Abstrak:
Sektor konstruksi memiliki risiko kecelakaan kerja tinggi. Studi ini mengembangkan pengukuran sistem manajemen k3 konstruksi menggunakan kriteria sistem manajemen keselamatan konstruksi sesuai peraturan Permen PUPR No.10/2021, dapat digunakan dalam menilai keselamatan di industri konstruksi. Metodologi menggunakan pendekatan mixed-method exploratory design. Hasil studi menunjukkan bahwa perusahaan BUMN karya yang diobservasi menerapkan sistem manajemen keselamatan konstruksi berdasarkan peraturan yang berlaku dan dilaksanakan berbeda-beda sesuai visi misi perusahaan. Meskipun pengukuran belum dilakukan dengan aplikasi khusus, namun penelitian menunjukkan bahwa pengukuran dapat dilakukan melalui aplikasi yang dikembangkan. Faktor-faktor kompetensi, SOP, dan SMKK 10/21 ditemukan mempunyai pengaruh yang signifikan terhadap penerapan sistem manajemen keselamatan konstruksi.

This study focuses on developing a measurement for assessing the construction safety management system in the construction sector, known for its high risk of work accidents. The researchers used the criteria outlined in Permen PUPR No.10/2021 to evaluate safety in the industry. The study employed a mixed-method exploratory design approach. The findings indicate that state-owned construction companies adhere to the construction safety management system based on relevant regulations, albeit with variations depending on each company's vision and mission. While a specific application of the measurement has not been conducted, the study demonstrates that it is feasible to measure safety in construction through the developed tool. Abstrak:
Posisi geografis Indonesia pada jalur Cincin Api Pasifik menimbulkan risiko bencana alam tinggi bagi sektor pertambangan batubara yang beroperasi di wilayah rawan. Sektor pertambangan batubara belum memiliki instrumen penilaian komprehensif untuk mengevaluasi Emergency Management Capability (EMC) yang terintegrasi dengan Sistem Manajemen Keselamatan Pertambangan (SMKP). Penelitian ini bertujuan mengembangkan instrumen EMC guna mengukur kesiapsiagaan organisasi terhadap potensi kondisi darurat seperti ledakan, longsor, dan banjir. Melalui desain mixed methods dengan pendekatan explanatory sequential, pengembangan instrumen ini merujuk pada Keputusan Direktur Jenderal Mineral dan Batubara Kementerian Energi dan Sumber Daya Mineral No. 185 (2019) dan No. 10 (2023), serta International Organization for Standardization (ISO) 22320:2018. Hasil penelitian menunjukkan bahwa EMC terdiri dari lima variabel, yaitu identifikasi dan penilaian potensi keadaan darurat, pencegahan, kesiapsiagaan, respons, dan pemulihan keadaan darurat. Instrumen terbukti valid dan reliabel. Pembobotan berbasis Confirmatory Factor Analysis (CFA) menunjukkan kesiapsiagaan keadaan darurat sebagai dimensi dominan (35%). Penelitian ini juga menghasilkan klasifikasi tingkat kapabilitas EMC ke dalam lima level, yaitu dasar, reaktif, terencana, proaktif, dan adaptif atau resilient. Selain itu, hasil pengukuran EMC dapat diintegrasikan ke dalam audit SMKP Minerba. Instrumen AJENG diharapkan dapat digunakan sebagai alat evaluasi untuk memetakan kesiapan organisasi, memperkuat mitigasi, dan meningkatkan pengelolaan kedaruratan pada pertambangan batubara.

Indonesia’s location along the Pacific Ring of Fire exposes coal mining operations to significant disaster risks. However, the sector lacks a comprehensive instrument for assessing Emergency Management Capability (EMC) that is integrated with the Mining Safety Management System (SMKP). This study aimed to develop an EMC assessment instrument to measure organizational preparedness for potential emergencies, including explosions, landslides, and flooding. A mixed-methods study using an explanatory sequential design was conducted. Instrument development was guided by the Decree of the Director General of Mineral and Coal No. 185 of 2019, No. 10 of 2023, and ISO 22320:2018. The results identified five EMC dimensions: emergency identification and risk assessment, prevention, preparedness, response, and recovery. The AJENG instrument demonstrated satisfactory validity and reliability. Confirmatory Factor Analysis (CFA) indicated that preparedness was the most influential dimension, accounting for 35% of the overall construct. The study also established five EMC capability levels: Basic, Reactive, Planned, Proactive, and Adaptive (Resilient). Furthermore, EMC assessment results can be integrated into the SMKP audit framework. The AJENG instrument is expected to support organizational preparedness assessment, strengthen risk mitigation, and enhance emergency management performance in the coal mining sector.

