Abstrak:
Klub Motor X merupakan salah satu klub motor yang anggotanya tersebar di seluruhIndonesia, namun untuk penelitian kali ini hanya dibatasi wilayah Jabodetabek saja.Klub motor ini memiliki agenda rutin yaitu melakukan turing. Dalam perjalananturing tersebut, ditemukan berbagai risiko yang dapat menimbulkan terjadinyakecelakaan. Meski tidak tercatat secara detail, diketahui telah ada kasus kecelakaanbahkan sampai memakan jiwa selama yang terjadi selama perjalanan turing.Penelitian ini dilakukan untuk mengetahui gambaran risiko apa saja yang dapatmenyebabkan terjadinya kecelakaan dengan menggunakan metode Human FactorAnalysis Classification System (HFACS). Kemudian permasalahan terkait gegagalandari sistem pertahanan HFACS yang banyak ditemukan adalah adversephysiological states, adverse mental states, personal readiness, routine violations,dan perceptual errors. Hasil analisis penelitian menyarankan tindakan perbaikanpada tingkat individu untuk kegagalan aktif dan tingkat organisasi untuk kegagalanlaten guna mengurangi atau bahkan menghilangkan kesalahan-kesalahan yang telahteridentifikasi sehingga akan memperkuat ketahanan sistem terhadap terjadinyarisiko kecelakaan.Kata kunci:Keselamatan Berkendara, Turing, Sistem Pertahanan, HFACS

Abstrak:
Operasi Pemboran dan Kerja Ulang PT XYZ terus melakukan inovasi program keselamatan kerja untuk menurunkan tingkat kecelakaan kerja agar mendekati nihil, namun kecelakaan kerja masih terjadi yang salah satu penyebabnya diakibatkan oleh perilaku pekerja yang tidak selamat (unsafe action). Inovasi untuk menurunkan jumlah unsafe action adalah penggunaan CCTV real time 24/7 yang terintegrasi (iCCTV) ke ruang kontrol yang dilengkapi dengan observer yang secara langsung memberikan pengawasan, umpan balik dan coaching kepada pekerja di lapangan yang diobservasi. Penelitian ini bertujuan untuk melakukan analisa gambaran tren unsafe action setelah pemakaian iCCTV terhadap waktu pada kasus berulang, segmen waktu kerja, penghentian kerja, tipe bahaya, jenis pekerjaan dan status tindak lanjut dari temuan kasus. Metode penelitian yang digunakan adalah case study restrospektif dengan mengunakan data sekunder yang diambil dari hasil pengamatan unsafe action selama kurun waktu Maret 2018 sampai dengan Desember 2019, serta data primer hasil wawancara observer. Jenis penelitian ini adalah semi kuantitatif dari studi kasus pelaksanaan iCCTV pada Unit Pemboran dan Kerja Ulang PT XYZ. Hasil penelitian menunjukkan bahwa setelah penggunaan iCCTV terjadi penurunan tren jumlah unsafe action terhadap waktu pada kasus berulang, segmen waktu kerja, penghentian kerja, tipe bahaya, jenis pekerjaan dan status tindak lanjut temuan kasus. Kesimpulannya adalah bahwa penggunaan iCCTV menurunkan tren unsafe action pekerja Pemboran dan Kerja Ulang PT XYZ melalui mekanisme pengawasan, umpan balik dan coaching yang bersesuaian dengan teori Efek Hawthorn, Teori ABC, serta kurva Bradley DuPont

After the implementation of iCCTV, the results showed that the trend of unsafe action is decreasing for recurrent case, work time segment, work stop, hazard types, work types and unsafe case finding follow up action status. As conclusion, the implementation of iCCTV reduce the unsafe action trends through supervision, feedbacks and coaches mechanism that is in alignment with Hawthorne Effect, ABC Theory, and Bradley DuPont Curve
