ABSTRAK Nama : Alifianti Lestari Program Studi : Kajian Administrasi Rumah Sakit Judul : EVALUASI PENGEMBANGAN PUSKESMAS KECAMATAN MENJADI RSUD KELAS D DENGAN STUDI KASUS DI 4 RSUD JAKARTA PUSAT TAHUN 2017 Penelitian ini dilakukan dengan pendekatan kualitatif yang secara umum bertujuan untuk mendapatkan hasil evaluasi pengembangan Puskesmas Kecamatan menjadi Rumah Sakit Kelas D di Jakarta Pusat tahun 2017. Penelitian dilakukan selama dua bulan ( April – Mei 2017 ) dengan mengambil lokasi di 4 RSUD Jakarta Pusat yaitu : RSUD Kemayoran, RSUD Cempaka Putih, RSUD Johar Baru dan RSUD Sawah Besar. Pengumpulan data melalui data primer berupa wawancara mendalam, yang dilengkapi dengan telaah dokumen dan observasi lapangan, kemudian data sekunder yang berasal dari 4 RSUD di Jakarta Pusat. Hasil penelitian menunjukkan banyak penyebab yang sangat mempengaruhi layanan kesehatan yang dberikan di 4 RSUD Jakarta Pusat. Seperti ketersediaan Dokter Spesialis yang tidak mudah mau bekerjasama dengan RSUD Kelas D di Jakarta Pusat. Salah satunya dikarenakan belum adanya kesepakatan pendapatan dokter spesialis. Selain itu kelengkapan sarana prasarana juga berpengaruh dalam pemberian layanan kepada masyarakat, lokasi Rumah sakit yang kurang strategis berpengaruh juga terhadap pemasaran layanan, serta kebijakan pengembangan Rumah sakit dengan layanan unggulan sesuai kebutuhan masyarakat sekitarnya. Sehingga pada penelitian ini dapat disimpulkan bahwa 4 RSUD di Jakarta Pusat cukup berhasil menjadi RSUD Kelas D di tahun 2017 dengan terus memperbaiki dan melengkapi sumber daya manusia termasuk Dokter Spesialis dan melengkapi sarana prasarana kesehatan rumah sakit yang belum terpenuhi secara optimal. ABSTRACT Name : Alifianti Lestari Study Program : Magister of Hospital Administration Title : EVALUATION OF PUSKESMAS KECAMATAN DEVELOPMENT BECOMING CLASS D HOSPITAL WITH CASE STUDY IN 4 HOSPITALS IN CENTRAL JAKARTA 2017. This research was conducted with qualitative approach which generally aimed to get the evaluation result of development of Puskesmas Kecamatan into Class D Hospital in Central Jakarta in 2017. The research was conducted for two months (April - May 2017) by taking the location in 4 Central Jakarta Public Hospital namely: Kemayoran Hospital, Cempaka Putih Public Hospital, Johar Baru Hospital and RSUD Sawah Besar. Data collection through primary data is in-depth interview, completed with document review and field observation, then secondary data coming from 4 hospitals in Central Jakarta. The results showed many causes that greatly affect the health services provided in 4 Central Jakarta hospitals. As the availability of Specialist Doctors who are not easy to cooperate with RSUD Class D in Central Jakarta. One of them is because there is no specialist doctor's income agreement. In addition, the completeness of infrastructure facilities also affect the provision of services to the public, the location of hospitals that are less strategic also affect the marketing of services, as well as development policies of hospitals with excellent service according to the needs of the surrounding community. So in this study it can be concluded that 4 hospitals in Central Jakarta succeeded in becoming RSUD Class D in 2017 by continuously improving and completing human resources including Specialist Doctor and equipping hospital health infrastructure facilities that have not been fulfilled optimally. Service Quality is very important in creating patient satisfaction. This study aims to determine the criteria of patients on hall of residence services, the relationship of dimensions of service quality with patient satisfaction and to determine which dimensions have the most influence on patient satisfaction. This study carried out univariate, bivariate and multivariate analyzes. With a total sample of 150 respondents. It was found that the characteristics of patients for the age with the highest category were in the range of 30-49 years, namely 54%, the sex of the most respondents was 78%, the history of the highest education was SMA 53.3%, the work of the most respondents was IRT, the highest frequency of income was income per month Rp.3,000,000 to Rp. 4,000,000. For bivariate analysis it was found that from the five dimensions of four-dimensional service quality had a significant effect on patient satisfaction: reliability (p value 0.001), responsiveness (p value <0.0001), emphaty (p value <0.0001) and tangible (p value <0.0001). And the multivariate analysis found that the dimensions of reliability and empathic affect patient satisfaction.

Dari hasil penghitungan inimenunjukan pembiayaan terbesar adalah pada operasional yaitu sebesar 54% dari totalbiaya. Pengeluaran terbesar adalah belanja pegawai yaitu sebesar 26% dari total biaya,pemulihan biaya di NICU sebesar 85 % yang menggambarkan NICU mengalami defisit.Dalam mengatasi defisit tersebut manajemen RSUD dapat membuat kebijakan yangberkaitan dengan penetapan biaya satuan serta membuat langkah strategis antara lainmeningkatkan utilisasi, efisiensi, mengurangi lama hari rawat dan melibatkan perawatpelaksana. Rumah sakit dapat meningkatkan pemanfaatan NICU salah satunya denganmenjadi rujukan pelayanan NICU di DKI maupun rujukan nasional yang bekerjasamadengan asuransi swasta dan rumah sakit lainnya

This thesis discuss about the cost recovery rate analysis at NICU RSUD Koja in 2016.The existing problem is the amount of expense that should be borne by the hospital onNICU. This research is qualitative analysis by using quantitative data and interviews.the method use is step down and SWOT analysis.
From the results of this calculationshows that the largest financing is on the operational that is equal to 54% of the totalcost. The largest expense is employee expenditures that is equal to 26% of the total cost,cost recovery in the NICU described that the NICU is experiencing deficit. In addressingthe deficit management of RSUD can create a policy with regard to the costing unit aswell as making strategic steps such as improving utilization, efficiency, reducing lengthof stay and involving nurses. Hospitals can improve the utilization of NICU such asbecoming a referral NICU service in DKI as well as national reference that work togetherwith private insurance and other hospitals.

Mechanical ventilator is one of the important aspects and frequently used for treatment of critical patients in the Intensive Care Unit (ICU). The duration for using mechanical ventilator is important factors caused VAP. This research aimed to find the correlation between duration of mechanical ventilator use and Ventilator Associated Pneumonia (VAP) case in ICU RSUD Tarakan Hospital Jakarta 2014 2017. This was an analitic research with observational study (cross-sectional design). Sampling method used consecutive sampling by observed medical records of patients who used mechanical ventilator at least 48 hours and treated at least 3 days in ICU RSUD Tarakan Hospital Jakarta January 2014 December 2017, as many as 106 sampels. Data was analyzed by chi-square, t-test independent, and logistic regression. Prevalention of Ventilator Associated Pneumonia (VAP) was about 17,9%. Duration of mechanical ventilator use and duration of treatment had significant correlation to VAP. While age, basic disease, sleep position, surgery and antibiotic treatment insignificant to VAP. Multivariate analysis showed that duration of mechanical ventilators use variable was the most dominant variable related to VAP OR=5,265 (95% CI: 1,084-25,548)
