ABSTRAK Penelitian dilatarbelakangi sering ditemukan ketidaklengkapan berkas rekam medis di unit rawat inap RSUD Kota Bekasi. Bertujuan menentukan pengaruh serta besarannya perilaku kepatuhan dan kompetensi dokter dalam mengelola rekam medis di unit rawat inap RSUD Kota Bekasi tahun 2013. Desain penelitian kuantitatif, cross-sectional, analisa Structural Equation Modeling-Partial Least Square. Uji chi-square variasi total jawaban variabel perilaku dan variabel kompetensi terhadap karakteristik responden dengan tingkat signifikansi α = 0,05 (CI 95%) menunjukkan P-values (asymp sign > 0,05). Hasil PLS pengaruh (rho) perilaku (69%) dan kompetensi dokter (38%) terhadap kinerja pengisian rekam medis. R-square 0,43 dan Q-square predictive 33%.
ABSTRACT Backed by the research often found incomplete medical record file in unit inpatient hospitals city of Bekasi. Determining the magnitude and influence the behavior of compliance and competence in managing physician medical record unit inpatient hospitals in Bekasi by 2013. Quantitative research design, cross-sectional, Structural Equation Modeling analysis-Partial Least Square. Chi-square test variations total answer variable and variable behavior competence with respect to the characteristics of the respondents with a level of significance of α = 0.05 (CI 95%) shows P-values (asymp sign > 0.05). Results PLS influence (rho) behavior (69%) and competence of physicians (38%) against performance charging medical record. R-square value of 0.43 and Q-square predictive 33%.
Background : Globalisation and boundless information have naturally matured patients’ criticism behaviour. Those develop patients’ knowledge and understanding how to claim medical treatments. They would like to demand services in very high quality level and not hesitate to take legal action for any procedure errors. They sue doctors and hospital should they feel being treated that may harm their body. Since medical record states patient’s identities, anamnesis, physical test, result of supporting test, and list of medicine consumed, the quality of medical records simply describe the service quality and act as fundamental verification in facing a patient complaints in court. In RSUD Bekasi, evaluation of medical record forms shows that there is a high degree of incompleteness of filing medical records. Therefore, this research subjects to explore the grade of incompleteness. Method : The research performs with qualitative descriptive approach in which retrospective analysis for six forms in obstetric unit during 2006. Incompleteness filing is analysed along with factors that suppose involved. Qualitative approach is applied through in-depth interview to unit staffs who have competencies and responsibilities in processing medical records. Result: It confirms that all medical records are not filled completely and in some cases, it found that forms are not in place. Two of seventy-two (approximately 3%) forms of follow-up are not found in medical records. Furthermore, three of seventy-two or 4% forms of result of supporting test are out of medical records. In the course of in-depth interview, it can be concluded that the factors that may cause incompleteness are limited staff in related education background, poor condition of medical records processing room, lack of supporting equipment, half-done in implementing standard operating procedures, lack of socialization of regulations and quidances medical records, and lack of supervision from top management. Coclusion: In order to decrease the number of incompleteness medical records, it is strongly suggested to train more medical records staff, to re-condition processing room and to furnish with supporting equipment, to modificate medical record, and to socialise regulation and guidance in filling medical records. It is also recommended to maximise medical records committee in supervising and controlling function. References : 27 (1980- 2007)
Metode: Penelitian ini merupakan penelitian kuantitatif cross sectional dimana variabel bebas dan variabel terikat diukur secara bersamaan. Metode pengambilan sampel yang akan digunakan adalah teknik sampling jenuh, jadi jumlah sampel yang digunakan adalah keseluruhan jumlah populasi yaitu 80 sampel.
Result: Melalui analisis multivariate menunjukkan bahwa variabel jumlah pasien dan insentif memiliki p value masing-masing 0,009 dan 0,041, dimana p value < 0,05. Sedangkan variabel lainnya nilai p value > 0,05.
Simpulan: Hasil penelitian menunjukkan bahwa variabel-variabel yang memiliki hubungan yang signifikan terhadap tingkat kepatuhan pengisian rekam medis di Rumah Sakit Bali Royal adalah variabel jumlah pasien dan variabel insentif

Introduction: The hospital is one of the facilities or institutions of health service providers (health providers) and in accordance with Law No. 36 of 2009 on Health. One of the conditions that are still often discussed and encountered the current obstacle is the completeness of medical records. This problem is also the attention of researchers after seeing and doing initial research at Bali Royal Hospital. It was found that the completeness of the medical record at Bali Royal Hospital Inpatient Unit is still below the standard set by Ministry of Health stated in Kepmenkes RI No.129 / MENKES / SK / II / 2008 which is 100%. The completeness of medical records for inpatients is crucial to the patient's healing process and maximizing hospital services to patients. Where the level of compliance of doctors in filling out medical records at RS BROS still not reached the target of the hospital that is 90%
Method: This research is a cross sectional quantitative research where independent variable and dependent variable are measured simultaneously. Sampling method to be used is a saturated sampling technique, so the number of samples used is the total population of 80 samples.
Results: Through multivariate analysis showed that the variable number of patients and incentives had p values of 0.009 and 0.041, respectively, where p value 0,05.
