Menurut World Health Organization (WHO), sekitar 12,2 juta kasus baru stroke terjadi setiap tahun secara global, dan lebih dari 6,5 juta orang meninggal akibat stroke setiap tahunnya. Di Indonesia, berdasarkan hasil Survei Kesehatan Indonesia (SKI) tahun 2023, prevalensi stroke pada penduduk usia ≥15 tahun tercatat sebesar 1,32%, dengan proporsi lebih tinggi pada laki-laki dan kelompok usia lanjut. Tujuan penelitian ini untuk mengetahui hubungan antara hipertensi, diabetes melitus dengan kejadian stroke pada penduduk DKI Jakarta. Penelitian ini merupakan studi analitik dengan pendekatan cross-sectional menggunakan data SKI 2023. Analisis dilakukan dengan regresi logistik multivariat dengan dua pendekatan yaitu, variabel hipertensi self-reported dan menggunakan hasil pengukuran tekanan darah pada 8.942 responden menggunakan pendekatan non-weighted pada multivariat. Hasil multivariat baik pendekatan self- report maupun hasil pengukuran hipertensi menjadi faktor risiko terkuat penyebab stroke dengan (OR = 13,09; 95% CI: 7,06–24,28; p < 0,001) dan (OR = 6,13; 95% CI: 2,62–14,31; p < 0,001), Laki-laki memiliki risiko stroke 2 kali lipat dibanding perempuan (p = 0,005). Usia ≥60 tahun memiliki OR tertinggi yaitu 9,09 (95% CI: 3,38–24,44; p < 0,001). Diabetes hanya signifikan dalam model pengukuran dengan OR = 6,59 (p < 0,001), untuk aktivitas fisik terbukti protektif. Dapat disimpulkan hipertensi baik berdasarkan self-report maupun hasil pengukuran merupakan prediktor kuat kejadian stroke. Aktivitas fisik memiliki efek protektif yang signifikan. Penggunaan data tekanan darah terukur memberikan estimasi risiko yang lebih konservatif namun stabil. Hasil ini menegaskan pentingnya deteksi dini hipertensi dan promosi aktivitas fisik dalam strategi pencegahan stroke.
According to the World Health Organization (WHO), approximately 12.2 million new cases of stroke occur globally each year, with over 6.5 million deaths attributed to stroke annually. In Indonesia, based on the 2023 Indonesian Health Survey (SKI), the prevalence of stroke among individuals aged ≥15 years was recorded at 1.32%, with higher proportions among males and the elderly population. This study aimed to determine the association between hypertension, diabetes mellitus, and stroke among residents of DKI Jakarta. This was an analytical cross-sectional study using SKI 2023 data, involving 8,942 respondents. Multivariate logistic regression was conducted using two approaches: self-reported hypertension and measured blood pressure. Both models were analyzed without weighting. The results showed that hypertension was the strongest risk factor for stroke, both in the self-reported model (OR = 13.09; 95% CI: 7.06–24.28; p < 0.001) and the measured blood pressure model (OR = 6.13; 95% CI: 2.62–14.31; p < 0.001). Males had twice the risk of stroke compared to females (p = 0.005). Individuals aged ≥60 years had the highest risk (OR = 9.09; 95% CI: 3.38–24.44; p < 0.001). Diabetes mellitus was significantly associated with stroke only in the model using measured blood pressure (OR = 6.59; p < 0.001). Physical activity was found to have a significant protective effect. In conclusion, hypertension—both self-reported and based on measured blood pressure is a strong predictor of stroke. Physical activity plays a significant protective role. Using objectively measured blood pressure yields a more conservative but stable risk estimate. These findings underscore the importance of early hypertension detection and the promotion of physical activity in stroke prevention strategies.
Background: Low Birth Weight (LBW) remains a global health issue. Although it has decreased by 1,9% from the previous decade, this reduction is still insufficient to achieve the Global Nutrition Targets. LBW predominantly occurs in low- and middle-income countries. According to the 2023 Indonesian Health Survey report, the prevalence of LBW in Indonesia stands at 6,1%. A high proportion of infants with LBW in a population indicates a complex health problem, as it reflects long-term maternal malnutrition, poor health conditions, and inadequate maternal healthcare services during pregnancy. Objective: To determine the factors associated with the incidence of LBW in Indonesia based on the 2023 Indonesian Health Survey data. Methods: This study performed a secondary analysis using data from the 2023 Indonesian Health Survey, employing a cross-sectional study design. The research sample consisted of 57.512 live-born infants from single pregnancies (singleton births). Univariate, bivariate, and multivariate analyses were performed using the Chi-Square test and multiple logistic regression. Results: The prevalence of LBW in Indonesia based on the 2023 SKI data was 5%. Factors independently associated with the incidence of LBW in Indonesia after adjusting for other variables were gestational age (APOR = 7,274; 95% CI: 6,312–8,382), maternal nutritional status (APOR = 1,898; 95% CI: 1,586–2,271), infant's sex (APOR = 0,733; 95% CI: 0,644–0,833) , ownership of Family Prosperity Card/KKS (APOR = 1,212; 95% CI: 1,024–1,436), cooking fuel (APOR = 1,217; 95% CI: 1,018–1,456), place of residence (APOR = 0,816; 95% CI: 0,710–0,937), and middle-class economic status (APOR = 0,795; 95% CI: 0,663–0,952). Conclusion: This study highlights the critical importance of improving nutritional and health status for both mothers and infants.
