Abstrak:
Fenomena Microsleep pada operator Dump Truck di industri pertambangan merupakan salah satu tantangan keselamatan kerja krusial yang dipicu oleh karakteristik pekerjaan monoton, durasi kerja panjang, dan sistem kerja shift. Pengawasan manual dan pendekatan administratif konvensional memiliki keterbatasan dalam mendeteksi penurunan kewaspadaan secara Real-Time. Penelitian ini bertujuan untuk menganalisis angka kejadian Microsleep sebelum dan setelah penerapan sistem Defender berbasis AI serta mengevaluasi tingkat efektivitas teknologi tersebut dalam menurunkan angka kejadian Microsleep operator Dump Truck di PT X. Metodologi yang digunakan adalah metode campuran sekuensial eksplanatori (Mixed Methods Sequential Explanatory Design). Data kuantitatif diperoleh dari kuesioner berskala Likert yang disebarkan kepada 201 operator Dump Truck (HD) dan dianalisis menggunakan program SPSS, serta dilengkapi data sekunder laporan Fatigue Incident Record periode 2020–2025. Data kualitatif dieksplorasi melalui wawancara mendalam bersama 8 pengawas lapangan dan 5 personel Safety, Health, and Environment (SHE). Hasil penelitian menunjukkan bahwa sebelum implementasi sistem Defender (periode 2020–2023), angka kejadian Microsleep tergolong sangat tinggi dengan rata-rata 1.262 kejadian per tahun. Setelah penerapan sistem Defender berbasis AI, jumlah kejadian Microsleep turun drastis menjadi 415 kejadian pada tahun 2024 dan mencapai titik terendah sebesar 207 kejadian pada tahun 2025 (rata-rata 311 kejadian). Hasil analisis data sekunder membuktikan tingkat efektivitas sistem sebesar 75,36% dalam menekan kejadian Microsleep. Diskusi menekankan bahwa alarm visual dan audio Real-Time dari Defender secara signifikan mempercepat deteksi dini dan memicu respons pengawasan proaktif, meskipun tantangan teknis seperti False Alarm akibat debu atau APD masih ditemukan. Dengan demikian, penelitian ini menyimpulkan bahwa penerapan teknologi monitoring fatigue Defender berbasis AI terbukti sangat efektif menurunkan angka kejadian Microsleep operator Dump Truck di PT X dan memperkuat implementasi Fatigue Risk Management System (FRMS).

The phenomenon of Microsleep among Dump Truck operators in the mining sector presents a critical occupational safety challenge, primarily driven by monotonous tasks, extended working hours, and shift work schedules. Conventional manual supervision and administrative controls have proved inadequate for detecting alertness degradation in Real-Time. This study aims to analyze the Microsleep incidence rates before and after the implementation of the AI-based Defender system and to evaluate the effectiveness of this technology in reducing Microsleep among Dump Truck operators at PT X. The methodology employed a Mixed Methods Sequential Explanatory Design. Quantitative data were gathered through Likert-scale questionnaires distributed to 201 Dump Truck (HD) operators, analyzed via SPSS software, and supplemented by secondary corporate Fatigue Incident Records from 2020 to 2025. Qualitative insights were extracted from in-depth interviews involving 8 field supervisors and 5 Safety, Health, and Environment (SHE) personnel. The findings indicate that prior to the system's integration (2020–2023), the frequency of Microsleep was notably high, averaging 1,262 incidents per year. Following the deployment of the AI Defender system, Microsleep occurrences fell sharply to 415 in 2024 and reached an all-time low of 207 incidents in 2025 (averaging 311 events). Secondary data analytics revealed a 75.36% effectiveness rate in mitigating Microsleep occurrences. The discussion highlights that Defender’s Real-Time audio-visual alerts substantially expedited early detection and triggered proactive supervisory interventions, despite enduring technical constraints like dust-induced or PPE-induced False Alarms. In conclusion, this study demonstrates that the implementation of the AI-powered Defender fatigue monitoring system is highly effective, reducing Microsleep incidence among Dump Truck operators at PT X by 75.36% and bolstering the corporate Fatigue Risk Management System (FRMS).

