Abstrak:
Penerapan keselamatan pasien di Rumah Sakit Setia Mitra belum berjalandengan optimal maka perlu pengukuran budaya keselamatan pasien tertutama dikalangan perawat. Penelitian ini bertujuan untuk mengetahui gambaran persepsiperawat tentang budaya keselamatan pasien dan bagaimana penerapan sasarankeselamatan pasien di Rumah Sakit Setia Mitra. Penelitian deskriptif ini dilakukandengan pendekatan kuantitatif dan kualitatif. Populasi adalah seluruh perawat yangada di Rumah Sakit Setia Mitra yaitu 77 perawat dengan sampel 68 perawat.Hasil penelitian menunjukkan persepsi perawat tentang budaya keselamatanpasien di Rumah Sakit Setia Mitra yaitu 72.1% baik. Dimensi paling banyakdipersepsikan baik yaitu dimensi operan sebesar 75% dan paling sedikitdipersepsikan baik adalah dimensi dukungan manajemen sebesar 1.5%. Secarastatistik usia (p value = 0.048), masa kerja (p value = 0.016) dan jabatan perawat (pvalue = 0.049) memiliki hubungan dengan persepsi perawat terhadap budayakeselamatan pasien (p<0.05). Pada penerapan sasaran keselamatan pasien yaitu55.15% memahami dengan baik bagaimana penerapan sasaran keselamatan pasien diRumah Sakit Setia Mitra. Disarankan agar tim keselamatan pasien yang ada lebihdioptimalkan tugas dan fungsinya sehingga budaya keselamatan pasien di rumahsakit dapat terus ditingkatkan.Kata Kunci : Budaya Keselamatan Pasien, Persepsi Perawat, Penerapan SasaranKeselamatan Pasien

Abstrak:
Praktik cuci tangan merupakan faktor kunci dalam menurunkan dan mencegah infeksi terkait pelayanan kesehatan. Masih rendahnya praktik cuci tangan di kalangan petugas kesehatan menyebabkan angka infeksi di rumah sakit masih cukup tinggi. Penelitian ini bertujuan mengidentifikasi elemen yang menjadi penyebab masih rendahnya praktik cuci tangan enam langkah melalui pendekatan elemen strategi implementasi multimodal WHO di Unit Rawat Inap Rumah Sakit Mitra Medika Batanghari. Dengan pendekatan kualitatif, Hasil penelitian menunjukkan praktik cuci tangan enam langkah baik (89,6%) dan ketersediaan handrub berbahan dasar alkohol merupakan faktor yang paling dominan berkontribusi terhadap pelaksanaan praktik cuci tangan enam langkah.

Abstrak:
Rumah sakit sebagai tempat pendidikan dan penelitian kedokteran, merupakan tempat kerja yang unik dan kompleks dapat menjadi sumber bahaya bagi tenaga kesehatan. Tujuan penelitian ini mengetahui persepsi perawat terkait masa kerja, sikap, pengalaman, pelatihan dan pengetahuan terhadap risiko kesehatan kerja. Design penelitian ini cross sectional, menggunakan kuesioner, analisis data mengunakan uji chi square. Hasil analisis menunjukkan hubungan pengetahuan dengan persepsi risiko kesehatan bermakna dengan nilai P Value = 0,015 (P Value < a), variabel lain tidak bermakna, namun memiliki kecenderungan terhadap risiko kesehatan. Disarankan bagi rumah sakit mengadakan Pelatihan K3 untuk meningkatkan persepsi, pengetahuan, keterampilan dalam mengenali bahaya atau hazard di lingkungan kerja, sehingga perawat di Rumah Sakit Mitra Keluraga Bekasi Barat menjadi sehat dan produktif.

