Cakupan kegiatan keperawatan komunitas di Kabupaten Cirebon masih rendah, sedangkan perawat sebagai peiaksananya merupakan tenaga kesehatan terbanyak dibandingkan dengan tenaga kesehatan lainnya Perawat melakukan asuhan keperawatan komunitas seharusnya dengan pendokummentasian atau pencatatan yang merupakan panduan sehingga kegiatannya terarah dan terpadu sesuai dengan masalah yang ditemukan. Tujuan penelitian ini adalah ingin mengetahui kualitas pencatatan asuhan keperawatan komunitas di Kabupaten Cirebon tahun 2006 dan faktor-:fuktor yang mempengaruhinya. Penelitian ini adalah penelitian kuantitatif dengan menggunakan rancangan cross-sectionaL Data dikumpulkan dari 7I perawat puskesmas dengan menggunakan perhitungan besar sampel Lot quality assurance sampling (I:QAS-Lot) secara sistematic random sampling, selain dilak:ukan wawancara juga dilakukan pemerik:saan catatan asuhan keperawatan masing-masing responden 5 dokumen. Penelitian dilakukan pada bulan September 2006 dengan menggunakan analisis univariat dan analisis jalur (path analysis). Dari data yang dikumpulkan diperoleh kualitas pencatatan dengan baik sebesar 59,2 %. Hasil pemodelan dengan analisis jalur temyata kepemimpinan merupakan variabel utama yang mempunyai pengaruh sebesar 46,8 % terhadap kualitas pencatatan asuhan keperawatan, variabel lain yang mempunyai pengaruh terhadap kualitas pencatatan asuhan keperawatan adalah pengetahuan sebesar 3 I %, sikap sebesar 17,6 %, imhalan sebesar 11,6 %, dan variabe1 masa kelja melalui sikap sebesar 4%. Kepemimpinan kepala puskesmas mampu meningkatkan lrualitas pencatatan asuhan keperawatan yang dilakukan karyawannya, selain itu kepemimpinan dapat meningkatkan pengetahuan, mengatur imbalan yang diberikan, dan dapat merubah sikap karyawannya. Bagi Dinas Kesehatan yang mempunyai kewajiban membina : kepala puskesmas maka harus selalu membina dan mengevaluasi kinelja kepala puskesmas.
Scope of community nursing activity in district of Cirebon has undervalued. In fact, number of nurse resources there larger than another medical profession. They to be organized in community nursing activity and nursing activity record-keeping as principal guide, they will working in systematic way and well integrated according to the problem raised from public health service. This research aim to determine the quality of community nursing activity record-keeping in district of Cirebon for year of 2006 and the influencing factors within. Research belong to quantitative research with cross-sectional design. Data collecting using Lot quality assurance sampling (LQAS-Lot) with systematic random sampling method by surveying and interviewing 71 nurse from local public health services, including checking of 5 document from each respondent's nursing record.' Research taken during september 2006 and using univariat analysis and path analysis. The result shows the quality of record-keeping which noticed as good are 52,2 % in value. Modelling result from path analysis put leadership as main variable which influence the quality of nursing record-keeping at 46,8%. Another variable which having influence on quality of nursing record-keeping are nurse's knowledge at 31%, nurse's attitude at 17,6%, rewards at 11,6% and working period at 4%. Leadership skills of local public service's head can improve the quality of record-keeping of community nursing activity by its employees (nurses). Besides,and change in attitude. District public health service which has responsibility in training and developing officer to become head of local public health service should control and monitor their working performance continuously leaderships skills direct to improvement of knowledge, remuneration management, and change in attitude. The process of communication, collaboration and coordination have a majorimpact on the effectiveness of the organization and an important element in theachievement of quality health services. The purpose of this study to analyzepatterns of communication, collaboration and coordination in Puskesmas IbrahimAdjie - Bandung, which has implemented a quality standard ISO 9001: 2008 andas the best health center in 2016 in West Java. The research method uses aqualitative approach is confirmatory. To maintain the validity of the data wasperformed using triangulation sources and methods of data collection is done byin-depth interviews to four people who are important in the process, focus groupdiscussions by six staff, observation and study of the document. The resultsshowed there is a pattern of all levels and channels of communication. Thepattern of broad-spectrum collaboration is secondary. Coordination patterns arestrengthening and expansion. Barriers that often happens, the choice of prioritydelivery of information, the dual role, misunderstanding, trouble harmonize timeactivities with other agencies, the repetition of the process of coordination whenthere is change of