ABSTRAK Budaya keselamatan pasien baru mulai tumbuh di RSU Manuaba setelah dicanangkan dan dibentuknya Tim Keselamatan Pasien Rumah Sakit RSU Manuaba tahun 2009. Belum berjalan dengan baiknya sistem pelatihan yang ada terutama dalam konsep keselamatan pasien menggambarkan belum adanya upaya rumah sakit dalam meningkatkan mutu rumah sakit terutama mutu SDMnya. Oleh karena itu peneliti ingin mengetahui kesiapan perawat dalam menerapkan konsep keselamatan pasien di Rumah Sakit. Penelitian ini menggunakan menggunakan desain operational research dengan pendekatan kualitatif jumlah sampel sebanyak 51 orang, yang merupakan jumlah total perawat RSU Manuaba. Hasil penelitian ini menunjukkan bahwa bahwa untuk saat ini perawat belum siap untuk menerapkan konsep keselamatan pasien di rumah sakit, kebijakan keselamatan pasien dan SOP sudah ada tapi belum disosialisasikan dengan baik dan berkelanjutan. Ada perubahan nilai pengetahuan dan sikap perawat ketika diukur sebelum dan sesudah diberikan pelatihan. Anggaran mengenai pendidikan dan pelatihan keselamatan pasien belum ada. Pihak rumah perlu meningkatkan pendidikan dan pelatihan mengenai konsep keselamatan pasien agar terwujud budaya keselamatan pasien di lingkungan perawat RSU Manuaba. Patient safety culture has just begun at Manuaba General Hospital after declerated and performed patient safety team at 2009. The Patient Safety concept had not worked out properly,the hospital had not strongly forced to improve quality of human resources. The study investigated the preparation of nurses in managing patient safety concept at Manuaba General Hospital. The study used research operational design with qualitative method, total sample were 51 nurses who worked at Manuaba General Hospital. The result showed nurses had not ready to implemantation patient safety concept, patient safety policy and standart operational procedure had established but not been good and continously sosialized. There was proggression of knowledge and attitude of nurses pre and post training.There was no budget for patient safety training. The manangement of Hospital need to increase education anda training of patient safety to create it concept as a culture in Manuaba General Hospital. Pada era globalisasi saat ini terdapat kecenderungan peningkatan kejadian tidak diharapkan (adverse event). Berdasarkan dari hal tersebut dikembangkan program untuk lebih memperbaiki proses pelayanan yang kemudian dikenal dengan program keselamatan pasien (patient safety). Namun pada kenyataannya ketika tiba pada pengaplikasian program tersebut di lapangan maka rumah sakit akan kembali menemui berbagai masalah. PSBH merupakan suatu pendekatan yang dapat membantu melaksanakan upaya pemecahan masalah yang terjadi di rumah sakit. Sasaran PSBH ada tiga hal yaitu meningkatkan mutu pelayanan, meningkatkan keselamatan pasien (patient safety) serta meningkatkan efisiensi biaya. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang mendukung keberhasilan pendekatan PSBH dalam meningkatkan keselamatan pasien di rumah sakit kemudian faktor-faktor tersebut akan dikaitkan dengan teori manajemen, teori kepemimpinan serta teori motivasi. Selain itu juga dilakukan penelitian mengenai kesinambungan kegiatan PSBH di rumah sakit. Penelitian dilakukan di Rumah Sakit Pusat Pertamina dan Rumah Sakit Dr.Sardjito dengan 14 informan. Metode penelitian yang digunakan adalah metode kualitatif yaitu wawancara mendalam dan telaah dokumen. Analisis data dilakukan dengan metode analisis isi yaitu membandingkan hasil penelitian dengan teori kepustakaan. Hasil penelitian menunjukkan bahwa PSBH memiliki perencanaan kegiatan yang terstrukur dan terperinci yang disusun dalam bentuk Plan Of Action. Penyusunan POA ini sesuai dengan teori fungsi manajemen. Tipe kepemimpinan yang mendukung keberhasilan PSBH adalah kepemimpinan yang memberikan kebebasan bagi para anggota timnya untuk mengemukakan pendapat dan menitikberatkan pada diskusi kelompok. Tipe kepemimpinan ini sesuai dengan tipe kepemimpinan demokratis. Faktor pendukung motivasi yang mempengaruhi keberhasilan PSBH adalah karena rasa tanggung jawab yang dimiliki terhadap pasien. Sedangkan kegiatan PSBH sebagian besar telah berhasil disinambungkan di kedua rumah sakit dengan cara melegalkan kegiatan tersebut dalam bentuk SOP. Kesimpulan dari penelitian ini adalah pendekatan PSBH tersebut dapat berhasil dikarenakan adanya perencanaan yang jelas dan terstruktur kemudian didukung dengan kepemimpinan yang demokratis dari masing-masing problem solver. Selain itu faktor tanggung jawab terhadap pasien juga mendorong keberhasilan PSBH tersebut. Sedangkan kegiatan PSBH dapat disinambungkan karena kegiatan-kegiatan tersebut dapat dirasakan manfaatnya baik bagi pasien maupun bagai tenaga kesehatan terkait.
