Pemerintah saat ini berfokus untuk meningkatkan pelayanan kesehatan yang bermutu dan berkualitas bagi masyarakat baik di fasilitas pelayanan kesehatan primer maupun rujukan. Pelayanan kesehatan yang bermutu dan berkualitas membutuhkan jaminan alat kesehatan yang berkualitas dan laik pakai. Disisi lain alat kesehatan merupakan salah satu aset terbesar dalam pelayanan kesehatan. Saat ini alat kesehatan masih belum terkelola dengan baik sebagaimana data dari WHO bahwa 50% peralatan kesehatan tidak berfungsi dikarenakan tidak tepat dalam perencanaan, kurangnya Sumber Daya Manusia yang mampu mengoperasikan dan memelihara dengan baik. Berdasarkan data kementerian kesehatan bulan September tahun 2023 menunjukkan masih ada sebanyak alat kesehatan yang rusak/tidak berfungsi sebanyak 14,4% dan sebanyak 2,25% alat kesehatan kondisi baik tapi tidak dioperasionalkan. Penelitian ini bertujuan untuk mengetahui penyelenggaraan manajemen pemeliharaan alat kesehatan puskesmas tahun 2023 di tiga kabupaten/kota rovinsi Jawa Timur. Penelitian menggunakan pendekatan proses yaitu input, proses dan output. Metode penelitian yaitu kualitatif dengan desain studi kasus komparatif yang membandingkan penyelenggaraan manajemen pemeliharaan alat kesehatan Puskesmas di Wilayah Kabupaten Malang, Kota Blitar dan Kabupaten Bangkalan Provinsi Jawa Timur. Data primer diperoleh melalui wawancara mendalam, observasi lingkungan dan data sekunder melalui telaah dokumen yang bersumber dari Aplikasi Sarana dan Prasarana Alat Kesehatan (ASPAK), data laporan dari kemenkes, dinas kesehatan dan puskesmas. Informan penelitian ini sejumlah 33 orang mulai dari tingkat Pusat dan daerah (Dinas Kesehatan Provinsi, Kabupaten/Kota, dan Puskesmas). Hasil manajemen pemeliharaan alat kesehatan puskesmas di tiga kabupaten/kota provinsi Jawa Timur berdasarkan input yaitu SDM, anggaran, suku cadang, metode dan peralatan mayoritas masih kurang. Faktor proses mulai dari perencanaan, pengorganisasian, pelaksanaan, dan evaluasi belum optimal dilaksanakan. Output dari pemeliharaan alat kesehatan di tiga kabupaten/kota sangat bervariasi ada cukup berhasil dan ada yang masih kurang. Diperlukan pengelolaan pemeliharaan alat kesehatan yang baik mulai dari sisi input sampai proses untuk mendapatkan hasil yang optimal dalam penyelenggaraan pemeliharaan alat kesehatan puskesmas sehingga akan meningkatkan penyediaan alat kesehatan yang laik pakai.
The government is currently focusing on improving quality health services for the community in both primary and secondary health care facilities. Quality health services require a guarantee of quality and usable medical devices. On the other hand, medical devices are one of the biggest assets in health services. Currently, medical devices are still not well managed as data from WHO that 50% of medical equipment is not functioning due to improper planning, lack of human resources who are able to operate and maintain properly. Based on data from the Ministry of Health in September 2023, it shows that there are still as many damaged / non-functioning medical devices as 14.4% and as many as 2.25% of medical devices in good condition but not operationalised. This study aims to determine the implementation of health centre medical equipment maintenance management in 2023 in three districts / cities in East Java province. The research used a process approach, namely input, process and output. The research method is qualitative with a comparative case study design that compares the implementation of health centre medical equipment maintenance management in Malang Regency, Blitar City and Bangkalan Regency, East Java . Primary data was obtained through in-depth interviews, environmental observations and secondary data through document review sourced from the Application for Medical Equipment Facilities and Infrastructure (ASPAK), report data from the Ministry of Health, health offices and health centres. The informants of this study were 33 people from the central and regional levels (Provincial Health Office, District / City, and Puskesmas). The results of the maintenance management of health centres in three districts / cities of East Java province based on inputs, namely human resources, budgets, spare parts, methods and equipment, the majority are still lacking. Process factors ranging from planning, organising, implementing, and evaluating have not been optimally implemented. The output of medical device maintenance in three districts / cities varies greatly, some are quite successful and some are still lacking. Good management of medical device maintenance is needed from the input side to the process to get optimal results in the implementation of health centre medical device maintenance so that it will increase the provision of medical devices that are fit for use.