Abstrak:
Kelelahan dalam operasi tambang merupakan isu yang serius dan merupakan kontributor signifikan untuk terjadinya kecelakaan. Secara hukum perusahaan yang mengoperasikan tambang batubara harus mengembangkan dan mengimplementasikan strategi untuk mengendalikan setiap risiko keselamatan dan kesehatan yang berhubungan dengan kelelahan pada pekerja. Penelitian dilakukan untuk mengembangkan instrumen penilaian kinerja manajemen risiko kelelahan di perusahaan tambang batubara di Indonesia. Pendekatan studi merupakan gabungan antara pendekatan kualitatif untuk menemukan indikator kinerja dan kuantitatif untuk menguji validitas, reliabilitas dan kualitas instrumen penilaian kinerja manajemen risiko kelelahan di perusahaan tambang batubara. Sampel sebanyak 90 perusahaan tambang batubara yang ada di Indonesia. Penelitian menghasilkan instrumen penilaian kinerja manajemen risiko kelelahan di perusahaan tambang batubara yamg terdiri dari 31 indikator, dengan validitas dan reliabilitas instrumen sudah memenuhi persyaratan. Hasil analisis kurva ROC diperoleh cut off point 73 dan AUC 71,3% yang artinya skor kinerja MRK memiliki kekuatan prediksi sedang untuk terjadinya kecelakaan karena kelelahan. Ditemukan kinerja manajemen risiko kelelahan di perusahaan tambang batubara dengan kategori kurang baik 45,6% dan baik 54,4% Kategori kinerja manajemen risiko kelelahan berkategori kurang baik paling banyak terdapat pada perusahaan yang jumlah karyawannya Fatigue in mining operations is a serious issue and a significant contributor to accidents. According to the law, companies operating coal mines must develop and implement strategies to control any safety and health risks associated with worker fatigue. This research was conducted to develop an instrument for assessing the performance of fatigue risk management in coal mining companies in Indonesia. The study approach combines a qualitative approach to find performance indicators and a quantitative one to test the validity, reliability, and quality of fatigue risk management performance assessment instruments in coal mining companies. The sample is 90 coal mining companies in Indonesia. The study produced a tool for assessing the performance of fatigue risk management in coal mining companies consisting of 31 indicators, with the validity and reliability of the instrument meeting the requirements. The results of the ROC curve analysis obtained a cut off point of 73 and an AUC of 71.3%, which means that the Fatigue Risk Management performance score has moderate predictive power for accidents due to fatigue. It was found that the fatigue risk management performance in coal mining companies was in the poor category (45,6% and 54,4% good). From the type of company permits, the fatigue risk management performance category was in the good category, the most in companies with IUJP permit types 80% and the poor category the most in companies with Production Operation IUP permit types 73,7%. It was found that there was a relationship between fatigue risk management performance with the number of employees and the type of company permit. In order to measure, monitor, and evaluate the performance of fatigue risk management in coal mining companies, it is hoped that the Indonesian Ministry of Energy and Mineral Resources and Indonesian coal mining companies will implement the fatigue risk management performance evaluation tool.