Komunikasi dan edukasi merupakan komponen fundamental pelayanan rumah sakit yang berhubungan langsung dengan keselamatan pasien. WHO melalui Global Patient Safety Action Plan 2021–2030 menegaskan bahwa 10–18% pasien rawat inap global mengalami insiden yang merugikan akibat kegagalan transfer informasi. Di Indonesia, meskipun 92% dari 3.178 rumah sakit telah terakreditasi pada April 2024, kepatuhan pelaporan Indikator Nasional Mutu (INM) hanya mencapai 69% pada tahun 2023, mencerminkan kesenjangan antara status akreditasi dan konsistensi mutu di lapangan. LARS DHP, yang telah mengakreditasi 563 rumah sakit sejak 2021, belum pernah dijadikan subjek penelitian khusus terkait capaian standar Komunikasi dan Edukasi (KE), sehingga terdapat kesenjangan informasi yang perlu diisi. Penelitian ini bertujuan menganalisis capaian pelaksanaan 7 standar komunikasi dan edukasi pada rumah sakit terakreditasi mitra LARS DHP serta mengidentifikasi faktor-faktor yang berhubungan dengan capaian tersebut, meliputi karakteristik rumah sakit (jenis, kelas, kepemilikan, letak geografis), kebijakan organisasi, sistem informasi rekam medis, serta pendidikan dan pelatihan SDM. Penelitian menggunakan desain kuantitatif cross-sectional berbasis data sekunder. Sampel berjumlah 34 rumah sakit yang dipilih dengan teknik total sampling. Data bersumber dari dokumen survei akreditasi LARS DHP menggunakan instrumen STARKES 2024. Analisis dilakukan secara deskriptif dan uji Chi-Square untuk menguji hubungan antar variabel. Hasil penelitian menunjukkan Standar KE 6 (Komunikasi Berkelanjutan) memiliki capaian terendah, dengan hanya 35,29% rumah sakit berstatus Terpenuhi Lengkap (TL), mengindikasikan kelemahan koordinasi edukasi antara rumah sakit dan FKTP. Dari seluruh variabel yang diuji, hanya kepemilikan rumah sakit yang menunjukkan hubungan signifikan (p < 0,05), di mana rumah sakit swasta berpeluang 10,31 kali lebih tinggi mencapai TL dibandingkan rumah sakit pemerintah. Variabel lainnya tidak bermakna secara statistik. Capaian Komunikasi dan edukasi secara umum baik pada standar struktural dan regulatif, namun lemah pada standar yang memerlukan implementasi aktif dan koordinasi lintas institusi. Diperlukan intervensi terarah pada RS pemerintah dan kelas D, akselerasi digitalisasi rekam medis, penguatan jejaring komunitas, serta pelatihan komunikasi efektif bagi seluruh PPA
Communication and education constitute fundamental components of hospital services that are directly linked to patient safety. Through the Global Patient Safety Action Plan 2021–2030, the World Health Organization (WHO) affirms that 10–18% of inpatients worldwide experience adverse incidents resulting from failures in information transfer. In Indonesia, despite 92% of 3,178 hospitals having achieved accreditation as of April 2024, compliance with reporting the National Quality Indicators (Indikator Nasional Mutu/INM) reached only 69% in 2023, reflecting a gap between accreditation status and actual quality consistency in the field. LARS DHP (Lembaga Akreditasi Rumah Sakit Damar Husada Paripurna), which has accredited 563 hospitals since 2021, has never been the subject of dedicated research concerning the attainment of Communication and Education (CE) standards, leaving an information gap that warrants further investigation. This study aims to analyze the level of compliance with seven communication and education standards among LARS DHP-accredited partner hospitals, and to identify factors associated with such compliance, including hospital characteristics (type, class, ownership, and geographic location), organizational policies, medical record information systems, and human resource education and training. The study employed a quantitative cross-sectional design based on secondary data. The sample comprised 34 hospitals selected through total sampling. Data were sourced from LARS DHP accreditation survey documents using the STARKES 2024 (Standar Akreditasi Rumah Sakit/Hospital Accreditation Standards 2024) instrument. Analysis was conducted descriptively, with Chi-Square tests applied to examine associations between variables. The findings indicate that CE Standard 6 (Continuous Communication) recorded the lowest level of compliance, with only 35.29% of hospitals achieving Fully Met (Terpenuhi Lengkap/TL) status, signaling weaknesses in educational coordination between hospitals and Primary Health Care Facilities (Fasilitas Kesehatan Tingkat Pertama/FKTP). Among all variables examined, only hospital ownership demonstrated a statistically significant association (p < 0.05), with private hospitals being 10.31 times more likely to achieve Fully Met status compared to government-owned hospitals. All other variables were not statistically significant. Overall, communication and education compliance was satisfactory with respect to structural and regulatory standards but remained weak in standards requiring active implementation and cross-institutional coordination. Targeted interventions are needed for government hospitals and Class D facilities, alongside accelerated digitalization of medical records, strengthening of community networks, and effective communication training for all Authorized Healthcare Professionals (Profesional Pemberi Asuhan/PPA).

