This study was aim to assess the effect of time to Antiretroviral Treatment (ART) onCD4 response failure in TB-HIV coinfection patients. This study was conductedfrom May to June 2016 at Infectious Disease Hospital Sulianti Saroso. This studyused cohort restrospective design with one and half year time to follow up. Studypopulation were TB-HIV coinfected patients, noted as a naive ART patient inmedical records from january 2010-november 2014. A total 164 patients ≥ 15 yearsold, had Anti Tuberculosis Treatment (ATT) 2 weeks before ART and had minimum2 CD4 sell count laboratorium test results. The cumulative probability of CD4response failure among TB-HIV co-infected patients was 14,43%. Hazard rate ofCD4 response failure was 767 per 10.000 person year in early ART (2-8 weeks afterATT) versus 474 per 10.000 person year in delayed ART (8 weeks after ATT) arm(p=0,266). In multivariate analysis using time independent cox regression test, rateof CD4 responses failure was lower in patients with delayed ART until 8 weeks afterATT than early ART 2-8 weeks after ATT. (Adjusted HR=0,502 ; 0,196-1,287 ; Pvalue=0,151) controlled by types of ARV regiments and classification of TB cure.Keywords: TB-HIV Co-infection, Antiretroviral treatment, CD4 response failure
HIV/AIDS impact to increased susceptibility to other diseases infections whichlead to death. The death of AIDS is also a problem, especially in Indonesia.According to UNAIDS, Indonesia is included in the list of countries where deathsfrom AIDS do not decline or rate of less than 25% of his descent. This research isobservational research, design with cross sectional. This research aims to know thedescription and the main factors which related to mortality of AIDS HIV/AIDS ininpatient unit RSUPN Dr Cipto Mangunkusumo in 2008-2012. The sample of thisresearch are 207 patients. Data collected by utilizing the patient's medical record datato see the independent variables consisted of gender, age, job, CD4 levels, risk factorsof transmission, the amount of illness suffered, nutritional status, history of centralnervous disorders, drug consumption history ARV consumption, and psychologicalconditions to be linked with the status of a patient's death related with HIV/AIDS.The data analysis done to multivariate analysis with prediction model. The resultsshowed that the AIDS death prevalence reach up to 28.5%. The results ofMultivariate analysis obtained 4 variables related to the death of AIDS, poornutritional status (OR=4,75) with 95% CI (2,278-9,917), central nervous disorderhistory (OR=1,82) with 95% CI (1,025-3,251), the number of illnesses suffered morethan 5 disease (OR=4,09) with 95% CI (1,854-9,043), and CD4 levels. CD4 levelsbecame the most influential factors towards AIDS deaths with a value of 5, 9 OR and95% CI (2,096-17,106). From the results can be recommended the efforts toincreased awareness toward control CD4 blood levels for HIV/AIDS patients andother supporting efforts to prevent deaths of AIDS such as improved quality ofnutrition AIDS patients, screening and early detection of central nervous disorders,and prevention of complications of the disease.Keywords: HIV/AIDS, Death, CD4 level, main determinant
As neonatal care is improving and survival rate is better, clinicans are now focus on optimizing postnatal growth. This retrospective study involved preterm infant in RSUPN Cipto Mangunkusumo from 2018-2020 which are inborn and survive at discharge. Exclusion criteria are infants with congenital anomalies and discharged at request. Severe neonatal morbidites are hyaline membrane disease, persistent ductus arteriosus, bronchopulmonary dysplasia, necrotizing enterocolitis, proven neonatal sepsis, and intraventricular hemorrhage. Extrauterine growth restriction is defined as the difference between discharge weight and birth weight more than -0.8 z-score based on Fenton Preterm Growth Chart. Controlled covariates are gestational age, sex, intrauterine growth status, history of resuscitation, prolonged use of invasive ventilation, and postnatal steroid. From 275 subjects, 93 of them had severe neonatal morbidities (33.82%) and 82 had extrauterine growth restriction (29.83%). Severe neonatal morbidities increase risk of extrauterine growth restriction by 1.61 (95%CI 1.05-2.46) after being controlled by covariates. In conclusion, severe neonatal morbidities is an independent risk factor of extrauterine growth restriction among preterm infants.