Abstrak:
Kebijakan manajemen untuk pasien gawat darurat klinis yang dilaksanakan oleh Public Safety Center (PSC) Indonesia telah ditetapkan dengan peraturan Menteri Kesehatan RI No. 19 tahun 2016. Penilaian manajemen gawat darurat klinis untuk PSC 119 di Indonesia perlu dilakukan untuk perbaikan manajemen gawat darurat klinis secara berkelanjutan. Penulis telah menyusun indikator penilaian manajemen gawat darurat klinis melalui kajian literatur dan forum grup diskusi dengan diikuti tenaga ahli PSC di Indonesia. Dari kajian literatur dan forum grup diskusi serta dilakukan uji analisisanalisis faktor konfirmatori dengan data yang terkumpul dari responsden, diketahui 28 indikator penilaian yang valid dan reliable (dapat diandalkan) untuk manajemen gawat darurat klinis di PSC dan 8 variabel yang menjadi aspek penilaian manajemen gawat darurat klinis di PSC. Dalam tulisan ini disampaikan hasil uji validitas dan reliabilitas dari analisis faktor konfirmatori terhadap 88 responsden di PSC. Semua variable, yaitu aspek kebijakan, perencanaan, implementasi gawat darurat, sistem komunikasi dan pertolongan, sistem transportasi gawat darurat, sistem rujukan, tinjauan ulang manajemen, dan pelayanan gawat darurat dapat dinyatakan bahwa semua pertanyaan atau indikator dalam setiap variabel valid dan reliable dengan uji analisis faktor konfirmatori. Diketahui bahwa terdapat hambatan dan tantangan pada pelaksanaan manajemen gawat darurat klinis di PSC berdasarkan data nasional. Hasil penelitian ini dapat menjadi masukan untuk pengembangan manajemen gawat darurat klinis secara berkelanjutan. Management policy used when administering clinical emergency patients by Indonesia’s Public Safety Center (PSC) is regulated in Minister of Health Regulation No. 19 of the year 2016. Therefore, clinical emergency management assessment for Indonesia’s 119 PSCs is crucial for continous subsequent improvements. By reviewing literatures, conducting group discussions with experts from the PSCs and analyzing confirmatory factors, 8 variables with 28 valid and reliable indicators to assess the clinical emergency management of PSCs are found. The results of the validity and reliability test of confirmatory factor analysis were presented to 88 PSC responsdents. On all variables, namely aspects of policy, planning, emergency implementation, communication and rescue systems, emergency transportation systems, referral systems, management reviews and emergency services, it can be stated that the question items or indicators in each variable are valid and reliable with confirmatory factor analysis. All obstacles and challenges in implementing clinical emergency management in PSC will be an improvement and development of clinical emergency management. Abstrak:
Pendahuluan: Penyakit Paru Obstruktif Kronik (PPOK) merupakan masalah kesehatan masyarakat yang sering tidak terdeteksi pada tahap awal, terutama di fasilitas kesehatan tingkat pertama. Dibutuhkan instrumen skrining yang valid, sederhana dan mudah yang sesuai dengan karakteristik masyarakat Indonesia untuk meningkatkan penemuan kasus secara dini. Penelitian ini bertujuan menyusun dan mengembangkan instrumen skrining PPOK baru, yaitu TAMI (insTrumen skrining PPOK pAda Masyarakat berIsiko), serta membandingkan validitasnya dengan instrumen PUMA (Prevalence Study and Regular Practice, Diagnosis and Treatment Among General Practitioners in Populations at Risk of COPD). Metode: Penelitian menggunakan desain exploratory sequential mixed methods, dimulai dengan penelitian kualitatif dan dilanjutkan dengan kuantitatif. Pada penelitian kualitatif menggunakan pendekatan qualitative case study dalam kerangka general inquiry, dilakukan melalui wawancara mendalam dan panel expert judgment untuk menyusun instrumen baru. Penelitian kuantitatif menggunakan desain studi kasus-kontrol, melibatkan 200 responden (100 kasus dan 100 kontrol) berisiko PPOK yang berkunjung ke Poliklinik Paru RSUD Jakarta pada Januari-Mei 2025 untuk pengisian instrumen dan pemeriksaan spirometri sebagai gold standard. Analisis mencakup uji bivariat dan multivariat, serta uji sensitivitas, spesifisitas, nilai prediksi positif (PPV), nilai prediksi negatif (NPV), Youden Index, dan area under the curve (AUC). Hasil: Instrumen TAMI menunjukkan kinerja diagnostik yang baik dengan AUC 0,773, serta keseimbangan sensitivitas dan spesifisitas yang stabil. Instrumen PUMA memiliki AUC 0,736 dengan sensitivitas dan spesifisitas lebih rendah dibandingkan TAMI. Nilai PPV dan NPV yang lebih besar pada TAMI juga menunjukkan kemampuan diskriminasi yang lebih baik dalam mengidentifikasi dan membedakan kasus PPOK dibandingkan PUMA. Kesimpulan: Instrumen TAMI terbukti valid dan dapat menjadi alat skrining PPOK yang lebih sesuai digunakan di fasilitas kesehatan tingkat pertama (FKTP) di Indonesia. TAMI juga lebih praktis sebagai instrumen skrining PPOK untuk penggunaan rutin di masyarakat. Implementasi TAMI di FKTP diharapkan meningkatkan penemuan kasus PPOK dan memperkuat upaya pencegahan serta edukasi pada masyarakat berisiko.