Abstrak:
Rumah sakit sebagai fasilitas kesehatan tingkat lanjut diharapkan dapat memberikan pelayanan paripurna. Dalam prosesnya itu sejalan dengan tujuan Akreditasi Rumah Sakit agar mendapatkan pengakuan mutu serta mengutamakan keselamatan pasien. Tujuan penelitian ini adalah mengetahui kesiapan pemenuhan standar Pencegahan dan Pengendalian Infeksi (PPI) menurut Standar Nasional Akreditasi Rumah Sakit (SNARS) edisi 1 di rumah sakit Mitra dengan menggunakan langkah-langkah dalam problem solving cycle. Penelitian ini adalah riset operasional dengan metode kualitatif dimana pengumpulan datanya dilakukan dengan wawancara mendalam dan telaah dokumen. Hasil penelitan menunjukkan bahwa dari sisi input terhadap sumber daya manusia, sarana dan prasarana, penganggaran dan instrumen sudah maksimal meski dengan berbagai keterbatasan dan kondisi rumah sakit yang masih baru beroperasional. Dalam sisi proses untu pemenuhan kualifikasi sumber daya manusia (SDM) sudah cukup meski pembekalan terhadap pelatihan masih minim dan terbatas pelatihan internal ataupun studi banding ke rumah sakit lain. Pengadaan sarana prasana juga masih menggunakan prioritas yang berhubungan langsung dengan pelayanan seperti pengadaan handrub dan hand soap program cuci tangan dan unit Central Sterile Supply Department (CSSD), londri dan gizi untuk peralatan dan ruangan yang sesuai standar PPI. Pembiayaan masih terkendala karena ketersediaan dana yang terbatas namun mampu dioptimalkan. Pelaksanaan instrumen yang meliputi monitoring evaluasi dinilai masih belum maksimal namun sudah berjalan dengan baik. Sebagai output capaian pemenuhan standar PPI melalui self assessment dari semua bagian input dinilai sudah cukup dan mampu untuk menghadapi porses akreditasi rumah sakit. Kesimpulannya kesiapan SDM, sarana prasarana, kebijakan/ regulasi, penganggaran serta instrumen Standar PPI sebagian besar sudah terpenuhi dan telah siap menghadapi survei akreditasi rumah sakit. Saran bagi Komite PPI dan Infection Prevention and Control Nurse (IPCN), Keperawatan, Manajemen Rumah Sakit serta Tim Akreditasi untuk dapat berkoordinasi secara berkesinambungan agar mendapat umpan balik, melakukan sosialisasi rutin terkait edukasi standar PPI baik kepada staf maupun pasien dan keluarga pasien sehingga mutu rumah sakit terhadap PPI dapat terus dipertahankan dan ditingkatkan.

Procurement of infrastructure is also still using priority directly related to services such as the procurement of hand rubs and hand soap hand washing programs and Central Sterile Supply Department (CSSD) units, laundry and nutrition for equipment and rooms that comply with Infection Prevention and Control standards. Financing is still constrained due to the limited availability of funds but can be optimized. The implementation of the instrument which includes monitoring evaluation is considered to be still not maximal but has gone well. As an output, the achievement of meeting Infection Prevention and Control standards through self-assessment from all parts of the input is considered sufficient and able to deal with the hospital accreditation process.
