This research examines work-related accidents in the mining industry categorized as injury cases, using the Human Factors Analysis and Classification System in Mining Industry (HFACS-MI). The mining industry is known as a high-risk sector, where serious incidents such as major injuries and fatalities frequently occur despite investigations and preventive measures. The study aims to identify the main causal factors of workplace accidents based on the HFACS-MI framework, which classifies human errors and systemic weaknesses within mining organizations. Using data from injury-related accident cases in 2024, both qualitative and quantitative analyses were conducted to assess contributing factors from the operator level up to the organizational level.The findings reveal that the majority of accidents were triggered by unsafe acts, particularly skill-based errors, indicating deficiencies in workers' basic competencies. Additionally, latent failures, such as inadequate supervision and organizational inefficiencies, were also found to play a significant role.
Penelitian ini mengkaji kecelakaan kerja di industri pertambangan yang tergolong dalam kategori cedera, menggunakan metode Human Factors Analysis and Classification System in Mining Industry (HFACS-MI). Industri pertambangan dikenal sebagai sektor dengan risiko tinggi, di mana insiden besar seperti cedera kerja serius dan kematian sering kali terjadi meskipun telah dilakukan investigasi dan tindakan pencegahan.
Studi ini bertujuan untuk mengidentifikasi faktor-faktor penyebab utama kecelakaan berdasarkan pendekatan sistem HFACS-MI yang mengklasifikasikan kesalahan manusia dan kelemahan sistemik dalam organisasi tambang. Berdasarkan data kecelakaan kasus cedera Tahun 2024, analisis dilakukan secara kualitatif dan kuantitatif terhadap berbagai faktor, dari tingkat operator hingga organisasi.
Hasil penelitian mengungkapkan bahwa kecelakaan paling banyak dipicu oleh unsafe acts, terutama skill-based errors, yang menunjukkan kelemahan pada kompetensi atau keterampilan dasar pekerja. Selain itu, faktor latent failures seperti lemahnya supervisi dan ketidakefisienan sistem organisasi juga berperan penting.

Coal mining industry is an industry with high risks of occupational healthand safety. Hence, it is considered important to conduct a risk assessment andevaluation on occupational health and safety in a coal mining industry. Theexpected result of the assessment and evaluation is an improvement of significantefforts in decreasing occupational accident and illness rate, and also fatality rate incoal mining activities.Focus of this study was OHS risk assessment and evaluation on miningactivity in PT. Adaro Indonesia Site Tanjung Tabalong South Kalimantan in 2013.This study was analytical descriptive using observational approach. This studyused Job Hazard Analysis for hazard and risk identification, and semi-quantitativerisk assessment method by multiplication of consequences, probability, andexposure score. The result of this study showed that mining activity in PT. AdaroIndonesia Site Tanjung Tabalong has 182 OHS risks.Key words: risk, coal, mining activities.

The activities of the gold mining industry are capital-intensive, labor-intensive, and technology-intensive industries. The unharmonious interactions between these three aspects can lead to work accidents and occupational diseases that are closely related to fatigue. The purpose of this study is to determine the factors associated with the prevalence of fatigue in workers at PT. X Year 2017. A cross-sectional study design was used in this study using the Occupational Fatigue Exhaustion Recovery (OFER) questionnaire with a target of 288 respondents from all departments at PT. X. It is known that the prevalence of worker fatigue at PT. X as many as 136 workers experiencing fatigue (36.8%) and as many as 182 workers did not experience fatigue (63.2%). From the statistical test results found two significant variables i.e. job satisfaction variables (p-value = 0.003, OR = 2.140), and job stress variables (p-value = 0,000). In conclusion, the most influential factor for fatigue is psychosocial risk factors. In order to solve this problem, it is necessary to establish a sustainable fatigue management management system, to establish an open dialogue on the management of psychosocial hazards in the workplace, and to implement effective communication and cooperative working culture at every level of the organization.

Coal Mining Operator is one of the high-risk occupations in experiencing fatigue. Thisstudy aim to determine factors associated with fatigue on coal mining operators througha systematic literature review method. Factors studied were shift work, work duration,and workload with covariate of individual factors (age, sleep quantity, sleep quality, andcircadian rhythm) and factors of work & work environment (rest periods, work hours,and work period). This research is an exploratory study with a descriptive methodthrough a systematic literature review of the literature in according to the researchcriteria. This systematic literature review is conducted through the identification,extraction, synthesis, and interpretation of data obtained from 11 selected literature. Theresukt showed that there was an influence between shift work, work duration, andworkload on fatigue in coal mining operators.
ABSTRAK Nama : MOCHAMAD FEBIYANTO Program Studi : Magister Keselamatan dan Kesehatan Kerja Judul : Analisis Profil Persepsi Risiko Supir (operator) Truk Angkut Batubara di PT X Tahun 2017 Sebagian besar kecelakaan di jalan angkut batubara perusahaan pertambangan PT X pada tahun 2016 melibatkan supir (operator) truk angkut batubara yang penyebab utamanya adalah tindakan tidak aman dari supir. Tindakan tidak aman ini dipengaruhi oleh persepsi risiko yang dimiliki oleh supir. Penelitian ini bertujuan untuk melakukan analisis dan melihat profil persepsi risiko supir (operator) truk angkut batubara terhadap faktor penyebab kecelakaan di PT X tahun 2017. Pengukuran profil persepsi risiko menggunakan 6 dimensi psikometri, yaitu kesukarelaan terhadap risiko, kesegeraan akibat, pengetahuan tentang risiko, pengendalian risiko, ketakutan, dan keparahan akibat. Data dikumpulkan dari 121 partisipan yang berasal dari 6 perusahaan jasa pertambangan kontraktor PT X yang melakukan kegiatan pengangkutan batubara. Hasil dari penelitian ini menunjukkan supir (operator) truk angkut batubara di PT X memiliki persepsi risiko yang baik pada 5 dimensi dari 6 dimensi psikometri yang diukur, sehingga secara umum profil persepsi risiko supir adalah baik. Penelitian ini juga menunjukkan usia memberikan perbedaan yang bermakna pada nilai rata-rata persepsi risiko dimensi pengetahuan tentang risiko, ketakutan, dan keparahan akibat. Sedangkan lama masa kerja dan asal perusahaan tidak memberikan perbedaan yang bermakna pada nilai rata-rata persepsi risiko. Kata kunci: Persepsi risiko; psikometri; pertambangan; supir truk
ABSTRACT Name : MOCHAMAD FEBIYANTO Study Program : Magister Keselamatan dan Kesehatan Kerja Title : Risk Perception Analysis of Coal Haul Truck Drivers at PT X Year 2017 Most of the accidents on PT X's coal haul roads in 2016 involve coal haul truck drivers whose main cause is the unsafe act of the drivers. This unsafe act is influenced by the risk perception that the driver has. This study aims to analyze and see the profile of the risk perception of coal haul truck drivers to the factors that cause accidents in PT X year 2017. Measurement of risk perception profile using 6 dimensions of psychometric, namely voluntariness of risk, immediacy of effect, knowledge about risk, control over risk, common-dread, and severity of consequences. Data were collected from 121 participants from 6 mining service contractors PT X who conducted coal hauling activities. The results of this study indicate that the coal hauling truck drivers in PT X has a good risk perception on the 5 dimensions of the six dimensions of the measured psychometry, so that in general the risk perception profile of the driver is good. The study also showed age to have significant differences in the risk perception’s mean value of knowledge about risk, common-dread, and severity of consequences. While the work experience and the origin of the company does not provide a meaningful difference in the mean value of risk perception.
