Abstrak:
Pemerintah Indonesia telah menyusun kebijakan nasional eliminasi penularan HIV, sifilis, dan hepatitis B dari ibu ke anak sejak tahun 2017 melalui pemeriksaan triple eliminasi yang diintegrasikan dalam pelayanan antenatal care (ANC). Meskipun kebijakan telah tersedia, implementasinya masih menunjukkan variasi antarprovinsi, termasuk di Pulau Jawa dan Bali. Proporsi pemeriksaan triple eliminasi lengkap di Pulau Jawa dan Bali sebesar 76,10%, dengan proporsi tertinggi di Bali (92,23%) dan terendah di Jawa Barat (59,70%). Sebelum memperhitungkan faktor individu dan kontekstual kabupaten/kota, 16,38% variasi pemeriksaan triple eliminasi lengkap dapat dijelaskan oleh perbedaan antarkabupaten/kota. Hasil analisis multilevel menunjukkan bahwa pendidikan ibu (AOR=1,402; 95% CI: 1,167–1,685; p=0,000), kepemilikan Jaminan Kesehatan Nasional (AOR=1,357; 95%CI: 1,113–1,656; p=0,002), partisipasi dalam kelas ibu hamil (AOR=1,397; 95%CI: 1,169–1,670; p=0,000), dan kunjungan ANC sesuai standar (AOR=1,650; 95%CI: 1,395–1,951; p=0,000) berhubungan dengan odds yang lebih tinggi untuk memperoleh pemeriksaan triple eliminasi lengkap. Sebaliknya, peningkatan usia ibu (AOR=0,980;95%CI: 0,965–0,996; p=0,016) dan ANC rutin di praktik bidan mandiri/klinik bersalin (AOR=0,727; 95% CI: 0,576–0,917; p=0,007) berhubungan dengan odds yang lebih rendah. Rasio bidan, rasio ATLM, rasio puskesmas per 1.000 ibu hamil, dan rasio kapasitas fiskal kabupaten/kota tidak berhubungan dengan odds pemeriksaan triple eliminasi lengkap. Temuan penelitian ini menunjukkan bahwa faktor individu lebih berperan terhadap peluang ibu hamil memperoleh pemeriksaan triple eliminasi lengkap. Namun, setelah memperhitungkan seluruh variabel dalam model, masih terdapat 13,61% variasi antarkabupaten/kota yang belum dapat dijelaskan oleh faktor-faktor kontekstual kabupaten/kota yang dianalisis dalam penelitian ini.

The Indonesian government has implemented a national policy to eliminate mother-to-child transmission of HIV, syphilis, and hepatitis B since 2017 through triple elimination screening integrated into antenatal care (ANC) services. Despite the existence of this policy, its implementation continues to vary across provinces, including those in Java and Bali. The proportion of pregnant women who received complete triple elimination screening in Java and Bali was 76.10%, with the highest proportion in Bali (92.23%) and the lowest in West Java (59.70%). Before accounting for individual- and district/city-level contextual factors, 16.38% of the variation in complete triple elimination screening was attributable to differences between districts/cities. Multilevel analysis showed that maternal education (AOR=1.402; 95% CI: 1.167–1.685; p<0.001), National Health Insurance (JKN) membership (AOR=1.357; 95% CI: 1.113–1.656; p=0.002), participation in pregnancy classes (AOR=1.397; 95% CI: 1.169–1.670; p<0.001), and receiving ANC according to the recommended standard (AOR=1.650; 95% CI: 1.395–1.951; p<0.001) were associated with higher odds of receiving complete triple elimination screening. In contrast, increasing maternal age (AOR=0.980; 95% CI: 0.965–0.996; p=0.016) and routinely receiving ANC at independent midwifery practices or maternity clinics (AOR=0.727; 95% CI: 0.576–0.917; p=0.007) were associated with lower odds of receiving complete triple elimination screening. The ratios of midwives, medical laboratory technologists, primary health centers per 1,000 pregnant women, and district/city fiscal capacity were not associated with the odds of receiving complete triple elimination screening. These findings indicate that individual-level factors play a greater role in determining pregnant women's likelihood of receiving complete triple elimination screening. However, after accounting for all variables in the model, 13.61% of the variation between districts/cities remained unexplained, suggesting the presence of other district/city-level contextual factors that were not included in this study.

