ABSTRAK Nama : Ida Ayu Eka Tirta Arini Program Studi : Kajian Administrasi Rumah Sakit Judul : Kelengkapan Pengisian Informed Consent Pada Pasien Operasi di Rumah Sakit Risa Sentra Medika Mataram Tahun 2016 Tesis ini membahas tentang kelengkapan pengisian informed consent pada pasien operasi di Rumah Sakit Risa Sentra Medika tahun 2016 dan faktor – faktor yang berhubungan. Penelitian ini merupakan penelitian kuantitatif dengan rancangan Cross Sectional Study. Sampel diambil secara acak, yaitu 235 lembar informed consent pada rekam medis pasien operasi bulan Januari hingga Desember 2016 di Rumah Sakit Risa Sentra Medika serta pengisian kuisioner oleh 10 orang dokter spesialis. Hasil penelitian menunjukkan bahwa rata – rata pengisian lembar informed consent pada pasien operasi di Rumah Sakit Risa Sentra Medika tahun 2016 adalah 21 item terisi atau sebesar 61% dan bagian yang paling banyak tidak terisi adalah bagian pemberian informasi yaitu sebesar rata – rata 7 item terisi (41%). Bagian informasi yang paling jarang diberikan adalah informasi tentang alternatif tindakan, yaitu hanya sebanyak 51 lembar dari 235 lembar informed consent yang didata (21,7%). Berdasarkan uji korelasi Spearman Rank, faktor yang memiliki hubungan dengan kelengkapan pengisian informed consent tersebut adalah faktor motivasi dan jenis tindakan operasi. Hal yang dapat disarankan adalah melakukan penyegaran pengetahuan tentang informed consent dan komunikasi efektif kepada dokter spesialis; lembar informed consent juga perlu dikaji ulang agar sesuai aturan identifikasi pasien. ABSTRACT Name : Ida Ayu Eka Tirta Arini Study Program : Master of Hospital Administration Title : The Completeness of Informed Consent Sheets In Surgery Patients at Risa Sentra Medika Hospital Mataram in 2016 This thesis discusses the completeness of informed consent in surgery patients at Risa Sentra Medika Hospital in 2016 and related factors. This research is a quantitative research with cross sectional study design. Samples were taken at random, ie 235 informed consent sheets in medical records of surgery patients from January to December 2016 at Risa Sentra Medika Hospital and filled out questionnaires by 10 specialist doctors. The results showed that the average of filling of informed consent sheet in operation patients at Risa Sentra Medika Hospital in 2016 was 21 items filled or 61% and the most unfilled part was the information giving section which was 7 items in average ( 41%). The most rarely informed section of information is information on alternative actions, ie only 51 sheets of 235 informed consent sheets (21.7%). Based on Spearman Rank correlation test, factors that are related to the completeness of the filling of informed consent are motivational factors and types of surgical measures. It may be suggested to refresh knowledge of informed consent and effective communication to a specialist; the informed consent sheet also needs to be reviewed to fit the patient identification rules. Kegiatan logistik di rumah sakit mempunyai peran yang sangat besar karena berkaitan dengan semua unit pelayanan di rumah sakit. Pembelanjaan terbesar rumah sakit setiap bulannya adalah untuk pembelian obat-obatan dan bahan habis pakai. Banyak dan beragamnya item obat yang harus disiapkan untuk pelayanan sehingga mempunyai nilai investasi yang paling besar dengan persediaan lainnya, di Rumah Sakit Risa Sentra medika sekitar 49-56% pembelanjaan obat dalam trimester pertama tahun 2012 dibandingkan dengan total biaya operasional Rumah Sakit Risa Sentra Medika. Gudang farmasi RS Risa Sentra Medika belum melakukan perhitungan safety stock yang sesuai sehingga sering terjadi kekosongan stock. Oleh karena itu, maka untuk menjaga agar stock selalu tersedia saat dibutuhkan maka perlu di adakan sistem pengendalian persediaan obat yang sesuai seperti pengendalian persediaan dengan safety stock. Jenis penelitian ini adalah analitik kualitatif untuk melihat perhitungan safety stock di Rumah Sakit Risa Sentra Medika selama periode Januari hingga Maret 2012. Obat antibiotik yang fast moving di bulan januari dengan jumlah 84 item dengan nilai persediaan sebesar Rp 126.889.911, pada bulan februari sebanyak 93 item dengan nilai persediaan Rp 135.524.014 dan pada bulan maret 2012 di dapatkan 85 item antibiotik yng tergolong fast moving dengan nilai persediaan Rp 117.021.085 berarti jumlah total pembelian dalam tiga bulan adalah Rp 379.435.010 sehingga didapatkan rata-rata pembelian perbulan adalah Rp 126.478.337. Data penjualan ini diolah menjadi rencana anggaran pembelian obat-obatan khususnya golongan antibiotika yang fast moving. Peneliti menyarankan untuk perhitungan safety stocknya menggunakan metode pemakaian maksimum dikurangi pemakaian rata-rata dikalikan dengan lead time. Metode ini sederhana namun bisa diterapkan. Peneliti juga menyarankan untuk membuat rencana anggaran berdasarkan omset.
