The increasing number of BPJS patients affects the length of service time in the outpatient installation of BPJS at Hermina Bekasi Hospital. Issues affecting the length of outpatient service can be identified by knowing all business processes in the service process using the Lean Six Sigma method. This research is a qualitative research with descriptive design with stages of DMAIC Define, Measure, Analize, Improve, and Control. The results of this study suggest that during the service process, the average length of service process is 200 minutes with a percentage of valuable activities value added of 17 and NVA activity of 83. The largest waste is waiting in the transfer of each stage in the service. Causes of long waiting time include lack of availability of human resources, hospital information systems that have not been integrated, and facilities and infrastructure that have not been adequate. The research also proposed improvements in the form of integrated information system application for registration process, improvement of work environment using 5 S method, proposed SPO making for doctors, room layout changes in pharmacy depot, and installing schedule of drug taking hours. This study uses the Narrative Review method concerning the analysis of theLean Thinking concept towards improving the waiting time for patient services in theOutpatient Hospital in Indonesia. The purpose of this study was to describe theimplementation of the Lean Thinking concept in overcoming the waiting time for patientservices in the Hospital Outpatient Installation. In order to obtain test-worthyliterature, researchers used the PRISMA guidelines. The researcher obtained 12literatures using qualitative and quantitative research methods. The results of the studyconclude that the Lean concept can prove that the outpatient service process ofhospitals in Indonesia is still not Lean. There are 7 literatures that show that Lean iseffective in reducing the waiting time of outpatient services at the hospital as evidencedby adopting Lean's main principles, namely Standardize work and Heijunka. In order toachieve the Lean condition, the Hospital must align the implementation of the 5 Leanprinciples (customer value, value stream, flow, pull, perfection) with organizationalculture, mindset, Lean method, cultivate the 5 S, by involving all parties in the hospitalto solve problems consistently and continuously.Keywords:Outpatient Waiting Time; Lean Hospital.
Outpatient services as the leading unit which displays image of thehospital and the largest contributor to patient must be properly managed,one of which is improving wait-times. Wait-times is one of thedimensions of quality in health care. This research aims to analyze thewait-times in outpatient services at Rumah Sakit Ibu Dan Anak Aulia for2014. This research is using the qualitative and quantitative methods inoutpatient services, starts from registration, polyclinic, pharmacy, andcashier.The results showed that approximate time for registration is 8.77 minutesat peak hours and 7.37 minutes at non peak hours. The longest wait-timesis in the children's polyclinic with approximate wait-times about 63.82minutes at peak hours and 40.27 minutes at non peak hours. Theapproximate wait-times in pharmacy at peak hours is 21.39 minutes forgeneric medicine and 33.92 minutes for personalized medicine, whileapproximate wait times at non peak hours is 17.50 minutes for genericmedicine and 31.23 minutes for personalized medicine. At cashier, theapproximate wait times is 15.09 minutes in peak hours and 9.43 minutesin non peak hours.All wait times is already referred to Minimum standard of services forhospital according to Kepmenkes no. 129/2008 except for the polyclinic'swait times. Wait times is influenced by some factors, consists of man,material, method, machine, and money. At polyclinic, the long wait timesis mainly due to human resources factor, which is indiscipline doctor topractice hours, so the result of this research suggests the management tore-analyze and restore the agreement with the doctors to shorten the waittimes so the quality of service in the future will improve.
Latar Belakang: Waktu tunggu rawat jalan adalah waktu yang diperlukan seorang pasien mulai dari saat mendaftar sampai dengan dipanggil untuk diperiksa oleh dokter spesialis. Merupakan gambaran dari kemampuan fasilitas sarana dan prasarana serta SDM dalam memberikan pelayanan kepada masyarakat. Di RSUD H Hanafie, waktu tunggu masih menjadi kendala dalam pelayanan terhadap pasien poliklinik. Masih cukup sering didapatkan komplain dari pasien baik yang disampaikan langsung maupun lewat kotak saran. Tujuan: Penelitian ini bertujuan untuk mengetahui waktu tunggu pasien rawat jalan, waktu tunggu pendaftaran, waktu tunggu rekam medik, waktu tunggu spesialis serta mengetahui faktor-faktor penyebabnya. Serta mengetahui persepsi dan harapan pasien terhadap pelayanan rawat jalan. Metode: Penelitian ini menggunakan metode Cross Sectional, dengan pendekatan kuantitatif dan kualitatif. Teknik sampel Systemic Random Sampling, Instrumen penelitian Formulir Observasi, Pedoman wawancara mendalam dengan informan, pedoman wawancara mendalam (FGD). Data kuantitatif di analisa univariat dan multivariat menggunakan software SPSS versi 26. Hasil: Rata-rata waktu tunggu 109±25 menit, waktu tunggu pendaftaran 51±20 menit, waktu tunggu rekam medik 20± 9 menit dan waktu tunggu spesialis 38±15 menit. Waktu tunggu poli terbaik adalah poli Syaraf dan Poli Paru dengan waktu rata-rata 99 dan 98 menit dan berbeda bermakna dengan P masing-masing 0,012 dan 0,010. Terdapat sejumlah faktor yang menyebabkan waktu tunggu memanjang, baik kendala SDM, sarana dan prasarana serta regulasi. Background: Outpatient waiting time is the time it takes the patient from registering until being called to be examined by a specialist. This is an overview of the ability of facilities and infrastructure as well as human resources in providing services to the community. At H. Hanafie Hospital, waiting time is still an obstacle in providing services to patients at the polyclinic. Complaints of patients are still quite common, either directly or through the suggestion box. Objectives: This study aims to determine outpatient waiting time, registration waiting time, medical record waiting time, specialist waiting time, and to determine the factors that cause it. As well as knowing the patient's perceptions and expectations of outpatient services. Method: This research uses cross sectional method, with quantitative and qualitative approaches, Systemic Random Sampling, Research Instrument Observation Form, Guidelines for in-depth interviews from informants, and in-depth interview guidelines (FGD). Quantitative data were analyzed univariate and multivariate using SPSS version 26. Result: The average waiting time was 109 ± 25 minutes, registration waiting time was 51 ± 20 minutes, medical record waiting time was 20 ± 9 minutes and specialist waiting time was 38 ± 15 minutes. The best waiting time for the polyclinic was the neurology department and the pulmonology department with a mean time of 99 and 98 minutes and significantly different with P 0.012 and 0.010, respectively. There are a number of factors that lead to prolonged waiting times, including human resource constraints, facilities and infrastructure and regulations.