Abstrak:
ISPA (Infeksi Saluran Pernapasan Akut) merupakan salah satu penyebab utama morbiditas balita di Indonesia, dengan prevalensi sebesar 36,9% pada tahun 2024 dan menunjukkan peningkatan dibandingkan tahun sebelumnya. Berdasarkan data Kementerian Kesehatan, prevalensi ISPA pada balita tertinggi terdapat di Pulau Jawa (38,4%-49,1%). Penelitian ini bertujuan mengidentifikasi wilayah hotspot dan coldspot ISPA di Pulau Jawa serta menganalisis determinannya menggunakan data SSGI 2024. Hotspot didefinisikan sebagai wilayah dengan kejadian tinggi yang dikelilingi wilayah berkejadian tinggi, sedangkan coldspot merupakan kondisi sebaliknya. Analisis pola spasial dilakukan menggunakan Moran's I dan Getis-Ord Gi*, sementara hubungan antarvariabel dianalisis dengan regresi logistik multilevel tiga level (individu, rumah tangga, dan kabupaten/kota). Hasil analisis, diketahui adanya autokorelasi spasial (Moran's I=0,220; p=0,001) dengan 15 kabupaten/kota sebagai hotspot yang sebagian besar berada di Jawa Barat dan Banten, serta 7 kabupaten/kota sebagai coldspot yang berada di Jawa Timur bagian timur dan sebagian Jawa Tengah. Wilayah hotspot meningkatkan risiko ISPA (OR=1,725; 95%CI:1,05-2,52) dan coldspot bersifat protektif (OR=0,58; 95%CI: 0,33-1,04). Faktor risiko signifikan lainnya meliputi jenis kelamin laki-laki (OR=1,05; 95%CI: 1,00-1,10), usia (OR=1,03; 95%CI: 1,00-1,04), wasting (OR=1,15; 95%CI: 1,03-1,29), underweight (OR=1,31; 95%CI: 1,19-1,43), overweight (OR=0,75; 95%CI: 0,63-0,89), pendidikan ibu rendah (OR=1,11; 95%CI: 1,05-1,18), ibu bekerja (OR=0,94; 95%CI: 0,89-0,99), sosial ekonomi kuintil terbawah (OR=1,16; 95%CI: 1,04-1,28), kepadatan hunian tidak memenuhi syarat (OR = 1,17; 95%CI: 1,07-1,29), dan kepadatan penduduk (OR = 1,04; 95%CI: 1,00-1,08). Nilai ICC rumah tangga 0,589 dan kabupaten/kota 0,064 pada full model mengindikasikan adanya variasi antar klaster dan membenarkan penggunaan model multilevel. Distribusi ISPA di Pulau Jawa menunjukkan pola spasial yang bermakna dengan perbedaan risiko yang signifikan antara wilayah hotspot dan coldspot berdasarkan faktor individu, keluarga, dan wilayah. Temuan ini menegaskan perlunya intervensi kesehatan berbasis wilayah dengan prioritas pada kabupaten/kota hotspot.

Acute Respiratory Infections (ARI) represent one of the leading causes of morbidity among children under five in Indonesia, with a prevalence of 36.9% in 2024, representing an increase from the preceding year. Based on data from the Ministry of Health, the highest prevalence of ARI among children under five is found in Java Island (38.4%–49.1%). This study aimed to identify hotspot and coldspot areas of ARI in Java Island and to analyze their determinants using data from the 2024 SSGI. Hotspots are defined as high-incidence areas surrounded by neighboring areas of similarly elevated incidence, whereas coldspots represent the opposite condition. Spatial pattern analysis was conducted using Moran's I and Getis-Ord Gi*, while associations between variables were examined using three-level multilevel logistic regression at the individual, household, and district/city levels. The results revealed the presence of spatial autocorrelation (Moran's I = 0.220; p = 0.001), with 15 districts and cities identified as hotspots, predominantly located in West Java and Banten, and 7 districts and cities identified as coldspots, situated in eastern East Java and parts of Central Java. Hotspot classification was associated with a significantly increased risk of ARI (OR = 1.725; 95% CI: 1.05–2.52), while coldspot classification was found to be protective (OR = 0.58; 95% CI: 0.33–1.04). Other significant risk factors included male sex (OR = 1.05; 95% CI: 1.00–1.10), age (OR = 1.03; 95% CI: 1.00–1.04), wasting (OR = 1.15; 95% CI: 1.03–1.29), underweight (OR = 1.31; 95% CI: 1.19–1.43), overweight (OR = 0.75; 95% CI: 0.63–0.89), low maternal education (OR = 1.11; 95% CI: 1.05–1.18), maternal employment (OR = 0.94; 95% CI: 0.89–0.99), lowest socioeconomic quintile (OR = 1.16; 95% CI: 1.04–1.28), household overcrowding not meeting standard requirements (OR = 1.17; 95% CI: 1.07–1.29), and population density (OR = 1.04; 95% CI: 1.00–1.08). The intraclass correlation coefficients (ICC) at the household level (0.589) and district/city level (0.064) in the full model indicated the presence of between-cluster variation, thereby justifying the use of a multilevel modeling approach. The distribution of ARI in Java Island demonstrated a significant spatial pattern, with meaningful risk differences between hotspot and coldspot areas attributable to individual, household, and regional factors. These findings underscore the need for area-based health interventions, with priority given to districts and cities identified as hotspots.