Background: Chronic Obstructive Pulmonary Disease (COPD) is a major public health problem that is often underdiagnosed in its early stages, particularly in primary healthcare settings. A valid, simple, and user-friendly screening instrument tailored to population characteristics in Indonesia is needed to improve early case detection. This study aimed to develop a new COPD screening tool, TAMI (insTrumen skrining PPOK pAda Masyarakat berIsiko), and to compare its validity with the PUMA instrument (Prevalence Study and Regular Practice, Diagnosis and Treatment Among General Practitioners in Populations at Risk of COPD). Methods: This study employed an exploratory sequential mixed-methods design, beginning with a qualitative phase followed by a quantitative phase. The qualitative phase used a qualitative case study approach within a general inquiry framework, conducted through in-depth interviews and expert judgment panels to construct the new instrument. The quantitative phase applied a case–control study design involving 200 respondents (100 cases and 100 controls) at risk for COPD who attended the Pulmonary Clinic of RSUD Jakarta between January and May 2025. All respondents completed the screening instruments and underwent spirometry as the gold standard. Data analysis included bivariate and multivariate testing, as well as assessment of sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), Youden Index, and area under the curve (AUC). Results: The TAMI instrument demonstrated strong diagnostic performance with an AUC of 0.773 and a stable balance between sensitivity and specificity. The PUMA instrument showed a lower AUC of 0.736, along with lower sensitivity and specificity compared with TAMI. Higher PPV and NPV values for TAMI further indicated superior discriminatory ability in identifying and excluding COPD cases compared with PUMA. Conclusion: The TAMI instrument is valid and suitable for use as a COPD screening tool in primary healthcare facilities in Indonesia. It also offers practical advantages for routine community-based screening. Implementation of TAMI in primary healthcare settings is expected to enhance early COPD detection and strengthen preventive and educational efforts among at-risk populations.

Abstrak: Kelelahan kerja berdampak besar terhadap kinerja, keselamatan, dan kesehatan tenaga kesehatan rumah sakit. Penelitian ini mengembangkan model sistem manajemen risiko kelelahan berbasis pendekatan kualitatif dan kuantitatif melalui studi literatur, FGD, wawancara, dan observasi. Hasilnya adalah model ICHAFIT (Integrated Collaboration Healthcare Adaptability for Fatigue Intervention and Tracking) dengan lima elemen utama dan strategi pencegahan berbasis data. Terdapat 24 indikator valid dan reliabel untuk menilai implementasinya. Model ini berfungsi sebagai kerangka konseptual dan alat praktis, serta menghasilkan policy brief untuk advokasi kebijakan nasional.
Work fatigue significantly affects hospital workers' performance, safety, and health. This study developed a fatigue risk management model using qualitative and quantitative approaches through literature review, FGD, interviews, and observations. The result is the ICHAFIT model (Integrated Collaboration Healthcare Adaptability for Fatigue Intervention and Tracking) comprising five key elements and a data-driven prevention strategy). It includes 24 valid and reliable indicators to assess implementation. ICHAFIT serves as both a conceptual framework and practical tool, and also produced a policy brief to support national advocacy for fatigue risk management in hospitals.

