Gizi buruk merupakan masalah kesehatan yang menjadi beban bagi negara-negara berkembang termasuk Indonesia. Pada anak-anak, gizi buruk dapat mengakibatkan terhambatnya pertumbuhan, rentan terhadap penyakit terutama penyakit infeksi, serta dapat pula mengakibatkan penurunan kecerdasan. Sedang pada orang dewasa, kekurangan gizi dapat menyebabkan penurunan produktifitas serta penurunan daya tahan, sehingga mudah terkena penyakit. Di Kabupaten Sambas, hasil pemantauan status gizi (PSG) balita tahun 2003 sampai dengan tahun 2005 menunjukan adanya kecenderungan kasus gizi buruk dan gizi kurang yang terus meningkat dari tahun ke tahun. Selama informasi yang dihasilkan dari sistem informasi gizi baru berupa data cakupan program penanggulangan, belum mengarah pada kondisi wilayah mana yang menjadi prioritas program penanggulangan gizi buruk serta tindakan apa yang akan dilakukan untuk penanggulangan gizi buruk tersebut. Hal ini menyebabkan kurang efektifnya program yang direncanakan. Penelitian ini bertujuan untuk mengembangkan adanya suatu sistem pendukung keputusan untuk program penanggulangan gizi buruk pada balita di Kabupaten Sambas yang dapat membantu dalam proses pengambilan keputusan dengan menggunakan data- data yang telah ada, sehingga keputusan yang diambil dapat lebih efektif dalam penanggulanan gizi buruk. Penelitian yang dilakukan merupakan pengembangan sistem dengan metodologi Structure System Analysis and Design (SSAD) atau metodologi yang berorientasi data (Data Oriented Methodologies). Metodologi ini menekankan pada karakteristik data yang akan diproses. Penelitian ini juga menggunakan Data Flow Diagram (DFD) sebagai alat untuk menggambarkan sistem yang sedang berjalan ataupun sistem yang akan dikembangkan. Hasil penelitian ini menunjukan bahwa pelaksanaan Permantauan Wilayah Setempat (PWS) Gizi di Kabupaten Sambas sudah sesuai prosedur. Permasalahan yang dihadapi pada sistem informasi gizi di Kabupaten Sambas adalah : (1) Laporan dari Puskesmas masih sering terlambat dan tidak tepat waktu (2) Minimnya tenaga pengelola gizi di yang hanya berjumlah 2 orang (3) Data belum dianalisis secara terintegrasi, analisis masih dilakukan secara manual sehingga sering terjadi kesalahan perhitungan baik dalam jumlah maupun hasil akhir dalam bentuk prevalensi. (4) Keluaran yang dihasilkan hanya terbatas pada informasi cakupan program. Untuk mengatasi permasalahan tersebut, diupayakan pembinaan administratif kepada Puskemas, peningkatan kualitas pengelola program gizi, serta dukungan sarana dan prasarana dalam upaya peningkatan pengelolan informasi gizi di Kabupaten Sambas. Sistem Pendukung Keputusan (SPK) yang dihasilkan berupa, pengembangan basis data pemantauan pertumbuhan balita dan pemantauan status gizi (PSG). Keluaran yang dihasilkan sistem berupa data pencapaian cakupan program penanggulangan gizi buruk dalam bentuk tabel, grafik maupun skala prioritas wilayah maupun skala prioritas program dalam bentuk pemetaan sederhana.
Malnutrition is known as one of health problems that still a burden in most developing countries, including Indonesia. It manifests to children in causing growth disorder, vulnerability to some diseases, especially infectious one, and also decreasing the child intelligence. Meanwhile, toward the adult, malnutrition can cause on reducing the productivity, as well as reducing the body resistance that make them vulnerable to some diseases. At Kabupaten of Sambas, the result of the state of nutrition monitoring (PSG) toward under-five in the year of 2003 to 2005 showed from year to year that there is a trend on the increasing of cases on malnutrition and under-nutrition. However, in dealing with the situation, during the malnutrition management program, there has no decision been made in which region will be the priority of the program and what action should be done in order to improve the condition. The situation that produce an ineffective process on program that has been planned. The study has a purpose on developing a decision supporting system for the malnutrition management program toward under-five at Kapubaten of Sambas, by assisting the process on decision making with some existing data at the region, in order to have an effective way on managing the malnutrition problems. The study is using a system development with Structure System Analysis and Design (SSAD) method, or Data Oriented Method, which is emphasized on data characteristic processed. The study is also developing the Data Flow Diagram System. The result of the study on the implementation of Nutrition Local Monitoring Area (NLAM) at Kabupaten Sambas showed that (1) The report from Puskesmas is mostly still delayed and always not on-time; (2) Inadequacy on nutrition management personnel, which is only 2; (3) The existing data has not been well integrated analyzed, and usually using manually, in which make the erroneous on calculation and result for producing the prevalence measurement; (4) The outcome of the NLAM is only limited to the result of the program coverage. Therefore, in order to cope with those issues mentioned above, a capacity building for Puskesmas administration, and quality improvement for nutrition informatics personnel are proposed. The Decision Supporting System (DSS) that has been made is consisting of the development of data base on under-five growth monitoring and state nutrition monitoring (SNM). The outcome of the system development is the data of program coverage on malnutrition management program, in the form of a simple mapping.