Abstrak:
Pada tahun 2023, lebih dari 50% kasus baru HIV di Indonesia berada di pulau jawa. Berdasarkan faktor risiko yang teridentifikasi, penyebaran terbesar berasal dari homoseksual 31% yang terdiri atas 30% kelompok LSL dan 1% Waria. Studi ini bertujuan untuk dapat mengetahui apa determinan sosial perilaku yang berhubungan dengan kejadian HIV pada kelompok LSL dan Waria di Pulau Jawa berdasarkan data STBP 2018. Desain Studi yang digunakan adalah kuantitatif dengan pendekatan studi cross-sectional pada 2555 responden LSL dan 1967 responden Waria. Hasil studi ini menunjukkan prevalensi HIV pada kelompok LSL di Pulau Jawa sebesar 19%, dan pada kelompok Waria sebesar 13,6%. Pada kelompok LSL, Usia ≥ 26 Tahun, status perkawinan yang belum kawin, status sifilis positif, dan status TBC positif merupakan faktor risiko pada kejadian HIV pada kelompok LSL di Pulau Jawa. Positif TBC menjadi faktor risiko paling dominan dengan peluang sebesar 3x meningkatkan terinfeksi HIV. Sementara itu, pada kelompok Waria, pengetahuan HIV yang rendah, penggunaan NAPZA suntik, status sifilis positif dan status TBC positif merupakan faktor risiko pada kejadian HIV pada kelompok Waria di Pulau Jawa. Positif sifilis menjadi faktor risiko paling dominan dengan peluang sebesar 3,8x meningkatkan terinfeksi HIV. Kementerian kesehatan dapat meningkatkan layanan berupa petunjuk teknis (Juknis) layanan terpadu HIV-Sifilis-TBC khusus populasi kunci atau pedoman peran layanan berbasis komunitas. Selain itu peningkatakan integrasi layanan pengobatan baik IMS maupun TB dengan HIV dapat dilakukan untuk memudahkan LSL dan Waria. LSM dapat meningkatkan layanan skrining "Jemput Bola" di ruang aman, serta meningkatkan pengetahuan HIV yang komprehensif pada Waria seperti pertemuan berkala dan juga penentuan community leader. Memperkuat upaya pendampingan untuk meningkatkan akses dan retensi layanan HIV, IMS, dan TB bagi LSL dan Waria

Java Island, as the most populous region in Indonesia, has become the epicenter of HIV/AIDS transmission. In 2023, it was reported that more than 50% of newly identified HIV cases in Indonesia occurred in Java. Based on identified risk factors, the largest proportion of cases (31%) was attributed to homosexual transmission, comprising 30% among men who have sex with men (MSM) and 1% among transgender women. This study aims to identify the social and behavioral determinants associated with HIV infection among MSM and transgender women in Java, using data from the 2018 Integrated Biological and Behavioral Survey (IBBS). The study employed a quantitative design with a cross-sectional approach involving 2,555 MSM and 1,967 transgender women respondents. The findings showed that HIV prevalence among MSM in Java was 19%, while among transgender women it was 13.6%. Among MSM, the significant risk factors for HIV infection were age ≥ 26 years, being unmarried, positive syphilis status, and positive tuberculosis (TB) status. TB was the most dominant risk factor, increasing the likelihood of HIV infection by threefold. Among transgender women, risk factors included low HIV knowledge, injecting drug use, positive syphilis status, and positive TB status. Syphilis was identified as the most dominant risk factor, increasing the risk of HIV infection by 3.8 times. The Ministry of Health needs encouraged to enhance services through the development of integrated technical guidelines for HIV–Syphilis–TB services tailored to key populations, as well as community-based service frameworks. Detection and treatment services for sexually transmitted infections (STIs) and TB should be integrated and made specifically accessible to MSM and transgender women. Civil society organizations can strengthen community-based screening services through mobile outreach strategies in safe spaces and increase comprehensive HIV knowledge among transgender women through regular meetings and the empowerment of community leaders. Strengthening peer support and outreach programs is also crucial to improve access to and retention in HIV, STI, and TB services for MSM and transgender populations.