Logistics activities in hospitals have a very big role as it pertains to all service units in hospitals. Largest expenditure is the hospital every month for the purchase of medicines and consumables. Many and varied items that drugs should be prepared for the ministry which has the largest value of investments with other supplies, in Risa Sentra Medika Hospital of approximately 49-56% drug spending in the first trimester of 2012 compared to the total operational cost of Risa Sentra Medika Hospital. Risa Sentra Medika Hospital pharmacy logistic has not made the appropriate safety stock calculations that are common stock blanks. Therefore, it is to keep the stock is always available when needed it is necessary to invent a drug inventory control system such as inventory control in accordance with safety stock. This type of qualitative research is to see the analytic calculation of safety stock in Risa Sentra Medika Hospital from January to March 2012. Antibiotic drugs are fast moving in January to 84 the number of inventory items with a value of Rp 126 889 911, in February a total of 93 items with a value of Rp 135 524 014 inventories in March 2012 and get 85 items in yng antibiotic belonging to the fast moving supply of Rp 117 021 085 the total number of purchases within three months is Rp 379 435 010 so we get the average purchase per month is Rp 126 478 337. Sales data is processed into the budget plan the procurement of medicines particularly fast moving class of antibiotics. Researchers suggest stocknya safety calculations using the maximum use of reduced consumption multiplied by the average lead time. The method is simple but can be applied. Researchers also suggested creating a budget plan based on turnover.
Pemanfaatan pelayanan bersalin bersifat personal dan dapat dipengaruhi oleh mutu layanan rawat jalan di rumah sakit. Sebanyak 77,5% pasien ibu hamil di unit obstetri rawat jalan RS Risa Sentra Medika tidak memilih bersalin di rumah sakit tersebut. Tujuan penelitian ini untuk mengetahui faktor-faktor yang mempengaruhi pemanfaatan pelayanan bersalin pada pasien ibu hamil di unit obstetri rawat jalan Rumah Sakit Risa Sentra Medika. Dengan desaign cross sectional dengan jumlah sampel 104 reponden. Untuk menggali lebih mendalam permasalahan rendahnya pemanfaatan pelayanan bersalin, penelitian dilengkapi dengan pendekatan kualitatif melalui diskusi kelompok terarah dengan pihak manajemen mengenai permasalahan yang ada. Jarak dan waktu tempuh menjadi variabel yang berhubungan dengan pemanfaatan pelayanan bersalin dari hasil penelitian dengan pendekatan kuantitatif. Berbeda dengan hasil pendekatan kualitatif yang memperlihatkan bahwa justru fasilitas yang lebih mempengaruhi pemanfaatan pelayanan bersalin pada pasien ibu hamil di unit obstetri rawat jalan di rumah sakit ini. Rekomendasi yang dapat diajukan dari penelitian ini diharapkan untuk memperbaiki fasilitas di unit obstetri rawat jalan RS Risa Sentra Medika atau merelokasinya sehingga pasien ibu hamil akan merasa lebih nyaman. Diharapkan manajemen RS Risa Sentra Medika melakukan Customer Relationship Management pada pasien ibu hamil yang melakukan antenatal care di unit obstetri rawat jalan RS Risa Sentra Medika, serta melakukan pendirian klinik rawat inap atau klinik bersalin sebagai jejaring RS Risa Sentra Medika, dengan terlebih dahulu melakukan studi kelayakan.