Abstrak:
Realisasi dana Bantuan Operasional Kesehatan (BOK) di Puskesmas Kota Serang dari tahun 2014-2016 selalu mencapai 100% setiap tahun dan 30% digunakan untuk kegiatan KIA. Namun, capaian cakupan pelayanan kesehatan ibu dan anak khususnya kunjungan antenatal K4 menunjukkan penurunan dan belum mencapai target (75%). Penelitian kualitatif ini dilakukan di Dinas Kesehatan, 2 (dua) Puskesmas dengan cakupan K4 tinggi dan 2 (dua) Puskesmas dengan cakupan K4 rendah. Hasil penelitian menunjukkan bahwa Puskesmas kekurangan sumber daya manusia dan sarana prasarana untuk program KIA, dana operasional untuk kegiatan preventif dan promotif hanya mengandalkan dana BOK, pengawasan pencatatan pelaporan bidan masih lemah, serta frekuensi pergantian kader yang relatif tinggi. Kata Kunci: Bantuan Operasional Kesehatan, Puskesmas, Kunjungan Antenatal K4 The Health Operational Aid Fund (BOK) has been 100% absorped during 2014- 2016 in health centers in Serang city. Out of the total fund, 30% has been used for KIA program activities. However, coverage of maternal and child health services especially K4 antenatal visit tend to decrease and has not reached the target (75%). This qualitative research was conducted in District Health Office, 2 (two) health centers with high K4 coverage and 2 (two) Puskesmas with low K4 coverage. The result showed that Puskesmas has shortage in human resources and infrastructure facilities for KIA program, while funding to support preventive and promotive activities was only rely on BOK funds. Abstrak:
Antibiotik merupakan obat yang paling banyak digunakan dan sekitar 40-62%antibiotik digunakan secara tidak tepat antara lain untuk penyakit-penyakit yang sebenarnya tidak memerlukan antibiotik. Penggunaan antibiotik yang tidak tepatakan berdampak terjadinya resistensi bakteri terhadap antibiotik, morbiditas dan mortalitas dan juga berdampak negatif terhadap ekonomi dan sosial. Salah satu kebijakan untuk mengatasi permasalahan tersebut adalah dengan pemantauanpenggunaan antibiotik dengan menggunakan indikator peresepan berdasarkanindikator penggunaan obat yang telah dikembangkan oleh WHO. Di Indonesia,pemantauan penggunaan antibiotik di puskesmas dilakukan terhadap ISPA nonpneumonia dan diare non spesifik. Penelitian ini dilakukan untuk membuat modelsistem informasi yang dapat mendukung manajemen puskesmas dalam memantaupenggunaan antibiotik secara terus menerus.Penelitian dilakukan menggunakan desain penelitian kualitatif denganmenerapkan pendekatan model prototyping dalam membangun model sisteminformasi. Penelitian ini menghasilkan rancangan basis data dan desain prototypedari sistem informasi pemantauan penggunaan antibiotik bagian farmasi puskesmas. Sistem Informasi Pemantauan Penggunaan Antibiotik Puskesmas dapat diimplementasikan secara bertahap disesuaikan dengan kondisi dan kemampuan puskesmas. Pada tahap pertama dapat dikembangkan pada ruang obatdengan resep sebagai sumber data dan proses evaluasi dilakukan dengan menganalisis dokumen rekam medis jika indikator penggunaan antibiotik dipuskesmas telah melebihi standar yang telah ditetapkan.

Prevalensi karies gigi anak masih sangat tinggi di Kota Bekasi. Hal ini sangat memprihantinkan karena anak-anak merupakan salah satu modal utama dalam membangun masa depan bangsa. Salah satu cara dalam membantu mengatasi masalah tersebut adalah melalui program UKGS. Program UKGS di Kota Bekasi dilaksanakan oleh semua Puskesmas, termasuk Puskesmas Mustikajaya. Tujuan dari penelitian ini adalah untuk mengetahui hasil implementasi program UKGS pada masa adaptasi kebiasaan baru di Puskesmas Kecamatan Mustikajaya pada tahun 2023. Penelitian ini dilakukan dengan metode penelitian kualitatif melalui wawancara mendalam dengan pihak-pihak yang terlibat dalam program UKGS di Mustikajaya. Tidak ada perbedaan komponen hasil implementasi antara masa sebelum pandemi dengan setelah pandemi. Hasil penelitian menunjukkan bahwa sosialisasi dilakukan oleh Dinas Kesehatan Kota Bekasi kepada Puskesmas Mustikajaya dan dilanjutkan ke sekolah. Terdapat SOP pelaksanaan program UKGS yang bersumber dari Kementerian Kesehatan. Sumber daya manusia dirasakan masih kurang tercukupi, fasilitas dan pendanaan berasal dari BLUD dan BOK. Terdapat SK penunjukkan penanggung jawab program UKGS. Para responden menyatakan berkomitmen penuh pada program UKGS. Paket I dan II Program UKGS Puskesmas Mustikajaya memiliki nilai sebesar 99,8% dan Paket III memiliki nilai 26,9%. Hal ini menandakan program UKGS belum berjalan dengan maksimal dan perlu ada perbaikan dan peningkatan pelayanan UKGS lebih lanjut.
Dental caries children’s prevalence in Bekasi City is still very high. This is a concerning matter since children are one of the main assets to build the nation’s future. UKGS become one of the solutions to overcome the problem. The UKGS program in Bekasi city is implemented by all Community Health Centers (CHC), including Mustikajaya CHC. The aim of the research is to determine the results of the implementation of UKGS program during the adaptation of new habit period at the Mustikajaya CHC in 2023. There is no difference in the components of implementation results between the prepandemic and post-pandemic periods. Qualitative methods was conducted in the research by using depth interview method through the parties that involved in the UKGS program in Mustikaya PHC. The results showed that the socialization was carried by the Bekasi Public Health Office to Mustikajaya CHC and continued to schools. There is an SOP for the UKGS program that made by Ministry of Health. Human resources are still inadequate, facilites and funds come from BLUD and BOK. A decree was appointed to the people in charge of the program. The respondents were fully commited to the UKGS program. The value of UKGS Package I and II are 99,8% while the Package III is 26,9%. This indicates that the UKGS program is not running optimally and need a further improvement and enhancement of UKGS services.