Abstrak:

Latar Belakang: Prevalensi HIV pada waria di Indonesia meningkat signifikan (28,9% pada 2023). Penelitian ini menganalisis perbedaan determinan penggunaan kondom secara konsisten antara waria yang membeli dan menjual seks. Metode: Analisis sekunder data Survei Terpadu Biologis dan Perilaku (STBP) 2023 dengan desain cross-sectional pada 2.103 responden waria menggunakan regresi logistik. Hasil: Penggunaan kondom secara konsisten lebih tinggi pada waria penjual seks (38,3%) dibanding pembeli seks (12,3%). Pada pembeli seks, determinan utama adalah usia <42 tahun dan kemudahan akses kondom (AOR=6,65). Pada penjual seks, faktor yang berpengaruh adalah pendidikan, tinggal dengan pasangan seks, paparan informasi HIV, riwayat IMS, tidak konsumsi alkohol sebelum seks, dan keikutsertaan program HIV. Kesimpulan: Terdapat perbedaan determinan perilaku antara kedua kelompok. Intervensi harus fokus pada perluasan akses kondom fisik bagi pembeli seks dan penguatan program edukasi serta komunitas bagi penjual seks.


Background: HIV prevalence among transgender women (Waria) in Indonesia has risen sharply to 28.9% in 2023. This study analyzes the differing determinants of consistent condom use between those who buy and sell sex. Methods: A secondary analysis of the 2023 Integrated Biological and Behavioral Survey (IBBS) was conducted using a cross-sectional design and logistic regression on 2,103 respondents. Results: Consistent condom use was higher among sellers (38.3%) than buyers (12.3%). For sex buyers, key determinants were age <42 years and ease of condom access (AOR=6.65). For sex sellers, significant factors included education, living with a partner, HIV information exposure, STI history, avoiding alcohol before sex, and HIV program participation. Conclusion: Behavior determinants differ between the two groups. Interventions should prioritize expanding physical condom access for buyers and strengthening educational and community-based programs for sellers.

Abstrak:
Lansia adalah kelompok yang rentan di masa pandemi COVID-19. Oleh karena itu diperlukan pemetaan vaksinasi untuk masyarakat lansia, terutama di Pulau Jawa, sehingga dapat menganalisis cakupan vaksinasinya di tiap Kab/Kota. Sehingga tujuan dari penelitian ini adalah melakukan analisis spasial cakupan vaksinasi COVID-19 dosis 1 dan dosis 2 pada sasaran prioritas lansia di Pulau Jawa sejak awal diadakan vaksinasi pada bulan Januari tahun 2021 sampai bulan Mei tahun 2022.Analisis spasial sebagai metode peneltian yang memetakan objek berdasarkan posisi geografis menjadi alat bantu yang tepat dalam Decision Support System dalam menganalisis pemetaan cakupan vaksinasi COVID-19 dosis 1 dan dosis 2 di Pulau Jawa. Hasil yang diperoleh menunjukkan adanya autokorelasi spasial antar wilayah di Pulau Jawa. Oleh karena itu dapat disimpulkan bahwa persentase jumlah lansia yang dilakukan Vaksin dosis 1 dan Vaksin dosis 2 di 118 kabupaten/kota di Pulau Jawa cenderung berkelompok (cluster) pada lokasi yang berdekatan. Hasil ini diharapkan dapat menjadi pertimbangan bagi pemerintah daerah dan dinas kesehatan dalam mengambil langkah selanjutnya dalam memastikan cakupan vaksinasi COVID-19 yang merata untuk tiap Kab/Kota di Pulau Jawa.

The elders are a vulnerable group during the COVID-19 pandemic. Therefore, it is necessary to map out vaccinations for the elderly, especially in Java, to analyze the vaccination coverage in each district/city. The purpose of this study is to conduct a spatial analysis of the coverage of COVID-19 vaccination doses one and dose two at the elderly priority targets on Java Island since the initial vaccination was held from January 2021 until May 2022. Spatial analysis is research method that maps objects based on geographical position. This method becomes the suitable tool in the Decision Support System to analyze the mapping of COVID-19 vaccination coverage doses one and two on the island of Java. The results obtained indicate that there is a spatial autocorrelation between regions in Java. Therefore, it can be concluded that the percentage of the elderly who received dose one and dose two vaccines in 118 districts/cities on the island of Java tended to be clustered in adjacent locations. This result is expected to be a consideration for local governments and health offices in taking the next step in ensuring equitable COVID-19 vaccination coverage for each district/city on the island of Java.

