational accidents of lifting activities on land rig operations in PT ‘X’ using the Human Factor Analysis and Classifications System (HFACS) method in 2014 - 2018. The type of research methodology is qualitative research with a descriptive design. The final result shows that the unsafe act layer is the most ineffective layer that contributing to almost all occupational accident cases which is 45 of 49 total cases of occupational accidents. Error is the sub-layer of unsafe act which has the highest number of contributions to occupational accident cases with total 39 cases. On the other side, the organizational influences layer is the second layer that has high contribution to accident which is 26 of 49 total cases of occupational accidents. The organizational process is the sub-layer of organizational influences which contributing to 23 cases of occupational accident. The third layer which has contribution to accident is unsafe supervision. The unsafe supervision has contribution to accident which is 16 of 49 total cases of occupational accidents. Inadequate supervision and planned inappropriate operation are the sub-layer of inadequate supervision which contribute to the accident cases for 10 cases equally. The layer of preconditions for unsafe actions is the effective layer which has contribution to occupational accident cases which is 8 of 49 total cases of occupational accidents. Personnel factor is the sub-layer of preconditions for unsafe actions which contribute to 7 cases of occupational accidents. According to the result, researcher recommend that corrective action must be taken at each layer of HFACS as the safety protection system, both latent failures and active failures with the emphasis on improvement, which start from the organizational influences layer, followed by the unsafe supervisions layer, and then unsafe actions layer, while the improvement on the layer of precondition for unsafe actions becomes the last improvement. Improvement to organizational influences layer, unsafe act layer, and unsafe supervisions layer will have a positive influence on the layer of precondition for unsafe actions.
ABSTRAK Infeksi virus dengue masih merupakan masalah kesehatan di Indonesia saat ini termasuk di Kota Banjarmasin dengan angka kematian yang tinggi, Tahun 2011 dilaporkan CFR 8,3% dimana sebagian besar pasien DBD ini dirawat di RSUD ULIN dan RSUD Ansari Saleh Banjarmasin, sementara penegakkan diagnosis sering sulit, apalagi dalam menilai apakah pada akhirnya akan terjadi shock (Dengue Shock syndrome) atau tidak. Peningkatan hematokrit, penurunan angka trombosit, leukosit dan serta perilaku pasien sebelum dirawat (lamanya sakit, rujukan) biasanya terjadi sebelum demam turun dan sebelum terjadinya shock. Hal ini merupakan diagnostik yang penting dan prognosis yang berharga dalam mendeteksi Dengue Shock Syndrome. Sehingga dengan mengetahui faktor resiko ini dapat mencegah/ mengurangi kematian Metode: Penelitian bersifat observasional dengan disain kasus kontrol. Kasus adalah penderita yang didiagnosis DSS berdasarkan diagnosis dokter yang merawat. Sedangkan kontrol adalah penderita yang didiagnosis sebagai tersangka DBD oleh dokter yang merawat. Data penelitian diperoleh dari data rekam medis dan formulir Kewaspadaan Dini Rumah Sakit (KD-RS) yang dirawat di RSUD ULIN dan RSUD Ansari Saleh dalam periode bulan April 2010 sampai Maret 2012. Rancangan analisis ditujukan untuk memperoleh nilai Odds Ratio (OR) dilanjutkan dengan multivariat analisis untuk mengetahui faktor risiko yang dapat mendeteksi DSS sejak dini. Hasil Penelitian: Variabel yang signifikan secara statistik dan di masukkan ke dalam prediksi model akhir adalah Jenis Kelamin perempuan (OR=3,250 95% CI=1,178-8,970), hematokrit ≥25,97% (OR=7,86 95% CI=2,748-22,500) , leukosit ≤ 4764,47 (OR=3,826 95% CI=1,375-10,647), lama sakit ≥4 hari (OR=3,146 95% CI=1,179-8,397) dan rujukan dari puskesmas (OR=4,543 95% CI=1,700-12,139).Variabel yang paling dominan yang berhubungan dengan kejadian Dengue shock syndrome adalah hematokrit. Dari hasil tersebut disarankan agar tenaga kesehatan dan akademisi perlu meningkatkan standar pelayanan penyakit yang lebih efektif dan efesien yang berisiko terjadinya Dengue Shock Syndrome.
