Penelitian ini merupakan penelitian kualitatif yang didahulukan oleh studi dokumenter (documentary study) dengan memeriksa kelengkapan rekam medis menggunakan alat bantu daftar tilik yang dibuat oleh peneliti. Motivasi ekstrinsik akan mempengaruhi tenaga medis dalam pengisian rekam medis, diantaranya kerjasama tim dan beban kerja. Motivasi intrinsik seperti pengetahuan dan persepsi dalam pengisian rekam medis juga akan mempengaruhi ketidaklengkapan pengisian rekam medis di suatu pelayanan kesehatan. Motivasi dari pekerja kesehatan berpotensi mempengaruhi penyediaan layanan kesehatan. Semangat rendah diantara pekerja dapat merusak kualitas penyediaan layanan, sementara kehadiran pekerja yang bermotivasi tinggi dan berkualitas merupakan aspek kunci dari kinerja sistem kesehatan.

A medical record is containing data and information to be made by a health service that contains a patient's medical history during the patient is receiving services, from anamnesis, history of the disease, physical examination and other investigations, diagnosis, treatment to action medical services. The completeness of a medical record is desperately needed as a valid proof that a patient is properly receiving health care, especially medical resume can be used as hospital claim sheets to insurance in collaboration with hospitals. The low number of completeness medical record in Bhakti Yudha Depok Hospital is the basis for the researcher to conduct the research. The incomplete filling of medical records is due to many factors, including the motivation possessed by the medical personnel responsible for the medical record.
This research is a qualitative research which takes precedence by documentary study by examining the completeness of medical record using the check list tool made by the researcher. Extrinsic motivation will affect medical personnel in filling out medical records, including teamwork and workload. Intrinsic motivation such as knowledge and perception in filling medical record will also affect the incomplete filling of medical record in a health service. The motivation of health workers has a potential affect the provision of health services. Low morale among workers can damage the quality of service provision, while the presence of highly motivated and qualified workers is a key aspect of health system performance.

Salah satu indikator untuk menunjukkan mutu pelayanan kesehatan di rumah sakit adalah data atau informasi dari rekam medis yang baik dan lengkap. Rekam medis yang lengkap dapat memberikan gambaran secara keseluruhan tentang pasien yang akan digunakan untuk berobat kembali. Rekam medis adalah bukti otentik jika terjadi tuntuntan di pengadilan. Penelitian ini bertujuan untuk memperoleh gambaran mengenai karakteristik dokter dalam kelengkapan pengisian rekam medis dan faktor-faktor ekstrinsik yang mempengaruhinya. Jenis penelitian ini adalah metode observasional. Data diambil secara potong lintang sebanyak 50 rekam medis selama bulan September-November 2011 yang diisi oleh 10 dokter spesialis, serta dilakukan wawancara langsung dengan menggunakan kuesioner. Hasil analisis menunjukkan variabel beban kerja (Pvalue=0,001), prosedur kerja (Pvalue 0,001), dan supervisi teknis (Pvalue=0,036) mempengaruhi langsung kelengkapan pengisian rekam medis. Variabel yang paling dominan mempengaruhi kelengkapan rekam medis adalah variable dengan nilai coeff. B paling besar yaitu status prosedur kerja dengan coeff B=13,7. Saran yang diberikan kepada RS. Hermina Depok ialah melakukan sosialisasi dan pelatihan pengisian rekam medis secara rutin. Selainitu, melakukan pemeriksaan dan pengawasan dalam hal pengisian rekam medis secara tepat dan sesuai dengan periode yang ditetapkan. One of the indicators of the health service quality in the hospital is the comprehensive information from patients’ medical records. A comprehensive medical record could display the whole patient’s condition which can be used for their next treatment. A medical record, in addition, is also important legal evidence used in the court, provided there is any lawsuit. This research is carried out to illustrate the link between Doctors’ individual characteristics with the completion of patients’ medical records together with other external related factors. Observational study is the method used in the research, with cross sectional data from 50 patients’ medical records in September-November 2011, filled out by 10 specialist doctors; as well as data collection from interview with the use of questionnaires. The Results indicate: workload variable (Pvalue=0,001), work procedures (Pvalue 0.001), and supervisory techniques (Pvalue=0,036) affecting the completion of the medical records directly. The most dominant variable affecting the completion of health records is the one with the highest Coeff B value work procedures with Coeff B value=13.7. Suggestion given to RS HerminaDepok is to perform regular training and socialization, as well as to carry out checks and supervision in regard to the correct completion of patients’ medical records based on the indicated period. Background : Globalisation and boundless information have naturally matured patients’ criticism behaviour. Those develop patients’ knowledge and understanding how to claim medical treatments. They would like to demand services in very high quality level and not hesitate to take legal action for any procedure errors. They sue doctors and hospital should they feel being treated that may harm their body. Since medical record states patient’s identities, anamnesis, physical test, result of supporting test, and list of medicine consumed, the quality of medical records simply describe the service quality and act as fundamental verification in facing a patient complaints in court. In RSUD Bekasi, evaluation of medical record forms shows that there is a high degree of incompleteness of filing medical records. Therefore, this research subjects to explore the grade of incompleteness. Method : The research performs with qualitative descriptive approach in which retrospective analysis for six forms in obstetric unit during 2006. Incompleteness filing is analysed along with factors that suppose involved. Qualitative approach is applied through in-depth interview to unit staffs who have competencies and responsibilities in processing medical records. Result: It confirms that all medical records are not filled completely and in some cases, it found that forms are not in place. Two of seventy-two (approximately 3%) forms of follow-up are not found in medical records. Furthermore, three of seventy-two or 4% forms of result of supporting test are out of medical records. In the course of in-depth interview, it can be concluded that the factors that may cause incompleteness are limited staff in related education background, poor condition of medical records processing room, lack of supporting equipment, half-done in implementing standard operating procedures, lack of socialization of regulations and quidances medical records, and lack of supervision from top management. Coclusion: In order to decrease the number of incompleteness medical records, it is strongly suggested to train more medical records staff, to re-condition processing room and to furnish with supporting equipment, to modificate medical record, and to socialise regulation and guidance in filling medical records. It is also recommended to maximise medical records committee in supervising and controlling function. References : 27 (1980- 2007)
Metode: Penelitian ini merupakan penelitian kuantitatif cross sectional dimana variabel bebas dan variabel terikat diukur secara bersamaan. Metode pengambilan sampel yang akan digunakan adalah teknik sampling jenuh, jadi jumlah sampel yang digunakan adalah keseluruhan jumlah populasi yaitu 80 sampel.
Result: Melalui analisis multivariate menunjukkan bahwa variabel jumlah pasien dan insentif memiliki p value masing-masing 0,009 dan 0,041, dimana p value < 0,05. Sedangkan variabel lainnya nilai p value > 0,05.
Simpulan: Hasil penelitian menunjukkan bahwa variabel-variabel yang memiliki hubungan yang signifikan terhadap tingkat kepatuhan pengisian rekam medis di Rumah Sakit Bali Royal adalah variabel jumlah pasien dan variabel insentif

