Abstrak:
Robot merupakan sebuah mesin yang kompleks dan rumit dengan kemampuanuntuk bergerak dengan berbagai kecepatan dan berbagai arah. Interaksi antaraorang, robot dan lingkungan berkontribusi menciptakan resiko yangmembahayakan manusia maupun properti yang berada di sekitar robot.Berdasarkan informasi dari situs resmi departemen tenaga kerja Amerika Serikatsejak tahun 1984 hingga tahun 2013 telah terjadi 37 kecelakaan fatal di tempatkerja terkait robot. Tesis ini membahas penilaian risiko yang dilakukan padapekerjaan pengoperasian dan maintenance robot di PT X. Identifikasi bahaya padapenelitian ini menggunakan metode analisa bahaya pekerjaan (JSA/JHA) danpenghitungan nilai risikonya dengan menggunakan metode analisa risko semikuantitatif. Hasil penelitian ini memberikan gambaran tingkat risiko pekerjaanpengoperasian dan maintenance robot, dimana risiko tertinggi adalah tertabraklengan robot baik saat pekerjaan pengoperasian maupun maintenance robot.Tingkat risiko tersebut didapat dari kombinasi kemungkinan kejadian(probability) dan keparahan dampak (severity). Selain itu dihasilkan rekomendasiuntuk mengendalikan sisa risiko yang ada.

Tesis ini merupakan penelitian yang bersifat deskriptif analitik, untuk mengetahui tingkat bahaya dan risiko yang ada di divisi ERS (equipment repair and service), departemen MOSC (Maintenance Operation Support and Certification), pada sebuah perusahaan yang bergerak dibidang minyak dan gas (Oil and Gas Company). Hasil penelitian ini memberikan gambaran tingkat risiko untuk masing-masing shop yang ada di divisi ERS, dimana tingkat risiko tertinggi ada pada PSV shop, dan hot tapping shop yang bekerja di lokasi pipa minyak produksi. Pengolahan data dilakukan dengan menggunakan JSA. Semua potensi bahaya yang diperoleh akan dilakukan penilaian consequences, probability, dan exposure. Metode analisis risiko yang digunakan adalah kualitatif dan semikuantitatif yang mengacu pada AS/NZS 4360 : 2004. Selain itu dihasilkan rekomendasi-rekomendasi untuk mengendalikan risiko-risiko yang ada sehingga sampai risiko dikategorikan menjadi risiko yang dapat diterima.
This thesis is a descriptive analytic study, to determine the level of hazards and risks that exist in ERS (equipment repair and service) division MOSC (Maintenance Operation Support and Certification) departement, in Oil and Gas Company. The study provides an overview of the level of risk for each shop in the ERS division, where there is the highest risk level at PSV shop, and hot tapping shop working on production of oil pipeline location. Data processing is done by using the JSA. All potential hazard assessment earned will be consequences, probability, and exposure. Risk analysis methods used are qualitative and semiquantitative which refers to the AS / NZS 4360: 2004. Moreover resulting recommendations to control the risks that exist so until the risk is categorized into acceptable risk.

