Insidens kasus tuberkulosis (TBC) anak di Indonesia diperkirakan mencapai 11,7% dari total kasus TB. Tidak semua individu terpapar TBC akan menjadi sakit, namun kemungkinan reaktivasi lebih tinggi pada anak , terutama pada anak di bawah lima tahun. Kontak serumah lebih berisiko. Nutrisi baik makro maupun mikro mempengaruhi kejadian TBC pada anak. Studi menyatakan Vitamin D dan Seng berperan dalam peningkatan imunitas. Namun penelitian tentang pemberian suplementasi vitamin D dan Seng serta upaya perbaikan nutrisi dalam pencegahan infeksi TBC pada balita belum dilakukan. Penelitian ini bertujuan untuk mengetahui efek pemberian suplementasi dan konseling diet dalam pencegahan infeksi TBC pada balita kontakserumah TBC paru terkonfirmasi bakteriologis di DKI Jakarta. Kami melakukan penelitian quasi eksperimen pada balita kontak serumah TBC paru bakteriologis di 25 kecamatan di DKI Jakarta. Kelompok intervensi diberikan vitamin D 400-600 IU/hari dan Seng 10-20mg/hari tergantung usia selama 3 bulan serta konseling diet pada orang tua. Balita yang masuk dalam sampel adalah balita yang tidak terinfeksi dan atau sakit TBC, tidak gizi buruk, HIV negative dan tidak menderita penyakit kronis lain. Setiap bulan dilakukan recall diet 24 jam untuk mengukur nutrisi dan status gizi. Setelah 3 bulan akan dihitung balita yang terinfeksi dan tidak dengan menggunakan tes tuberculin. Analisa multivariat dilakukan dengan GEE. Berdasarkan hasil penelitian, insidenss kumulatif infeksi TBC pada kelompok intervensi 5% sedangkan pada kelompok kontrol 23%. Pemberian intervensi meningkatkan konsumsi vitamin D pada balita yakni dari 3 mcg menjadi 14,9 mcg dan Seng 3.8 mg menjadi 18.2 mg. Pada balita terinfeksi, konsumsi vitamin D dan Seng lebih rendah. Besarnya risiko kejadian infeksi TBC dengan pemberian intervensi adalah 0.22 kali ( 95% CI 0.08-0.57 p. value 0.002). Pemberian suplementasi vitamin D dan Seng serta konseling diet menurunkan kejadian infeksi TBC pada balita kontak serumah hingga 78%. The incidence of childhood tuberculosis (TB) in Indonesia is estimated to be 11.7% of total TB cases. Not all individuals exposed to TB will become ill, but the likelihood of reactivation is higher in children, especially children under five years old. Household contacts are more at risk. Both macro and mikro nutriens influence the incidence of TB in children. Studies suggest that vitamin D and zinc play a role in boosting immunity. However, research on vitamin D and zinc supplementation and nutritional improvement efforts in preventing tuberculosis infection in children under five years of age has not been conducted. This study aims to determine the effect of supplementation and dietary counseling in preventing TB infection in young children with bacteriologically confirmed pulmonary TB in DKI Jakarta. We conducted a quasi-experimental study among infants with bacteriologically confirmed pulmonary TB home contacts in 25 subdistricts in DKI Jakarta. The intervention group received vitamin D 400-600 IU/day and Zinc 10-20mg/day depending on age for 3 months, as well as nutritional counseling for parents. Included in the sample were infants who were not infected and/or sick with TB, not malnourished, HIV negative, and not suffering from any other chronic diseases. A 24-hour dietary recall to measure diet and nutritional status was conducted every month. After 3 months, infected and uninfected children were counted using the tuberculin test. Multivariate analysis was performed using GEE. Based on the results of the study, the cumulative incidence of TB infection was 5% in the intervention group and 23% in the control group. The intervention increased vitamin D consumption in toddlers from 3 mcg to 14.9 mcg and zinc from 3.8 mg to 18.2 mg. Vitamin D and zinc intake was lower among infected infants. The risk of TB infection with the intervention was 0.22 times (95% CI 0.08-0.57 p. value 0.002). Provision of vitamin D and zinc supplementation and dietary advice reduced the risk of TB infection. Tuberkulosis (TBC) merupakan penyakit menular yang menjadi salah satu penyebab utama kesakitan dan kematian di dunia. Di Indonesia peningkatan jumlah kasus TBC dari tahun 2020-2022 sejalan dengan peningkatan kasus TBC anak. Peningkatan kasus TBC anak juga terjadi di Kabupaten Nagekeo dengan cakupan penemuan TBC anak di tahun 2022 yaitu 70%. Adanya pasien TBC anak merupakan indikator masih berlangsungnya penularan TBC di suatu komunitas. Anak yang kontak dengan penderita TBC dewasa sangat berisiko untuk terinfeksi TBC. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan infeksi TBC pada anak di Kabupaten Nagekeo tahun 2020-2022. Penelitian ini menggunakan desain studi cross sectional dan analisis regresi logistik untuk mengestimasi risiko dengan memanfaatkan data sekunder pada Sistem Informasi Tuberkulosis (SITB) Dinas Kesehatan Kabupaten Nagekeo. Dari 239 anak yang memiliki riwayat kontak dengan penderita TBC, sebesar 5.44% memiliki riwayat infeksi TBC. Dari hasil analisis multivariat, ditemukan bahwa anak 5 tahun lebih berisiko menderita TBC dibanding anak >5 tahun [prevalence odds ratio (POR) 5,74 (95% CI: 1,66 – 19,85]. Kontak dengan penderita TBC yang memiliki riwayat gagal berobat sebelumnya juga berasosiasi dengan peningkatan risiko infeksi TBC pada anak dibanding dengan kontak dengan pasien TBC baru [POR 5,17 (95%CI : 1,17 – 22,70)]. Upaya promotif dan preventif harus terus dilakukan dalam rangka pencegahan infeksi TBC pada anak dengan meningkatkan komunikasi informasi edukasi (KIE), pemberian Terapi Pencegahan TBC (TPT) kepada semua kontak anak terutama 5 tahun serta dukungan kepatuhan pengobatan kepada penderita TBC dan pengawas minum obat (PMO).
