Globally, the mining industry has long been considered one of the most dangerous industries with great health and safety risks to its workers. The frequency of work accidents can be prevented by planning, implementing and monitoring Occupational Safety and Health (K3) in the workplace as well as the behavior of workers in particular and the community in general, who are starting to realize the importance of implementing OSH norms in the workplace. Maturity level of safety culture (safety maturity level) is very important for the prevention of unsafe behavior, especially various industrial sectors with high rates of injury and death. This concept aims to assist organizations in determining the maturity level of their current safety culture and identify the actions needed to improve their safety culture, so that the dimensions and aspects of safety culture need to be improved . The purpose of this study is to design a cultural measurement instrument that is appropriate to the conditions in the PT. XYZ with 10 research variables commitment & support, Leadership, policy & strategy, engagement & involvement, value, safety cost, competency & training, information & communication and safety performance, measured safety culture at PT. XYZ is based on a safety culture maturity level model, and looks at the relationship between safety culture dimensions. The method used in this study is a mix method, quantitative and literature study to select the dimensions of safety culture which are used as research variables with research sample of 123 people. The results were analyzed using Structural Equation Modeling (SEM). The results of the study show that the assessment of the safety maturity level with 10 (ten) dimensions of safety culture at PT. XYZ is at point 3.56 (Compliant to Proactive), there are 9 (nine) hypotheses that can be accepted, there is a significant relationship between Leadership (L) and Policy & Strategy (PS), Policy & Strategy (PS) dimension with Safety Cost (SC), Commitment & Support (CS) dimension with Process (P), Leadership dimension (L) with Engagement & Involvement (EI), Safety Cost dimension (SC) with Competency & Training (CT), Process dimension (P) with Information & Communication (IC), Engagement & Involvement (EI) dimension with Value (V), Competency & Training (CT) dimension with Safety Performance (SP) , Value (V) with Safety Performance (SP) and 1 hypothesis which is not accepted, there is no significant relationship between Information & Communication (IC) and Safety Performance (SP) dimensions.
Kecelakaan tambang di Indonesia menunjukkan beban yang perlu dikendalikan secara sistemik, melalui efektivitas pengukuran awal Safety Maturity Level (SML). Penelitian ini bertujuan menganalisis hubungan antara SML dan kinerja keselamatan pertambangan yaitu indikator kecelakaan (Frequency Rate dan Severity Rate), sekaligus menjelaskan faktor konteks yang memengaruhi kekuatan hubungan tersebut. Penelitian menggunakan desain mixed methods - explanatory sequential. Fase kuantitatif memanfaatkan data sekunder dari Kementerian ESDM (KESDM) tahun 2024 di 52 perusahaan tambang batubara tahap operasi produksi sesuai kriteria inklusi, yaitu skor SML berbasis KepDirjen Minerba nomor 10.K/MB.01/DJB.T/2023 dan data statistik kecelakaan tambang 2024. Uji normalitas data menunjukkan tidak terdistribusi normal, sehingga analisis menggunakan pendekatan nonparametrik Spearman rho (ρ) dan GLM gamma. Hasil kuantitatif mengindikasikan bahwa SML secara total (agregat) berkorelasi negatif dan signifikan dengan kinerja keselamatan (indeks kecelakaan tambang), namun kontribusi tiap dimensi SML tidak seragam. “Upaya Pengendalian” merupakan indikator yang paling konsisten bersifat protektif terhadap indeks kecelakaan, sedangkan “Partisipasi Pekerja” tidak menunjukkan hubungan yang signifikan namun korelasi negatif; sementara itu, “Tanggung Jawab Pimpinan” dan “Analisis Statistik”
menunjukkan hubungan negatif (bivariat) dan positif (multivariat), serta signifikan terhadap indeks kecelakaan tambang. Selanjutnya, temuan kualitatif dari data sekunder KESDM terhadap sub-sampling perusahaan menegaskan bahwa SML tinggi tidak selalu diikuti penurunan kecelakaan ketika terdapat kesenjangan implementasi pengendalian, terutama pada tata kelola kontraktor, kompetensi, dan konsistensi eksekusi di lapangan. Penelitian menyimpulkan bahwa SML berdampak pada penurunan kecelakaan terutama jika diwujudkan dalam pengendalian risiko yang nyata dan pembelajaran berbasis data yang ditutup dengan tindakan (closed-loop learning), bukan sekadar pemenuhan administratif.
