Seiring dengan berjalannya waktu, masyarakat pengguna BPJS merasa puas dengan inovasi pelayanan kesehatan melalui program BPJS. Namun, masalah justru terjadi pada pihak instansi kesehatan yang merupakan pihak pendukung program BPJS Kesehatan yang mengalami masalah pending claim. Permasalahan pending claim ini harus segera diatasi karena pending claim menyebabkan kerugian akibat pembiayaan pelayanan lebih besar daripada jumlah klaim yang dibayarkan. Penelitian ini bertujuan untuk mengidentifikasi faktor input, faktor proses, faktor hasil (output) dalam prosedur pengajuan klaim rawat inap pasien BPJS di RS Hermina Ciputat, dan mendapatkan gambaran yang menyebabkan pending claims (output) di RS Hermina Ciputat. Penelitian ini merupakan penelitian dengan pendekatan observasional deskriptif dengan metode kualitatif melalui wawancara mendalam dan telaah dokumen. Hasil penelitan menunjukkan bahwa faktor input yang mengakibatkan banyaknya pending claims di RS Hermina Ciputat antara lain: faktor Man (kompetensi ataupun pengetahuan dokter spesialis, dokter umum, dan tenaga koder yang kurang terkait klaim BPJS); Money (belum dilaksanakannya secara berkelanjutan evaluasi kinerja kepada dokter spesialis); Methods (belum semua ada dan maksimal untuk panduan praktik klinis/clinical pathways); Materials (aplikasi SIMRS tidak praktis dan lambat, jaringan internet lama); dan Machine (kurangnya sarana dan prasarana). Hasil lain dari penelitian menunjukkan bahwa faktor yang mengakibatkan pending claims di RS Hermina Ciputat antara lain: kesulitan dalam melengkapi bukti administrasi klaim dan kesalahan pengisian administrasi dengan tepat; pengisian berkas rekam medis yang tidak lengkap; kualitas pengisian resume medis dan pengisian resume medis yang tidak sesuai; kurang lengkapnya bukti-bukti penunjang klaim; pemberian kode untuk diagnosa primer dan sekunder yang tidak tepat; dan kesalahan pengentrian jenis perawatan.
BPJS users are satisfied with the innovation of health services through the BPJS program. However, the problem occurred on the health agencies which are supporter institutions of the BPJS Health program which experienced pending claims problem. This problem of pending claims must be addressed immediately because pending claims cause losses due to service costs are greater than the number of claims paid. This study aims to identify input factors, process factors, output factors in the procedure for submitting claims for BPJS inpatient claims at Hermina Ciputat Hospital and to obtain an overview of the causes of pending claims (output) at Hermina Ciputat Hospital. This study uses descriptive observational approach with qualitative methods through in-depth interviews and document review. The results of the study show that the input factors that result in the number of pending claims at Hermina Ciputat Hospital include: Man factor (competence or knowledge of specialist doctors, general practitioners, and coding staff who are not competent handling BPJS claims); Man factor (competence or knowledge of specialist doctors, general practitioners, and coding staff who are lacking in relation to BPJS claims); Money (no continuous performance evaluation or specialist doctors); Methods (not maximal impelementation of practice guidelines/clinical pathways); Materials (the SIMRS application is not user-friendly and slow, slow internet connection); and Machine (lack of facilities and infrastructure).Other results of the study show that the factors resulting in pending claims at Hermina Ciputat Hospital include: difficulties in completing proof of claim administration and administrative filling errors; incomplete filling of medical record files; the quality of filling out medical resumes and filling out medical resumes that are not appropriate; incomplete evidence supporting the claim; inappropriate coding for primary and secondary diagnoses; and errors in determining treatment type.