Abstrak:

Obesitas merupakan masalah kesehatan global dengan prevalensi yang terus meningkat, termasuk di Indonesia. Pada anak dan remaja, obesitas dipengaruhi oleh berbagai faktor, seperti genetik, lingkungan, pola makan, dan status sosial ekonomi. Namun, penelitian yang secara khusus mengkaji obesitas pada kelompok usia ini masih terbatas. Penelitian ini bertujuan untuk mengetahui prevalensi dan determinan obesitas pada anak dan remaja di Indonesia menggunakan data Survei Kesehatan Indonesia (SKI) 2023. Desain penelitian ini adalah potong lintang, dengan sampel terdiri dari 115.053 anggota rumah tangga berusia 5–19 tahun yang memenuhi kriteria inklusi dan eksklusi. Hasil menunjukkan prevalensi obesitas sebesar 7,9% (95% CI 7,6–8,1). Faktor-faktor yang secara signifikan berhubungan dengan peningkatan risiko obesitas meliputi: jenis kelamin laki-laki [AOR 1,56; 95% CI 1,451–1,678], pendidikan ibu tinggi [AOR 1,197; 95% CI 1,106–1,296], ibu bekerja [AOR 1,14; 95% CI 1,063–1,223], tinggal di perkotaan [AOR 1,27; 95% CI 1,176–1,370], status ekonomi teratas [AOR 1,791; 95% CI 1,548–2,032], aktivitas fisik rendah [AOR 1,534; 95% CI 1,230–1,913], konsumsi makanan olahan lebih dari satu kali sehari [AOR 1,27; 95% CI 1,009–1,242], serta konsumsi buah dan sayur minimal satu porsi per hari [AOR 1,142; 95% CI 1,060–1,227]. Temuan ini menunjukkan bahwa intervensi promosi kesehatan yang menargetkan faktor-faktor tersebut penting untuk mencegah dan mengendalikan obesitas pada anak dan remaja di Indonesia.

Among children and adolescents, obesity is influenced by various factors such as genetics, environment, dietary patterns, and socioeconomic status. However, research specifically focusing on obesity within this age group remains limited. This study aims to examine the prevalence and determinants of obesity among children and adolescents in Indonesia using data from the 2023 Indonesia Health Survey (SKI). A cross-sectional study design was employed, with a total sample of 115,053 household members aged 5–19 years who met the inclusion and exclusion criteria. The results showed an obesity prevalence of 7.9% (95% CI: 7.6–8.1). Factors significantly associated with increased obesity risk included: male gender [AOR 1.56; 95% CI: 1.451–1.678], higher maternal education [AOR 1.197; 95% CI: 1.106–1.296], working mothers [AOR 1.14; 95% CI: 1.063–1.223], urban residence [AOR 1.27; 95% CI: 1.176–1.370], highest socioeconomic status [AOR 1.791; 95% CI: 1.548–2.032], low physical activity [AOR 1.534; 95% CI: 1.230–1.913], consumption of processed food more than once a day [AOR 1.27; 95% CI: 1.009–1.242], and fruit and vegetable intake of at least one portion per day [AOR 1.142; 95% CI: 1.060–1.227]. These findings underscore the importance of targeted health promotion interventions addressing these factors to prevent and control obesity among children and adolescents in Indonesia.

