Early Breastfeeding Initiation (IMD) is an important step to guarantee exclusive breastfeeding which reduces infant mortality. IMD implemented in Cilegon City Hospital is recorded at 72.7% of the target of 100% in 2018. This study explores the factors that influence the implementation of IMD in RSUD Cilegon City with a qualitative approach, using in-depth interviews and document review An in-depth interview was carried out on 14 informants consisting of head of health services, head of maternity room, obstetrician and postpartum mothers. The final results showed that IMD was carried out quite well in normal labor, but not yet The method that needs improvement is IMD Standard Operating Procedure (SOP) and training in the hospital Another factor is the improvement of educational activities for pregnant women in primary health care facilities to improve maternal knowledge about IMD.

Breastfeeding is an easy way to reduce morbidity and mortality in infants and childrens. The delay in giving the first breast milk can be caused by the chosen method of birth such as vaginal or cesarean delivery. This study was conducted in a cross sectional and quantitative approach using the 2017 IDHS data to determine the relationship between cesarean delivery and Early Breastfeeding Initiation (EBFI) in DKI Jakarta Province. The sample used was women of childbearing age 15-49 years during the survey in DKI Jakarta Province who had their last child born in the last 2 years as many as 236 respondents. Univariate, bivariate statistical analysis was performed using chi-square (95% CI) and multivariate analysis with multiple logistic regression tests. The results of the analysis showed that mothers who gave birth by cesarean had 0.52 times [95% CI 0.27-1.01] times lower risk of having an EBFI than mothers who gave birth vaginally. Multivariate analysis in this study showed that mothers who delivered by cesarean had 0.54 times lower risk [95% CI 0.18-1.61] of having an EBFI compared to mothers who gave birth vaginally after controlled by variables such as education level, age, and economic status. It is concluded that policy making and providing interventions on how to implement and the importance of early breastfeeding initiation need to be carried out on mothers who have low levels of education, age under 35 years, low economic status, and who plan to give birth by cesarean delivery method.
Infants given the opportunity to early initiation of breastfeeding by suckling in thefirst hour after birth and making skin contact with a baby for at least 1 hour can reduceinfant mortality. Meanwhile, the role and commitment of health agencies and providers ofservices is needed to support the implementation of early initiation of breastfeeding. Toachieve this it is necessary to do innovations that can increase knowledge and skills ofmidwives.This study aims to determine the influence of the use of communication, information,education media in the implementation of early initiation of breastfeeding by midwife incommunity health centers East Jakarta 2018. Type of quasy experimental research with asample of 42 midwives divided into 21 midwives of the intervention group and 21midwives of the control group. The researcher made a communication, information,education media in the form of posters and booklets about the flow of IMD implementationstage in newborn care that is interesting, easy to remember and never before, then themedia is given to the intervention group. The study was conducted from April to May 2018.Collected data were analyzed univariat and bivariate using independent t-test.The results showed that, in univariate analysis, the mean value of early initiation ofbreastfeeding implementation score in intervention group was higher by 97.41 than themean score in the control group was 78.28. While the results of bivariate analysis withindependent t-test obtained p value = 0.001 which means the value of p <0.05 andstatistically there is a significant difference in the average score of early initiation ofbreastfeeding implementation between the intervention group and the control group.The scores of early initiation of breastfeeding implementation scores by the midwivesin the intervention group that received of communication, information, education mediawith poster and booklet media were significantly higher than the midwives in the controlgroup. So that there is influence of communication, information, education media media onearly initiation of breastfeeding implementation.Keywords: communication, information, education media, Midwife, early initiation ofbreastfeeding.
Prelacteal is foods given to newborns before breastfeeding is established or before breast milk comes out, one of the effects of prelacteal feeding practice is to increase mortality in infants. Efforts to reduce the behavior of prelacteal feeding practice is by implementing Early Initiation of Breastfeeding. Where one of the benefits of implementing EIB is stimulate the release of breast milk. This study uses data from the Indonesian Demographic and Health Survey on 2017 which aims to determine the correlation between the implementation of EIB with prelacteal feeding practice among newborns after being controlled by covariate variables (age, education, occupation, economic status, region of residence, frequency of Antenatal Care visits, birth attendants, type of delivery, and birthweight). The study design used was cross sectional with a total sample of 1,728 respondents. Data analysis used multiple logistic regression. The results showed that there was a correlation between the implementation of IMD with prelacteal feeding practice among newborns in Indonesia on 2017 (ρ-value = 0.0001) with an OR value of 5.20, which means that respondents who did not implement EIB had a 5.20 times more likely to do prelacteal feeding practice among newborns after being controlled by variables the region of residence and type of delivery. The recommendations of this research are technical guidelines and implementation guidelines of existing policies so that these regulations can be implemented by the regions.