ABSTRACT DHF is still a health problem in Indonesia is currently included in Banjarmasin city with a high mortality rate in 2011 was reported CFR 8.3% where the majority of dengue patients are treated at the Ulin Hospital and Ansari Saleh Hospital Banjarmasin, while the diagnosis is often difficult, especially in assessing whether it will eventually happen shock (dengue shock syndrome) or not. This is an important diagnostic and prognostic value in the detection of Dengue Shock Syndrome. So that by knowing these risk factors can prevent / reduce mortality. Methods: The study is an observational with case-control design. Cases are those who hospitalized and diagnosed as suspect Dengue haemorrhagic fever by clinicans using WHO criteria.Controls are those who hospitalized and diagnosed as suspect Dengue Haemorrhagic fever by the clinicans. Data were collected from medical records and (KD-RS) are treated in Ulin Hospital and Ansari Saleh Hospital in the period from April 2010 until March 2012. Analysis design is done to obtain Odds Ratio (OR) and followed by using multivariate logistic regression to determine risk factors that can detect early DSS. Consclusion: The significant variables in statistic manner and put into the final model predictions are increasing Female sex (OR=3,250 95% CI=1,178-8,970), haematocryt ≥25,97% (OR=7,86 95% CI=2,748-22,500) leukopenia ≤4764,47 (OR=3,826 95% CI=1,375-10,647), lengh of hospital ≥4 days (OR=3,146 95% CI=1,179-8,397) and referrals from Health centers (OR=4,543 95% CI=1,700- 12,139). From these results it is suggested that health professionals and academics need to improve service standards diseases more effectively and efficiently at risk of Dengue Shock Syndrome.
Latar Belakang : Gonore adalah penyakit menular seksual yang disebabkan oleh Neisseria gonorhoeae yang menginfeksi lapisan dalam uretra, leher rahim dan rectum. Pada wanita Gonore bisa naik kesaluran kelamin menginfeksi selaput di dalam panggul sehingga 40% timbul nyeri panggul dan dan gangguan reproduksi serta menjadi faktor risiko penting dalam transmisi HIV ( Kemkes RI ). Di Indonesia gonore merupakan jenis IMS yang tertinggi berdasarkan data yang ada pada WPS. Prevalensi Gonore pada WPS 49% (IBBS 2007), Gonore merupakan indikasi biologis tentang perilaku seks berisiko dan banyak terjadi di industri seks komersial Tujuan penelitian ini untuk mengetahui faktor yang berhubungan dengan kejadian infeksi gonore pada WPS yaitu umur, pendidikan, lama menjadi WPS, Masa kerja, Umur pertama hubungan sek, pemakaian kondom dan status HIV. Metode : Desain penelitian ini adalah potong Lintang. Data diperoleh dari Survey Terpadu Biologis dan Perilaku (STBP) tahun 2011. Data dianalisis secara bivariat untuk mendapatkan nilai Crude Odds Ratio dari faktor yang berhubungan dengan infeksi gonore, serta analisis multivariate untuk melihat variabel paling dominan kejadian infeksi gonore dilihat dari nilai Adjusted Odds Ratio. Hasil : faktor yang berhubungan dengan infeksi gonore. umur OR (0,95%CI); 1.41 (1.98 ? 1,65), jumlah pelanggan OR (0,95%CI);1.29 (1.09 ? 1.51) , lama menjadi WPS OR (0,95%CI); 136 (1.15 ? 1.59) dan status HIV OR (0,95%CI); 1.72 ( 1.33 - 2.22) merupakan faktor yang paling dominan berhubungan dengan kejadian infeksi gonore.
Background : Gonorrhea is a sexually transmitted disease caused by Neisseria gonorhoeae that infects the inner lining of the urethra, cervix and rectum. In women gonorrhea can infect the genital tract up to the membrane in the pelvis so that the 40% and pelvic pain and reproductive problems as well as being an important risk factor in HIV transmission (Kemkes RI). In Indonesia, gonorrhea is the highest type of STI prevalence data based on 49% of gonorrhea in the FSW (IBBS 2007), Gonorrhea is considered as an indication of biological resources in risky sexual behavior and still a lot happening in the commercial sex industry The aim of this study to determine factors associated with the incidence of gonorrhea infection in the FSW: age, education, length of work FSW, number of client, age at sex debut, condom use, HIV status. Methods : The study design is cross sectional. Data was obtained from Integrated Biological and Behaviour Survey (IBBS) in 2011. Data in the bivariate analysis to obtain the value of Crude Odds Ratio of factors associated with gonorrhea infection, and multivariate analysis to see the most dominant variable incidence of gonorrhea infection in view of the Adjusted Odds Ratio. Results : The factors related with gonorrhea infection. age OR1.41 0.95% CI 1.98 - 1.65), number of clients OR 1.28 95%CI 1.09 ? 1.51), length of work FSW OR 1:26 0.95% CI 1.15 to 1.59 and HIV status OR 1.72 0.95% CI 1:33 to 2:22 is the most dominant factor related with the incidence of gonorrhea infection.