Abstrak:
Minimum Dietary Diversity (MDD) merupakan indikator penting kualitas pemberian makanan pendamping ASI (MP-ASI) yang mencerminkan kucukupan keragaman konsumsi pangan anak usia 6–23 bulan. Capaian MDD di Indonesia masih rendah, sementara determinannya belum banyak dianalisis dengan mempertimbangkan pengaruh faktor individu dan konteks wilayah secara simultan. Penelitian ini menggunakan desain potong lintang dengan data Survei Status Gizi Indonesia (SSGI) 2024 yang melibatkan 70.561 anak usia 6–23 bulan dari 412 kabupaten/kota. Analisis dilakukan menggunakan regresi logistik multilevel dua tingkat untuk mengidentifikasi determinan MDD pada tingkat individu dan kabupaten/kota. Hasil penelitian menunjukkan sebanyak 48,3% anak memenuhi MDD. Faktor tingkat individu yang berhubungan dengan pemenuhan MDD meliputi usia anak, usia ibu, pendidikan ibu, status pekerjaan ibu, status sosial ekonomi rumah tangga, jumlah anggota rumah tangga, dan pemanfaatan layanan kesehatan anak. Sekitar 13,0% variasi pemenuhan MDD dijelaskan oleh perbedaan antar kabupaten/kota. Pada tingkat wilayah, indikator ketahanan pangan yang mencerminkan dimensi ketersediaan, keterjangkauan, dan pemanfaatan pangan berhubungan signifikan dengan pemenuhan MDD. Temuan ini menegaskan bahwa pemenuhan MDD pada anak usia 6–23 bulan di Indonesia dipengaruhi oleh faktor individu serta faktor kontekstual wilayah. Peningkatan capaian MDD memerlukan penguatan intervensi gizi spesifik dan sensitif melalui pendekatan multisektor yang mempertimbangkan karakteristik wilayah.

Minimum Dietary Diversity (MDD) is a key indicator of the quality of complementary feeding practices that reflects the adequacy of dietary diversity among children aged 6–23 months. The achievement of MDD in Indonesia remains low, while its determinants have rarely been examined by simultaneously considering individual- and area-level contextual factors. This study employed a cross-sectional design using data from the 2024 Indonesian Nutritional Status Survey (Survei Status Gizi Indonesia/SSGI), involving 70,561 children aged 6–23 months from 412 districts/municipalities. Data were analyzed using a two-level multilevel logistic regression model to identify the determinants of MDD at the individual and district/municipality levels. The results showed that 48.3% of children met the MDD criteria. At the individual level, MDD was significantly associated with the child's age, maternal age, maternal education, maternal employment status, household socioeconomic status, household size, and utilization of child health services. Approximately 13.0% of the variation in MDD was attributable to differences between districts/municipalities. At the area level, food security indicators representing the dimensions of food availability, food accessibility, and food utilization were significantly associated with MDD. These findings indicate that MDD among Indonesian children aged 6–23 months is influenced by both individual characteristics and area-level contextual factors. Improving MDD requires strengthening nutrition-specific and nutrition-sensitive interventions through a multisectoral approach that takes regional characteristics into account.

Abstrak:
Bayi usia dibawah satu bulan termasuk kedalam kategori usia dengan risiko gangguan kesehatan paling tinggi, risiko tersebut dapat berakibat fatal yaitu kejadian kematian apabila tidak mendapat pelayanan kesehatan yang memadai. Kematian neonatal di Provinsi Jawa Tengah, Provinsi Jawa Timur dan Provinsi Jawa Barat tersebut pada tahun 2022 mencapai 38,38% dari jumlah kematian neonatal pada tingkat nasional. Di Provinsi Daerah Istimewa Yogyakarta selama 4 tahun cenderung meningkat, dengan peningkatan terbesar terjadi di Tahun 2022 dengan peningkatan sebesar 16% dibandingkan Tahun 2021. Tujuan dari penelitian ini adalah untuk mengetahui determinan AKN yang dipengaruhi oleh kondisi spasial di Provinsi Jawa Tengah, Jawa Timur, Jawa Barat dan Daerah Istimewa Yogyakarta pada tahun 2022. Penelitian berdesain studi ekologi dengan unit analisis 105 kabupaten/kota di Provinsi Jawa Tengah, Jawa Timur, Jawa Barat dan Daerah Istimewa Yogyakarta. Data yang dianalisis merupakan data agregat yang bersumber dari data sekunder berupa Profil Kesehatan Provinsi Tahun 2022 dan hasil Susenas Tahun 2022 yang dipublikasikan oleh BPS. Pemodelan dengan metode GWR diketahui bahwa dari 6 variabel yang dianalisis dalam model, terdapat 3 variabel yang berpengaruh signifikan secara statistik dengan AKN yaitu variabel BBLR, kerapatan jalan, dan KN lengkap.

