There is an increase in maternal mortality in Indonesia. In 2023, only 57.8% of pregnant women received standardized antenatal care (ANC). In 2022, there were 5 maternal deaths in Pematangsiantar City. The coverage of ANC K6 reached 81.6%, but there were still 230 cases of complications in pregnancy and public health centers (PHC) whose coverage was still below the target. The following study aims to determine the factors that contribute to the low coverage of ANC in several PHCs in Pematangsiantar City. This study used a non-experimental design with a comparative descriptive study. Research informants were selected using purposive sampling and exit poll techniques. Research informants consisted of heads of PHCs, midwives, doctors and pregnant women. The variables are structure (human resources, facilities, disposable medical materials, funding, guidelines) and processes (planning, organizing, implementing, supervising). Data collection through in-depth interviews, observations and document reviews. The results of the study indicate that there is sampling bias. Factors that contribute are planning and implementation of ANC. Cooperation with the Population and Civil Registration Agency (Dukcapil) is needed to access the latest pregnant women data. Also, socialization of the importance of routine ANC visits to increase ANC participation of pregnant women.
A good quality ANC can reduce maternal mortality.The background of this research is the unknown midwives compliance in Bruno health centers and the unavailability of the baseline data. This study aims to determine the factors that affect midmives compliance in Bruno health centers, Purworejo in 2013. This study used a cross sectional design and quantitative methods.Based on bivariate analysis, there is a significant association between longer working with midwives compliance, very strong association in variable attitudes, strong association in variable facilities and supervision While variable knowledge and training was moderate associaton.
Puskesmas sebagai fasilitas pelayanan kesehatan tingkat pertama memiliki peran penting dalam mendukung program kesehatan masyarakat. Pemenuhan sumber daya manusia kesehatan (SDMK) yang mencakup jenis dan jumlah tenaga kesehatan sesuai standar menjadi syarat utama untuk menjamin pelayanan yang berkualitas. Penelitian ini bertujuan untuk menganalisis kecukupan jenis dan jumlah SDMK pada Puskesmas BLUD di Kota Depok tahun 2024 serta faktor-faktor yang memengaruhinya. Penelitian menggunakan pendekatan kualitatif dengan metode studi kasus melalui wawancara mendalam dan telaah dokumen pada enam Puskesmas terpilih serta instansi terkait di Kota Depok. Hasil menunjukkan bahwa masih terdapat kesenjangan antara jumlah SDMK eksisting dengan kebutuhan berdasarkan analisis jabatan dan beban kerja yang tertuang dalam dokumen perencanaan dan kebutuhan SDMK Kota Depok. Kesenjangan ini menyebabkan multitasking dan task shifting, yang berdampak pada beban kerja dan mutu layanan. Faktor yang memengaruhi pemenuhan SDMK meliputi sistem manajemen kepegawaian, kepemimpinan, kemitraan/jejaring, ketersediaan anggaran, pendidikan/pelatihan, serta kebijakan yang berlaku. Temuan juga menunjukkan bahwa fleksibilitas pengelolaan keuangan melalui BLUD belum dimanfaatkan secara optimal untuk mendukung pemenuhan SDMK. Diperlukan komitmen pemerintah daerah dalam perencanaan dan pengadaan SDMK secara strategis berbasis data dan analisis beban kerja untuk memastikan tersedianya tenaga kesehatan yang sesuai baik secara jumlah maupun jenis.
Community health centers (Puskesmas) as primary health care facilities play an important role in supporting public health programs. The fulfillment of health human resources (SDMK), which includes the type and number of health workers in accordance with standards, is a key requirement for ensuring quality services. This study aims to analyze the adequacy of the type and number of SDMK at BLUD Puskesmas in Depok City in 2024 and the factors that influence it. The study employs a qualitative approach using a case study method through in-depth interviews and document reviews at six selected Puskesmas and related institutions in Depok City. The results indicate that there is still a gap between the existing number of HHR and the needs based on job analysis and workload as outlined in Depok City's planning documents and HHR requirements. This gap leads to multitasking and task shifting, which impacts workload and service quality. Factors influencing the fulfillment of SDMK include human resource management systems, leadership, partnerships/networks, budget availability, education/training, and applicable policies. The findings also indicate that the flexibility of financial management through BLUD has not been optimally utilized to support the fulfillment of SDMK. A commitment from the local government is needed in strategic planning and procurement of SDMK based on data and workload analysis to ensure the availability of healthcare personnel that are appropriate in both quantity and type.
The Ministry of Health is implementing programs to improve the performance of healthresources through education and training, especially training of traditional health careworkers, through the training of acupressure services for Primary Health Care, butacupressure service has not been run in Primary Health Care. In South Jakarta, PrimaryHealth Care that have been providing acupressure service are only two (2). Thisresearch is a qualitative research, and aims to analyze the policy and implementation ofacupressure service in Primary Health Care and its obstacles. Informants in the studyamounted to 11 people, namely the Ministry of Health, Sudinkes South Jakarta, Head ofPrimary Health Care, Doctor, program implementer. Methods of data collection throughWM and document review. The result of research of input component have beenrunning, existence of support of Head of Puskesmas, service SOP, and SK ofassignment but not optimal rotation of staff become one of obstacle, component ofoutput and outcome not yet optimally. The communication aspect (clarity andconsistency) has not been effective about the existing policy regulation informationfrom the policy makers to the implementers, the financing aspect has not been supportedby local regulations, the bureaucratic aspects are still lacking coordination and thepolicy socialization from the Health Service to tribe of health service and PrimaryHealth Care.
Primary Health Center (Puskesmas) in the era of JKN plays an important role asFirst level Health Facilities filters refferal. As a gatekeeper, Primary Health Centerhave a responsibility to resolve cases of non-specialist medical thoroughly. Thisstudy aims to analyze the effectiveness of the Primary Health Center arerepresented by the refference value when linked with the availability of resourcesand Primary Health Center managerial leadership. This research was crosssectional design with quantitative approach. Sampling was collected by purposivesampling method to 25 puskesmas in South Tangerang. The results showed thatthe resources of Primary Health Center do not meet the standards that have beenset except for the drug. The availability of tools/technology and medical supportmaterial ranges level from 60-79%. While aspects of leadership managerialshould be highlighted is supervision, jobs delegation and communication betweenleaders and staff. From 25 Primary Health Center, there are 7 Primary HealthCenters with good effectiveness category which the reference value < 15%.Key word : Effectiveness, health service, primary health center (Puskesmas).