Mining accidents in Indonesia indicate a burden that must be controlled systemically through the effectiveness of early measurement of the Safety Maturity Level (SML). This study aims to analysis the relationship between SML and mining safety performance represented by accident indicators (Frequency Rate and Severity Rate) while also explaining contextual factors that influence the strength of this relationship. The research employs a mixed-methods design with an explanatory sequential approach. The quantitative phase utilizes secondary data from the Ministry of Energy and Mineral Resources (KESDM) for 2024, covering 52 coal mining companies at the production operation stage that met the inclusion criteria, namely SML scores based on the Director General of Mineral and Coal Decree No. 10.K/MB.01/DJB.T/2023 and 2024 mine accident statistics. Data normality tests showed non-normal distributions; therefore, analysis was conducted using nonparametric Spearman’s rho (ρ) and a gamma Generalized Linear Model (GLM). Quantitative results indicate that overall (aggregate) SML is negatively and significantly correlated with safety performance (the mining accident index), but the contribution of each SML dimension is not uniform. “Control Measures” emerged as the most consistently protective indicator against the accident index, whereas “Worker Participation” showed no significant relationship, although the correlation direction was negative. Meanwhile, “Leadership Responsibility” and “Statistical Analysis” demonstrated negative (bivariate) and positive (multivariate) relationships, and both were significant with respect to the mining accident index. Furthermore, qualitative findings derived from secondary KESDM data on a subsample of companies confirm that a high SML does not always translate into reduced accidents when gaps in the implementation of controls persist—particularly in contractor governance, competency, and consistency of execution in the field. This study concludes that SML contributes to accident reduction primarily when it is translated into tangible risk controls and data-driven learning that is closed with corrective action (closed-loop learning), rather than merely fulfilling administrative requirements.
The implementation of Occupational Health and Safety (OHS) in the company can be measured with how many accidents happened each year and many professionals have developed leading indicators as safety culture to prevent these. The company focused on oil and gas sector has also potential risk to fires, explosions, environmental poollution and other work accidents. This study aims to provide comprehensive overview of safety culture implementation in the workplace, in particular oil and gas refining company and will be utilized as safety behaviour to achieve target set by the company. The research study included 356 workers in both office and field through online survey asking for demographic items and safety climate dimensions. The statistical analysis was performed with independent-samples T test comparing safety climate items. The study resulted the safety climate in the dimensions of the organization, work and individual earned values of 4.23, 3.98 and 4.36, respectively. Based on the safety factor, Personal Priorities and Need for Safety (PPNS) in general having highest score perception among others, while work environment has lowest score. The mean comparison showed there was no significant among safety climates based on work location and education. Meanwhile the variable of management position indicated mean difference including management commitment, communication, supportive environment, involvement, personal priorities and need for safety, and work environment. In addition, Three categories of common finding from Safety Observation (PEKA): equipment and supplies around 61.29%, environmental conditions 25.32%, and Personal Protective Equipment 5.34%. From the measurement results of the Safety Culture Maturity Level at PT XYZ, it can be seen that PT XYZ is at a calculative level with a value of 3.04. In terms of position level: upper management 3.1 and lower management 2.98.
The chemical manufacturing industry is one of the industrial sectors with high hazard potential and complex work processes, requiring the implementation of a strong occupational safety system. One critical aspect that must be explored in depth is the level of safety culture maturity within a company. This study aims to analyze the maturity level of safety culture at PT. XYZ and identify the differences in perception and engagement among employees. The assessment was conducted using a questionnaire instrument consisting of various safety culture dimensions and elements, supported by data from interviews, focus group discussions, and document reviews. Based on the analysis results, the overall safety culture maturity level at PT. XYZ was found to be at level 3 or Calculative. This indicates that while the company has a structured and well-documented safety management system, safety values have not yet been fully internalized into daily work behaviors. In addition, the study found significant differences in perceptions across job levels and departments, where workers in operational roles had limited involvement in safety evaluation and organizational learning. Areas that require improvement include safety communication effectiveness, employee participation in incident analysis, and consistency in follow-up actions from findings and risk evaluations.
This thesis research analyzes the OHS climate at PT. XYZ, an oil and natural gas exploration and production company. Company data shows that there were 16 recordable injury (RI) cases in 2019, 9 RI cases in 2020 and 2 RI cases up to 31 August 2021 which were caused by lack of awareness, vigilance and complacency. OHS climate survey at PT. XYZ is carried out to get the latest OHS climate portrait, analyze weaknesses and provide recommendations for improvement to decrease the accidents. A cross-sectional study with a quantitative approach and research methods on OHS Climate was used by distributing questionnaires survey via Google Form and sample selection by proportional cluster random sampling involving 322 respondents. The survey results show that the average value of the company's OHS climate is 4.12 on a scale of 1-5 and average value for each variable >3.75 indicating that workers perceive that safety values have been optimally internalized as individuals, groups and organizations. The focus of improvement is on group variables which have 2 sub-variables below the average value. There was a significant difference in worker status group (Supportive Environment p-value 0.048, Involvement p-value 0.001) and work location group (Management Commitment p-value 0.018).