Babies aged under one month are included in the age category with the highest risk of health problems, this risk can have fatal consequences, namely death if they do not receive adequate health services. Neonatal deaths in Central Java Province, East Java Province and West Java Province in 2022 will reach 38.38% of the number of neonatal deaths at the national level. In the Yogyakarta Special Region Province, it has tended to increase over the past 4 years, with the largest increase occurring in 2022 with an increase of 16% compared to 2021. The aim of this research is to determine the determinants of AKN which are influenced by spatial conditions in the Provinces of Central Java, East Java, Java West and Special Region of Yogyakarta in 2022. The research has an ecological study design with analysis units of 105 districts/cities in the Provinces of Central Java, East Java, West Java and the Special Region of Yogyakarta. The data analyzed is aggregate data sourced from secondary data in the form of the 2022 Provincial Health Profile and the results of the 2022 Susenas published by BPS. Modeling using the GWR method shows that of the 6 variables analyzed in the model, there are 3 variables which have a statistically significant effect on AKN are the variables LBW, road density, and complete neonatal visit.

Stunting merupakan masalah kesehatan masyarakat dunia. Prevalensi Stunting di Indonesia pada tahun 2022 adalah 21,6%. Prevalensi stunting pada balita di Provinsi Sulawesi Barat pada tahun 2019 sebesar 39,3 persen dan turun pada tahun 2021 namun mengalami kenaikan dari tahun 2021 sebesar 1,2 persen yaitu dari 33,8 persen menjadi 35,0 persen atau sebanyak 479.699. Prevalensi stunting terendah tahun 2019, 2021 dan 2022 adalah di provinsi Bali dengan nilai berturut-turut adalah 14,3 persen lalu turun menjadi 10,9 persen dan turun 2,9 persen pada tahun 2022 menjadi 8 persen. Penelitian ini bertujuan untuk menganalisis komparasi determinan stunting pada anak usia 6-23 bulan di Provinsi Sulawesi Barat dan Provinsi Bali berdasarkan data SSGI Tahun 2022. Desain dalam penelitian ini adalah cross-sectional menggunakan data SSGI 2022. Sampel dalam penelitian ini anak usia 6-23 bulan di Provinsi Sulawesi Barat dan Bali yang terpilih menjadi responden SSGI 2022. Analisis data dilakukan menggunakan chi-square dan regresi logistik berganda. Hasil penelitian menunjukkan variabel yang berhubungan bermakna dengan stunting pada anak usia 6 – 23 bulan di Provinsi Sulawesi Barat, yakni Jenis kelamin dan berat bayi lahir rendah. Status IMD merupakan variabel counfounding. Variabel yang paling dominan berhubungan dengan stunting di Provinsi Sulawesi Barat adalah riwayat BBLR. Hasil analisis didapatkan odds ratio (OR) dari variabel BBLR adalah 2,640 (95% CI: 1,286-5,420), artinya peluang anak dengan riwayat BBLR lebih besar 2,64 kali lebih besar untuk mengalami stunting daripada anak dengan riwayat tidak BBLR setelah dikontrol variabel jenis kelamin sedangkan di Provinsi Bali, Jenis Kelamin, Berat Bayi Lahir Rendah, Pendidikan Ayah, Tinggi Badan Ibu, Umur Ibu, dan IMD yang berhubungan dengan stunting (p<0,05). Faktor yang paling dominan berhubungan dengan stunting di Provinsi Bali adalah IMD. Variabel jenis kelamin, BLLR, pekerjaan ibu dan sumber air minum, Riwayat pneumonia merupakan variabel counfounding. Anak yang tidak IMD (=1 jam) setelah dikontrol variabel, Pendidikan Ayah, Umur Ibu, dan Tinggi ibu.

Stunting is a global public health concern. The prevalence of stunting in Indonesia in 2022 is 21.6%. The prevalence of stunting among young children in West Sulawesi province was 39.3% in 2019 and decreased in 2021, but increased by 1.2% from 2021, from 33.8% to 35.0% or 479,699. The lowest prevalence of stunting in 2019, 2021 and 2022 was in Bali province with consecutive values of 14.3 percent, then decreased to 10.9 percent and decreased by 2.9 percent to 8 percent in 2022. This study aims to analyze the comparative determinants of stunting in children aged 6-23 months in West Sulawesi Province and Bali Province based on SSGI data in 2022. The design in this study was cross-sectional using SSGI 2022 data. The sample in this study were children aged 6-23 months in West Sulawesi and Bali provinces who were selected as SSGI 2022 respondents. The data were analyzed using chi-squared and multiple logistic regression. The results showed that the variables significantly associated with stunting in children aged 6-23 months in West Sulawesi Province were gender and low birth weight. IMD status is a cofounding variable. The most dominant variable associated with the incidence of stunting in West Sulawesi Province is a history of LBW. The results of the analysis obtained the odds ratio (OR) of the LBW variable is 2.640 (95% CI: 1.286-5.420), meaning that the chances of children with a history of LBW are 2.64 times greater to experience stunting than children with a history of not LBW after controlling for gender variables, while in Bali Province, The most dominant factor associated with the incidence of stunting in Bali Province was early initiation of breastfeeding. Gender, BLLR, maternal occupation and drinking water source, and history of pneumonia were cofounding variables. Children who did not initiate early breastfeeding (=1 hour) after controlling for the variables of father's education, mother's age, and mother's height.

