Abstrak:
Penelitian ini bertujuan untuk mengetahui gambaran hazard dan analisis risiko pada aktivitas maintenance wahana X. Penelitian ini besifat deskriptif dengan desain semikuantitatif dan menggunakan tahapan manajemen risiko AS//NZS: 4360. Tahapan kerja aktivitas maintenace didapatkan dari SOP dan wawancara mendalam. Analisis tingkat risiko menggunakan kriteria exposure, likelihood, consequence, dan risk level W.T. Fine. Aktivitas maintenance yang di analisis adalah harian dan mingguan (greasing). Hasil penelitian menunjukkan bahwa hazard terbanyak yang dihadapi oleh pekerja pada maintenance harian adalah hazard fisik (ketinggian dan listrik) dengan tingkat risiko tertinggi pada tahapan memeriksa neple dan oli hidraulik tentakel. Sementara untuk aktivitas greasing, hazard yang paling banyak adalah fisik (ketinggian) dengan tingkat risiko tertinggi pada tahapan kerja menuju lokasi penggantian grease. Adapun pengendalian yang telah dilakukan oleh perusahaan antara lain dengan pembagian shift kerja dan menyediakan kendaraan berupa golf car serta APD berupa; gloves, safety shoes, body harness, helm, dan googles.

Abstrak:
Indek standar pencemaran udara DKI Jakarta, parameter kritis terbanyak diKelapa Gading. Untuk mengestimasi risiko kesehatan dilakukan analisis risikokesehatan PM10 dan SO2 di Kelapa Gading. Konsentrasi risk agent diperolehstasiun pemantau udara BPLHD DKI Jakarta selama 365 hari. Berat badan, waktupajanan diukur dari 80 responden. Estimasi risiko kesehatan dinyatakan dalamRQ, dihitung dari intake risk agent dan dosis referensinya. Risiko kesehatandianggap berisiko bila RQ>1. Hasil penelitian menunjukkan RQ PM10 dan SO2pajanan realtime tidak berisiko. Pajanan lifespan konsentrasi maksimum PM10berisiko terhadap kesehatan populasi. Abstrak:
Pajanan agen risiko kesehatan dari lingkungan kerja berdampak pada timbulnya risiko penyakit akibat kerja sehingga pekerja menjadi tidak produktif. Oleh karena itu, untuk mengestimasi risiko kesehatan dari pajanan agen risiko berupa PM10 dari lingkungan kerja, sebuah penelitian analisis risiko telah dilakukan pada 70 orang pekerja industri readymix PT. X Plant Kebon Nanas. Penelitian ini menggunakan desain studi cross sectional dengan pendekatan analisis risiko. Risiko kesehatan akibat pajanan PM10 dihitung dengan membandingkan asupan PM10 dengan dosis referensi. Konsentrasi PM10 diukur pada 6 titik. Hasil konsentrasi tertinggi yaitu 0,407 mg/M3 dan terendah yaitu 0,167 mg/M3 dengan perhitungan konsentrasi rata-rata yaitu 0,289 mg/M3 . Hasil perhitungan risiko yang diterima saat ini (realtime) terdapat 21,4% pekerja yang berada dalam kategori berisiko. Sedangkan hasil estimasi risiko yang diterima seumur hidup (lifetime) hanya 2 orang pekerja yang dalam kategori tidak berisiko. Manajemen risiko yang dapat dilakukan adalah dengan menurunkan konsentrasi menjadi 0,08 mg/M3 . Dengan konsentrasi tersebut pekerja diestimasikan aman bekerja selama 11 jam per hari dan 362 hari per tahun.

Abstrak:
Penelitian ini bertujuan untuk memperoleh tingkat risiko K3 pada Praktikum Milling Tingkat I di ATMI Cikarang dengan menganalisis risiko K3 tersebut. Penelitian ini dilakukan dengan observasi lapangan, wawancara, dan telaah dokumen. Identifikasi bahaya dilakukan dengan metode Job Hazard Analysis, lalu melakukan analisis semi-kuantitatif dengan mempertimbangkan consequences, probability, dan exposure sehingga diperoleh tingkat risikonya, baik itu basic risk, exisiting risk, dan predictive risk. Hasil penelitian menunjukkan bahwa terdapat bahaya tertinggi yaitu cutter (baik yang berputar/rotating maupun yang tidak), chips/gram, dan metal fume. Oleh karena itu, masih diperlukan pengendalian tambahan untuk mengurangi tingkat risiko yang masih ada.