Abstrak:
Latar Belakang: Program nasional Deteksi Dini Hepatitis B (DDHB) untuk ibu hamil merupakan strategi utama untuk mencegah penularan hepatitis B dari ibu ke anak/ mother-to-child transmission (MTCT). Sekitar 90% bayi yang tertular dari ibu dengan HBsAg reaktif akan berkembang mengalami hepatitis B kronik. Imunisasi merupakan salah satu upaya pencegahan, namun belum bisa sepenuhnya mencegah penularan Hepatitis B pada anak. Penelitian ini bertujuan untuk menganalisis faktor risiko yang berhubungan terhadap hepatitis B pada anak, membuat model probabilitas kejadian hepatitis B pada anak, dan menelaah penerapan program DDHB sebagai tindakan pencegahan dan pengendalian MTCT. Metode: Penelitian ini menggunakan pendekatan concurrent mixed method, penelitian kuantitatif dengan desain studi kohort retrospektif dilakukan dengan melibatkan 166 pasangan ibu-anak, ibu dipastikan memiliki infeksi hepatitis B (HBsAg-positif) berdasarkan skrining saat melakukan ANC. Sedangkan penelitian kualitatif menggunakan desain studi kasus melalui indepth-interview kepada 23 informan. Penelitian ini dilaksanakan di Kota Makassar dan Kabupaten Gowa, Provinsi Sulawesi Selatan. Hasil: Analisis multivariat menggunakan GLM binomial link log dilakukan untuk menghitung risk rasio (aRR) dari faktor risiko yang berhubungan dengan hepatitis B pada anak. Hasil penelitian menunjukkan bahwa ibu dengan kadar HBV-DNA tinggi (>106 copies/mL) memiliki risiko lebih tinggi untuk menularkan HBV kepada anaknya (aRR=2,9; 95%CI=1,37-6,20). Anak yang tidak mendapat HBIg (aRR=5,6; 95%CI=2,28-13,76), tidak vaksin HB-0 (aRR=2,9; 95%CI=1,37-6,20), tidak vaksin HB-1 (aRR=10,4; 95%CI=5,23-20,87), dan tidak vaksin HB-2 (aRR=12,1; 95%CI=5,21-28,35) memiliki risiko infeksi HBV yang lebih tinggi. Adapun anak dengan kadar HBV-DNA ibunya yang tinggi (>106 copies/mL), tidak mendapat HBIg, tidak vaksin HB-0, tidak vaksin HB-1, dan tidak vaksin HB-2 memiliki probabilitas kejadian hepatitis B sebesar 94%. Kesimpulan: Temuan ini menekankan pentingnya program DDHB bagi ibu hamil dalam mengidentifikasi kadar HBV-DNA untuk memfasilitasi terapi antivirus sesuai kebutuhan. Begitupun pemberian HBIg dan vaksin hepatitis B (HB-0, HB-1, dan HB-2) kepada anak sangat efektif dalam mengurangi risiko penularan hepatitis B pada anak, menjadikannya sebagai strategi penting dalam mencegah infeksi hepatitis B pada anak. Saran: Program DDHB perlu dioptimalkan secara komprehensif, sejak tindakan skrining pada ibu hamil, melaksanakan pemeriksaan HBV-DNA pada ibu hamil yang HBsAg reaktif, serta meningkatkan cakupan pemberian HBIg dan vaksinasi hepatitis B (HB-0, HB-1, dan HB-2) pada anak.

With approximately 90% of infants born to HBsAg-positive mothers developing chronic hepatitis B, immunization remains crucial but does not fully eliminate the risk of transmission. This study aimed to analyze risk factors contributing to hepatitis B in children, develop a probability model for its occurrence, and evaluate the implementation of the DDHB program as a preventive and control measure for MTCT. Methods: This study employed an explanatory mixed-methods approach. The quantitative component used a retrospective cohort design involving 166 mother-child pairs, where mothers were confirmed to have hepatitis B infection (HBsAg-positive) through antenatal care (ANC) screening. The qualitative component utilized a case study design with in-depth interviews conducted with 23 informants. The research was carried out in Makassar City and Gowa Regency, South Sulawesi Province. Result: A multivariate analysis using a binomial GLM with a log link was conducted to calculate the adjusted risk ratio (aRR) for factors associated with hepatitis B in children. The results indicated that mothers with high HBV-DNA levels (>106 copies/mL) had a significantly increased risk of transmitting HBV to their children (aRR=2.9, 95%CI=1,37-6,20). Children who did not receive hepatitis B immunoglobulin (HBIg) (aRR = 5.6, 95%CI=2,28-13,76), did not vaccinate HB-0 (aRR = 2.9, 95%CI=1,37-6,20), did not vaccinate HB-1 (aRR = 10.44, 95%CI=5,23-20,87), or did not vaccinate HB-2 (aRR = 12.11, 95%CI=5,21-28,35) were at significantly higher risk of HBV infection. Additionally, children born to mothers with high HBV-DNA levels (>106 copies/mL) who did not receive HBIg, HB-0, HB-1, and HB-2 vaccines had a 94% probability of hepatitis B occurrence. Conclusion: These findings emphasize the importance of the DDHB program for pregnant women in identifying HBV-DNA levels to facilitate antiviral therapy as needed. Furthermore, the administration of HBIg and hepatitis B vaccines (HB-0, HB-1, and HB-2) to infants is highly effective in reducing the risk of MTCT, making it a vital strategy in preventing hepatitis B infections in children. Recommendation: The DDHB program should be comprehensively optimized, starting with screening for pregnant women, conducting HBV-DNA testing on pregnant women who are HBsAg reactive, and enhancing the coverage of HBIg administration and implementation of the hepatitis B vaccination (HB-0, HB-1, and HB-2) for children.

