The process of returning outpatient medical records at RSUP Fatmawati is not optimal enough, indicated by the process is not in accordance with existing regulations, medical records are late, and not returned from Outpatient Installation (IRJ) to Installation of Medical Record and Information Data Center (IRMPDI) at the same day when it used. The aim of this research is to get analysis result of outpatient medical records return process from IRJ to IRMPDI using lean six sigma approach. This research is operational research type that use quantity and quality design. The results of this research were described with the value stream mapping that showed lead time of outpatient medical records return process for 183,09 minutes, with value added 41,55 minutes (22,7%) and non value added 141,54 minutes (77,3%), the longest process occurs on returning of medical record form IRJ to IRMPDI due to the waiting of IRMPDI officers to take medical records. The biggest waste is waiting for amount of 124,16 minutes (87,72%) of the total waste. Based on fishbone diagram analysis it is known that the root cause of medical record problem mostly comes from man category. Based on that analysis, an improvement proposal will be given as the following development of work standarization, workload calculation, coordination meeting, devide the work into a certain period of time (implementation of heijunka), and improvement of supervision.

This research focuses on the procrastination behaviors of medical record officers in the provision of outpatient medical record files at Fatmawati Hospital in 2017. The aim of this research is to see the procrastination level of medical record officers and to analyze the individual, organizational, and psychological factors that may relate to the provision of outpatient medical record files at Fatmawati Hospital. This research uses quantitative methods using surveys with a cross sectional approach through a questionnaire, and also qualitative methods through in-depth interviews. The results of this study concludes that generally, the level of procrastination of medical record officers is in the medium to high category and that there are individual, organizational, and psychological factors associated with procrastination behaviors of medical record officers in the provision of outpatient medical record files at Fatmawati Hospital in 2017.

This study discusses the completeness of inpatient medical records at Fatmawati Fatmawati in December 2015. The purpose of this study was to analyze the completeness of the contents of the medical records of patients Inpatient as well as the factors relating to the charging of medical records. This research is a quantitative and qualitative methods of observation and in-depth interviews. The results, the average medical records of inpatient patients are 57.8%. One of the parameters to determine the quality of health services in the hospital is the quality of medical record services that is about the completeness of recording medical records. A good medical record quality indicator is the completeness of its content, accurate, punctual, and legal aspects fulfillment. A complete medical record will provide convenience for the provision of information in the hospital. The medical record services at Kasih Ibu General Hospital Denpasar in the implementation are still facing some problems, to wit the return of medical record file that is more than the deadline specified and the completeness of the medical record file. The evaluation results for the third quarter report in 2017 showed that the average length of medical record file returns from July to September 2017 which is more than 1x24 hours is 22% while less than 1x24 hours was 78%. On average incompleteness charging inpatient medical record file at the General Hospital Kasih Ibu Denpasar in July to September 2017 was 73%. The purpose of this study was to determine differences in the completeness of the document review is based on a retrospective review and concurrent review of the patient's complete medical record file Sectio Caesaria (inpasif action) and pneumonia (by conventional measures) Kasih Ibu Hospital in Denpasar. The type of research used in this research is using quantitative approach method by comparing two (2) population that is Retrospecttive Review and Concurrent Review. The study was conducted on the medical records of Sectio Caesaria patients and on the medical records of patients with Pneumonia. Determination of population is done based on clinical pathways in Kasih Ibu Hospital Denpasar. There is a difference to the completeness of the medical record file between the retrospective review and the concurrent review. The data obtained shows that the number of medical record completeness with concurrent review method is higher than in the retrospective review method. This is because the medical recorder to check the completeness of the file when the patient is still inpatient and provide a sign or note on the file that is not complete so that when the doctor visits it is easier to complete the medical records file

ABSTRAK Nama : Ni Wayan Sri Wahyuni Program Studi : Kajian Administrasi Rumah Sakit Judul : Ketepatan Waktu Penyelesaian Klaim Pasien Rawat Inap Jaminan Kesehatan Nasional di Rumah Sakit Ari Canti Kabupaten Gianyar Tahun 2017 Rumah Sakit Ari Canti kerap terjadi keterlambatan pengajuan klaim oleh pihak rumah sakit kepada BPJS Kesehatan yang mengganggu cash flow. Rumah sakit perlu melakukan kajian lebih lanjut melalui penelitian ini mengenai ketepatan waktu penyelesaian klaim pasien rawat inap JKN. Penelitian ini bertujuan untuk menganalisis faktor yang berpengaruh terhadap ketepatan waktu penyelesaian klaim pasien rawat inap Jaminan Kesehatan Nasional. Pendekatan yang dilakukan dengan kuantitatif dan kualitatif (mix methods) yang menggunakan desain potong lintang. Sampel berjumlah 209 dari berkas klaim dan 6 orang informan. Ketepatan waktu klaim pasien rawat inap JKN sebesar 65,4% yang tidak tepat 34,6%. Faktor – faktor yang berpegaruh secara signifikan terhadap ketepatan waktu klaim adalah ketersediaan SEP, kelengkapan berkas rekam medis, ketepatan waktu pengembalian berkas rekam medis, kesesuaian entri data berkas dan ketepatan waktu entri data. . ABSTRACT Name : Ni Wayan Sri Wahyuni Study Programe : Hospital Administration Analysis Title : Timeliness of Claim Settlement on Inpatient Patients of National Health Insurance at Ari Canti Hospital, Gianyar District 2017 Ari Canti Hospital often happens delay in filing claims by the hospital to The Organizer of Social Health Insurance that interfere with cash flow. The hospitals need to conduct further study through this research on the timeliness of settlement of claims of inpatients of National Health Insurance. This study aims to analyze the factors that affect the accuracy of the settlement of claims of inpatients of the National Healthcare. The research using a quantitative and qualitative approach (mixed methods) with using cross sectional techniques. Research sample was 209 of claim files and six informants. The time punctuality of National Health Insurance inpatient claim’s was 65.4% and not punctual around 34.6%. Factors significantly influencing the timeliness of claims are the availability of letters of patient eligibility, medical file completeness, the timeliness of the returning the medical record, the suitability of the data entry files and the timeliness of the data entry.