Abstrak:
Epidemi HIV di Indonesia masih terkonsentrasi pada populasi kunci, namun dalam satu dekade terakhir tren penularannya cenderung bergeser ke populasi umum. Perbedaan karakteristik epidemiologi kedua kelompok tersebut menunjukkan perlunya pendekatan spasial dalam pengendalian HIV. Penelitian ini bertujuan untuk menganalisis pola spasial dan faktor-faktor yang berhubungan dengan distribusi penemuan ODHIV baru pada populasi kunci dan populasi umum di 35 kabupaten/kota Provinsi Jawa Tengah tahun 2024. Penelitian ini menggunakan desain studi ekologi dengan data sekunder dari Sistem Informasi HIV/AIDS (SIHA) Dinas Kesehatan Provinsi Jawa Tengah tahun 2024, Profil Kesehatan, serta Badan Pusat Statistik. Analisis dilakukan menggunakan Global Moran's I, Local Indicators of Spatial Association (LISA), dan Spatial Autoregressive Model (SAR). Hasil penelitian menunjukkan bahwa distribusi ODHIV baru pada populasi kunci tidak memiliki autokorelasi spasial yang signifikan, sedangkan pada populasi umum ditemukan autokorelasi spasial positif yang signifikan (Global Moran's I = 0,2215; p = 0,016). Analisis LISA mengidentifikasi Kabupaten Karanganyar sebagai satu-satunya klaster high-high pada populasi umum. Model SAR merupakan model terbaik untuk populasi umum dan menunjukkan adanya ketergantungan spasial antarwilayah (ρ = 0,696; p < 0,001). Kepadatan penduduk merupakan satu-satunya faktor yang berhubungan signifikan dengan peningkatan penemuan ODHIV baru (p < 0,001), sedangkan faktor demografi lainnya, sosial ekonomi, dan akses pelayanan kesehatan tidak berhubungan signifikan. Penelitian ini menunjukkan bahwa distribusi penemuan ODHIV baru pada populasi umum dipengaruhi oleh faktor spasial, sedangkan pada populasi kunci tidak ditemukan pola spasial yang bermakna. Temuan ini mendukung pemanfaatan pendekatan spasial untuk mengidentifikasi wilayah prioritas, khususnya klaster high high, sehingga dapat menjadi dasar dalam perencanaan deteksi dini, penguatan layanan HIV dan alokasi sumber daya kesehatan yang lebih efektif di Provinsi Jawa Tengah.

This study  aimed to examine the spatial distribution and factors associated with the distribution of  newly identified people living with HIV (ODHIV) among key populations and the general  population across the 35 districts and municipalities of Central Java Province in 2024. An  ecological study design was employed using secondary data obtained from the 2024  HIV/AIDS Information System (SIHA) of the Central Java Provincial Health Office, the  Central Java Provincial Health Profile, and Statistics Indonesia. Spatial analysis were  performed using Global Moran's I, Local Indicators of Spatial Association (LISA), and  the Spatial Autoregressive (SAR) model. The findings indicated that the spatial  distribution of newly identified ODHIV among key populations did not exhibit significant  spatial autocorrelation. In contrast, significant positive spatial autocorrelation was  observed among the general population (Global Moran's I=0.2215; p=0.016). LISA  identified Karanganyar District as the only statistically significant high-high cluster.  Based on spatial diagnostic tests, the SAR model provided the best fit for explaining the  spatial variation in newly identified ODHIV among the general population and  demonstrated significant spatial dependence across neighboring areas (ρ=0.696;  p<0.001). Population density was the only area-level factor significantly associated with  an increased rate of newly identified ODHIV (p<0.001), whereas other demographic,  socioeconomic, and healthcare access variables were not significantly associated. These  findings suggest that the spatial distribution of newly identified ODHIV in the general  population is influenced by spatial dependence, whereas no meaningful spatial pattern  was identified among key populations. The results highlight the value of spatial analytical  approaches in identifying priority areas, particularly high-high clusters, to support  targeted HIV screening, strengthen HIV service delivery, and improve the allocation of  public health resources in Central Java Province.