Abstrak:

Kecelakaan tambang di Indonesia menunjukkan beban yang perlu dikendalikan secara sistemik, melalui efektivitas pengukuran awal Safety Maturity Level (SML). Penelitian ini bertujuan menganalisis hubungan antara SML dan kinerja keselamatan pertambangan yaitu indikator kecelakaan (Frequency Rate dan Severity Rate), sekaligus menjelaskan faktor konteks yang memengaruhi kekuatan hubungan tersebut. Penelitian menggunakan desain mixed methods - explanatory sequential. Fase kuantitatif memanfaatkan data sekunder dari Kementerian ESDM (KESDM) tahun 2024 di 52 perusahaan tambang batubara tahap operasi produksi sesuai kriteria inklusi, yaitu skor SML berbasis KepDirjen Minerba nomor 10.K/MB.01/DJB.T/2023 dan data statistik kecelakaan tambang 2024. Uji normalitas data menunjukkan tidak terdistribusi normal, sehingga analisis menggunakan pendekatan nonparametrik Spearman rho (ρ) dan GLM gamma. Hasil kuantitatif mengindikasikan bahwa SML secara total (agregat) berkorelasi negatif dan signifikan dengan kinerja keselamatan (indeks kecelakaan tambang), namun kontribusi tiap dimensi SML tidak seragam. “Upaya Pengendalian” merupakan indikator yang paling konsisten bersifat protektif terhadap indeks kecelakaan, sedangkan “Partisipasi Pekerja” tidak menunjukkan hubungan yang signifikan namun korelasi negatif; sementara itu, “Tanggung Jawab Pimpinan” dan “Analisis Statistik”
menunjukkan hubungan negatif (bivariat) dan positif (multivariat), serta signifikan terhadap indeks kecelakaan tambang. Selanjutnya, temuan kualitatif dari data sekunder KESDM terhadap sub-sampling perusahaan menegaskan bahwa SML tinggi tidak selalu diikuti penurunan kecelakaan ketika terdapat kesenjangan implementasi pengendalian, terutama pada tata kelola kontraktor, kompetensi, dan konsistensi eksekusi di lapangan. Penelitian menyimpulkan bahwa SML berdampak pada penurunan kecelakaan terutama jika diwujudkan dalam pengendalian risiko yang nyata dan pembelajaran berbasis data yang ditutup dengan tindakan (closed-loop learning), bukan sekadar pemenuhan administratif.


Mining accidents in Indonesia indicate a burden that must be controlled systemically through the effectiveness of early measurement of the Safety Maturity Level (SML). This study aims to analysis the relationship between SML and mining safety performance represented by accident indicators (Frequency Rate and Severity Rate) while also explaining contextual factors that influence the strength of this relationship. The research employs a mixed-methods design with an explanatory sequential approach. The quantitative phase utilizes secondary data from the Ministry of Energy and Mineral Resources (KESDM) for 2024, covering 52 coal mining companies at the production  operation stage that met the inclusion criteria, namely SML scores based on the Director  General of Mineral and Coal Decree No. 10.K/MB.01/DJB.T/2023 and 2024 mine accident statistics. Data normality tests showed non-normal distributions; therefore, analysis was conducted using nonparametric Spearman’s rho (ρ) and a gamma Generalized Linear Model (GLM). Quantitative results indicate that overall (aggregate) SML is negatively and significantly correlated with safety performance (the mining accident index), but the contribution of each SML dimension is not uniform. “Control Measures” emerged as the most consistently protective indicator against the accident index, whereas “Worker Participation” showed no significant relationship, although the correlation direction was negative. Meanwhile, “Leadership Responsibility” and “Statistical Analysis” demonstrated negative (bivariate) and positive (multivariate) relationships, and both were significant with respect to the mining accident index. Furthermore, qualitative findings derived from secondary KESDM data on a subsample of companies confirm that a high SML does not always translate into reduced accidents when gaps in the implementation of controls persist—particularly in contractor governance, competency, and consistency of execution in the field. This study concludes that SML contributes to accident reduction primarily when it is translated into tangible risk controls and data-driven learning that is closed with corrective action (closed-loop learning), rather than merely fulfilling administrative requirements.