Stroke merupakan salah satu penyebab utama kematian di Indonesia. Individu dengan diabetes mellitus tipe 2 memiliki risiko lebih tinggi untuk mengalami stroke dibandingkan populasi umum. Namun, penelitian terkait faktor-faktor kejadian stroke pada penderita diabetes mellitus tipe 2 di Indonesia masih terbatas dan umumnya dilakukan di tingkat rumah sakit, sehingga cakupan populasinya kecil dan tidak merepresentasikan kondisi secara nasional. Oleh karena itu, tujuan penelitian ini adalah untuk mengetahui gambaran faktor kejadian stroke pada penderita diabetes mellitus tipe 2 di Indonesia. Data berasal dari Survei Kesehatan Indonesia 2023 dengan menggunakan desain studi potong lintang. Responden terdiri dari 17.186 penderita diabetes mellitus tipe 2 di Indonesia. Analisis menggunakan uji kai kuadrat untuk jenis data kategorik dan uji-t independent untuk jenis data numerik berdistribusi normal/Mann Whitney untuk jenis data numerik yang tidak berdistribusi normal. Hasil penelitian ini menemukan bahwa prevalensi kejadian stroke pada penderita diabetes mellitus tipe 2 di Indonesia sebesar 4,5%. Hasil penelitian juga menemukan asosiasi perbedaan proporsi antara beberapa faktor secara individual terhadap kejadian stroke. Pada faktor sosiodemografi, usia ≥ 55 Tahun (POR: 1,768; 95% CI: 1,378 – 2,267), jenis kelamin laki-laki (POR: 1,475; 95% CI: 1,179 – 1,845), tingkat pendidikan rendah (POR: 0,556; 95% CI: 0,416 – 0,743), dan wilayah tempat tinggal di perdesaan (POR: 0,748; 95% CI: 0,576 – 0,972); faktor metabolik, kadar tekanan darah tinggi (hipertensi) (POR: 1,549; 95% CI: 1,203 – 1,995); faktor klinis, usia pertama kali didiagnosis DM ≤ 43 tahun (POR: 0,514; 95% CI: 0,358 – 0,736) dan lama menderita DM 5 – 9 Tahun (POR: 1,363; 95% CI: 1,037 – 1,791) dan ≥ 10 Tahun (POR: 1,322; 95% CI: 1,009 – 1,731); dan faktor perilaku, konsumsi makanan berisiko (POR: 0,603; 95% CI: 0,462 – 0,787) menunjukkan perbedaan proporsi yang berarti antara masing masing faktor terhadap kejadian stroke. Diharapkan pemangku kebijakan dapat mempertimbangkan faktor-faktor tersebut untuk menerapkan kebijakan atau program yang dapat menurunkan prevalensi kejadian stroke pada penderita diabetes mellitus tipe 2 di Indonesia.
Stroke is one of the leading causes of death in Indonesia. Individuals with type 2 diabetes mellitus are at a higher risk of experiencing stroke compared to the general population. However, research on stroke risk factors among type 2 diabetes mellitus patients in Indonesia remains limited and mostly on hospital-based, resulting in a small population scope that does not represent the national condition. Therefore, this study aims to examine the factors associated with stroke among type 2 diabetes mellitus patients using data from the 2023 Indonesian Health Survey with a cross-sectional design, involving 17,186 respondents. The analysis used the chi-square test for categorical data and the independent t-test for numerical data with a normal distribution, or the Mann-Whitney test for numerical data that are not normally distributed. This study found that the prevalence of stroke among individuals with type 2 diabetes mellitus in Indonesia was 4.5%. The results also showed differences in proportions between several single factors and the occurrence of stroke: sociodemographic factors such as age ≥ 55 years (POR: 1,768; 95% CI: 1,378 – 2,267), male (POR: 1,475; 95% CI: 1,179 – 1,845), low education level (POR: 0,556; 95% CI: 0,416 – 0,743), and residing in rural areas (POR: 0,748; 95% CI: 0,576 – 0,972); metabolic factor was high blood pressure (POR: 1,549; 95% CI: 1,203 – 1,995); clinical factors such as being diagnosed with diabetes at ≤ 43 years old (POR: 0,514; 95% CI: 0,358 – 0,736) and diabetes duration of 5 – 9 years (POR: 1,363; 95% CI: 1,037 – 1,791) and ≥ 10 years (POR: 1,322; 95% CI: 1,009 – 1,731); and behavioral factor was consumption of high-risk foods (POR: 0,603; 95% CI: 0,462 – 0,787). These findings highlight the need for policymakers to consider these factors in developing strategies and programs to reduce the prevalence of stroke among individuals with type 2 diabetes mellitus in Indonesia.