officials such as district or village heads, the delay in theapproval of program activity reports from the district and village. Suggestions areto continue to maintain the existing pattern and increase, the need for advocacyfor the strengthening of human resources, the need for a MoU, it is necessary totransfer the pattern of the process that has been ongoing basis to the health centerpersonnel. Pelaksaanaan asuhan keperawatan merupakan standar pelayanan yang harus dilakukan oleh perawat dalam melaksanakan pelayanan terhadap masyarakat dalam hal ini pasien. Dalam pelaksanaannya khusus pada perawat di Puskesmas masih belum optimal dilaksanakan. Tujuan penelitian ini untuk mendapatkan informasi tentang pelaksanaan asuhan keperawatan dan faktor-faktor yang berhubungan dengan pelaksanaannya pada perawat Puskesmas di Kabupaten Sumedang tahun 2002. Penelitian yang digunakan adalah penelitian deskriptif analitik yang menggunakan metode survai dan telaah dokumen dan dilakukan dengan pendekatan potong lintang. Populasi pada penelitian ini adalah perawat puskesmas di Kabupaten Sumedang. Jumlah populasi 274 orang, jumlah sampel 107 orang, cara pengambilan sample dengan sistematik random sampling. Hasil penelitian bivariat menunjukan bahwa (1) Ada hubungan antara variabel individu, meliputi tingkat pendidikan dengan pelaksanaan asuhan keperawatan, golongan/pangkat dengan pelaksanaan asuhan keperawatan dan mesa kerja dengan pelaksanaan asuhan keperawatan. (2) Ada hubungan antara variabel psikologis yaitu sikap dengan pelaksanaan asuhan keperawatan. (3) Ada hubungan antara variabel organisasi meliputi pelatihan dengan pelaksanaan asuhan keperawatan, sarana dan prasarana dengan pelaksanaan asuhan keperawatan. Hasil penelitian multivariat didapatkan bahwa variabel yang berhubungan paling erat dengan pelaksanaan asuhan keperawatan pada perawat puskesmas di Kabupaten Sumedang tahun 2002 adalah variabel psikologis yaitu sikap dengan variabel organisasi yaitu pelatihan. Diantara keduanya mempunyai hubungan yang erat dan model interaksi. Dari hasil penelitian ini dapat disimpulkan bahwa hubungan antara sikap dan pelatihan mempunyai tingkat keeratan yang paling kuat dibandingkan dengan pendidikan, golongan/pangkat, sarana & prasarana, dan masa kerja. Mengingat bahwa sikap dan pelatihan mempunyai keeratan hubungan yang paling kuat, disarankan untuk peningkatan pelatihan asuhan keperawatan secara berkesinambungan pada perawat puskesmas di Kabupaten Sumedang dan menanamkan suasana kerja yang mendukung salah satunya dengan meningkatkan sikap perawat dari aspek kognisi, konisi dan apeksi sehingga terlaksananya asuhan keperawatan pada pasien sehingga pelayanan keperawatan menjadi lebih berkualitas dan sesuai dengan harapan masyarakat pada umumnya dan pasien pada khususnya.
The Factors Related to Treatment Upbringing Execution at Nurses Puskesmas in Sumedang District, a Study in all Puskesmas in Sumedang District, West Java, year 2002The implementation of upbringing treatment is a standard services that should be conducted by nurses in executiag services to society in this case patiens. In its special execution at nurses in puskesmas hasn't been conducted optimally yet. The aim of this research is to get information about treatment upbringing execution and factors related to execution at nurses in puskesmas, in Sumedang district, year 2002. The kind of research that's used is analytic descriptive research by using survey method and document study and conducted with transversal crosscut approach. The population in this research is puskesmas nurses in Sumedang district, The amount of population is 174 samples (people), and the way of intake samples by systematicly sampling random. The result of bivariat research shows that (1) there is a relationship among individual variables, covering education storey level with treatment upbringing execution and year of services with treatment upbringing execution. (2) there is a relationship among psychological variables that is attitude with treatment upbringing execution. (3) There is arelationship among organizational variables, including training with treatment upbringing execution. The result of multivariate research is known that closet corresponding variables with treatment upbringing execution. At puskesmas nurses in Sumedang district is psychological variables that's attitude with organizational variables, training. Between both have a close relationship and interaction models. From the result of this research can be cloncluded that the relationship between training and attitude have a strongest relationship compared by education, factions/ranks, facilities and basic facilities and a period of work. Considering that attitude ang training have a close relationship, suggested to make training increased at nurses and it gives ondosif condition in puskesmas, so that attitude for treatmen upbringing execution become a better thing.