The Utilization of maternity service are personal and could be influence by the quality of outpatient service in hospitals. As total of 77.5% of pregnant women patients in outpatient obstetric unit at Risa Sentra Medika Hospital choose not to dilivered at this hospital. The goal of this study is to determine the factor affecting the utlization of maternity service for pregnant patients in aoutpatient hospital obstetric unit at Risa Sentra Mediak Hospital. The study was condected with a cross sectional design, the number of samples taken by 104 respondents. To probe about the low utilization of maternity service, this study equipped with qualitative research through focus group discussions with management and a quantitative study. The quantitative study results that distance and time as factors related to service utilization. Through a qualitative approach to the results obtained show a different matter where the facilities is a further factor affecting the utilization of maternity service that may be filed. Recommendations of the study were expected the Risa Sentra Medika Hospital managemant to improve the facilities on an outpatient obstetric unit or relocated so that the pregnant patient will feel more comfortable. Also encourage Risa Sentra Medika Hospital management to maintain Customer Relationship Management in pregnant patients who did antenatal care in hospital outpatient obstetric unit at Risa Sentra Medika Hospital. Another option is to established inpatient clinic or birthing center as a network for Risa Sentra Medika, by first conducating a feasibility study.
Dokter adalah tenaga kesehatan yang memiliki peran dan otoritas dalam penulisan resep obat untuk pasien. Proses seleksi dan pemilihan obat seharusnya dilakukan secara rasional dan mengikuti pedoman panduan obat yang ditetapkan oleh World Health Organization. Masih banyak ditemui peresepan obat di luar formularium di Rumah Sakit Risa Sentra Medika, 70 % dari 100% target penggunaan formularium. Penelitian ini bertujuan mengetahui motivasi dokter dalam penulisan resep. Faktor yang mempengaruhi dilihat dari faktor motivasi instrinsik (persepsi, kepentingan, dan aspirasi) faktor ekstrinsik (diagnosis, konsistensi, dan kerjasama), organisasi (kepemimpinan, sosialisasi, supervisi, fee), industri farmasi (promosi dan imbalan), serta implementasi kebijakan. Penelitian ini dilakukan dengan pendekatan kualitatif dan desain studi kasus. Data primer diperoleh dari wawancara mendalam dan dilengkapi dengan observasi dan telaah dokumen sebagai bentuk triangulasi. Penelitian ini menyimpulkan bahwa motivasi dokter dalam menuliskan resep dipengaruhi oleh banyak faktor antara lain, diagnosis penyakit, kondisi keuangan pasien, imbalan dari hasil kerjasama dengan industri farmasi. Imbalan dari pihak luar memberikan pengaruh yang kuat pada dokter dalam menuliskan resep. Sebagai saran untuk tindak lanjut, diperlukan peraturan yang jelas mengenai penerapan formularium oleh Direktur Rumah Sakit Risa Sentra Medika. Hal lain yang penting untuk diperhatikan dalam proses pelaksanaan penggunaan formularium adalah pemberian imbalan dan sanksi yang jelas bagi para dokter.
Doctor are health profesionals who have a role and authority in prescribing drugs to patients. Selection process and selection of drugs should be done rationally and follow the guidelines established drug guidelines by the World Health Organization. There are mostly found outside the formulary prescriptions at Risa Hospital Medical Center, 70% of the 100% target of the use of formularies. This study aims to find motivation in the prescribing physician. Factors influencing views of intrinsic motivation factors (perceptions, interests, and aspirations), extrinsic factors (diagnosis, consistency, and cooperation), organization (leadership, socialization, supervision, fee), the pharmaceutical industry (promotion and compensation), as well as policy implementation. The research was conducted with a qualitative approach and case study designs. Primary data obtained from interviews and observations and is equipped with a document review as a from of triangulation. This study concluded that the motivation of doctors in prescribing is influenced by many factors, among others, the diagnosis of diseases, the financial condition of the patient, the rewards of collaboration with the pharmaceutical industry. Remuneration from an outside party provides a strong influence on the prescribing physician. As a suggestion for a follow-up, needed clear rules for the application of the formula by the Director Risa Hospital Medical Center. Another important point to consider in the process of implementing the use of formularies is giving a clear rewards and sanctions for physicians.

In today's highly competitive market in hospitals, marketing strategies are needed that focus more on the objectives of their activities on partnership relationships. The implementation of customer relationship marketing aims to find the lifetime value (LTV) of customers and further improve operational efficiency. The purpose of this thesis is to know the role of customer care to customer relationship marketing of patient companies and insurance IKS RS Risa Sentra Medika Mataram. The identification and evaluation of this marketing strategy is done by system approach. The type of this research is qualitative descriptive research. The results of this study found that has been done customer relationship marketing efforts through customer care activities, has not been optimal. In that case, can be seen from the performance of human resources, organizational structure, budget, facilities that support customer care activities that are not according to standards. In the implementation of customer relationship marketing apply 4 concepts that are developing trust, bonding commitment, communication and conflict handling, these efforts have been done even though they have not met the target. It requires commitment from all parties in the organization, from the board of directors to the executor level, as well as synergistic cooperation from the internal company and also outside parties.