Conclusion: The results showed that the variables that have a significant relationship to the compliance level of medical record at Bali Royal Hospital are variable of patient number and incentive variable

Medical records are an element that must be fulfilled in a hospital with a good medical record management system that will automatically improve the quality of service for both patients and hospitals. This thesis discusses what factors influence nurse compliance in returning inpatient medical record files at the Kalawa Atei Mental Hospital in 2021. Where there are 3 factors, namely individual factors, pshycology factors and organization factors. This research uses quantitative research with cross sectional method. Data in the form of secondary data obtained from medical records and primary data from questionnaires involving the entire population of nurses who served in inpatient rooms from January to February 2021. The results showed a significant relationship between age, years of service, employment status and knowledge. It is hoped that this research can be input for hospitals with the same characteristics, especially the management of the Kalawa Atei Mental Hospital in compliance with returning medical records of inpatients
This research analyzed the outpatient medical record services at RSUD GunungJati Cirebon. The purposes of this study were to determine the length of time ittakes in a series of processes of outpatient medical record services and to identifyobstacles. The type of research employed was qualitative research. The resultsshowed that the average length of was 151 minutes. The longest process occurredin the search stage of outpatient medical records. The time required in the processof outpatient medical record services was still within the old category, i.e. >10minutes, which was affected by the lack of training on storage, and the non-optimum use of medical record storage cabinets in the storage section.Keywords: medical records, outpatient, length of time
Pelaporan kesalahan pelayanan merupakan usaha untuk memperbaiki sistem pelayanan dalam mencapai pelayanan yang aman. RSUD Kab Bekasi dalam mengembangkan program keselamatan pasien sejak tahun 2009, yang terlihat dari laporan tahunan program keselamatan pasien, terdapat indikasi perlunya peningkatan kesadaran setiap personil dalam melaporkan kesalahan pelayanan, termasuk perawat pelaksana di unit rawat inap rumah sakit. Penelitian ini bertujuan untuk mengukur persepsi perawat pelaksana dalam melaporkan kesalahan pelayanan serta mencari hubungannya dengan budaya keselamatan pasien, gaya kepemimpinan, dan kerja tim. Penelitian dirancang dengan disain cross sectional dengan menggunakan kuesioner sebagai alat ukur. Pengambilan data dilakukan pada bulan November 2011.Responden merupakan keseluruhan perawat pelaksana di unit rawat inap RSUD Kab. Bekasi dan didapatkan 77 kuesioner yang dapat dianalisa. Data yang diperoleh dianalisa secara univariat dan multivariat dengan menggunakan metode component based structural equation modeling dengan aplikasi komputer SmartPLS. Hasil penelitian menunjukkan budaya keselamatan pasien, gaya kepemimpinan, kerja tim dan persepsi pelaporan kesalahan pelayanan oleh perawat dalam penilaian sedang. Didapatkan pula adanya pengaruh baik secara langsung maupun tidak langsung budaya keselamatan pasien, gaya kepemimpinan, dan kerja tim terhadap persepsi pelaporan kesalahan pelayanan oleh perawat. Total pengaruh sebesar 89%. Persamaan linier yang didapat dari penelitian ini adalah persepsi pelaporan kesalahan = 0,12.budaya keselamatan pasien + 0,30.kepemimpinan transaksional ? 0,22.kepemimpinan transformasional + 0,37.kerja tim + 0,26. Dari penelitian ini dapat disimpulkan perlunya peningkatan faktor-faktor yang terbukti memberikan pengaruh positif terhadap peningkatan pelaporan dapat menjadi dasar usaha perbaikan. Terdapat pula faktor-faktor lain yang tidak masuk dalam model penelitian ini yang mempengaruhi perawat dalam melaporkan kesalahan pelayanan yang masih perlu digali agar pelaporan kesalahan pelayanan di masa depan dapat meningkat.
Reporting errors is an attempt to improve the system in achieving a safe service. From a report in 2010 in RSUD Kab. Bekasi seen that the number of cases or incidents reported has increased, but still needs to improve awareness of any personnel, including nurse in inpatient units. The aim of this study is to measure the nurse?s perception in the reporting of sevice delivery errors and to find a relationship between the behavior to other factors: patient safety culture, leadership style, and team work. This study was using cross-sectional design by questionnaire as a measuring tool. Data was collected in November 2011 from the entire nurse at the inpatient unit of the hospital as respondens. There are 77 questionnaires that can be analyzed. The data obtained were analyzed using multivariate methods by component-based structural equation modeling with computer applications SmartPLS. The results of this study suggest patient safety culture, leadership style, teamwork and the perception of service delivery error reporting by nurses are in intermediate conditions. It was found that there are relationship obtained either directly or indirectly from patient safety culture, leadership style, and teamwork to service delivery error reporting by nurses. This research model can explain the real state of 89%. Linier equation from this model is reporting perception = 0,12.patient safety culture + 0,30.transactional leader ? 0,22.transformational leader + 0,37.team work+ 0,26. From this study it can be concluded that factors that are proven to provide positive influence of this research can be the basis of improvement efforts. In addition, there are other factors that are not included in this study that should be considered that better reporting of medical errors.