Ischemic heart disease and stroke are still the leading causes of death worldwide in the past 15 years. In Indonesia, cardiovascular disease increases with increasing years. Cardiovascular disease also has an impact on financing, which is based on BPJS data from 2014-2018 which continues to increase both in the number of cases and costs. A large amount in this case is followed by a large financing burden. As a result of this deficit in the balance sheet, payment of claims was hampered. In addition to the deficit factor, internal hospital administration factors and verification of the BPJS are also the cause of claims not being paid. This study aims to identify problems and solutions in the JKN claim file that is pending in the integrated heart center of Cipto Mangunkusumo Hospital for the period July - December 2019. The research design used is qualitative with a case study approach. The results showed that the problems were incomplete resume, multitasking employees, limited resources, error in the distribution of claim files, information systems that could not accommodate the needs, and external factors from BPJS Kesehatan. Solutions that can be done are increasing commitment and priority, building a special unit for the management of JKN services, providing and improving the capabilities of professional and reliable human resources, providing adequate facilities and budget, implementing a simple and firm system, creating a real time, transparent and integrated, and improves adaptability to external responses
This paper emphasizing the company improvement to solving pending claim issues which almost reach 60% from whole claim in every month period, as the intervention for internal management PT CIGNA Insurance. Internal variable that being used during this research are 3 main factors; (1) Input Factor consist of human resource, SOP, facility, communication coordination; (2) Process Factor consist of claim input, claim review and claim investigation; (3) Output Factor consist of the pending process of payment for claim reimbursement. This research conducted with descriptive qualitative method, with 6 main respondent from the management for the indepth interview process. The results show that PT CIGNA Insurance still need improvement within their management, especially to increase the human resource quality and quantity, review Service Level Agreement, facilitating the claim team with documentation and information system also creating good communication through social media as the alternative way to educate claimant with an attractive way.
Latar belakang: Ketidaklengkapan rekam medis merupakan salah satu penyebab sehingga berkas klaim sering kali tidak lengkap atau tidak tepat waktu. Banyaknya klaim yang tidak berhasil berhubungan dengan penundaan pembayaran klaim JKN oleh BPJS Kesehatan menggangu cash flow RSKD Duren Sawit. Oleh karena itu, rumah sakit perlu melakukan penelitian tentang cara pengisian lengkap rekam medis yang baik. Tujuan: Mengidentifikasi faktor-faktor yang berhubungan dengan kelengkapan dokumentasi rekam medis dan bagaimana faktor-faktor tersebut mempengaruhi proses pembayaran klaim BPJS untuk pasien yang menjalani rawat inap non jiwa di RSKD Duren Sawit dan bagaimana upaya untuk mengurangi klaim pending serta mencegah klaim pending berulang. Metode: Penelitian ini menggunakan pendekatan deskriptif analitik dengan metodologi penelitian kualitatif, dilaksanakan wawancara mendalam dengan informan yang dianggap dapat memberikan informasi yang akurat dan relevan untuk studi tersebut yang melibatkan pengumpulan dan detail dari data klaim pending yang belum terselesaikan, dibagi berdasarkan berbagai aspek masalahnya. Hasil: Penelitian ini menunjukkan bahwa tingkat kelengkapan berkas klaim BPJS pasien rawat inap di RSKD Duren Sawit cukup baik, terutama terkait identitas peserta. Namun, terdapat kelemahan signifikan dalam kesesuaian pengkodean yang memerlukan perbaikan. Penyebab utama klaim yang tertunda adalah ketidaksesuaian pengkodean dan kelengkapan hasil pemeriksaan penunjang. Ketidaklengkapan dokumen klaim dapat mengganggu arus kas rumah sakit, berpotensi mempengaruhi pembayaran gaji pegawai dan penyediaan obat-obatan. Penelitian ini merekomendasikan penguatan manajemen dan pelatihan bagi staf untuk meningkatkan kelengkapan dan akurasi berkas klaim, menekankan pentingnya peningkatan sistem dokumentasi dan pengelolaan rekam medis dalam mendukung kelancaran proses klaim BPJS.
Incomplete medical records are one of the causes of claim files often being incomplete or not submitted on time. The large number of unsuccessful claims related to the delays in payment of JKN claims by BPJS Kesehatan disrupts the cash flow of RSKD Duren Sawit. Therefore, the hospital needs to conduct research on how to properly fill out complete medical records. The objective is to identify the factors related to the completeness of medical record documentation and how these factors affect the BPJS claim payment process for patients undergoing non-psychiatric hospitalization at RSKD Duren Sawit, as well as efforts to reduce pending claims and prevent recurring pending claims. This study uses a descriptive analytical approach with qualitative research methodology, conducting in-depth interviews with informants deemed capable of providing accurate and relevant information for the study, involving the collection and details of unresolved pending claim data, categorized based on various aspects of the problems. The study shows that the completeness of BPJS claim files for inpatients at RSKD Duren Sawit is quite good, particularly regarding patient identity. However, there are significant weaknesses in coding conformity that require improvement. The main causes of pending claims are coding discrepancies and the completeness of supporting examination results. Incomplete claim documents can disrupt the hospital's cash flow, potentially affecting employee salary payments and the provision of medications. This study recommends strengthening management and training for staff to enhance the completeness and accuracy of claim files, emphasizing the importance of improving documentation systems and medical record management to support the smooth processing of BPJS claims.