Geographically, Indonesia indeed, lies on high-risk natural disaster, like earthquake, tsunami, landslide, flood, typhoon, drainage and epidemic diseases. Besides, there are other disasters might happen due to the awareness and failure in managing natural resources and environment, such as, haze, environment pollution, traffic accident and technology failure. One of disasters that often occurred in some region is social riot, caused by poverty, culture conflict, injustice in natural resources distribution, social economic gap, different ethnic, religion and races or the intervention of other countries. Health crisis response caused by those stated disasters can be handled quickly and accurately if it is supported by quick, accurate and exact information. In order to gain the quick, accurate and exact information above, it is necessary to develop information system that can be applied as the base of the appropriate decision making in conducting the intervention. The aim of the development of system is to form disaster information system to make an appropriate decision in responding to health crisis. The system development methodology applied is the approach System Development Life Cycle which consists of planning system and visible analysis, analysis toward system, system design and system implementation. Information system is conducted by identifying the opportunity of system development. Based on the analysis of economic visibility, techniques and organization Information System for Health Crisis Response Consequence of Disaster in Ministry of Health, Republic of Indonesia, may still be further developed. Designing system is organized by data based design output design in table and input design in report form. Some advantages of system developed are the use of form and data base management in data entry, use automatic will be more structured and quick process in data processing, report can be directly printed and data developed in table. Meanwhile, the constraint from the developing system is it is still needed to be further developed and has not been justified in implementation stage in Health Provincial Office and Health District Office. It is advisable that there is a supporting policy in regulating the implementation Information System for Health Crisis Response Consequence of Disaster, both in province and district, including feedback mechanism and sustainable control so that the reporting process of health crisis response of disaster can be operated well. The village is an area where the population has the readiness of resources and capabilities and the willingness to prevent and overcome health problems, disasters and emergencies independently. Preliminary studies conducted in Waru Jaya Village and Bojong Indah Village found several health problems that could threaten public health, namely stunting, Acute Respiratory Infection and hypertension. This research uses a qualitative approach with a policy perspective. The use of in-depth interview and observation (triangulation) methods is intended so that researchers gain a thorough understanding of the existing phenomena/events. This research was conducted in two villages, namely Waru Jaya Village as the focus and Bojong Indah Village as the model. The two villages are located in Parung District, Bogor Regency with different health problems. The results of this study showed that Bojong Indah Village was able to optimize all existing resources to improve the quality of health in her village. Meanwhile, Waru Jaya Village is still focusing on derivative programs provided by the Bogor Regency without adjusting to health problems in the field. It is hoped that the Ministry of Villages and Development of Disadvantaged Regions and Transmigration as well as the Bogor District Health Office can carry out more detailed evaluation and monitoring and help develop more applicable programs that can be implemented in villages according to their respective problems
Tujuan penelitian ini adalah untuk mengetahui perilaku seks pra nikah dan faktorfaktor yang mempengaruhi perilaku ini pada mahasiswa Akademi Kesehatan X di Kabupaten Lebak. Jenis penelitian yang dilakukan adalah kuantitatif dengan pendekatan cross sectional dengan jumlah sampel 143. Proporsi perilaku seks pranikah 60,1% dengan alasan tertinggi (14,7 % ) adalah kedua belah pihak samasama senang melakukan hubungan seks. Variabel yang memiliki hubungan bermakna adalah faktor sikap, paparan media pornografi, dan adanya peluang, sedangkan variabel yang paling dominan adalah paparan media pornografi. Diperlukan upaya untuk meningkatkan kegiatan penyuluhan perilaku seks pranikah baik di kampus maupun di luar kampus, membentuk dan meningkatkan layanan informasi dan konseling seks pra nikah di lingkungan institusi atau lingkungan masyarakat. The purpose of this research is to reveal premarital sexual behavior and factors that influence it on