Abstrak:
Industri pertambangan mineral dan batubara merupakan sektor berisiko tinggi yang masih mencatat kecelakaan fatal meskipun telah memiliki regulasi dan sistem manajemen keselamatan. Investigasi kecelakaan yang terlalu berfokus pada tindakan tidak aman pekerja berisiko mengabaikan faktor laten pada tingkat organisasi, pengawasan, kondisi kerja, dan sistem pengendalian. Penelitian ini bertujuan untuk mengeksplorasi accident archetypes atau pola kegagalan sistemik berulang pada kecelakaan fatal pertambangan mineral dan batubara di Indonesia melalui pendekatan Human Factors Analysis and Classification System for Mining Industry (HFACS-MI). Penelitian menggunakan desain kualitatif studi multi-kasus berbasis data sekunder berupa laporan investigasi kecelakaan tambang fatal tahun 2025 dari Kementerian ESDM. Dari 44 kasus fatalitas, tujuh kasus di regional Sumatera dan Kalimantan dipilih secara purposive berdasarkan kelengkapan data, keragaman aktivitas operasional, dan kompleksitas sistemik. Analisis dilakukan melalui directed content analysis, coding HFACS-MI, causal pathways, cross-case matrix, causal loop diagram, dan validasi expert judgement. Hasil penelitian menunjukkan bahwa organizational process, inadequate supervision, personnel factors, dan physical/technological environment muncul secara konsisten pada seluruh kasus yang dianalisis. Unsafe acts tidak berdiri sebagai penyebab tunggal, melainkan sebagai manifestasi akhir dari kondisi laten organisasi, pengawasan, kompetensi, dan lingkungan kerja. Penelitian ini mengidentifikasi empat accident archetypes, yaitu Cost-Driven Operational Normalization, Defense-in-Depth Collapse, Competence Illusion in High-Risk Engineering Tasks, dan Reactive Compliance Trap. Temuan ini menunjukkan bahwa pencegahan kecelakaan fatal perlu bergeser dari event-based intervention menuju structure-based intervention melalui penguatan pengawasan, critical control verification, preventive maintenance, verifikasi kompetensi, dan evaluasi efektivitas prosedur keselamatan di lapangan.

The mineral and coal mining industry is a high-risk sector that continues to experience fatal accidents despite the existence of safety regulations and management systems. Accident investigations that focus primarily on workers’ unsafe acts may overlook latent factors at the organizational, supervisory, working condition, and control-system levels. This study aimed to explore accident archetypes, or recurring patterns of systemic failure, in fatal mineral and coal mining accidents in Indonesia using the Human Factors Analysis and Classification System for Mining Industry (HFACS-MI). This research employed a qualitative multi-case study design based on secondary data from fatal mining accident investigation reports issued by the Ministry of Energy and Mineral Resources in 2025. Of 44 fatal accident cases, seven cases from the Sumatra and Kalimantan regions were purposively selected based on data completeness, operational diversity, and systemic complexity. Data were analyzed using directed content analysis, HFACS-MI coding, causal pathways, cross-case matrix, causal loop diagram, and expert judgement validation. The findings showed that organizational process, inadequate supervision, personnel factors, and physical/technological environment consistently appeared across all analyzed cases. Unsafe acts did not stand as single causes of accidents, but rather as final manifestations of latent organizational conditions, supervision, competence, and work environment. Four accident archetypes were identified: Cost-Driven Operational Normalization, Defense-in-Depth Collapse, Competence Illusion in High-Risk Engineering Tasks, and Reactive Compliance Trap. These findings suggest that fatal accident prevention should shift from event-based intervention toward structure-based intervention by strengthening supervision, critical control verification, preventive maintenance, competence verification, and evaluation of the actual effectiveness of safety procedures in the field.

