Abstrak: In utero tobacco exposure has been associated with fetal growth restriction, but uncertainty remains about critical windows of exposure and specific effects on body segments. In the present study, we aimed to examine the association of maternal smoking with fetal biometry in different stages of pregnancy. The study population comprised 2,478 fetuses from a Spanish birth cohort study that was established between 2003 and 2008. Biparietal diameter, femur length, abdominal circumference, and estimated fetal weight were evaluated at 12, 20, and 34 weeks of gestation. Fetal size and growth were assessed by standard deviation scores adjusted by maternal and fetal characteristics. Maternal smoking was assessed using questionnaire and a sample of urinary cotinine at week 32 of gestation. Associations were estimated using multiple regression analysis. Smokers at week 12 of gestation showed decreased fetal growth as reflected by all growth parameters at 20–34 weeks, leading to a reduced fetal size at week 34. The reduction was greatest in femur length, at -9.4% (95% confidence interval -13.4, -5.4) and least in abdominal circumference, at -4.4% (95% CI: -8.7, -0.1). Fetuses of smokers who quit smoking before week 12 showed reduced growth only in femur length (-5.5; 95% CI: -10.1, -0.9). Dose–response curves for smoking versus fetal growth parameters (abscissa: log2 cotinine) were linear for biparietal diameter and femur length.

Abstrak: In this prospective cohort study, based on 1,505 mother-infant pairs in rural Bangladesh, we evaluated the associations between early-life exposure to arsenic, cadmium, and lead, assessed via concentrations in maternal and child urine, and children's weights and heights up to age 5 years, during the period 2001-2009. Concurrent and prenatal exposures were evaluated using linear regression analysis, while longitudinal exposure was assessed using mixed-effects linear regression. An inverse association was found between children's weight and height, age-adjusted z scores, and growth velocity at age 5 years and concurrent exposure to cadmium and arsenic. In the longitudinal analysis, multivariable-adjusted attributable differences in children's weight at age 5 years were -0.33 kg (95% confidence interval (CI): -0.60, -0.06) for high (≥ 95th percentile) arsenic exposure and -0.57 kg (95% CI: -0.88, -0.26) for high cadmium exposure, in comparison with children with the lowest exposure (≤ 5th percentile). Multivariable-adjusted attributable differences in height were -0.50 cm (95% CI: -1.20, 0.21) for high arsenic exposure and -1.6 cm (95% CI: -2.4, -0.77) for high cadmium exposure. The associations were apparent primarily among girls. The negative effects on children's growth at age 5 years attributable to arsenic and cadmium were of similar magnitude to the difference between girls and boys in terms of weight (-0.67 kg, 95% CI: -0.82, -0.53) and height (-1.3 cm, 95% CI: -1.7, -0.89).

Abstrak: Iodine supplementation during pregnancy is a common practice in developed countries. However, scant evidence is available regarding the safety and effectiveness of maternal iodine supplementation with regard to child neuropsychological development. We previously reported an inverse association between iodine supplementation and the psychomotor development of infants in a birth cohort from Valencia, Spain. In the present study, we assessed this association in a wider sample of mother and child pairs from 3 other regions in Spain. Neuropsychological development was assessed using the Bayley Scales of Infant Development in 1,519 infants (median age, 16 months) between 2006 and 2009. In multivariate analyses, maternal consumption of 150 μg/day or more of iodine from supplements was related to a 1.5-fold increase in the odds of a psychomotor score less than 85 (95% confidence interval: 0.8, 2.9) and to a 1.7-fold increase in the odds of a mental score less than 85 (95% confidence interval: 0.9, 3.0). Findings previously reported in the Valencia cohort were only partially verified. The results of the present study suggest that, at least in these regions, iodine supplementation does not improve infant neuropsychological development at 1 year of age. Further research is needed on the risks and benefits of supplementary iodine for both maternal thyroid function and child neurodevelopment.

Abstrak:
Salah satu penyebab kesakitan dan kematian ibu adalah masih rendahnya pengetahuan ibu hamil. Guna mengatasi hal tersebut maka Kementerian Kesehatan RI merencanakan Kelas Ibu Hamil (KIH) yang bertujuan untuk meningkatkan pengetahuan ibu hamil. Penelitian ini dilakukan untuk mengetahui pengaruh KIH di Puskesmas Cipaku Kota Bogor terhadap pengetahuan kesehatan maternal. Penelitian ini menggunakan metode penelitian kuantitatif guna mengetahui pengaruh KIH terhadap pengetahuan serta kualitatif untuk mengetahui gambaran pelaksanaan KIH. Hasil penelitian menunjukan peningkatan pengetahuan sesudah pelaksanaan KIH, dimana peningkatan lebih tinggi terjadi pada kelompok intervensi. Selain itu terjadi penurunan retensi pengetahuan pada 7 hari setelah KIH sebesar 4%. Temuan kuantitatif ini didukung dengan temuan kualitatif yang menunjukan masih ditemukannya berbagai hambatan pada pelaksanaan KIH. Uji instrumen menunjukan kuesioner KIH belum memiliki validitas dan reliabilitas yang baik sehingga perlu dilakukan perbaikan.

One of the issues causing maternal morbidity and mortality is the lack of knowledge among pregnant mothers. In order to overcome this problem, the Ministry of Health of the Republic of Indonesia has planned a program named Kelas Ibu Hamil (KIH) or a class for pregnant mothers designed to improve their knowledge. This study aims at finding out the effects of KIH held at Puskesmas Cipaku Kota Bogor – on knowledge about maternal health. Using quantitative method, this study discovers how KIH affects knowledge; meanwhile, the qualitative method describes the implementation of KIH at this particular area. Results indicate increasing knowledge among pregnant mothers after the implementation of KIH, as more considerable improvemens were found within exposed group. However, 7 days after KIH, their knowledge retention decreased 4%. These quantitative results are supported by some qualitative findings, which indicate several obstacles coming across the program. Instrument test shows that the validity and reliability of the KIH questionnaire has not yet been confirmed yet, therefore requires further improvements
