PT. X is one of the companies engaged in the oil and gas distribution sector. The main process of PT. X is the receipt, hoarding and distribution of fuel oil (BBM) to the general public. In the operational process, PT. X uses the services of third parties (contractors) in carrying out the procurement process of goods and services, such as new installations and equipment maintenance. The procurement process of goods and services has different levels of risk according to the scope of work and the potential dangers faced. The large number of projects, the high pressure to work to meet production targets with a narrow time, operational activities that cannot stop, and coupled with the limited number of human resources to supervise aspects of work safety, can allow the emergence of substandard actions during the project. This study uses the theory of Loss Causation Model and Mixed Method by combining questionnaire and interview data to strengthen the research results. The factors studied are personal factors and work factors that can cause substandard action. From the results of the study, it was obtained that the factors of education, supervision, length of service and knowledge are factors that influence the emergence of substandard action in the PT. X. Several improvement programs are needed to follow up on the results of research based on questionnaires and interview results that deepen the condition of management control during the research.
Latar belakang: Penggunaan komputer dapat menimbulkan suatu keluhan kesehatan yang disebut dengan Computer Vision Syndrome (CVS), Sindrom ini dapat dipengaruhi oleh berbagai faktor risiko individual, lingkungan dan komputer.
Tujuan: Mengidentifikasi dan menganalisis faktor-faktor resiko ergonomik individual dan komputer yang berhubungan dengan kejadian Computer Vision Syndrome (CVS) pada pekerja pengguna komputer yang berkacamata dan pekerja yang tidak berkacamata.
Metode: Penelitian ini merupakan penelitian metode kualitatatif. Penelitian dilakukan pada bulan April - Mei 2013 di Unit Pelakasana dan Pelatihan. Sampel sebanyak 18 orang dengan kriteria tertentu, dibagi menjadi 2 kelompok pekerja berkacamata dan pekerja yang tidak berkacamata. Peneliltian dilakukan dengan wawancara langsung menggunakan kuesioner dan pengukuran.
Hasil: Faktor-faktor yang berhubungan dengan kejadian CVS adalah Kelembaban 71%, Pencahayaan kurang dari 300-500 lux (KEPMENKES nomor 1405/Menkes/SK/XI/2002), Usia lebih dari 40 tahun (das et al.), lama bekerja dengan komputer, dan jarak komputer dengan mata.
Kesimpulan: Gejala ekstraokuler pada pekerja pengguna kacamata bifocal melakukan retrofleksi leher sehingga leher tertekuk kebelakang yang menyebabkan keluhan nyeri pada leher. Penderita terbanyak bukan dari pengguna kacamata tetapi pada pekerja yang tidak berkacamata. Serta penderita CVS (berdasarkan kriteria anamnesa) di usia 25 tahun, kedua hal ini berkaitan dengan potur ergonomi pada saat kerja baik secara design tempat kerja, kondisi ruangan ataupun durasi kerja yang semuanya saling berkaitan sehingga menimbulkan gejala Computer Vision Syndrome (CVS).
Background: Computer usage could cause health complaints called Computer Vision Syndrome (CVS). This syndrome was influenced by individual and computer risk factors.
Aim: The objective of the study is to identify and to analyze individual and computer factors of computer Vision Syndrome (CVS).
Methods: This study was an observational study with methods qualitatively. The research was conducted in April-May 2013 in the Pelakasana and Training Unit. Sample of 18 people with certain criteria, divided into 2 groups of workers and workers who are not wearing glasses glasses. Peneliltian done by direct interviews using questionnaires and measurements.
Results: Factors associated with the incidence of CVS is Humidity 71%, less than the 300-500 lux lighting (KEPMENKES 1405/Menkes/SK/XI/2002), age over 40 years (das et al.), Long working computers, and computer distance by eye.
Conclusion: Extraocular symptoms in workers bifocal glasses users do retrofleksi neck so the neck is bent backwards which causes pain in the neck. Most patients but not from users goggles to workers who do not wear glasses. And people with CVS (based on criteria anamnesis) at the age of 25 years, these two things related to ergonomic posture at work both in design work, ambient conditions or duration of action that are all intertwined, giving rise to symptoms of Computer Vision Syndrome (CVS).