Human error pada proses penyortiran parsel manual merupakan kontributor signifikan terhadap kejadian misroute dalam operasional logistik, yang secara langsung memengaruhi efisiensi operasional dan kualitas layanan. Penelitian ini bertujuan untuk menilai keandalan manusia (human reliability) pada proses penyortiran manual di PT. XYZ menggunakan metode Human Error Assessment and Reduction Technique (HEART) yang diintegrasikan dengan model SHELL. Penelitian observasional dengan desain studi sebelum dan sesudah (before–after study) serta analisis komparatif pada dua kondisi: sebelum perbaikan proses manual sorting (18 sub-tugas, tanpa pemindaian wajib) dan sesudah perbaikan proses manual sorting (23 sub-tugas, dengan pemindaian wajib). Metodologi mencakup Hierarchical Task Analysis (HTA), identifikasi Error Producing Conditions (EPC) berbasis SHELL, klasifikasi Generic Task Type (GTT), penilaian Assessed Proportion of Effect (APOE) melalui Focus Group Discussion Subject Matter Expert yang melibatkan empat ahli, perhitungan Assessed Effect (AE), dan perhitungan Human Error Probability (HEP). Hasil penelitian menunjukkan bahwa rata-rata HEP menurun dari 0,142 menjadi 0,044 (penurunan sebesar 69,1%), sementara HEP tertinggi menurun dari 0,72 menjadi 0,160 (penurunan sebesar 77,8%). Misroute terkait penyortiran yang disebabkan oleh human error menurun dari 761 kasus (53,4%) menjadi 330 kasus (19,6%), atau turun sebesar 56,6%. Namun di sisi lain, proporsi misroute yang disebabkan oleh faktor sistem justru meningkat, yang mengindikasikan bahwa faktor kontributor bergeser dari human error menuju kesalahan sistem; sehingga peningkatan keandalan sistem tetap diperlukan untuk menjaga kinerja operasional secara keseluruhan. Temuan ini mengindikasikan bahwa perbaikan proses manual sorting, termasuk penerapan pemindaian wajib, secara substansial meningkatkan keandalan manusia, sekaligus menegaskan perlunya penguatan keandalan sistem secara berkelanjutan. Integrasi metode HEART dengan model SHELL terbukti efektif dalam mengidentifikasi faktor kontributor sistemik dan mengukur probabilitas human error, sehingga menjadi dasar dalam merancang intervensi pengurangan error yang tepat sasaran.
