Gangguan kesehatan mental di DKI Jakarta memiliki prevalensi tertinggi di Indonesia, mencapai 24,3%, dengan tingkat ketidakpatuhan pengobatan sebesar 51%. Penelitian ini dilakukan di RS Soeharto Heerdjan sebagai salah satu rumah sakit rujukan utama untuk penanganan masalah kesehatan mental di wilayah ini. Dengan tujuan untuk menganalisis determinan kepatuhan pengobatan dengan lima dimensi WHO. Studi kuantitatif ini menggunakan pendekatan cross-sectional. Data dikumpulkan melalui wawancara dengan kuesioner. Sebanyak 100 pasien rawat jalan yang berusia di atas 18 tahun dipilih secara purposive sampling. Penelitian ini dilakukan pada periode Februari hingga April 2025. Analisis data dilakukan dengan regresi logistik ganda. Hasil penelitian menunjukkan bahwa 52% responden memiliki kepatuhan baik terhadap pengobatan, dengan nilai rata-rata kepatuhan sebesar 80,36, yang dapat disimpulkan sebagai hasil yang baik. Analisis regresi logistik menunjukkan bahwa kesadaran dan sikap terkait penyakit (p=0,033) serta persepsi terhadap peran apoteker/petugas penyerah obat (p=0,005; adjusted OR = 4,49; 95% CI: 1,57–12,86) berhubungan secara signifikan dengan tingkat kepatuhan pengobatan. Variabel yang paling dominan adalah persepsi peran apoteker, di mana pasien dengan persepsi yang kurang baik berisiko 4,49 kali lebih tinggi untuk memiliki kepatuhan buruk dibandingkan mereka yang memiliki persepsi baik (aOR=4,49; 95% CI 1,57-12,86). R-Square penelitian adalah 0,191, yang berarti variabel kesadaran dan sikap terkait penyakit dan persepsi peran apoteker menjelaskan 19,1% variasi kepatuhan pengobatan. Karena itu, perlu peningkatan program komunikasi kepada apoteker untuk memberikan edukasi yang jelas dan mudah dimengerti kepada pasien, termasuk kolaborasi dengan dokter dalam menerangkan manfaat dan efek samping obat.

The aim of the study was to analyze the determinants of medication adherence based on the five dimensions of the WHO adherence framework. This quantitative study employed a cross-sectional approach. Data were collected through interviews using a structured questionnaire. A total of 100 outpatient participants aged over 18 years were selected using purposive sampling. The study was conducted from February to April 2025. Data analysis was performed using multiple logistic regression. Results showed that 52% of respondents demonstrated good adherence to medication, with a mean adherence score of 80.36, indicating a generally good outcome. Logistic regression analysis revealed that awareness and attitude toward the illness (p=0.033) and perception of the role of pharmacists/dispensing officers (p=0.005; adjusted OR = 4.49; 95% CI: 1.57–12.86) were significantly associated with medication adherence. The most dominant variable was the perception of the pharmacist’s role, where patients with poor perception were 4.49 times more likely to have poor adherence compared to those with good perception (aOR = 4.49; 95% CI: 1.57–12.86). The study’s R-squared value was 0.191, indicating that awareness and attitude toward illness and perception of the pharmacist’s role explained 19.1% of the variance in medication adherence. Therefore, it is essential to enhance communication programs for pharmacists to deliver clear and understandable education to patients, including collaboration with physicians in explaining medication benefits and side effects.

Krisis kesehatan jiwa di Indonesia terus meningkat dengan prevalensi gangguan jiwa berat mencapai 630 ribu orang dan lonjakan angka bunuh diri hingga 180% pada 2023, di tengah keterbatasan rasio psikiater yang hanya 1:250.000 penduduk. Kondisi ini menempatkan keluarga sebagai garda terdepan (family caregiver) yang memikul beban multi-dimensi. Penelitian kualitatif ini bertujuan mendeskripsikan secara mendalam proses appraisal dan coping mechanism keluarga di Rumah Jiwa Cipayung, Jakarta Timur, inovasi layanan komunitas di wilayah dengan kasus ODGJ yang tinggi (471 kasus). Menggunakan Model Transaksional Stres Lazarus dan Folkman, data dikumpulkan melalui wawancara mendalam dan observasi. Hasil penelitian menunjukkan bahwa pada tahap primary appraisal, keluarga awalnya memaknai perawatan sebagai situasi menekan (stressful) sebelum berproses menuju penerimaan sebagai ujian hidup. Dalam secondary appraisal, keterbatasan sumber daya internal keluarga diperkuat oleh dukungan eksternal dari tenaga kesehatan dan lingkungan Rumah Jiwa. Mekanisme koping yang digunakan merupakan kombinasi problem-focused coping melalui manajemen pengobatan serta emotion-focused coping melalui pendekatan religius dan dukungan sosial. Konteks layanan Rumah Jiwa Cipayung terbukti mentransformasi penilaian keluarga dari persepsi ancaman menjadi tantangan melalui dukungan sistem yang terstruktur. Penguatan sumber daya komunitas ini mendorong keluarga menggunakan mekanisme koping yang lebih adaptif, baik secara praktis maupun emosional, dalam merawat anggota keluarga dengan gangguan jiwa.
The mental health crisis in Indonesia is escalating, with severe mental disorder prevalence reaching 630,000 cases and a 180% surge in suicide rates in 2023, coupled with a critical psychiatrist ratio of only 1:250,000. This situation positions families as frontline caregivers (family caregivers) bearing multidimensional burdens. This qualitative study aims to describe the appraisal processes and coping mechanisms of families at Rumah Jiwa Cipayung, East Jakarta—a community-based service innovation in an area with high psychiatric cases (471 cases). Grounded in Lazarus and Folkman’s Transactional Model of Stress, data were collected through in-depth interviews and observations. Findings indicate that during primary appraisal, families initially perceive caregiving as a stressful situation before transitioning toward acceptance as a life trial. In secondary appraisal, limited internal resources are bolstered by external support from healthcare workers and the Rumah Jiwa environment. Coping mechanisms involve a combination of problem-focused coping through medication management and emotion-focused coping via religious approaches and social support. The Rumah Jiwa Cipayung service context proved to transform family appraisal from a perceived threat into a manageable challenge through structured systemic support. This strengthening of community resources enables families to employ more adaptive coping mechanisms, both practically and emotionally, in caring for family members with mental disorders.