The aim of this study was to determine the factors that led to occupational skindisease complaints on Sewing and Cutting workers at the Preparing/ Upper SoleDepartment, one of the shoe manufacturing in Sukabumi, May 2016. Out of the1.350 respondents, found that 777 workers suffering from occupational skindisease complaints, while 573 others do not suffer from this complaint. Using thesystematic random sampling technique, obtained a sample of 817 workers, ofwhich the result showed 58% of them suffer from occupational skin diseasecomplaints. Statistically there was no significant association between exposures toorganic solvents with occupational skin disease complaints in workers.Furthermore, the study result indicates that workers exposed to organic dust 2.5times are at risk of suffering from occupational skin disease complaints. Workerswith ≤ 3service years had 2.4 times the risk of developing occupational skindisease complaints compared to workers who have > 3 years of service. Workerswho have the habit of not washing their hands have 2.6 times the risk ofoccupational skin disease complaints. Workers who do not wearing gloves are atrisk 4.7 times of occupational skin disease complaints, and workers with a historyof allergies had 6.7 times risk to occupational skin disease complaints. Controlcan be done by educating the workers and do the monitoring, as well asadministrative control and provided the facilities in health promotion andoptimum preventive, such as to provide a sink, use appropriate PPE, screeningand do the treatment as well.
Construction is one of the sectors that has a high risk of work accidents, in 2020 this sector has 55.2% accident rate. Human factor is one of the factors causing work accidents, 70% of work accidents occur due to unsafe actions. The purpose of this study is to see the Project Z PT X risk perception level, the relationship between independent and dependent variable, and to explain the condition of risk perception that has been run by Project Z. This research is a semi-quantitative study with a cross-sectional design. The population of this study was 82 people with a sample size of 67 people. The results showed that most workers perception in Project Z were bad (55.7%). Risk voluntarily, immediacy of effect, knowledge of risk, catastrophic potential, and severity of consequences have the largest bad percentage, and there is a significant relationship on all research variables. Multivariate analysis explained that 4 variables had the most significant relationship, namely immediacy of effect, catastrophic potential, common dread, and severity of consequences. For short-term solutions, Project Z can improve supervision on high risk categorized job, for long term Project Z can improve hazard and risk identification and communicating them to all workers

This thesis discusses the analysis of work related dan non work related risk factorstowards fatigue of construction workers in a high-rise building project in the Jakarta.Many factors that cause accidents in the construction sector, one of them is fatigue canbe affected by work and non-job risk factors. Analysis of the relationship between riskfactors and fatigue that occurs becomes important as a baseline of data in an effort toreduce accidents in the construction sector. This research is a quantitative research withcross sectional design. The results of this study indicate that there is a significantrelationship between occupational risk factors: duration of work, psychosocial factors(effort, Reward, social support, job satisfaction, work stress) and non-work factors(quantity and quality of sleep) to the fatigue of Project X construction workers.

This study discusses the safety risk assessment in construction work atheight in Project Bogor Valley Residence & Hotel PT. X 2014. Risk assessment isfocused to the risk that will be experienced by workers on the job, especially atthe height of finishing, namely: painting by using the gondola, the installation ofrailing at the edge of the building up to a height of 20 floors, billboards usingscaffholding, and Passenger hoist.Risk assessment is done by analyzing the probable value, exposure andconsequences of each phase of the work which is then compared to the standardsemi-quantitative risk level W.T Fine. J to determine the level of risk involved ineach stage of the production process. The study was a descriptive analytic studyusing semi-quantitative method AS/NZS 4360:2004. Design research is anobservational cross-sectional approach. The collection of data obtained fromobservations and interviews. The study states that the level of risk in work atheight includes very high level, priority 1 level, substantial level, priority 3 levelandacceptable level.Keywords:AS/NZS 4360:2004, risk assessment, construction, work at height.