Health Academy X students in Kabupaten Lebak. The type of this research is quantitative with cross sectional approach with 143 samples. The proportion of premarital sexual behavior is 60.1% with the highest reason (14.7%) is that the couple enjoy having sex. Variables that have significant relation are behavior, the exposure to pornographic media, and opportunity. While the most dominant variable is the exposure to pornographic media. It is necessary to improve outreach activities of premarital sexual behavior in campus and out of campus, establish and improve information and counseling of premarital sex in the institution and society. Rumah Sakit merupakan salah satu fasilitas pelayanan kesehatan. Pelayanan kesehatan yang sebaik-baiknya harus ditunjang oleh data melalui Sistem Informasi Kesehatan (SIK) dimana informasi yang dihasilkan akan bermanfaat bagi kegiatan manajemen Rumah Sakit, selain untuk monitoring pelayanan kesehatan individual. Penggunaan informasi dalam tingkat manapun membutuhkan informasi yang berkualitas yaitu relevan, berguna pada waktu yang tepat, dapat ditelusuri dan bebas dari kesalahan.Sejak tahun 2000 Rumah Sakit Harum telah memulai uji coba implementasi Sistem Informasi Manajemen Rumah Sakit (SIMRS). Oleh karena itu dibutuhkan suatu studi mengenai pengembangan SIMRS di Rumah Sakit Harum melalui analisis tahap-tahap pengembangan sistem yang sudah dilakukan. Analisis tahap-tahap pengembangan sistem ini menggunakan dan mengkombinasikan model-model dari beberapa konsep sehingga menjadi rumusan kerangka konsep, yaitu SDLC (System Development Life Cycle) yang sering digunakan dalam pengembangan sistem informasi, PDCA (Plan-Do-Check-Act) yang sering digunakan dalam peningkatan kualitas berkesinambungan (Continuous Quality Improvement) serta model IPO (input process-output) yang dapat digunakan untuk perencanaan strategik sebagai salah satu kegiatan dalam pengembangan sistem informasi.Model PDCA digunakan sebagai template atau panduan dalam tahap-tahap pengembangan sistem informasi, dengan beberapa aspek yang berkaitan di dalamnya diambil dan aspek-aspek yang diberikan dalam model SDLC dan PDCA. Panduan tersebut dapat diaplikasikan atau diterapkan pada semua jenis dan ukuran organisasi agar menghasilkan sistem informasi yang sesuai dengan kebutuhan sehingga informasi yang diperoleh dari sistem yang dikembangkan tersebut dapat terus ditingkatkan kualitasnya, selain juga dapat meningkatkan kualitas sistem informasinya.Penelitian ini bertujuan untuk mendapatkan informasi mengenai tahap-tahap pengembangan yang dilakukan oleh RS Harum dalam pengembangan sistem informasinya serta membandingkan dengan tahap-tahap pada template pada kerangka konsep. Data yang dikumpulkan dalam studi ini adalah data kualitatif dengan telaah dokumen, observasi fisik dikonfirmasi dengan wawancara, serta wawancara mendalam. Informan dari studi ini adalah karyawan yang akan ditugaskan menjadi operator dalam sistem informasi yang berasal dari bagian akuntansi, rekam medis dan perawatan, dengan 2 karyawan yang diwawancara dari masing-masing bagian sehingga jumlah seluruh karyawan yang diwawancara adalah 6 karyawan serta 3 informan yang mewakili pihak manajemen serta 1 informan kunci.Dalam studi ini ditemukan bahwa belum dilakukan atau belum terpenuhinya beberapa komponen dalam sebagian tahap pengembangan sistem informasinya. Beberapa tahap yang belum terpenuhi tersebut berhubungan dengan dokumentasi yang menyimpan informasi perkembangan dan kendala yang dihadapi pada tiap tahap pengembangan; dokumentasi mengenai pelatihan dan model untuk operasional dan penanganan kesalahan (error handling); mengenai tim implementasi; mengenai penerimaan operator (user acceptance) serta mengenai organisasi pengelola sistem informasi.Proses pengembangan sistem informasi ini belum berjalan seluruhnya, atau masih melakukan beberapa proses uji coba. Untuk mendapatkan sistem informasi dengan fungsi yang maksimal dan sesuai dengan harapan atau tujuan dari pengembangan sistem informasi, sebaiknya dapat diperhatikan kegiatan-kegiatan yang dapat dilakukan dalam kegiatan uji coba sistem dan untuk selanjutnya dalam kegiatan peningkatan kualitas, dapat diterapkan tahap-tahap yang disediakan model-model peningkatan mutu. Dalam tahap-tahap pada metode PDCA maupun SDLC terdapat contoh kegiatan dan alat ukur yang dapat dipergunakan. Juga disarankan untuk membuat semacam tabel praktis (untuk assessment) yang dapat membantu pendokumentasian mengenai perkembangan atau kendala yang dialami dalam proses pengembangan sistem informasi yang kemudian dapat digunakan untuk kegiatan analisis, menugaskan tim mutu untuk melakukan assessment tersebut, mememperbaiki persepsi operator melalui pelatihan yang kegiatankegiatannya dapat menumbuhkan rasa keterlibatan mereka, serta mempertimbangkan pengorganisasian pengelolaan informasi dengan bentuk yang sesuai dengan kebutuhan Rumah Sakit Harum.