Kejadian kecelakaan di perusahaan tambang menimbulkan kerusakan properti, cedera ringan, berat, hingga kematian. Demikian pula PT X, yang merupakan perusahaan pertambangan, masih mengalami cedera akibat kecelakaan dengan nilai Total Recordable Injury Rate (TRIR) sebesar 0,168 di Tahun 2020 dan kemudian naik menjadi 0,176 pada Tahun 2021. Salah satu temuan penyebab kejadian kecelakaan ini adalah adanya kegagalan manajemen melakukan kontrol risiko. Oleh karena itu, penelitian ini dilakukan untuk menganalisis faktor manajemen terhadap kejadian fatalitas di PT X. Quinlan Pathway (2014) yang digunakan dalam analisis kejadian fatal menemukan bahwa ada 3 Quinlan Pathway yang berkontribusi terhadap aspek manajemen yaitu Pathway-4 failures in safety management systems and hazard plan, Pathway-5 failures in auditing, dan Pathway-9 poor management – worker communication and trust. Penelitian ini dilakukan dengan melakukan tinjauan dokumen kejadian kecelakaan dan audit Tahun 2023, kemudian dilanjutkan dengan survei kuisioner terhadap 396 responden dan wawancara dengan 29 orang yang terbagi ke dalam 11 Divisi di PT X. Hasil tinjauan dokumen kejadian kecelakaan dan audit Tahun 2023 menunjukkan bahwa Pathway 4 failures in safety management systems and hazard plan memiliki temuan paling tinggi. Selain itu, hasil triangulasi survei dan wawancara menunjukkan bahwa nilai Pathway-4 failures in safety management systems and hazard plan memiliki nilai yaitu 7,52 diikuti Pathway-5 failures in auditing memiliki nilai yaitu 7,55, dan nilai Pathway-9 poor management – worker communication and trust memiliki nilai yaitu 7,75. Hal ini menunjukkan bahwa terdapat peluang kegagalan manajemen berkontribusi terhadap kejadian fatalitas. Oleh karena itu, hasil penelitaian ini menitikberatkan rekomendasi terhadap manajemen PT X untuk melakukan risk management yang terintegrasi, melaksanakan investigasi secara memadai, memastikan tim auditor internal memiliki kompetensi yang memadai, serta membangun komunikasi manajemen dan karyawan untuk meningkatkan kepercayaan.
Accidents at mining companies cause property damage, minor and serious injuries and even death. This happened in PT X, which is a mining company, is still experiencing injuries due to accidents with a Total Recordable Injury Rate (TRIR) of 0.168 in 2020 and then increase to 0.176 in 2021. One of the findings that caused this accident was management's failure to control risks. Therefore, the objective in this research was analyze management factors regarding fatality incidents in PT X. The Quinlan pathway (2014) was used in the analysis and found 3 pathway contributing factor i.e. Pathway-4 failures in safety management systems and hazard plans, Pathway-5 failures in auditing, and Pathway-9 poor management – worker communication and trust. This research was carried out by reviewing accident document and audit documents in 2023, then continue with questionnaire survey of 396 respondents, and interviewing 29 people from 11 divisions in PT X. The results of a review of accident documents and audits document in 2023 showed that Pathway-4 failures in safety management systems and hazard plans have the highest findings. Apart from that, the results of survey and interview triangulation show that the Pathway-4 failures in safety management systems and hazard planning has a value of 7.52, followed by Pathway-5 failures in auditing has a value of 7.55, and Pathway-9 poor management – worker communication and trust has a value of 7. 75. These values show that there is a chance that management failure will contribute to fatalities. Therefore, the research outlines the recommendation to PT X management to carry out integrated risk management, to carry out adequate investigations, to ensure internal auditor team has adequate competence, and to build communication that creates trust between management and employee.

Abstrak:
Radiografer secara umum mempunyai tugas dan tanggung jawab untuk melakukanpemeriksaan pasien secara radiografi meliputi pemeriksaan untuk radiodiagnostiktermasuk kedokteran nuklir dan ultrasonografi (USG) dan melakukan tindakan proteksiradiasi dalam mengoperasikan peralatan radiologi. Penelitian ini bertujuan memprakirakanrisiko pajanan radiasi sinar-X pada pekerja radiasi di Departemen Radiologi RSUPN CiptoMangunkusumo. Dalam perhitungan prakiraan risiko pajanan radiasi sinar-X, dosis pajananradiasi sianr-X radiografer diperoleh dari hasil pengukuran film badge. Data pola aktifitas(lama kerja, frekuensi pajanan dan masa kerja) diperoleh berdasarkan hasil wawancara pada35 radiografer di Departemen Radiologi RSUPN Cipto Mangunkusumo. Berdasarkanperhitungan yang dilakukan, nilai rata-rata Excess Cancer Risk (ECR) lifetime (4,8E-2) danrealtime (1,9E-2). Karena secara teoretis karsinogenisitas tidak mempunyai nilai ambang ataunon threshold, maka prakiraan risiko dinyatakan unacceptable (dosis tidak dapat diterima)bila ECR < E4. Kisaran angka E-4 diperoleh dari nilai default karsinogenistas yang digunakanoleh US-EPA (1990). Berdasarkan perhitungan ECR lifetime dan ECR realtime diperolehgambaran prakiraan risiko efek karsinogenik yang terjadi pada radiografer di DepartemenRadiologi RSUPN CM, dinyatakan aceptable pada risiko kanker baik pada ECR lifetimemaupun realtime.Kata kunci : Analisis risiko, Radiasi, Sinar-X, Radiografer.

This study aims to estimated the risk ofX-ray radiation exposure to radiographer in the Department of Radiology RSUPN CiptoMangunkusumo using Environmental Health Risk Analysis (ARKL). In calculating theestimated risk forecasts ARKL, risk of X-ray radiation exposure dose radiographer obtainedfrom measurements of the film badge. Data patterns of activity (duration of work,frequency of exposure and years of work) obtained based on the results of a survey of 35radiographers in the Department of Radiology RSUPN Cipto Mangunkusumo. Based on thecalculations performed, the average value of Excess Cancer lifetime Risk (ELCR) is 4,8E-2and the value of Excess Real-time Cancer Risk (ERRC) the average is 1,9E-2. Becausetheoretically carcinogenicity has non-threshold value, then the forecast is declaredunacceptable when ECR < E-4. Range of numbers obtained from the E-4, carcinogenicitydefault values used by the US-EPA (1990).
