High number of fire incidence on building in Indonesia indicates less attention ofthe owner to the completeness of fire protection system in their building.Although there have been a protective device against fire but must be analyzedregarding the effectiveness and standard of such devices. This study uses CFSESsoftware for evaluating fire protection systems in government buildings. Thestudy revealed that the government building is still lacking in the safety variableof fire control, especially on vertical openings parameter. Difficulty in this studyis the lack of data from each building regarding fire safety systems.

Penelitian kualitatif ini bertujuan untuk mengembangkan penerapan piranti lunak POM-QM for Windows 3 pada antrean pasien rawat jalan di Poliklinik Penyakit Dalam RSU OKU Timur, Provinsi Sumatera Selatan. Pengamatan dilakukan untuk mendapatkan komponen penting dari waktu tunggu yang diperlukan untuk mengembangkan piranti lunak. Piranti lunak yang dikembangkan lalu diujicobakan. Hal ini berhasil memberikan informasi kepada pasien tentang lamanya waktu untuk menemui dokter spesialis guna pemeriksaan medis. Antrean pasien di ruang tunggu menjadi lebih tertib. Melalui wawancara mendalam, pasien dan manajer rumah sakit merasa puas dengan penerapan piranti ini.
This qualitative study aimed to develop POM-QM for Windows 3 software application on patients que at the outpatient Internal Disease Clinic of RSU OKU Timur, South Sumatera Province. Observations were conducted to obtain important components of waiting times, which were needed to developed the software. The developed software was tried out. It succeeded to provide information to the patients on length of time to be called by the specialist for medical examination. Patients’ que in the waiting room was more orderly. Through indepth interview, patients and hospital managers were satisfied with the application of this tool.
According to ILO convention (International Labour Organizations), petrochemicalinstallation is one among other industry with a major hazard risk involved in theindustrial process. PT Pupuk Kujang Involved ammonia in the production processsaved on 10000 MT capacity storage tank. Although there is no leaking history forammonia storage tank at PT Pupuk Kujang it does not mean the potential for theevent to happen became impossible. The ammonia storage process still process thepotential of a mishap, malfunction or damage that could jeopardize the integrity ofthe storage tank an eventually leakage could occur. With this background, anconsequence analysis required for PT Pupuk Kujang around the facility to understandthe hazard and risk possibly occur, so proper mitigation and emergency plan could beproduce.In this study, the analysis of the consequences of leakage performed using BREEZEIncident Analyst . This studies show that the occurrence and impact of leakage can becaused by the failure of major and minor failures were analyzed by Fault TreeAnalysis, The projection produced by BREEZE Incident Analyst show the maximumdispersion distance is more 262.6 m with 25 ppm ammonia concentration and164.6 m for 150 ppm ammonia concentration and 116.9 m for 750 ppm ammoniaconcentration. Areas at risk of dispersion of the distribution of ammonia in variousscenarios are Factory Cleaver 1A, 1B Kujang utility plant, cooling tower andammonia refrigerant.Keywords : Storage tank, Ammonia, dispersion, BREEZE Incident analyst, FaultTree Analysis