Study on the Quality of Information System Development in Harum Hospital, Jakarta Timur, 2001Hospital is functioned as health service facility. A fine health service ought to be supported by health data through Health Information System (HIS). Adjacent to that, information that was created also useful to Hospital's management, besides its value to monitor individual health care. The use of information at any level requires qualified information that is, relevant, timeliness, verifiability and error free.Since the year 2000, Harum Hospital had started implementation trial for its Hospital Information System development. The study is done to analyze every phase on information system development that has made by Hospital. The analysis on phases of hospital information system development employs and combines some models as conceptual frame, with each model represent its area of expertise, those are: SDLC (System Development Life Cycle) which is frequently applied in information system development, PDCA (Plan-Do-Check-Act) as a model that is more often than not applied to improve quality continuously and IPO (Input-Process-Output) model which is usually applied to do a strategic planning as one of the activities to develop information system. Qualified information is generated from appropriate information system. The activities in this study is using phases given by PDCA as a template and using some of its applicable aspects with other relevant aspects were taken from subjects in SDLC and TO. The blending of SDLC and IPO into PDCA approach is expected to be applicable to all type and size of organization so, the following information system development can produce suitable information system and as well, the quality of information system can be improved continuously.The purpose of this research is to comprehend the information system development phases that is done by Harum Hospital and how well-suited are those if compared to phases in the template. Data were collected qualitatively with document review, tangible observation with interview and in-depth interview. The information source was choosen from the employees that will be functioned as operators in the information system, they are each 2 employees from accounting department, medical record and nursing with the total of 6 employees, plus 3 information sources that stand for management and 1 key informant.It is shown in this study that phases which are done in the information system development from the first phase to the phase of trial run were not completed entirely if it was compared with phases given by SDLC and PDCA. Phases that are not accomplish suitably was quite important, those are about documentation that kept information on positive changes and obstacle that happens in every phase; documentation about training, operations manual and error handling guidance; about information system institution in the organization; about implementation team; and about user acceptance.The trial and implementation in this information system development activity are not totally occupied and are doing the trial phase by the time this study was completed. To obtain system with maximum function and system that fits to the expectations or objectives of the information system development, it is recommended to give more notification on activities that can be done in system trial, while for the next quality improvement activities, other steps provided by quality improvement's model can be implemented. There are some example of activities and measurement in PDCA and SDLC phases. Those activities are very flexible and very applicable. It is also suggested to make a practical table to manage an assessment and to help recording the improvement or obstacle that occurs in the information system development phases, which it can be used in evaluation activities, it is much useful as well to revise operator's perception and their acceptance about information system through training and evaluation, and also to consider an information organization with a proper form that meets Hospital's needs.