Abstrak:

Pendahuluan : Sistem Manajemen Keselamatan dan Kesehatan Kerja (SMK3) yang mengelola dan mengontrol keselamatan dan kesehatan kerja menjadi tanda tanya akan efektivitas penerapannya, dengan kondisi saat ini yakni Kecelakaan kerja yang masih terus meningkat, yang menyebabkan peningkatan biaya, berkurangnya produktivitas, hingga mempengaruhi daya saing suatu negara. Tujuan penelitian ini adalah untuk menilai tingkat penerapan Sistem Manajemen K3 dan menganalisis hubungannya dengan kinerja K3 (safety climate, incidence rate, frequency rate, dan severity rate) di perusahaan.
Metode : Penelitian ini menggunakan pendekatan kuantitatif dengan design cross sectional study pada perusahaan yang memenuhi kriteria penerapan SMK3 dan beroperasi di wilayah Kalimantan Timur, sebanyak 94 perusahaan. Total responden untuk pengukuran safety climate sebanyak 8.055 orang.
Hasil : Tingkat penerapan Sistem Manajemen K3 perusahaan di Kalimantan Timur berada pada Level 3/Konsisten yaitu penerapan SMK3 yang pendekatan sistematis dan konsisten, upaya pengendalian risiko dilakukan secara terstruktur, terukur secara kualitatif, integrasi antarprosedur dan manajemen risiko berjalan di semua divisi. Penerapan sistem pembelajaran melalui monitoring, laporan K3, dan proses perbaikan belum berjalan secara menyeluruh. Komponen tertinggi di Hazard control and prevention, dan skor terlemah di Education and training. Safety climate berada dalam kondisi baik, dengan skor tertinggi di dimensi 6 (Pembelajaran komunikasi dan inovasi) serta skor terendah di dimensi 5 (Prioritas keselamatan pekerja dan tidak ditoleransinya risiko bahaya). Faktor safety climate dan kinerja K3 (incidence rate dan frequency rate) memiliki hubungan signifikan dengan tingkat Sistem Manajemen K3 yang diterapkan di perusahaan. Terdapat hubungan antara tingkat penerapan SMK3 dengan safety climate, incidence rate, dan frequency rate berdasarkan tersertifikasi SMK3.
Kesimpulan : Semakin baik tingkat penerapan SMK3 pada suatu perusahaan maka akan menghasilkan kinerja K3 yang lebih baik. Pada perusahaan yang tersertifikasi SMK3 namun penerapan SMK3 masih pada tingkat Adhoc & Coping akan menghasilkan kinerja K3 yang buruk (safety climate membutuhkan perbaikan; incidence rate dan frequency rate cenderung meningkat).


Introduction: Work accidents that continue to increase has caused the increased costs, reduced productivity, and affect the competitiveness of a country, making the Implementation of Occupational Safety and Health Management System a question mark about its effectiveness. The purpose of this study was to assess the level of implementation of the OSH Management System and analyze its relationship with OSH performance (safety climate, incidence rate, frequency rate, and severity rate) in the company.
Method: This study uses a quantitative design with a cross-sectional study approach in companies that meet the criteria for implementing SMK3 (Occupational Safety and Health Management System) and operate in the East Kalimantan region, as many as 94 companies. Total respondents for safety climate measurement were 8,055 people.
Results: The level of implementation of the company's OSH Management System in East Kalimantan is at Level 3/Consistent, namely the implementation of SMK3 (OSH Management system) with a systematic and consistent approach, risk control efforts are carried out in a structured manner, measured qualitatively, integration between procedures and risk management runs in all divisions. The implementation of the learning system through monitoring, OSH reports, and improvement processes has not been carried out comprehensively. The highest component is in Hazard control and prevention, and the weakest score is in Education and training. The safety climate condition is in good condition, with the highest score in dimension 6 Learning, communication and innovation and the lowest score in dimension 5 Priority of worker safety and zero tolerance of hazard risks. Safety climate factors and OSH performance (incidence rate, frequency rate,) have a significant relationship with the level of OSH Management System implemented in the company. There is a relationship between the level of OHS implementation with safety climate, Incidence rate and frequency rate based on OHS certification.
Conclusion: The better the level of OHS implementation in a company, the better OHS performance will be. In companies that are SMK3 certified but the implementation of SMK3 is still at the Adhoc & Coping level, it will result in poor K3 performance (SC needs improvement; IR and FR tend to increase).
