Background : Globalisation and boundless information have naturally matured patients’ criticism behaviour. Those develop patients’ knowledge and understanding how to claim medical treatments. They would like to demand services in very high quality level and not hesitate to take legal action for any procedure errors. They sue doctors and hospital should they feel being treated that may harm their body. Since medical record states patient’s identities, anamnesis, physical test, result of supporting test, and list of medicine consumed, the quality of medical records simply describe the service quality and act as fundamental verification in facing a patient complaints in court. In RSUD Bekasi, evaluation of medical record forms shows that there is a high degree of incompleteness of filing medical records. Therefore, this research subjects to explore the grade of incompleteness. Method : The research performs with qualitative descriptive approach in which retrospective analysis for six forms in obstetric unit during 2006. Incompleteness filing is analysed along with factors that suppose involved. Qualitative approach is applied through in-depth interview to unit staffs who have competencies and responsibilities in processing medical records. Result: It confirms that all medical records are not filled completely and in some cases, it found that forms are not in place. Two of seventy-two (approximately 3%) forms of follow-up are not found in medical records. Furthermore, three of seventy-two or 4% forms of result of supporting test are out of medical records. In the course of in-depth interview, it can be concluded that the factors that may cause incompleteness are limited staff in related education background, poor condition of medical records processing room, lack of supporting equipment, half-done in implementing standard operating procedures, lack of socialization of regulations and quidances medical records, and lack of supervision from top management. Coclusion: In order to decrease the number of incompleteness medical records, it is strongly suggested to train more medical records staff, to re-condition processing room and to furnish with supporting equipment, to modificate medical record, and to socialise regulation and guidance in filling medical records. It is also recommended to maximise medical records committee in supervising and controlling function. References : 27 (1980- 2007)
Penelitian ini merupakan penelitian kualitatif yang didahulukan oleh studi dokumenter (documentary study) dengan memeriksa kelengkapan rekam medis menggunakan alat bantu daftar tilik yang dibuat oleh peneliti. Motivasi ekstrinsik akan mempengaruhi tenaga medis dalam pengisian rekam medis, diantaranya kerjasama tim dan beban kerja. Motivasi intrinsik seperti pengetahuan dan persepsi dalam pengisian rekam medis juga akan mempengaruhi ketidaklengkapan pengisian rekam medis di suatu pelayanan kesehatan. Motivasi dari pekerja kesehatan berpotensi mempengaruhi penyediaan layanan kesehatan. Semangat rendah diantara pekerja dapat merusak kualitas penyediaan layanan, sementara kehadiran pekerja yang bermotivasi tinggi dan berkualitas merupakan aspek kunci dari kinerja sistem kesehatan.

A medical record is containing data and information to be made by a health service that contains a patient's medical history during the patient is receiving services, from anamnesis, history of the disease, physical examination and other investigations, diagnosis, treatment to action medical services. The completeness of a medical record is desperately needed as a valid proof that a patient is properly receiving health care, especially medical resume can be used as hospital claim sheets to insurance in collaboration with hospitals. The low number of completeness medical record in Bhakti Yudha Depok Hospital is the basis for the researcher to conduct the research. The incomplete filling of medical records is due to many factors, including the motivation possessed by the medical personnel responsible for the medical record.
This research is a qualitative research which takes precedence by documentary study by examining the completeness of medical record using the check list tool made by the researcher. Extrinsic motivation will affect medical personnel in filling out medical records, including teamwork and workload. Intrinsic motivation such as knowledge and perception in filling medical record will also affect the incomplete filling of medical record in a health service. The motivation of health workers has a potential affect the provision of health services. Low morale among workers can damage the quality of service provision, while the presence of highly motivated and qualified workers is a key aspect of health system performance.

ABSTRAK Penelitian dilatarbelakangi sering ditemukan ketidaklengkapan berkas rekam medis di unit rawat inap RSUD Kota Bekasi. Bertujuan menentukan pengaruh serta besarannya perilaku kepatuhan dan kompetensi dokter dalam mengelola rekam medis di unit rawat inap RSUD Kota Bekasi tahun 2013. Desain penelitian kuantitatif, cross-sectional, analisa Structural Equation Modeling-Partial Least Square. Uji chi-square variasi total jawaban variabel perilaku dan variabel kompetensi terhadap karakteristik responden dengan tingkat signifikansi α = 0,05 (CI 95%) menunjukkan P-values (asymp sign > 0,05). Hasil PLS pengaruh (rho) perilaku (69%) dan kompetensi dokter (38%) terhadap kinerja pengisian rekam medis. R-square 0,43 dan Q-square predictive 33%.

Determining the magnitude and influence the behavior of compliance and competence in managing physician medical record unit inpatient hospitals in Bekasi by 2013. Quantitative research design, cross-sectional, Structural Equation Modeling analysis-Partial Least Square. Chi-square test variations total answer variable and variable behavior competence with respect to the characteristics of the respondents with a level of significance of α = 0.05 (CI 95%) shows P-values (asymp sign > 0.05). Results PLS influence (rho) behavior (69%) and competence of physicians (38%) against performance charging medical record. R-square value of 0.43 and Q-square predictive 33%.
This research analyzed the outpatient medical record services at RSUD GunungJati Cirebon. The purposes of this study were to determine the length of time ittakes in a series of processes of outpatient medical record services and to identifyobstacles. The type of research employed was qualitative research. The resultsshowed that the average length of was 151 minutes. The longest process occurredin the search stage of outpatient medical records. The time required in the processof outpatient medical record services was still within the old category, i.e. >10minutes, which was affected by the lack of training on storage, and the non-optimum use of medical record storage cabinets in the storage section.Keywords: medical records, outpatient, length of time