Human error in manual parcel sorting is a significant contributor to misrouting in logistics operations, directly affecting operational efficiency and service quality. This study aims to assess human reliability in the manual sorting process at PT. XYZ using the Human Error Assessment and Reduction Technique (HEART) integrated with the SHELL model. The research employed an observational before-after study design across two conditions: before the manual sorting process improvement (18 sub-tasks, without mandatory scanning) and after the manual sorting process improvement (23 sub-tasks, with mandatory scanning). The methodology comprised Hierarchical Task Analysis (HTA), SHELL-based Error Producing Conditions (EPC) identification, Generic Task Type (GTT) classification, fuzzy Assessed Proportion of Effect (APOE) scoring through a Subject Matter Expert Focus Group Discussion involving four experts, Assessed Effect (AE) computation, and Human Error Probability (HEP) calculation. The results showed that the mean HEP decreased from 0.142 to 0.044 (a reduction of 69.1%), while the highest HEP declined from 0.72 to 0.160 (a reduction of 77.8%). Sorting-related misrouting attributable to human error fell from 761 cases (53.4%) to 330 cases (19.6%), a reduction of 56.6%. Nevertheless, the proportion of misrouting attributable to system-related causes increased, indicating that the contributing factors shifted from human error toward system failures; thus, improving system reliability remains necessary to sustain overall operational performance. These findings indicate that the manual sorting process improvement, including the implementation of mandatory scanning, substantially enhanced human reliability, while underscoring the continued need to strengthen system reliability. The integration of the HEART method with the SHELL model proved effective in identifying systemic contributing factors and quantifying human error probability, providing a basis for designing targeted error-reduction interventions.

Penelitian ini membahas Manajemen risiko keselamatan dan kesehatan kerja dipabrik tahu x tahun 2012, penelitian ini bersifat deskriptif. Desain studi yangdigunakan merupakan desain studi berdasarkan standar AS/NZS 4360:2004 dengan metode semi kuantitatif menggunakan Job Hazard Analysis (JHA).Analisis risiko dilakukan menganalisis nilai konsekuensi, peluang serta frekuensidan dianalisis menggunakan metode Fine yang ada pada AS/NZS 4360:2004.Hasil penelitian menunjukkan bahwa ditemukan level risiko yang belumacceptable pada setiap proses pembuatan tahu yaitu very high, priority 1,substansial, dan priority 3. Oleh karena itu, diberikan rekomendasi yang bersifatengineering, administrative, serta penggunaaan alat pelindung diri.

Abstrak:
ABSTRACT Penelitian ini membahas tentang penilaian risiko keselamatan kerja pada pembuatan produk berbahan dasar baja ringan di PT Bumi Lancang Kuning Pusaka (BLKP) Pekanbaru tahun 2014. Penelitian ini bertujuan untuk menjelaskan tingkat risiko pada kegiatan di area produksi. Identifikasi bahaya dilakukan menggunakan Job Safety Analysis. Penilaian risiko dilakukan berdasarkan kriteria Fine yaitu nilai risiko berasal dari perkalian probability, consequence, dan exposure. Hasil penelitian menemukan 4 jenis bahaya di tempat kerja yaitu bahaya mekanik, fisik, psikososial, dan perilaku. Terdapat 4 risiko dengan kategori very high, 5 risiko kategori priority 1, 8 risiko kategori substantial, 2 risiko kategori priority 1, dan 2 risiko kategori acceptable pada penilaian risiko dengan mempertimbangkan pengendalian yang sudah ada di perusahaan. Setelah diberikan rekomendasi pengendalian ada 15 risiko diestimasi masuk kategori acceptable, 5 kategori priority 3, dan 1 kategori substantial.
ABSTRACT This research discusses the occupational safety risk assessment in the manufacturing of light gauge steel products at PT Bumi Lancang Kuning Pusaka (BLKP) Pekanbaru 2014. The aim of this research is to define the level of risk of the activities in the production area. Hazard was identified using Job Safety Analysis (JSA). Risk assessment was conducted using Fine criteria. Level of risk was the function of the multiplication of probability, consequence, and exposure (R=PxCxE). This results found that there are 4 hazards in the workplace, they are mechanical hazard, physical hazard, psychosocial hazard, and behaviroal hazard. The assessment of existing risks showed that there are 4 risks ranked as very high, 5 are priority 1 risks, 8 are substantial risks, 2 are priority 3 risks, and 2 are acceptable risks. The recommendations of control were given based on the existing risk and risk categories were estimated changed into 15 acceptable risks, 5 priority 3 risks, and 1 subtantial risk.