Abstrak:
Partisipasi pekerja merupakan salah satu faktor penting dalam keberhasilan implementasi Sistem Manajemen Keselamatan Pertambangan (SMKP). Tingkat partisipasi pekerja dipengaruhi oleh berbagai faktor, salah satunya adalah safety leadership yang diterapkan oleh pimpinan unit kerja. Penelitian ini bertujuan untuk menganalisis pengaruh gaya safety leadership terhadap partisipasi pekerja dalam implementasi keselamatan pertambangan di PT XYZ. Penelitian menggunakan desain explanatory sequential mixed methods. Tahap kuantitatif dilakukan dengan rancangan cross-sectional terhadap 699 responden yang dipilih menggunakan teknik proportionate stratified random sampling. Data dianalisis menggunakan uji korelasi Spearman dan regresi linear berganda. Tahap kualitatif dilakukan melalui wawancara mendalam, observasi, focus group discussion, dan telaah dokumen untuk menjelaskan dan memperdalam hasil kuantitatif. Hasil penelitian menunjukkan bahwa seluruh dimensi safety leadership memiliki hubungan positif dan signifikan dengan partisipasi pekerja, dengan hubungan terkuat pada task-centred leadership (r=0,673; p<0,001). Analisis multivariat menunjukkan bahwa democratic leadership dan task-centred leadership merupakan dua dimensi yang paling berpengaruh terhadap partisipasi pekerja (masing-masing β=0,318 dan β=0,292; p<0,001), diikuti relationship-centred leadership (β=0,187; p=0,004), sedangkan authoritarian leadership tidak berpengaruh signifikan (p=0,168). Model penelitian mampu menjelaskan 48,7% variasi partisipasi pekerja (Adjusted R²=0,487). Hasil pendalaman kualitatif menunjukkan bahwa kejelasan arahan kerja, konsistensi pengawasan keselamatan, komunikasi dua arah, keterlibatan pekerja dalam pengambilan keputusan, dan tindak lanjut terhadap masukan pekerja merupakan faktor yang mendukung peningkatan partisipasi pekerja. Penelitian ini menyimpulkan bahwa safety leadership berperan penting dalam meningkatkan partisipasi pekerja dalam implementasi keselamatan pertambangan, dengan democratic dan task-centred leadership sebagai dua dimensi yang paling dominan memengaruhi partisipasi pekerja.

Worker participation is a critical factor in the successful implementation of the Mining Safety Management System. The level of worker participation is influenced by various factors, one of which is the safety leadership practiced by supervisors and managers. This study aimed to analyze the influence of safety leadership styles on worker participation in the implementation of mining safety at PT XYZ. An explanatory sequential mixed-methods design was employed. The quantitative phase used a cross-sectional approach involving 699 respondents selected through proportionate stratified random sampling. Data were analyzed using Spearman correlation and multiple linear regression. The qualitative phase was conducted through in-depth interviews, observations, focus group discussions, and document review to explain and enrich the quantitative findings. The results showed that all dimensions of safety leadership were positively and significantly associated with worker participation, with the strongest relationship observed in task-centred leadership (r = 0.673; p < 0.001). Multivariate analysis revealed that democratic leadership and task-centred leadership were the two most influential dimensions affecting worker participation (β = 0.318 and β = 0.292, respectively; p < 0.001), followed by relationship-centred leadership (β = 0.187; p = 0.004), while authoritarian leadership was not significantly associated with worker participation (p = 0.168). The regression model explained 48.7% of the variance in worker participation (Adjusted R² = 0.487). Qualitative findings indicated that clear work instructions, consistent safety supervision, two-way communication, worker involvement in decision-making, and follow-up on worker feedback were key factors supporting increased worker participation. This study concludes that safety leadership plays an important role in enhancing worker participation in mining safety implementation, with democratic and task-centred leadership being the two most dominant dimensions influencing worker participation.