Tuberculosis (TB) is a communicable disease that is a major cause of ill health and one of the leading causes of death worldwide. In Indonesia, the increase in the number of TB cases from 2020-2022 is in line with the increase in TB cases in children. An increase in cases of TB in children has also occurred in Nagekeo Regency with a coverage of TB detection in children in 2022, namely 70%. The presence of pediatric TB patients is an indicator of ongoing TB transmission in a community. Children who are in contact with adult TB patients are at high risk of becoming infected with TB. This study aims to determine the factors related to TB infection in children in Nagekeo Regency in 2020-2022. This study used a cross-sectional study design and logistic regression analysis to estimate the risk by utilizing secondary data from the Tuberculosis Information System of the Nagekeo District Health Office. Of the 239 children who had a history of contact with TB sufferers, 5.44% had a history of TB infection. From the results of multivariate analysis, it was found that children 5 years were more at risk of suffering from TB than children >5 years [prevalence odds ratio (POR) 5.74 (95% CI: 1.66 – 19.85). A history of previous treatment failures is also associated with an increased risk of TB infection in children compared to contacts with new TB patients [POR 5.17 (95% CI: 1.17 – 22.70)]. Promotive and preventive efforts must continue to be carried out in the context of preventing TB infection in children by increasing communication, information and education (CIE), providing TB Prevention Therapy (TPT) to all child contacts especially 5 years and supporting treatment adherence to TB patients and drugs taking supervisors (DTS).
Abstrak Proporsi keberhasilan pengobatan pasien TBC di Jakarta Barat trend-nya mengalami penurunan dari tahun 2020 hingga 2022. Penelitian ini bertujuan untuk mengetahui hubungan kepatuhan pengobatan, co-infeksi HIV, dan riwayat pengobatan dengan ketidakberhasilan pengobatan TBC SO di Kota Jakarta Barat tahun 2022. Desain studi penelitian yaitu kohort retrospektif, bersumber dari laporan TB03.SO Sistem Informasi Tuberkulosis (SITB) Kota Jakarta Barat tahun 2022. Analisis penelitian meliputi analisis deskriptif, survival dengan Kaplan Meier, dan multivariat (cox regression). Dari 2116 pasien yang terdapat 1846 pasien menjadi sampel penelitian. Hasil penelitian menunjukkan insiden rate kumulatif: 4,9/1000 orang-minggu; probabilitas survival kumulatif: 70,5%. Pada kelompok negatif DM, tidak patuh minum obat HR: 47,78 kali (95% CI: 32,59-70,03; p-value: <0,001). Pada kelompok ada riwayat pengobatan, tidak patuh minum obat HR: 26,28 (95% CI: 12,54-55,03; p-value: <0,001) setelah dikontrol variabel jenis kelamin. Pada kelompok patuh pengobatan, pada saat pasien TBC SO memiliki riwayat pengobatan sebelumnya memiliki HR: 2,3 (95% CI: 1,06-5,01; p-value: 0,035). Diharapkan menguatkan koordinasi dengan poli lainnya (Poli HIV/PDP atau Poli Penyakit Dalam) untuk memantau keteraturan minum OAT dan juga obat untuk penyakit penyerta lainnya untuk kasus TBC dengan komorbid. Perlu dilakukan pemantauan efek samping, konsultasi, tatalaksana efek samping sesuai standar, dan juga follow up pengobatan pasien. Abstract The proportion of successful treatment for TB patients treated in West Jakarta has decreased by 83.40% (2020), 79.36% (2021), and 77.18% (2022) (22.82% failure) . This study aims to determine the relationship between treatment adherence, HIV co-infection, and previous treatment history with TB SO patient survival and treatment failure in West Jakarta City in 2022. The study design of this research is a retrospective cohort with data sourced from the TB03.SO System report. Information on Tuberculosis (TBC SO) for West Jakarta City for the period January-December 2022. The analysis used in this research is descriptive analysis, survival using Kaplan Meier, and multivariate using cox regression. Of the 2116 eligible patients in this study, 1846 patients were included in the research sample. The results showed that the cumulative incidence rate was 4.9/1000 person-weeks with a cumulative survival probability of 70.5%. in the DM negative group, when TB SO patients were non-compliant with taking medication HR: 47.78 times (95% CI: 32.59-70.03; p-value: <0.001) after controlling for the gender variable. The results of the multivariate analysis showed that in the group with no history of treatment, when TB patients did not adhere to taking medication, the HR was: 65.65 times (95% CI: 43.09-100.03; p-value: <0.001) after controlling for variables gender. In the group with a history of treatment, when TB patients did not comply with taking medication, the HR was 26.28 times (95% CI: 12.54-55.03; p-value: <0.001) after controlling for the gender variable. in the treatment adherent group, when TB SO patients had a history of previous treatment, the HR was: 2.3 times (95% CI: 1.06-5.01; p-value: 0.035). It is hoped that coordination with other polyclinics (HIV/PDP Polyclinic or Internal Medicine Polyclinic) will be strengthened to monitor the regularity of taking OAT and also medication for other comorbidities for TB cases with comorbidities, for example ARVs for HIV patients and DM therapy for DM patients. It is necessary to monitor side effects, consult, manage side effects according to standards, and also follow up on patient treatment so as to increase treatment compliance and reduce the rate of treatment failure.