Introduction: The hospital is one of the facilities or institutions of health service providers (health providers) and in accordance with Law No. 36 of 2009 on Health. One of the conditions that are still often discussed and encountered the current obstacle is the completeness of medical records. This problem is also the attention of researchers after seeing and doing initial research at Bali Royal Hospital. It was found that the completeness of the medical record at Bali Royal Hospital Inpatient Unit is still below the standard set by Ministry of Health stated in Kepmenkes RI No.129 / MENKES / SK / II / 2008 which is 100%. The completeness of medical records for inpatients is crucial to the patient's healing process and maximizing hospital services to patients. Where the level of compliance of doctors in filling out medical records at RS BROS still not reached the target of the hospital that is 90%
Method: This research is a cross sectional quantitative research where independent variable and dependent variable are measured simultaneously. Sampling method to be used is a saturated sampling technique, so the number of samples used is the total population of 80 samples.
Results: Through multivariate analysis showed that the variable number of patients and incentives had p values of 0.009 and 0.041, respectively, where p value 0,05.
Conclusion: The results showed that the variables that have a significant relationship to the compliance level of medical record at Bali Royal Hospital are variable of patient number and incentive variable

Bhakti Yudha Hospital has a turnover of health workers (doctors and nurses) morethan 10% per year. A high turnover rate will be bad for the hospital. The purposeof the analysis of factors related to intention to quit in order to provide a fact andrecommendation to the hospital for this problem.The sample was 100 nurses and25 doctors. The type of research is quantitative research followed by qualitativemethod. The results showed that 34% of health workers have a desire to get out ofthe hospital. From the multivariate analysis, there are four variables that have asignificant influence on the desire to quit the job is job satisfaction, age, theperception of compensation and benefits, and salary. The proportion of healthworkers with low job satisfaction has a greater tendency to intention to quit Theproportion of health workers who have a perception of compensation and benefitsthat are less appropriate, low salaryand the younger tend to quit the job.Jobunsatisfaction also caused by management of employees and career path system isless good.