Abstrak:
Industri pertambangan merupakan sektor dengan tingkat risiko kecelakaan kerja yang tinggi sehingga diperlukan upaya pengendalian risiko yang efektif untuk meningkatkan kinerja keselamatan. Salah satu program yang diterapkan oleh PT X Kontraktor Tambang Batubara di Kalimantan Timur adalah Safety Friendly Program, yang terdiri atas kegiatan observasi keselamatan, inspeksi keselamatan, dan manajemen menyapa. Penelitian ini bertujuan untuk menganalisis efektivitas Safety Friendly Program terhadap penurunan nilai Accident Frequency Rate (AFR) sebagai indikator kinerja keselamatan perusahaan. Penelitian ini menggunakan pendekatan kuantitatif dengan desain time series comparative. Data yang digunakan merupakan data sekunder berupa laporan pelaksanaan Safety Friendly Program dan data statistik kecelakaan kerja periode Kuartal III Tahun 2025 hingga Kuartal I Tahun 2026. Analisis data dilakukan melalui analisis univariat, bivariat, dan multivariat menggunakan perangkat lunak statistik. Uji normalitas dilakukan menggunakan metode Shapiro-Wilk sebelum pengujian hipotesis. Hasil penelitian menunjukkan bahwa implementasi Safety Friendly Program dilaksanakan secara konsisten selama periode penelitian. Aktivitas observasi, inspeksi, dan manajemen menyapa menunjukkan keterlibatan aktif manajemen dan pekerja dalam upaya peningkatan keselamatan kerja. Pada periode yang sama, nilai AFR menunjukkan tren penurunan dari 10,57 pada Kuartal III Tahun 2025 menjadi 8,02 pada Kuartal IV Tahun 2025 dan 3,07 pada Kuartal I Tahun 2026. Temuan ini mengindikasikan bahwa pelaksanaan Safety Friendly Program berkontribusi terhadap penurunan frekuensi kecelakaan kerja selama periode intervensi. Kesimpulan penelitian menunjukkan bahwa Safety Friendly Program efektif sebagai upaya preventif dalam meningkatkan kinerja keselamatan dan menurunkan nilai Accident Frequency Rate (AFR) pada PT X Kontraktor Tambang Batubara di Kalimantan Timur. Program ini dapat dijadikan strategi berkelanjutan dalam memperkuat budaya keselamatan, meningkatkan partisipasi pekerja, dan mendukung pencapaian target zero accident.

The mining industry is recognized as a high-risk sector with significant  occupational accident potential, requiring effective risk control measures to  improve safety performance. One of the initiatives implemented by PT X, a coal  mining contractor in East Kalimantan, is the Safety Friendly Program, which  consists of safety observations, safety inspections, and management engagement  activities (management walk-around). This study aims to analyze the effectiveness  of the Safety Friendly Program in reducing the Accident Frequency Rate (AFR) as  an indicator of organizational safety performance. This study employed a quantitative approach using a comparative time-series  design. Secondary data were collected from Safety Friendly Program  implementation reports and occupational accident statistics covering the period  from the third quarter of 2025 to the first quarter of 2026. Data analysis was  conducted using univariate, bivariate, and multivariate statistical methods. A  Shapiro-Wilk normality test was performed prior to hypothesis testing. The results indicate that the Safety Friendly Program was implemented  consistently throughout the study period. Safety observation activities,  inspections, and management engagement demonstrated active participation from  both management and workers in promoting workplace safety. During the same  period, the AFR showed a declining trend, decreasing from 10.57 in the third  quarter of 2025 to 8.02 in the fourth quarter of 2025 and further to 3.07 in the  first quarter of 2026. These finding indicates that the implementation of the Safety  Friendly Program contributed to a reduction in the frequency of work-related  accidents during the intervention period. The study concludes that the Safety Friendly Program is an effective preventive  strategy for improving safety performance and reducing the Accident Frequency  Rate (AFR) at PT X. The program can serve as a sustainable approach to  strengthening safety culture, increasing worker participation, and supporting the  achievement of a zero-accident objective.
