Program Jaminan Kesehatan Nasional (JKN) menempatkan fasilitas kesehatan tingkat pertama (FKTP) sebagai garda terdepan dalam pelayanan kesehatan melalui sistem rujukan berjenjang. Namun, dalam implementasinya, masih ditemukan tingginya angka rujukan dari FKTP ke fasilitas kesehatan rujukan tingkat lanjut (FKRTL). Penelitian ini bertujuan untuk membandingkan karakteristik rujukan peserta JKN pada dua Puskesmas di Jakarta Selatan, yaitu Puskesmas dengan angka rujukan tertinggi (Jagakarsa) dan terendah (Setiabudi) sepanjang tahun 2024. Penelitian ini dilakukan dengan wawancara mendalam, observasi, dan telaah dokumen. Hasil penelitian menunjukkan bahwa perbedaan angka rujukan dipengaruhi oleh beberapa faktor seperti jumlah dan rate ketersediaan dokter, kecukupan fasilitas penunjang, ketersediaan laboratorium klinik, serta ketersediaan obat-obatan. Meskipun kedua Puskesmas telah memiliki fasilitas penunjang yang sesuai dengan Permenkes No. 43 Tahun 2019, keterbatasan SDM, beban kerja, dan diagnosa medis pasien turut mendorong terjadinya rujukan yang sebenarnya dapat dihindari. Penelitian ini menyarankan perlunya evaluasi berkala terhadap rate dokter dan manajemen fasilitas, serta penguatan tata kelola layanan dan distribusi tenaga kesehatan agar sistem rujukan berjalan lebih efisien dan tepat sasaran.
The National Health Insurance (JKN) program places primary healthcare facilities (FKTP) at the forefront of health services through a tiered referral system. However, in its implementation, there is still a high rate of referrals from FKTP to advanced referral health facilities (FKRTL). This study aims to compare the referral characteristics of JKN participants at two Puskesmas in South Jakarta, namely Puskesmas with the highest (Jagakarsa) and lowest (Setiabudi) referral rates throughout 2024. This research was conducted using in-depth interviews, observation, and document review. The results showed that the difference in referral rates was influenced by several factors such as the number and ratio of doctor availability, adequacy of supporting facilities, availability of clinical laboratories, and availability of medicines. Although both Puskesmas have supporting facilities in accordance with Permenkes No. 43 of 2019, limited human resources, workload, and patient medical diagnoses also encourage referrals that can actually be avoided. This study suggests the need for periodic evaluation of doctor ratios and facility management, as well as strengthening service governance and health worker distribution to make the referral system run more efficiently and on target.

Klinik pratama sebagai salah satu Provider Pelayanan Kesehatan (PPK) BPJSKesehatan dari sektor swasta berperan penting dalam memberikan pelayanankesehatan terhadap peserta Jaminan Kesehatan Nasional (JKN). Namun, partisipasiklinik pratama dalam menjadi Fasilitas Kesehatan Tingkat Pertama (FKTP) BPJSKesehatan di beberapa wilayah masih rendah apabila dilihat berdasarkan rasiodokter/peserta. Hal tersebut menyebabkan terjadinya penumpukan peserta JKN padaPuskesmas. Penilitian kualitatif ini bertujuan untuk mengetahui variabel yangmempengaruhi keputusan klinik pratama dalam menjadi FKTP BPJS Kesehatan.