Penanganan kasus KEP pada Balita tidak bisa hanya dilakukan dengan langkah-langkah pencegahan, tetapi harus sekaligus dilakukan intervensi gizi, antara lain dengan pemberian tambahan konsumsi makanan. Untuk menyusun perencanaan program atau intervensi gizi diperlukan identifikasi masalah gizi dan kebutuhan yang diperlukan dengan melakukan analisis situasi kesehatan. Hasil analisis situasi kesehatan yang akurat membutuhkan datalinformasi yang cukup baik kuantitas maupun kualitas. Sistem Pencatatan dan Pelaporan Puskesmas program gizi (SP3-LB3.1) yang berjalan selama ini belum menghasilkan data/informasi program gizi yang lengkap, cepat dan akurat. Oleh karenanya pemanfaatan hasil luaran SP3-LB3.1 oleh pengelolah program gizi di tingkat Dinkes Kabupaten belum optimal, sehingga tidak dapat memenuhi kebutuhan informasi tingkat manajemen pelaksana dalam penyusunan programlintervensi terhadap permasalahan KEP pada Balita. Disamping itu, SP3-LB3.1 bukan merupakan satu-satunya pelaporan yang harus dibuat oleh Puskesmas, tetapi masih terdapat laporan lain (F III Gizi) yang diminta langsung oleh pengelola program gizi Dinkes Kabupaten. Hal ini selain menjadi beban bagi Puskesmas juga mengakibatkan adanya duplikasi data gizi antara pemegang program gizi dengan data pada pengelola SP3-LB3.1. Sistem Pencatatan dan Pelaporan Program Gizi (SP3G) merupakan pengembangan dari SP3-LB3.1, yang diharapkan menghasilkan datalinformasi mengenai cakupan keberhasilan program gizi di Puskesmas secara cepat, lengkap, dan akurat. mengenai cakupan keberhasilan program gizi di Puskesmas secara cepat, lengkap, dan akurat. Pengembangan sistem ini didukung dengan adanya perubahan fungsi Dinas Kesehatan Kabupaten dalam era otonomi daerah, dari technical control menjadi technical support. Dimana Dinas Kesehatan kabupaten mempunyai kewenangan dalam pengembangan Sistem Kesehatan sesuai dengan kebutuhannya sendiri. Pengembangan SP3G dilaksanakan dengan menetapkan kebutuhan data/informasi, dan indikator, mendesain sistem pengolahan dan penyajian data, mendesain format input dan output laporan, serta perancangan program aplikasinya. Pengumpulan data/informasi dilakukan melalui wawancara dan observasi terhadap komponen sistem. Pengoptimalan fungsi Sub Bagian Perencanaan sebagai pengelola data program kesehatan khususnya masalah gizi, serta pelaksanaan mekanisme umpan balik akan lebih mengoptimalkan pelaksanaan SP3G dalam menghasilkan informasi program gizi yang berkualitas, sehingga dapat mendukung manajemen program gizi di tingkat Dinas Kesehatan Kabupaten, baik dalam perencanaan, monitoring, dan evaluasi program.
Management Information System Development of Protein Energy Malnutrition For Children 0-5 Years at The Health Departement of Banjarnegara District To solve the case of protein energy malnutrition (PEM) for children 0-5 years is not only through prevention, but also through nutrition intervention program, for example by giving additional food. To compose nutrition intervention program or planning, officer should identify nutrition problem along with its needs through analyzing health condition. Its accurate result needs qualified data 1 information in terms of quality and quantity. SP3-LB3.1 (Nutrition recording and reporting program used at public health center/PHC) which is used currently does not produce data 1 information which is complete, instant and accurate. Consequently, performing SP3-LB.1 results used by nutrition analyst at District Health Officer is still not so optimal that it does not fulfill information which is needed by management executive level in order to compose nutrition intervention program/planning to solve PEM for children 0-5 years. In addition, SP3-LB3.1 is not the only reporting program which is composed by PT-IC. The other report is F III - nutrition which is asked directly by the nutrition program executive of District Health Office. These all become burden for PHC. In addition, it causes nutrition data to be duplicated among nutrition program executives and SP3-LB3.I executives. SP3G (the system of nutrition recording and reporting program is developed from SP3-LB3.1) which is designed in order to produce data / information about the coverage of PHC nutrition program achievement rapidly, completely and accurately. The system development is supported by functional changes of district health office from technical control into technical support in distract authonomy era. With this changes, District Health Office has an authority to develop health system based on its own needs. Developing SP3G is conducted deciding data/information needs along with their indicators, designing data performing and processing system, designing input and output reporting format and designing its application program. Data/information collection is conducted through interviewing and observing system components. Optimizing the function of Sub Sector Planning office as the executive of health program data especially for nutrition along with its feed back mechanism application will maximize SP3G application in order to produce qualified nutrition program information so that it supports nutrition program management at District Health Office in perspective of planning, monitoring, and program evaluation.