In current globalization era theme is tendency of increasing adverse event. Based on that fact a program was developed to fix the services which is known as patient safety program. In reality, when it comes to the application of the program, hospitals will also find some difficulties. PSBH is an approach which can help to resolve a problem in hospitals. There are three aims of PSBH which are increasing the quality of services, increasing patient safety and increasing the cost efficiency in hospitals. The aim of this research is to identify the factors influencing the success of problem solving for better hospitals in increasing patient safety at hospitals. The factors will then be connected to theory of management, theory of leadership and theory of motivation. Beside that a researh is performed toward the continous activities of PSBH at hospitals. Research is performed in Rumah Sakit Pusat Pertamina and Rumah Sakit Dr.Sardjito with 14 informant Research method used is qualitative method that is indepth interview and document study. Data analysis conducted with content analysis method which is comparing the result with bibliography theory. The result of the research shows that PSBH has a structurized and well organized plan which is called Plan of Action. The arrangement of the plan of action is equivalent to the theory of management. The type of leadership influencing the success of PSBH is leadership that gives the freedom to the members of the team to give their opinion and focusing in group discussion. This type of leadership is associated to the democratic type. The motivating factor that influencing the success of PSBH is the responsibility toward patients. The continous activities of PSBH mostly have been sussessfully done by legalisation of the Standard Operating Procedure. The conclusion from this research is the success of PSBH are influenced by the well organized plan of action and supported by the democratic type of leadership from each of problem solver. Beside, the responsibility toward patient also influencing the succes of PSBH. The reason PSBH can be done continously in hospitals is because the advantages of the activities can be feel either by the patient or by the health administrator.
Tujuan: Penelitian ini bertujuan untuk melakukan pengkajian berbagai faktor-faktor yang mempengaruhi kinerja perawat dalam melakukan dokumentasi asuhan keperawatan di Unit Rawat Inap RSU Bintang
Metode: Penelitian ini menggunakan pendekatan kuantitatif dengan analisis cross sectional yang dilakukan pada perawat di instalasi rawat inap RSU Bintang. Penelitian ini melakukan evaluasi terhadap hubungan dari motivasi, kepemimpinan kepala ruangan terhadap kinerja perawat dalam melakkukan pendokumentasian asuhan keperawatan.
Hasil: Penelitian ini menunjukkan bahwa terdapat hubungan antara asppek motivasi kerja yaitu persepsi peran (OR: 12,38; p=0,001), desain pekerjaan (OR: 8,72; p=0,002), kondisi kerja (OR: 29,2; p<0,001), dan pengebangan karir (OR: 7,31; p=0,005) terhadap kinerja perawat. Kemudian berdasarkan variabel kepemimpinan kepala ruangan, aspek kredibilitas (OR: 12,91; p<0,001) dan komunikasi (OR: 8,8; p=0,003) memiliki hubungan dengan kinerja perawat dalam melakukan dokumentasi. Melalui analisis multivariate ditemukan bahwa persepsi peran merupakan faktor yang paling berpengaruh dalam kinerja perawat dalam melakukan dokumentasi asuhan keperawatan (aOR: 19,2); p<0,05).
Simpulan: persepsi peran, desain pekerjaan, kondisi kerja, dan pengembangan karir, kredibilitas kepala ruangan, dan komunikasi kepala ruangan merupakan faktor motivasi yang mendukung untuk tercapainya kinerja perawat yang baik dalam melakukan pendokumentasian asuhan keperawatan di unit rawat inap RSU Bintang.
Background: One of the duties of a nurse is to provide nursing care. What is meant by nursing care is the elaboration of the target regarding the level of performance, the quality of the structure, the process of giving nursing care, to obtain a result which can then be assessed. In the Bintang Hospital itself the nurse's performance evaluation has not been done properly and correctly, and in random inspection there is always an incomplete medical record for nursing care, so this is a reflection of the lack of performance of nurses. On the other hand documentation is an indicator of hospital quality, so that it becomes a problem if documentation is not done completely and accurately.
Objective: This study aims to study various factors that influence the performance of nurses in conducting nursing care documentation in the Star Hospital RSU Inpatient Unit
Method: This study used a quantitative approach with cross sectional analysis carried out on nurses at the inpatient hospital of Bintang Hospital. This study evaluates the relationship of motivation, leadership of the head of the room to the performance of nurses in documenting nursing care.
Results: This study shows that there is a relationship between work motivation aspects, namely perceptions of role (OR: 12.38; p = 0.001), job design (OR: 8.72; p = 0.002), work conditions (OR: 29.2; p <0.001), and career development (OR: 7.31; p = 0.005) on nurse performance. Then based on head room leadership variables, aspects of credibility (OR: 12.91; p <0.001) and communication (OR: 8.8; p = 0.003) have a relationship with the performance of nurses in conducting documentation. Through multivariate analysis it was found that perceptions of role were the most influential factors in nurse performance in carrying out nursing care documentation (aOR: 19,2); p <0.05).
Conclusions: perceptions of roles, job design, working conditions, and career development, credibility of the head of the room, and communication of the head of the room are supporting motivational factors to achieve good nurse performance in documenting nursing care in the inpatient unit of Bintang Hospital.
Medical records are an element that must be fulfilled in a hospital with a good medical record management system that will automatically improve the quality of service for both patients and hospitals. This thesis discusses what factors influence nurse compliance in returning inpatient medical record files at the Kalawa Atei Mental Hospital in 2021. Where there are 3 factors, namely individual factors, pshycology factors and organization factors. This research uses quantitative research with cross sectional method. Data in the form of secondary data obtained from medical records and primary data from questionnaires involving the entire population of nurses who served in inpatient rooms from January to February 2021. The results showed a significant relationship between age, years of service, employment status and knowledge. It is hoped that this research can be input for hospitals with the same characteristics, especially the management of the Kalawa Atei Mental Hospital in compliance with returning medical records of inpatients
